10 Top Tips for Hospital Discharge 1), Ask the staff caring for you on the ward when you’re likely to be getting home. They should be able to give you an expected date of discharge. 2), When you have a discharge date, arrange for a relative or friend to collect you or let the staff know if they need to make other transport arrangements for you. 3), Make sure you have keys to get in! 4), Don’t forget to ask for any medical certificates you may need whilst on the ward. 5), Some hospitals have a Discharge Lounge where you can wait before you’re collected. This is a comfortable, safe and relaxing environment with magazines and refreshments.
7), Your Discharge Letter will be given to you with your medications when they are ready – the Discharge Lounge can speed this process. 8), It may be helpful to ask a friend or relative to stay with you or visit regularly. 9), If this is not possible, make sure you have plenty of food, drink and other essentials at home. 10), If you have any queries, ask the nurse in charge of the ward/department.
