2020 was a difficult year for many reasons. Stress levels were elevated. Consuming the wrong foods and drinks. Not exercising. Skipping our doctor visits. All in all, just a year to forget. Now that it is 2021, let us start the year fresh. Manage stress. It is near impossible to eliminate stress from our lives: From busy work schedules, to family obligations, to financial worries; sources of stress and anxiety are everywhere. There is a strong connection between stress and illness: Stress promotes higher cortisol levels, which has been linked to weight gain. Also, stress can play a huge role in many digestive problems, particularly GERD and IBS. Research shows that anxiety, depression, and stress provoke symptoms in up to 60 percent of people with IBS! Stress management tools such as deep breathing, relaxation and hypnosis can be extremely helpful.

Eat in moderation. Keep balance, variety, and moderation in your eating habits. Take your time and try not to eat too fast: Slowing down and eating more mindfully helps prevent overeating. You will feel full much sooner than if you will if you race through your meals. Remember, it takes your brain 20 minutes to realize that your stomach is full. Exercise. Getting physical activity will not only help shed pounds, but it can also help moderate stress and boost your mood, which in turn will help combat IBS and GERD. Wintertime activities include skating, sledding, skiing, walking in the mall or outdoors, dancing and working out in a gym or at an exercise class. Bundle up and take a walk around the neighborhood and you will burn 324 calories in an hour. Try to maintain a set schedule for exercise to help boost your metabolism, mood, and energy. Regular screenings. If you are 50 years of age, it is time to get screened for colorectal cancer. Colonoscopies help detect colon cancer. No other colon cancer screening is as thorough and comprehensive to detect cancer and remove precancerous lesions as a colonoscopy. It is the most effective means of prevention, diagnosis, and treatment. Do not ignore your symptoms. A change in bowel habits that lasts more than a few weeks; a feeling of having to have a bowel movement that does not go away; rectal bleeding; stomach discomfort; unexplained weakness or fatigue; and unexplained weight loss. Colon Cancer Screening- One Day Can Save Your Life According to data from the American Cancer Society, colon cancer is the 3rd most common cause of cancer-related death for both men and women. That is a disappointing statistic, because colon cancer is one of the most preventable cancers. Unlike many other cancers, there is an easy and effective way to screen for colon cancer. Colonoscopy is readily available and effective. Large, national studies have shown that colonoscopy reduces an individual’s risk of dying from colon cancer by nearly 70%.

Colonoscopies help detect colon cancer. No other colon cancer screening is as thorough and comprehensive to detect cancer and remove precancerous lesions. It is the most effective means of prevention, diagnosis, and treatment. Colon cancer is typically (but not always) caused by abnormal growths called polyps, or adenomas, that develop very gradually over time to become cancerous. During the colonoscopy, the physician will look for and remove these polyps. By removing polyps early, the risk of developing cancer is reduced. Without colonoscopy, the polyps continue to grow, abnormal cells continue to change, and cancer may develop. Colonoscopies can be used to diagnose colitis, which is a chronic inflammation of the colon, and treatment can be prescribed immediately. Colonoscopies can be used to diagnose diverticulosis, which are pockets in the intestinal wall. Colonoscopies can detect bleeding lesions which can then be treated. Colonoscopies are one of the best ways to help diagnose the cause of abdominal pain. Symptoms like anemia, weight loss, abdominal pain, and /or cramping could arise from many different causes. A colonoscopy can help determine if symptoms are related to a colon disease.

Health & & Wellness Wellness Health HGA’s 9 Step Prescription for Good Digestive Health Improve Your Vitality & Eliminate Discomfort The key to good nutrition depends on two things: Eating the right foods and maintaining a strong digestive system that allows your body to break down and absorb the nutrients from the foods you eat In order to keep your digestive system in optimum shape, follow these guidelines: 1. Focus on fiber. Ideally, you should be eating 20 to 35 grams of fiber — or about 14 grams of fiber for every 1,000 calories you eat. Fiber is the workhorse that slows down digestion and gives your body time to absorb the glucose it needs to produce energy. 2. Drink lots of water. Water helps move nutrients into your organs and tissues. It helps regulate your body temperature. An adequate amount of fluid also softens your stool for easier elimination. 3. Avoid processed foods. Many processed foods have little or no fiber, are too high in sodium, and often contain preservatives and other additives that you may be sensitive to. Some processed foods also contain lactose, which can give you gas. 4. Cut down on fat. Foods that are naturally high in fat or are fried in fat alter the digestive process and can lead to constipation or diarrhea. No more than 30% of the calories you consume should be fat; and only one-third of fat calories should be saturated fat. 5. Limit your alcohol consumption. Drinking too much alcohol can have a negative impact on every organ in your body. 6. Add yogurt to your food plan. Your digestive system contains healthy bacteria, known as probiotics, which help your digestion and fight off disease. Both yogurt and kefir contain probiotics. Eating them on a regular basis can help replace good bacteria lost by eating a poor diet, illness, stress and some medications and medical treatments. 7. Reduce your stress. Stress can be harmful to your digestion. It can cause your esophagus to go into spasms, your stomach to produce more acid and your colon to respond in a way that causes a change in your bowel habits and peptic ulcers. 8. Maintain a healthy weight. Your weight affects the efficiency of your digestive system. Even a few extra pounds can cause acid reflux, a condition where stomach contents flow back into your esophagus and cause heartburn. Losing weight will not only help you feel better, it can also prevent more serious conditions, such as hiatal hernia, non-alcohol fatty liver disease and colorectal cancer. 9. Exercise! Regular exercise improves your digestion by increasing blood flow to all your organs. It can help you lose weight and reduce stress. It can also strengthen the abdominal muscles and improve colon transit times along your gastrointestinal tract, helping your colon work more efficiently to eliminate food waste.

HEMORRHOIDAL TREATMENT Over half the population will experience some type of hemorrhoidal problem in their lifetime. Can People With Peanut Allergies Get The Covid Vaccine? Presented by Alexandra Gallagher Starting August 1, 2021, Alexandra will be a student for PreMed/Biology at the University of Miami.

By Aristos Georgiou NEWSWEEK 12/18/20 The Pfizer/BioNTech COVID-19 vaccine is now being rolled out across the United States as part of the largest vaccination campaign in American history. But can people with allergies to food—such as peanuts—get the shot? According to guidance from the U.S. Centers for Disease Control and Prevention (CDC,) people who have a history of food, pet insect, venom, environmental, latex or oral medication allergies can receive the Pfizer/ BioNTech vaccine.

The American College of Allergy, Asthma, and Immunology has said that the vaccine should not be administered to anyone with a known history of severe allergic reaction one of the components called polyethylene glycol or PEG in particular. This substance has previously been documented as a a cause of anaphylaxis by scientists, although these reactions are rare. The CDC says that individuals who have a history of severe allergic reaction to any vaccine or injectable therapy can still get the Pfizer/ BioNTech vaccine but they should take extra precautions. The agency recommends that such individuals should undergo a risk assessment and discuss the potential harms of taking the vaccine with their doctors. Those with a history of only mild allergic reactions to vaccines or injectable therapies can take the Pfizer/BioNTech vaccine without these extra precautions, according to the CDC.

The CDC recommend that vaccine providers should observe people with a history of anaphylaxis—a severe, potentially life-threatening allergic reaction—due to any cause for 30 minutes after the individual receives the shot. Everyone else should be observed for 15 minutes. “If you have a severe allergic response—for instance people have peanut allergies or egg allergies or food allergies—you can still get this vaccine, you just need to wait for 30 minutes in the area where you got the vaccine so if you do have an allergic reaction someone would be able to give you a shot of epinephrine to reduce it,” Dr. Paul Offit, who sat on the FDA advisory board that approved the vaccine, told reporters during a a Poynter Institute webinar on Monday. “And then we for everybody else, we make a recommendation we always make with vaccines, which is that whenever you get a vaccine, you should hang around for about 15 minutes to make sure you didn’t have a severe allergic reaction because one out of every million doses roughly of vaccines is complicated by severe allergic reaction,” Offit said When it comes to allergies, the only people who shouldn’t get the vaccine are those who have a history of severe reactions to any component of the Pfizer/BioNTech vaccine, according to the CDC. An advanced honors student, Alexandra has been a straight A student since kindergarten. Alexandra is a member of the National Honor Society, a Duke TIP Scholar, received honorable mention two years in a row at Canterbury School’s Science Fair, Regional First Place at HOSA for Nutrition and a Science Olympian member. Alexandra aspires to be a surgeon specializing in female cancer research and helping children with Treacher Collins Syndrome, a condition that affects the development of bones and other tissues of the face. Alexandra was diagnosed by Dr. Robert Wood, of the John Hopkins, at the age of 4 with a most severe peanut allergy after she went anaphylaxis after eating a peanut product. Alexandra feels the best way to prevent a severe reaction is education on food labeling, and fellow student education on this deadly condition. Dr. Richard Arrigo is a board-certified gastroenterologist who graduated from New York College of Osteopathic Medicine of New York Institute of Technology and then proceeded to complete his Residency at the University of Medicine and Dentistry of New Jersey- New Jersey Medical School and then his fellowship at Rutgers New Jersey Medical School. Prior to his fellowship, Dr. Arrigo was in practice with the Liver Transplant program in Newark, NJ where he managed patients with severe liver disease. Dr. Arrigo is trained to perform Endoscopic Ultrasound (EUS), a procedure that uses sound waves to create visual images of the digestive tract, and Endoscopic Retrograde Cholangiopancreatography (ERCP), a procedure that is used to diagnose diseases of the gallbladder, biliary system, pancreas, and liver. She graduated from RWJ Medical School at Rutgers University and then proceeded to complete her residency at Thomas Jefferson University Hospital. She then went on to complete her fellowship in Gastroenterology and Hepatology at Cooper University Hospital. “During my training, I observed many of my patients struggling with not only their chronic medical problems such as GERD, diabetes, HTN, but also obesity. I started to realize that the root of many of these chronic medical conditions was in fact his/her underlying obesity. Currently, I treat patients with a myriad of gastrointestinal disorders ranging from gastroesophageal reflux to gallstones to fatty liver. All of these diagnoses can be linked to obesity. Because of this desire to help my patients not only in the field of gastroenterology, but also their overall well-being, I decided to obtain my board certification in Obesity Medicine. I hope to incorporate teaching patients about obesity, nutrition and exercise in my practice as a gastroenterologist in hopes of helping them lose weight, and hopefully, help them improve their overall health.”

Heartburn triggers: What’s burning you? Heartburn or acid reflux symptoms include chronic cough, chest pain and burning. Knowing your triggers, such as certain foods, medications, obesity, or even stress, can help prevent heartburn. Heartburn treatment may include medications, home remedies, or diet changes. The specific triggers for heartburn differ from person to person. Marinara sauce may always spell trouble for you, but your spouse may lick the plate clean and sit back with a satisfied belly and a smile. Despite its name, heartburn has nothing to do with the heart. Some of the symptoms, however, are like those of a heart attack or heart disease. Heartburn is an irritation of the esophagus -- the tube that connects your throat and stomach. It’s caused by stomach acid. This leads to a burning discomfort in your upper belly or below your breastbone. What Causes Heartburn? Heartburn symptoms can start because of a problem with a muscular valve called the lower esophageal sphincter (LES). It’s located where the esophagus meets the stomach -below the rib cage and slightly left of center. Normally, with the help of gravity, the LES keeps stomach acid right where it should be -in your stomach. When it’s working right, the LES opens to allow food into your stomach or to let you belch, then closes again. But if the LES opens too often or doesn’t close tightly enough, stomach acid can seep into the esophagus and cause a burning sensation. If your LES doesn’t tighten as it should, there are often two things that contribute to the problem. One is overeating, which puts too much food in your stomach. Another is too much pressure on your stomach, often due to obesity, pregnancy, or constipation. What can lead to heartburn may surprise you. Stay away from foods you know will give you heartburn. It’s not just about the food you eat. How and when you exercise and what you take to relieve your aches and pains may also cause that burning feeling. The key to taming the flame is to understand what triggers your own personal symptoms. Heartburn triggers: Heartburn and diet Fatty foods are big no-no’s if you suffer from heartburn. High-fat foods sit around in your belly longer. This makes your stomach produce more acid, irritating your digestive system. And fatty and greasy foods lead to a

lazy, relaxed LES. So not only do you have more irritating stomach acids, you’re more likely to have the contents splash back up your throat resulting in heartburn pain. Several foods and drinks can cause the LES to relax. Food and drinks that commonly trigger heartburn include: • alcohol, particularly red wine • black pepper, garlic, raw onions, and other spicy foods • chocolate • citrus fruits and products, such as lemons, oranges and orange juice • coffee and caffeinated drinks, including tea and soda • peppermint • tomatoes However, unless these foods are causing you heartburn you don’t have to avoid them. To prevent heartburn after meals: • Don’t overeat. Eat five or six small meals each day, instead of several large meals. • Don’t eat before bedtime. Allow 2 hours to digest your food before lying down. This allows time for the food to pass out of the stomach and into the small intestine, rather than having it back up into the esophagus. Lying down makes digestion difficult and makes heartburn more likely. Exercising and Heartburn Crunches and ab work can trigger heartburn. Body positions that involve bending over increase pressure on the abdomen, thrusting stomach acids back up into the esophagus. So, you feel the burn -- but not the type you’d expect from going to the gym. Keep in mind that leg lifts also work the abdominal muscles and may aggravate heartburn symptoms too. Activities such as headstands and yoga moves like downward dog can reverse the natural flow of stomach contents and may trigger heartburn. Jarring exercises, such as jogging or aerobics, can slosh stomach contents around and upward if you have a weak LES. Heartburn is no reason not to exercise. In fact, weight loss from exercise can help heartburn. But never exercise on a full stomach. Doing so increases abdominal pressure, which makes heartburn more likely. Food takes several hours to digest so it really is a matter of what works for you. Most experts recommend waiting about two hours after eating before working out. Heartburn triggers: Heartburn and medications Many different medications can trigger heartburn or make heartburn worse. An aspirin

here or there is not likely to lead to that fiery feeling. But regular use of aspirin or a popular class of painkillers called nonsteroidal antiinflammatory drugs (NSAIDs) may irritate the esophagus. NSAIDs include ibuprofen, naproxen, and prescription Cox-2 inhibitors such as Celebrex. These drugs are often used to treat arthritis. Several different types of blood pressure medicines can also cause heartburn. Many blood pressure and heart disease medicines, including calcium channel blockers and nitrates, relax the LES muscle, making it easy for your stomach acids to retreat backwards. Tracking heartburn triggers Heartburn can limit your menu choices, interrupt your sleep, and interfere with your daily activities. Keeping tabs on what you eat and when you eat will help your doctor determine what’s causing your symptoms.

Then together, you can figure out how to prevent them. You can track heartburn triggers by keeping a heartburn diary. Make sure you include notes about portion size and if you have pain after eating a certain food. Understanding your heartburn triggers and learning how to avoid them can help you avoid the discomfort of heartburn. An episode of heartburn every now and then is usually nothing to worry about. Dr. Richard Arrigo is a board-certified gastroenterologist who graduated from New York College of Osteopathic Medicine of New York Institute of Technology and then proceeded to complete his Residency at the University of Medicine and Dentistry of New Jersey- New Jersey Medical School and then his fellowship at Rutgers New Jersey Medical School. Prior to his fellowship, Dr. Arrigo was in practice with the Liver Transplant program in Newark, NJ where he managed patients with severe liver disease. Dr. Arrigo is trained to perform Endoscopic Ultrasound (EUS), a procedure that uses sound waves to create visual images of the digestive tract, and Endoscopic Retrograde Cholangiopancreatography (ERCP), a procedure that is used to diagnose diseases of the gallbladder, biliary system, pancreas, and liver. Dr. Arrigo is also trained and certified to perform the Orbera Weight Loss Balloon procedure.

Alcohol-Related Liver Disease Care… Helping you prevent or cope with liver damage Your liver plays a vital role in your overall health. It is the organ that stores the vitamins, sugar, and iron you need for energy. It filters your blood from harmful toxins and waste. It also controls cholesterol, removes harmful bacteria, and metabolizes some of the medications you take. Dr. Kenneth DiGregorio is a board-certified gastroenterologist at Hunterdon Gastroenterology Associates. He received his medical degree from the Medical College of Virginia and then proceeded to complete his residency at Temple University Hospital and his fellowship at the University of Pittsburgh Medical Center. Dr. DiGregorio has received advanced training in both biliary endoscopy and the specialty of liver disease. Dr. Ramon Alberto Rosa is a graduate of the University of Santo Domingo in his native Dominican Republic. He completed his post-graduate medical education at the University of Pennsylvania Graduate Hospital in Philadelphia with a residency in internal medicine (1991-1994), Chief Medical Resident (19941995), and cardiology fellowship (1995-1998). Dr. Rosa has practiced cardiology in Sussex County since July 1998. He has served as Medical Director of the Cardiac Catheterization Laboratory and as Chief of the Department of Cardiology at Beebe Medical Center in Lewes, DE. Dr. Rosa is trained in non-invasive cardiology, Nuclear Transesopageal Echocardiograms, as well as invasive diagnostic procedures and pacemaker implantations. Coronavirus-induced heart injury, lessons learned….. so far It has become clear that involvement of the cardiovascular system is common in COVID-19 infected patients. Around 25-33 % of hospitalized patients will show evidence of myocardial injury as defined by abnormal cardiac enzymes, detected on regular blood tests. The affected patients are generally older and have a higher incidence of hypertension, diabetes mellitus, coronary artery disease and heart failure. In addition, myocardial injury is associated with a more severe COVID-19 infection that often times requires the use of ventilators and is associated with higher in-hospital mortality. This disease affects multiple organ systems, in addition to the heart and vessels, lungs, kidneys, liver, intestines and brain. Several mechanisms of myocardial injury have been described with this coronavirus: Direct viral invasion of the heart cells (Myocytes), blood clots in both large and small vessels, inflammation mediated injury, stress-induced cardiomyopathy and actual heart attacks. Most of these

cases have been observed in prolonged coronavirus cases, longer than 14 days as a general rule. Coronavirus enters the human cells by attaching to the ACE-2 receptor on the cell surface through the virus spike protein, in the finger-like projections that make this virus so characteristically familiar to all of us. These receptors are abundant in lung, vessels and heart cells. The virus takes over the cells mechanisms to replicate itself and infect adjacent cells, rapidly extending to multiple organs. It is the body’s inflammatory response to counter the virus that causes the clinical syndrome that we see with fever, cough, general malaise, progressing to pneumonia, kidney failure, heart disease and other organ systems. The inflammatory response can lead to direct damage to the inside of small and large vessels in the major organs causing blood clots to form and create hypoxia (lack of adequate oxygen levels) at the different tissue levels. This is the main reason why the pneumonia caused by

the COVID-19 infection has turned out to be so different from typical ones. Current treatment for COVID-19 infection includes antivirals such as remdesivir, antiinflammatories such as steroids, anticoagulants such as heparin and convalescent plasma. The reality is that the vast majority of COVID-19 patients will have either none to mild symptoms. Those with preexisting conditions

(lung disease, heart disease, high blood pressure, obesity or diabetes) as well as older than 65 will be more predisposed to developing a more serious form of the condition can progress to severe lung failure or heart injury with its potential for a prolong disease or will be in the less than 1 % of patients than will not survive the infection. Brush Up on Pet Dental Care A Healthy Mouth Starts with Clean Teeth BY ELIZABETH POPE, DVM

Healthy teeth and gums play an important role in the overall health and well-being of our companion animals. Periodontal disease is one of the most commonly diagnosed conditions in adult cats and dogs; in fact, by age three, 80 percent of dogs and 70 percent of cats have some evidence of dental disease. Without good dental hygiene, your pet may have bad breath, drool, and experience pain while chewing. Left untreated, dental disease can result in pain, gum infection and tooth loss and eventually, bacteria can enter the bloodstream and cause liver, kidney and heart valve disease. Pets are good at hiding dental pain, so it’s important to try to prevent periodontal disease before it begins. Fortunately, your veterinarian can examine your pet’s oral health at their annual check up! If periodontal disease is present, your veterinarian will recommend a comprehensive oral health exam and treatment. A full oral examination cannot safely be performed on our pets while they are awake, so your pet should be under anesthesia so that we can safely and comprehensively examine, clean and polish their teeth. Pre-anesthesia blood work helps us identify possible problems that may not be evident during your pet’s physical examination and if the test results are abnormal, we will take special precautions to address these concerns prior to your pet’s cleaning.

Pet dental exams and cleanings are just like those for humans: Teeth are charted and probed for pockets and gum disease and then x-rays are taken to reveal things like abscesses, bone loss and impacted teeth. Calculus is removed from the tooth’s surface and under gum lines and then teeth are scaled with an ultrasonic scaler and polished. If unhealthy teeth are found on examination or on x-rays it may be recommended that those teeth are extracted or follow up care may be needed. The result? A healthy, clean and pain-free mouth! To help keep your pet’s mouth healthy between cleanings: •

This is best introduced in our young patients and make it a positive experience by giving him his favorite treat or playing his favorite game afterwards. Switch to a VOHC (Veterinary Oral Health Council) approved dental treat or a dental diet. Add in a water additive to help fight plaque and promote healthy gums.

Meet the Team of D R. E LIZABE TH POPE Dr. Elizabeth Pope is a small animal veterinarian and owner of South Branch Veterinary Services. Elizabeth Pope is a New Jersey native and went on to receive a Bachelor of Science degree from Washington & Lee University in Virginia and her medical degree from Oklahoma State University, Center for Veterinary Health Sciences. Dr. Pope has advanced training in abdominal ultrasound and dentistry as well as special interests in internal medicine and oncology. She resides in Hunterdon County on a small cattle farm with her husband Nick, a fellow small animal veterinarian, and their son Benjamin. At South Branch Veterinary Services she places a large emphasis on communication, preventative care and routine diagnostic testing; routine diagnostic testing is the best way to see potentially dangerous changes that may need to be addressed. “The entire hospital staff is constantly working to learn the best way to treat our patients. I am proud to be part of a group of individuals who strive to provide the highest level of veterinary care. As your pet’s veterinarian, I aim to give your pet the best possible care in an environment that they can enjoy. D R. R AC HAEL G OR DON Dr. Rachael Gordon is a small animal veterinarian who was raised in New Jersey and is a graduate of Rutgers University. She attended the College of Veterinary Medicine in Dublin, Ireland where she received her Bachelor of Veterinary Medicine. While in Ireland, Dr. Gordon managed her own veterinary hospital and served on the board of the Veterinary Ireland Companion Animal Society. She has been practicing veterinary medicine for over 15 years and is one of three veterinarians at South Branch Veterinary Services. In addition to treating dogs and cats, she is one of only a few veterinarians in the area with a special interest and extensive experience treating exotics pets and birds. Dr. Gordon has special training in abdominal ultrasound as well as special interest in orthopedic conditions and canine reproduction. Along with her colleagues, she works closely with local rescue organizations to provide care for pets in shelters and in foster homes. Dr. Gordon lives in Hunterdon County on a farm where she spends her free time working with her Portuguese Water Dogs which she shows and performs agility trials, and rescues donkeys.

D R. C ATHER INE R OTSIDES Dr. Catherine Rotsides was born and raised in New Jersey. She currently calls Union County home where she lives with family and two cats. Dr. Rotsides graduated from Stevens Institute of Technology with a degree in chemical biology and then attended veterinary school at St. George’s University in Grenada. After receiving her veterinary degree, Dr. Rotsides performed advanced training through a small animal medicine and surgery internship at Garden State Veterinary Specialists in Tinton Falls, New Jersey. After her internship, she worked as an emergency and critical care veterinary for several years. Dr. Rotsides continues to have a special interest in emergency medicine as well as interests in cardiac conditions specific to the dog and cat and soft tissue surgery. Dr. Rotsides has advanced training in small animal dentistry and enjoys providing the highest level of dental care for her patients. She values the bond between a pet and their owner and her extensive experience in emergency medicine has been a huge asset in her treatment of medical and surgical cases at South Branch Veterinary Services.

Everyone needs a pal...especially now! By Leah Jacobson, President Many companion animals find themselves homeless through no fault of their own. People surrender the family pet to shelters or rescue groups for a variety of reasons: moving and can’t find pet friendly housing, divorce, a new baby, allergies, changing work schedules or unable to afford vet care. Others simply tire of caring for their pet, and open the car door in an unfamiliar location near a farm or on the side of the road, dumping them like a bag of trash.

Rescuing a dog or cat saves them from the heartbreaking loneliness of abandonment. When shelters run out of room, euthanasia is sadly a real possibility with space needed for incoming animals. Rescue groups offer a lifeline by getting these animals out of harm’s way and freeing up shelter space. Rescue groups initially take the animal in, then volunteers feed and care for it, and take it to the vet to get it spayed / neutered and a thorough health checkup. After adoption readiness is certified, a foster family provides love and care while these loving animals wait for their forever home.

Cats and dogs that come to us have often been members of a loving family. Sometimes they are found loose as strays. Even if we don’t know what their lives were like, we can get clues from their behavior: responding to certain commands, being housetrained, being comfortable walking on a leash, in a home, on a couch or a bed. Sometimes they’re scared or shy, and just need patience and TLC to blossom. Gateway Regional Rescue based in Stewartsville, NJ, accepts animals from people who can no longer keep their pets, good Samaritans who find a stray, and over-crowded shelters hoping to extend their rescue outreach. Since 2006 we have facilitated a weekly transport from high-volume shelters in Virginia that brings at risk animals to rescues and nokill shelters in NJ / NY that finds them homes. We have transported over 15,000 animals to safety!

The Live-In Caregiver Deborah Walsh,

Executive Director, HomeWorks Most of us understand the benefits of having a live-in caregiver and remaining in the comfort of one’s home. Consider for a moment the perspective of the caregiver who must move out of their own home, leave family behind, and move into a client’s home. We certainly understand the point of view of the client but want to take a moment to view it from the standpoint of the caregiver and to highlight the work that they do. Both the caregiver and client come from different backgrounds and personal histories. A caregiver must attend to a client’s most personal needs. A client must allow the caregiver to be privy to her personal life. Both are difficult. Yet, each needs the other and they compromise to make the situation mutually beneficial. Cultural differences, food choices, routines and more, come into play. A caregiver must navigate the client’s family relationships and discover how to fit into the family dynamic. She often advocates and speaks to family with and for the client as she gains more knowledge of the client’s needs and routines. We take for granted our dining routines; eating and mealtime is significant for all of us. In the caregiving situation, food is a key part of the equation for both the caregiver and the client. Often the caregiver is from a different culture. Buying familiar food and

meal preparation is something that takes considerable work so that both are content with the living situation. This process, the give and take, is significant in building the relationship. Companionship is another foundational element to building the relationship. Our caregivers spend time playing board games, cards, puzzles and more to interact with their clients. Going to dinner or lunch together or to church services and the beauty salon are also often on the day’s agenda. This special time together fosters the bond. The nurse who oversees the situation is always in touch and is essential in supporting this relationship. He or she will guide the caregiver and the client in getting past small hurdles and issues. We are often pleased to see the interesting combinations of caregiver and client who come together with HomeWorks. It is not unusual to have a caregiver from Africa preparing an Italian dish for a client or a client from the Mid-West enjoying a Jamaican dish. It can be an enriching life experience. At United Methodist Communities HomeWorks, we understand that both the client and the caregiver are entering a very new situation. We work hard to make sure personalities are compatible and the relationship can develop in a positive way. The tenderness, patience, and love expressed by our live-in caregivers are very special and often mimic a familial bond. Caregivers are sometimes alone with a client at end of life; they comfort the client, sometimes holding them in their arms at their last breath. They grieve with the family, and yet they continue to do their difficult work and start the process again with someone new. This is the life of a live-in caregiver. If either you or your future spouse has or may inherit substantial assets, or if either of you has children from previous marriages, you may want to consider a prenuptial agreement. A prenuptial agreement is a binding contract between future spouses that defines the rights, duties, and obligations of the parties during marriage and in the event of legal separation, annulment, divorce, or death. A prenuptial agreement typically addresses the following areas: • Assets and liabilities--What assets will each of you bring into the marriage? What liabilities do each of you have (e.g., credit card/mortgage debt)? • Contributions of each partner--Will there be particular consideration given for special contributions that either of you make (e.g., one spouse limiting his or her career)? • Divorce--If you and your future spouse divorce, will there be alimony or a lump-sum payment? How will you divide assets purchased from joint funds? • Estate planning--Who gets what at the death of either spouse?

Discuss your financial history

Marriage is the union of two separate individuals ... and their finances. While talking about money can be a stressful topic for many couples, you’ll want to sit down and discuss your financial history and your future spouse’s financial history before you merge your money. Start out by taking stock of each of your respective financial situations. You should each make a list of your individual assets (e.g., investments, real estate) and any liabilities (e.g., student loans, credit card debt) you may have. This is also the time to address items such as how much each of you earns and if either of you has additional sources of income (e.g., interest, dividends).

Agree on a system for budgeting/maintaining bank accounts

Right now, you are probably accustomed to managing your finances in a way that is comfortable for you and you alone. Once you are married, you and your spouse will have to agree on a system for budgeting your money and paying your bills together as a couple. Either of you can agree to be in charge of managing the budget, or you can take turns keeping records and paying the bills. If both of you are going to be involved in the budgeting process, make sure that you develop a record-keeping system that both of you understand and agree upon. In addition, you’ll want to keep your records in a joint filing system so that both of you can easily locate important documents. Once you agree on a budgeting system, you’ll be able to establish a budget. Begin by listing all of your income and expenses over a certain time period (for example, monthly). Sources of income can include things such as salaries and wages, interest, and dividends. Expenses can be divided into two categories: fixed (e.g., housing, utilities, food) and discretionary (e.g., entertainment, vacations). Be sure to include occasional expenses (e.g., car maintenance) as well. To help you and your future spouse stay on track with your budget: • Try to make budgeting part of your daily routine • Build occasional rewards into your budget (e.g., going to the movies) • Examine your budget regularly and adjust/make changes as needed This might also be a good time to decide whether you and your future spouse will combine your bank accounts or keep them separate. While maintaining a joint account does have its advantages (e.g., easier recordkeeping and lower maintenance fees), it is sometimes more difficult to keep track of the flow of money when two individuals have access to a single account.

If you do decide to combine your accounts, each spouse should be responsible for updating the checkbook ledger when he/she writes a check or withdraws funds. If you decide to keep separate accounts, consider opening a joint checking account to pay for household expenses.

Map out your financial future together An important part of financial planning as a couple is to map out your financial future together. Where do you see yourself next year? What about five years from now? Do you want to buy a home together? If you decide to start a family, would one of you stay at home while the other focuses more on his or her career? Together you should make a list of short-term financial goals (e.g., paying off wedding debt, saving for graduate school) and long-term financial goals (e.g., retirement). Once you have decided on your financial goals, you can prioritize them by determining which ones are most important to each of you. After you’ve identified which goals are a priority, you can set your sights on working to achieve them together. Resolve any outstanding credit/debt issues Since having good credit is an important part of any sound financial plan, you’ll want to identify any potential credit/debt problems either you or your future spouse may have and try to resolve them now rather than later. You should each order copies of your credit reports and review them together. You are entitled to a free copy of your credit report from each of the three major credit reporting agencies once every 12 months (go to annualcreditreport.com for more information). For the most part, you are not responsible for your future spouse’s past credit problems, but they can prevent you from getting credit together as a couple after you are married. Even if you’ve always had spotless credit, you may be turned down for credit cards or loans that you apply for together if your future spouse has a bad track record with creditors. As a result, if you find that either one of you does have credit issues, you might want to consider keeping your credit separate until your or your future spouse’s credit record improves.

Consider integrating employee and retirement benefits

If you and your future spouse have separate health insurance coverage, you’ll want to do a cost/benefit analysis of each plan to see if you should continue to keep your health coverage separate. If your future spouse’s health plan has a higher deductible and/or co-payment or fewer benefits than those offered by your plan, he or she may want to join your health plan instead. You’ll also want to compare the premium for one family plan against the cost of two single plans. In addition, if both you and your future spouse participate in an employer-sponsored retirement plan, you should be aware of each plan’s characteristics. Plans may differ as to matching contributions, investment options, and loan provisions. Review each plan together carefully and determine which plan provides the better benefits. If you can afford to, you should each participate to the maximum in your own plan.

Assess your insurance coverage needs

While you might not have felt the need for life and disability insurance when you were single, once you are married you may find that you and your future spouse are financially dependent on each other. If you don’t have life or disability insurance, you will want to have policies in place in order to make sure that your future spouse’s financial needs will be taken care of if you should die prematurely or become disabled. If you already have life and disability insurance, you should reevaluate the adequacy of your existing coverage and be sure to update any beneficiary designations as well. You should also take a look at your auto insurance coverage. Check your policy limits and consider pooling your auto insurance policies with one company (your insurance company may give you a discount if you insure more than one car with them). As for renters/homeowners insurance, you’ll want to make sure your personal property and possessions are adequately covered. NOTE: If you and your future spouse obtain joint credit, both of you will become responsible for 100% of the debt.

Social Distancing -Alone Together We are living in times that have never been experienced before. In the past century, there have been just four pandemics. The last one was in 1918. Life, as we know it has been put on hold. Events cancelled, school is now in a virtual landscape, and undoubtedly someone you know has been affected by this disaster. Thanks to modern technology, we are still able to connect with our friends and loved ones either through a Google hangout, Zoom, or House party. In dark times, it is more important than ever to be a beacon of hope for your friends and family. Families all over the nation are spending more time together than they have in years. With the absence of full calendars, practices and other after school activities, there is more time to connect. When this time passes (and it will) most of us will come out better for it; more spiritually connected and definitely closer to our loved ones. Although there is currently no vaccine to prevent COVID-19, the best way to prevent the spread is to avoid being exposed. Those who are in close contact with one another, usually within six feet, spread it from person to person. Respiratory droplets are produced when an infected person coughs or sneezes. The droplets can land in the mouths or noses of people who are nearby or possibly inhaled in the lungs. Another silver lining The County Woman Magazine

is that once you have contracted the virus it is unlikely you can get it a second time. Health officials have said that coronavirus antibodies are able to prevent a person who had the infection from getting it a second time. The best course of action at the present time is to minimize your chances of contacting the virus. We can do that in the following ways: • Washing your hands with soap and water for at least 20 seconds, especially after being in a public place. • You can use a hand sanitizer (if soap and water are not readily available) that contains at least 60% alcohol. • Avoid touching your face with unwashed hands • Sneeze into your elbow • Clean and disinfect frequently touched surfaces daily – this includes doorknobs, light switches, keyboards, and phones.

It is easy to feel helpless during times like this. One way to alleviate those feelings are to try and help where you can and right now our nation is facing a major shortage in blood donations. Donating blood is essential to the health of a community and the need right now is dire. The Red Cross has expressed concern over future blood shortages. If you can, give blood so they can maintain a sufficient blood supply for the weeks to come. Never have we been given this respite from our daily schedules. The stress and anxiety from the unknowns can be daunting. Try and take some time for self-care and making yourself a priority during the days ahead.
