DEAR DR. ROACH: After being on Paxil for about 15 years, it wasn’t working anymore. My doctor decreased my dose, and I slowly started Viibryd. The first week I felt great. When I increased the dose of Viibryd, I began feeling sick. I had a bad headache, anxiety and couldn’t sleep, and my muscles began jerking. I went to the doctor, and she had me stop immediately, prescribed Deplin for seven days, and said that I’d had serotonin syndrome. When I looked it up, I found it can be fatal if the SSRI is not stopped. I was off all SSRIs for about 10 days but started feeling unconfident and fragile, so I began taking 25 mg of Paxil CR. My doctor seemed concerned that the serotonin syndrome could return, and advised me to take Deplin along with the Paxil. How common is this syndrome? I am weaning myself off the Paxil, but what if I need an antidepressant? -- M.M. ANSWER: Serotonin syndrome is more common than often is suspected. It is an abnormality of brain chemistry caused most often by medications, especially selective serotonin reuptake inhibitors (SSRIs), like paroxetine (Paxil) and vilazodone (Viibryd). You had many symptoms of serotonin syndrome. Serotonin syndrome may consist of mood changes such as anxiety and restlessness, but a person may develop confusion and disorientation. Abnormalities of the muscles, such as rigidity and the uncontrollable jerking you mention, are very common. Finally, sweating, high body temperature, vomiting and diarrhea also may be part of the syndrome. Treatment is stopping the medication that caused the symptoms. Many experienced practitioners will use the same or similar medications that caused the serotonin syndrome at a lower dose if it is the only effective treatment, but this requires expertise and careful monitoring. Deplin, a brand of methylfolate, is a B-type vitamin that can make SSRIs work better. It’s not associated with serotonin syndrome, as far as I have been able to find. *** DEAR DR. ROACH: A diagnosis of “Cameron erosions of the esophagus” was made after my gastroscopy. I was not able to find that term at nlm.nih.gov/medlineplus. Have you ever heard of this? -- A.M. ANSWER: To be honest, no. However, I did look it up, and found that Cameron erosions are found only in people with a hiatal hernia. A hiatal hernia isn’t like an umbilical or groin hernia; in a hiatal hernia, the stomach goes upward into the chest through the diaphragm, the sheet of muscle that separates your chest from your abdomen. It is our primary muscle for breathing. There normally is a hole in the diaphragm (the hiatus), but in a hiatal hernia, the structures that hold the stomach in place weaken and allow it to go into the chest. The part of the stomach inside the chest is prone to bleeding, and the areas that bleed can be seen by the endoscope and are called Cameron erosions, or ulcers. Small hiatal hernias usually can be managed with medication as necessary. However, large hernias and those with a history of bleeding may require surgical repair. An expert is needed to decide whether surgery is required. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. Readers may email questions to ToYourGoodHealth@med.cornell.edu. MOMENTS IN TIME • The History Channel • On April 18, 1906, an earthquake estimated at close to 8.0 on the Richter scale strikes San Francisco, toppling buildings, igniting fires and breaking water mains. An estimated 3,000 people died and 30,000 buildings were destroyed.

An Easy Way to Add to Your Barbecue Repertoire Grilling is one of the great American rites of summer, but your average burgers and brats can only take you so far. If you’re looking for a way to take your barbecue experience to another level, adding a smoker to your arsenal can really help you bring more flavor to your gatherings. You might think using a smoker would be labor-intensive and time-consuming compared to using a standard grill, but smoking is actually easier than you might think. Smokers are also versatile and comparable in price to a nice grill. “Smoking is a great method for adding flavor to ribs, wings or a pork roast,” said cookbook author John McLemore. “It’s also as simple as preparing your meat, placing it in the smoker and letting it cook while you spend time with your guests on the patio. Once it’s done, your meat is ready to serve.” McLemore offers the following tips for those who are looking to try their hand at smoking: • Before you cook anything in your new smoker, season it by filling it with a handful of wood chips and setting it to its highest temperature with the air vents open. Let it run for about two hours, adding wood chips twice during the process. • Part of the fun of a smoker is that you can use multiple types of wood chips to add different flavors - from standard mesquite and hickory, as well as the less common, but flavorful, apple or pecan. Repeat the process of seasoning your smoker with each new flavor. • An electric smoker can make for simple and convenient barbecuing. Features such as a digital thermostat and remote timer make it easier to monitor your roast. • If your meat reaches your desired temperature before you are ready to serve, simply wrap it in aluminum foil and reduce the smoker temperature until mealtime. • Overloading the smoker with food may cause uneven cooking and extend cooking time. It’s best to allow enough space for proper heat circulation. • If you’re smoking wet ribs, apply barbecue sauce about an hour before you are finished cooking and wrap with aluminum foil. If you prefer a dry rub, apply the seasoning before placing the meat in the smoker. • Barbecue sauce isn’t the only thing you can use to flavor meat. For example, try mixing things up by complementing your pork roast with peanut butter to add a delicious nutty flavor.

I want all those who have worked tirelessly in the severe weather to restore power, those keeping the roads clear as possible, those Emergency and 1st responders having to risk their lives to help others to know your efforts are not unnoticed. You are very much appreciated. Truly owed a big debt of gratitude. Well done and may you be truly Blessed. Hope after all this a little extra rest. Thank you to the Bloomfield Town Workers who put the snow fence up on Route 444/Victor Holcomb Rd every year. The snow fence makes a huge difference when it is snowing and blowing. Went to breakfast at T’s Family Restaurant in Webster recently. Waiting for waitress to bring our bill, she said do you know that lady who just left in this booth? We did not; she asks if we are sure, yes we do not know her, she told us that she had paid for our breakfast. What a wonderful

leave my valuables in plain sight. There’s the old saying: outa sight, outa mind. Leaving something valuable out does tempt unscrupulous people, precisely why my stuff is locked up and out of sight. Sorry it happened to you, but being more careful protects your valuables better.

surprise, have heard that people do this type of thing, but never have experienced it. Thank you so much, and we hope you and your granddaughter had a wonderful day too. We too will pass it on to someone. During the bad weather the last few weeks, I was blessed with caring family, great neighbors and concerned friends. I want to say “Thank You” for ALL you did and for being there. They say you can’t choose anyone to be your relative, friends or neighbors - but I wouldn’t change anyone! Thank You! Kitty Englert West Henrietta Last night my doorbell rang and (to my delight) it wasn’t someone selling roof repair or tree removal service. It was a local Fairport Village Brownie Girl Scout, taking orders for cookies. Instead of placing an order, I made a cash donation on the spot. Later, I discovered a package of (delicious) ‘Thank You’ cookies in my mailbox! What a refreshing and welcome surprise! I did not get the Scout’s name, but I wish I could thank her personally for delivering something more scrumptious than cookies that day! Kudos to The Girl Scouts organization for encouraging the practice, and instilling the value of appreciation in their youngsters.

For those who use [local roads] as a pass through, please slow down. The speed limit is 40 MPH, not 50 or above. This is a residential street and not a speed way. Many residents walk, jog and walk their dogs on this road. It is also getting dangerous to even cross the road so we can get our mail. It boils down to inconsiderate motor vehicle operators. There is also a stop sign. This means that you stop and not drive through it. Use common sense. Maybe it is time we have the County Sheriffs sit in our driveways and give you tickets, so you are even later to wherever, you are headed. If you hit someone, they could lose their life. SLOW DOWN and be CONSIDERATE. To the pizza complaint, contact your local BBB bureau and eat elsewhere. To the stolen lawn mower complaint: it’s not elder abuse. Things get stolen from people any age. Bottom line if you don’t want your stuff stolen, lock it up. If you had a garage sale, box things up and lock your mower up. I don’t

To the person who got what looks like chocolate on the Jayne Ann Krentz Book, Illusion Town. I took the book out after you and ended up having to pay your fine for the damage. It would be nice if you would donate $40 to the Public Library. Regarding Vent about Elevator and Weight the person is OLDER. When people get older, they say things like that. If the person is old cut some slack. It also says judge not lest you be judged. You don’t know who will make it and who won’t, so you yourself stop judging. The problem is people get offended at the littlest things. Only you can decide whether to let it roll off your back or make an issue of it. I think there are more important things in this world that deserves to be angry about. I have a weight problem, if someone says something I don’t come down to their level and have to retort back. By letting it go, I forgive. So you need to forgive this person and show some mercy.

Advice from the Pros: Do Not Neglect Auto Service The average age of a personal vehicle on the road today is more than 11 years, according to recent news reports. In tough economic times, it’s natural to rein in spending. But the experts at the nonprofit National Institute for Automotive Service Excellence (ASE) warn that putting off routine auto maintenance or ignoring service intervals is false economy. Small issues can morph into more expensive repairs. For example, failure to replace worn brake pads or a faulty oxygen sensor -- both relatively easy services for qualified auto technicians -can result in expensive brake rotor service or costly replacement of the vehicle’s catalytic converter. Frequently neglected items include oil changes, tire and brake service, wheel alignment, air and fuel filters, and transmission service, according to ASE-certified auto technicians. Besides, a well-maintained vehicle gets better gasoline mileage, pollutes less, is safer to operate, and lasts longer than one that’s neglected, notes ASE.

585-237-2750 Rebecca Y. M. Barnhoorn March 31, 2017

Rebecca Y. M. (Becky) Barnhoorn, 78, of Honeoye, beloved mother, grandmother and great-grandmother, passed away peacefully, March 31, 2017 at F.F. Thompson Hospital, Canandaigua. Born on February 19, 1939 in Bath, NY, as Laura Jane Wilson, she was given up for adoption by her natural parents, the late Orin and Helen Jane (Williams) Wilson, when she was 8 months old. On August 26, 1940, she was formally adopted by the late Robert and Isabel (Snay) Wilcox of Bath, and her name was legally changed to Rebecca Yvonne Wilcox. She later added Mary to her given name. During her youth in Bath, Rebecca went to Haverling Central School. She was active in the Girls Scouts, and the Centenary Methodist Youth Fellowship, participating in the choir and as a pianist. Because of a lack of love by her adoptive mother, Rebecca left the Wilcox home. She lived her late teen years in foster homes in New York City, Hornell and Rochester. She completed her education at Nazareth Academy in Rochester, graduating in 1959. On November 5, 1960, Rebecca married Bart Joseph Barnhoorn at the Church of the Blessed Sacrament in Rochester; Bart predeceased her on May 3, 1996. Rebecca worked as a nursing assistant at St. Ann’s Nursing Home, Rochester; Elm Manor Nursing Home, Canandaigua; Honeoye Lake Home for Adults; and Sisters of St. Joseph’s Infirmary, Rochester. She also provided private duty in-home nursing care. In the 1970s, Becky served as a member of the Committee for the Handicapped at Honeoye Central School. She also was an organist at the former St. Michael’s Church, Livonia Center. On December 17, 1990, after over half century of separation, Rebecca was united with her biological brother Paul Douglas Watkins. The reunion was made possible by the persistent research of Rebecca’s son, Stephen. In addition to her biological parents and husband, she was predeceased by her brother and sister-in-law, Paul and Shirley Watkins, paternal half-sister, Thelma Jennie Wilson, and niece Colleen Jane Watkins. Rebecca will be sorely missed by five children, Michael J. Barnhoorn of Honeoye Falls, Daniel J. (Andrea) Barnhoorn of Scottsville, Stephen A. Barnhoorn of Honeoye, Bart J. Barnhoorn of Lima, and Toni (James) Sloan of Bloomfield; five grandchildren, Nickki Lee Barnhoorn, Lydia Anne Guthrie, USA Sgt. Suzanne W. Barnhoorn-Beardslee, Elliott J. Guthrie, and Christopher W. Barnhoorn; great-grandson, Elijiah Joseph Perrin; many nieces and nephews. A memorial service will be held on Saturday, April 8, 2017, 10:30 AM at St. Mary’s Church in Honeoye, Father Hoan Dinh, officiating. Interment at St. Mary’s Cemetery, Honeoye. In lieu of flowers, contributions may be made in Rebecca’s memory to the Richmond Fire Department, 4830 County Rd. 36, P.O. Box 469, Honeoye, NY 14471. Arrangements by Kevin W. Professional Women of the Finger Lakes have announced their annual Women of Distinction event to be held on May 3rd at the Inn on the Lake in Canandaigua. The event recognizes outstanding women in the categories of Professional Achievement, Entrepreneurial, Volunteer Service, Women Helping Women and Outstanding Youth. Olympic Gold medalist Megan Musnicki will be the keynote speaker for the event. Musnicki is a two-time Olympic champion as a member of the women’s eight rowing team in both 2012 and 2016. “The Women of Distinction Event recognizes outstanding women who contribute to their professional and volunteer organizations,” said event committee co-chair Rhonda Vaccaro. “It’s always an inspiring night.” Proceeds from the event support the Professional Women of the Finger Lakes Scholarship program, supporting local women and High School students for nearly fifty years. Scholarships range from $500 to $1,000 and have topped $80,000 in the last ten years. “The adult women learners are really the heart and soul of our work,” said Scholarship Committee Co-chair Amy Morrisey. “These range from women who stayed home to raise their families to those who experienced difficult times in their lives and are not what you would consider traditional students. Supporting these women as they return to school and continue their education is inspiring to see.”

Professional Women of the Finger Lakes have been active in the community since the 1950’s, when they were first founded as the Canandaigua Chapter of Business and Professional Women. The group expanded to include the entire Finger Lakes region and changed their name in 2010. Today’s membership includes over fifty women who are business leaders, volunteers and philanthropists in their communities. Goals of the group include networking, professional development, community involvement, and work-life balance. Events are held monthly and include networking events, professional development seminars, and social events. Membership is open to all women in the Finger Lakes region- regardless of work status. For more information on Professional Women of the Finger Lakes and the scholarship program, please visit their website www.pwflnys.org.

Avoid aches and pains when gardening People who have not spent much time in a garden may not consider this rewarding hobby much of a threat to their health. But as veteran gardeners can attest, gardening can contribute to nagging aches and pains that can force even the most ardent green-thumbers indoors. Gardening is a physical activity that, despite its peaceful nature, can be demanding on the body. Thankfully, there are several ways that gardening enthusiasts can prevent the aches and pains that can sometimes pop up after long days in the garden. · Use ergonomic gardening tools. Ergonomic gardening tools are designed to prevent the types of aches, pains and injuries that can cut gardeners’ seasons short. Gardening injuries can affect any area of the body, but injuries or aches and pains affecting the back, wrists and hands are among the most common physical problems gardeners endure. Look for ergonomic tools that reduce the strain on these areas of the body. Even arthritis sufferers who love to garden may find that ergonomic tools make it possible for them to spend more time in their gardens without increasing their risk for injury. · Alternate tasks. Repetitive-strain injuries can affect gardeners who spend long periods of time performing the same activity in their gardens. By alternating tasks during gardening sessions, gardeners can reduce their risk of suffering repetitive strain injuries. Alternate tasks not just on muscle groups worked, but also level of difficulty. Remember to include some simple jobs even on busy gardening days so the body gets a break. · Take frequent breaks. Frequent breaks can help combat the stiffness and muscle aches that may not appear until gardeners

finish their gardening sessions. Breaks help to alleviate muscles or joints that can become overtaxed when gardening for long, uninterrupted periods of time. When leaning down or working on your hands and knees, stand up to take breaks every 20 minutes or the moment aches and pains start to make their presence felt. · Maintain good posture. Back injuries have a tendency to linger, which can keep gardeners indoors and out of their gardens. When gardening, maintain good posture to prevent back injuries. Gardening back braces can protect the back by providing support and making it easier for gardeners to maintain their posture. Tool pouches attached to gardening stools or chairs also can be less taxing on the back than gardening belts tied around the waist. Gardening might not be a contact sport, but it can cause pain if gardeners do not take steps to prevent the onset of muscle aches and strains when spending time in their gardens.

Once the invitation is extended, make a reservation. Even if the client cannot make it, it’s possible to cancel the reservation. When choosing a restaurant, look for one with an established reputation, and preferably one you have patronized in the past. Avoid restaurants with loud music or ones that are considered trendy, as such locales are often louder and less conducive to conversation. Another thing to discuss when making the reservation are the seating arrangements. If the restaurant is known for its views, reserve a table with a great view and make sure the clients get the best seats at the table. Clients should not be looking at the wall or sitting in a busy location, such as outside the kitchen or near the bar or restroom.

BE SAFE, HEALTHY AND HAPPY IN THE WORKPLACE WHILE PREGNANT The first thing pregnant working women should know is that they have the right to work while pregnant. In the United States, it has been illegal to discriminate against pregnant women in the workplace since 1978, when the Pregnancy Discrimination Act was passed. But in spite of those laws, some employers may be less receptive to their female employees’ pregnancies, with some insisting they harbor good intentions, such as the woman’s safety. Businesses cannot fire employees for being pregnant, and many courts have ruled that decisions about the safety of the woman and fetus are up to the employee and her doctor, not her boss. On the same token, a company cannot refuse to hire a person simply because she’s pregnant.
