Many of us are approaching this usually joyous time of year with a sense of unease. A new wave of COVID restrictions is impacting the outlook for together time with loved ones, and there seems to be more uncertainty rather than less so far into the pandemic. The darkest hour is always just before dawn, and there is no more important time to remain optimistic. A viable vaccine is expected in circulation by late this year, and it is likely that in person activities will return to normal as 2021 progresses. Adversity is rarely a cause for celebration, but it is almost always a source of positive change. For us, the Fall touchless version of the HC Woman was a big success. Most COVID sensitive distribution points like doctor’s offices, spas and salons are now displaying these as the only material in their location. We expect further developments in our distribution format over the winter months that will further enhance ease of access and readership. The Publisher

R. Alberto Rosa, M.D., FACC Dr. Ramon Alberto Rosa is a graduate of the University of Santo Domingo in his native Dominican Republic. He completed his post-graduate medical education at the University of Pennsylvania Graduate Hospital in Philadelphia with a residency in internal medicine (1991-1994), Chief Medical Resident (19941995), and cardiology fellowship (1995-1998). Dr. Rosa has practiced cardiology in Sussex County since July 1998. He has served as Medical Director of the Cardiac Catheterization Laboratory and as Chief of the Department of Cardiology at Beebe Medical Center in Lewes, DE. Dr. Rosa is trained in non-invasive cardiology, Nuclear Transesopageal Echocardiograms, as well as invasive diagnostic procedures and pacemaker implantations. Dr. Rosa is board certified by the American Boards of Internal Medicine and Cardiology. He is also a Certified Aviation Medical Examiner and is Board Certified in Nuclear Medicine.

Coronavirus-induced heart injury, lessons learned….. so far It has become clear that involvement of the cardiovascular system is common in COVID-19 infected patients. Around 25-33 % of hospitalized patients will show evidence of myocardial injury as defined by abnormal cardiac enzymes, detected on regular blood tests. The affected patients are generally older and have a higher incidence of hypertension, diabetes mellitus, coronary artery disease and heart failure. In addition, myocardial injury is associated with a more severe COVID-19 infection that often times requires the use of ventilators and is associated with higher in-hospital mortality. This disease affects multiple organ systems, in addition to the heart and vessels, lungs, kidneys, liver, intestines and brain. Several mechanisms of myocardial injury have been described with this coronavirus: Direct viral invasion of the heart cells (Myocytes), blood clots in both large and small vessels, inflammation mediated injury, stress-induced cardiomyopathy and actual heart attacks. Most of these

cases have been observed in prolonged coronavirus cases, longer than 14 days as a general rule. Coronavirus enters the human cells by attaching to the ACE-2 receptor on the cell surface through the virus spike protein, in the finger-like projections that make this virus so characteristically familiar to all of us. These receptors are abundant in lung, vessels and heart cells. The virus takes over the cells mechanisms to replicate itself and infect adjacent cells, rapidly extending to multiple organs. It is the body’s inflammatory response to counter the virus that causes the clinical syndrome that we see with fever, cough, general malaise, progressing to pneumonia, kidney failure, heart disease and other organ systems. The inflammatory response can lead to direct damage to the inside of small and large vessels in the major organs causing blood clots to form and create hypoxia (lack of adequate oxygen levels) at the different tissue levels. This is the main reason why the pneumonia caused by

the COVID-19 infection has turned out to be so different from typical ones. Current treatment for COVID-19 infection includes antivirals such as remdesivir, antiinflammatories such as steroids, anticoagulants such as heparin and convalescent plasma. The reality is that the vast majority of COVID-19 patients will have either none to mild symptoms. Those with preexisting conditions

(lung disease, heart disease, high blood pressure, obesity or diabetes) as well as older than 65 will be more predisposed to developing a more serious form of the condition can progress to severe lung failure or heart injury with its potential for a prolong disease or will be in the less than 1 % of patients than will not survive the infection. Prevention and early detection and attention remain the key factors. Wear your masks when out

in public places, especially indoors and keep your distance and your hands clean. Protective eye wear is also important. In the latter months of this pandemic, effective treatment protocols have been developed, and an effective vaccine is coming in 2021. Seek attention early if you are exposed or suspect you may have COVID-19. Do not fear medical care, this virus is here to stay a while longer, and we are learning to live with it!

Health & Wellness Maria E. Cruz, D.D.S. Dr. Cruz grew up in sunny San Juan, Puerto Rico. She attended The University of Maryland in College Park where she earned a B.S. degree in Zoology. She went on to graduate from Georgetown University School of Dentistry in Washington D.C. Dr. Cruz is very excited exploring and enjoying her new life in Salisbury, MD. Her outside interests include exercise, classical music and traveling with her daughter, a law student in NYC.

Dr. Richard J. McCann, D.M.D., P.A. Dr. McCann grew up in Northeastern Pennsylvania and attended King’s College where he earned a B.S. degree in Biology. He went on to graduate with honors from Temple University School of Dentistry, Philadelphia, Pennsylvania. Dr. McCann is Northeast Regional Board Certified. Obstructive Sleep Apnea in Women Presented by Maria E. Cruz, D.D.S.

O

Obstructive Sleep Apnea (OSA) is a condition in which relaxation of the throat muscles cause blockage of the airways while sleeping. This causes people to have interrupted sleep because of difficulty in breathing. OSA most commonly affects people between 30 and 60 years old and in women, especially after menopause. Risk factors include obesity, large tonsils, large tongue, small jaw, deviated nasal septum, family history, smoking and alcohol. If left untreated OSA can result in health problems like high blood pressure, stroke, diabetes, depression and heart attack among others. OSA has traditionally been seen as a male disease. However, the importance of diagnosing OSA in women is increasingly being recognized. It has been suggested that women report difference symptoms than men. Typically symptoms for men are sleepiness and snoring. Woman with OSA complain of general tiredness, fatigue, insomnia, restless legs depression, nightmares, anxiety and heart palpitations. Dentists can and do play an essential role in the screening and care of patients with OSA and are well positioned to identify the ones at greater risk. Working in conjunction with our colleagues

in medicine, we have methods of treating this disorder after proper diagnosis. Diagnosis is often based on symptoms, physical examination and is confirmed through a sleep study.

Empowering Seniors To Prevent Healthcare Fraud The Delaware Senior Medicare Patrol (SMP) mission is to empower and assist Medicare beneficiaries, their families, and caregivers To Prevent, Detect, and Report health care fraud, errors, and abuse through outreach, counseling and education. Medicare loses over $60 billion each year while affecting countless beneficiaries nationwide. The Delaware SMP teaches Medicare beneficiaries, caretakers as well as their family members how to protect themselves and others from Medicare fraud. Treating your Medicare card and number like it’s your credit card is the first step in keeping scammers at bay. Remember to be cautious of offers of “free” medical services and/or equipment and understand Medicare will never contact you to sell you anything. Reviewing your Medicare Summary Notice (MSN) is the first step in detecting Medicare fraud and abuse. When checking your MSN, be certain the services received are what’s indicated on the MSN. When detecting fraud and abuse, thoroughly look over your MSN to ensure it does not reflect services never received, double billing or services which your doctor has not ordered. Create a health care journal and/or medical diary detailing scheduled medical appointments, prescriptions, medical procedures along with your medical provider’s contact information. Maintaining accurate information within a health care journal will assist with the detection of fraud, errors and abuse.

Little Teeth Big Problems!

After all, dentistry is very much in your face type of deal, literally! Dentists are usually well inside the boundaries of personal space. We also like to have our patients reclined back in a vulnerable position with a bright light pointing right at their face and that is before any of the less enjoyable stuff begins! The wonderful tastes, the yummy smells, the delightful numbness and the occasional gagging! My first experience at the dentist was when I was around 8 years old. I can vividly remember when the assistant called my name and asked me to follow her to the dental chair. I did so without hesitation, my head held high. But it was not long before my bravery started to wane, by the time I made it to the chair

my head was still held high but merely to be on the lookout and prepared to fight or flight! Within a few minutes of being sat down the dental assistant starts to head toward my chair with a Stainlesssteel tray on hand and metal instruments making an ominous sound as they roll around on the tray. I see my opportunity to escape vanishing with every step she takes and before she could even put the tray down, I bolted out of the chair and I was out of there! The visit was not successful, and the experience gave me valuable insight into the fears and anxieties children can experience. Pediatric dentistry is a dynamic field with a wide range of treatment options, philosophies and objectives. Easterseals Supported Employees Are Essential By Natalie Scott Easterseals Supported Employment program has been an essential part of our community, Easterseals participant’s lives, and the companies who benefit from the hard working employees in the program. Easterseals Supported Employment gives adults with intellectual disabilities, including autism, the choice to experience the community through individualized employment. The program matches participants with jobs in local businesses so they can earn competitive wages with the assistance of job coaching and training. Billy has been a stellar employee for 24 years and continued to work hard at Bayhealth Hospital through the pandemic. “I am essential! During the COVID-19 Pandemic, I continued to work fulltime,” Billy says. “I work hard to make sure our hospitals are clean and properly disinfected because I care about our patients and my coworkers. I want to keep them safe.” Unfortunately, the unemployment rate for individuals with disabilities is more than double that of workers without disabilities. Many people with disabilities want to work, but have not been able to secure jobs because of accessibility and attitudinal barriers. As a result, labor force participation for individuals with disabilities is less than 1/3 of general labor force participation. Acupuncture & Immune System Dr. Tracy Hudson Dr. Hudson is a ’95 Cape Henlopen High School graduate and continued her educational career at the University of North Carolina at Wilmington where she completed her Bachelor’s degree in Physical Education. While attending UNCW, Dr. Hudson suffered an injury playing softball and she sought treatment with a local chiropractor. After a few adjustments, Dr. Hudson was back on the field. Covid-19 primarily affects the lungs but also can have heart and muscle/joint complications. People that have been affected directly by the coronavirus or who have limited physical activity due to self-isolation may have similar issues – increased fatigue, weakness, difficulty breathing, decreased endurance, and reduced balance. People who have this weak core, often have a difficult time correcting this weakness, as it is difficult to do typical core strengthening exercises, such as sit ups, pushups, and planks. But with the Trusculpt flex, patients can lie in comfort and have the abdominal and back muscles exercised for them. For example, in a 45 minute treatment session, the TruSculpt flex will perform the equivalent of 54,000 crunches! Other areas typically treated by TruSculpt flex are the thighs, calves, shoulders, biceps and triceps. The TruSculpt flex can also treat the buttock area for a non-surgical buttock lift. Multiple areas can be treated at once. How does the TruSculpt flex work? The TruSculpt flex utilizes direct muscle stimulation, administered through small pads that are placed over the muscles to be treated. The direct stimulation causes the muscles to contract involuntarily, much faster and harder than can be accomplished through normal exercise. Multiple body areas are typically treated at the same time, for example, the abdomen and low back are treated at the same time, both arms, both legs, are treated at the same time. HEMORRHOIDAL TREATMENT Over half the population will experience some type of hemorrhoidal problem in their lifetime. You may be suffering in silence because you are too embarrassed to discuss the problem, or worry that the cure will be more painful than the problem itself. Now, advances in medicine have made hemorrhoid treatment fast and virtually painless. FDA Approves First Drug for Treatment of Peanut Allergy for Children Presented by Alexandra Gallagher Today the U. S. Food and Drug Administration approved Palforzia [Peanut (Arachis hypogaea) Allergen Powder-dnfp] to mitigate allergic reactions, including anaphylaxis, that may occur with accidental exposure to peanuts. Treatment with Palforzia may be initiated in individuals ages 4 through 17 years with a confirmed diagnosis of peanut allergy and may be continued in individuals 4 years of age and older. Those who take Palforzia must continue to avoid peanuts in their diets. “Peanut allergy affects approximately 1 million children in the U. S. and only 1 out of 5 of these children will outgrow their allergy. Because there is no cure, allergic individuals must strictly avoid exposure to prevent severe and potentially life-threatening reactions,” said Peter Marks, M. D., Ph.D., director of the FDA’s Center for Biologics Evaluation and Research. “Even with strict avoidance, inadvertent exposures can and do occur. When used in conjunction with peanut avoidance, Palforzia provides an FDA-approved treatment option to help reduce the risk of these allergic reactions in children with peanut allergy.” Peanut allergy is a condition in which the body’s immune system mistakenly identifies even small amounts of peanut as harmful. Allergic reactions to peanut are unpredictable in occurrence and in how they present, with some individuals experiencing severe reactions from even trace amounts. Physical symptoms can develop within seconds of exposure and may include skin reactions (e.g., hives, redness or swelling), digestive discomfort, or more dangerous reactions, such as constriction of the throat and airways, and loss of adequate blood flow to vital organs of the body. Antihistamines and epinephrine can be used to treat allergic reactions, but severe reactions can be fatal even with appropriate, prompt treatment. Palforzia cannot be used for the emergency treatment of allergic reactions, including anaphylaxis. Treatment with Palforzia consists of three phases: Initial Dose Escalation, UpDosing, and Maintenance. The Initial Dose Escalation phase is given on a single day. The Up-Dosing phase consists of 11 increasing dose levels and occurs over several months. Initial Dose Escalation, and the first dose of each Up-Dosing level, are administered under supervision of a healthcare professional in a healthcare setting with the ability to manage potentially severe allergic reactions, including anaphylaxis. While anaphylaxis can occur at any time during Palforzia therapy, patients are at highest risk during and after the Initial Dose Escalation and the first dose of each Up-Dosing level. During Up-Dosing, if the patient tolerates the first dose of an increased dose level, the patient may continue that dose level daily at home. After a patient completes all Up-Dosing levels, they may begin the daily maintenance dose. Patients who experience certain allergic reactions due to Palforzia may need to discontinue treatment or have their dosing schedule modified. Palforzia is a powder that is manufactured from peanuts and packaged in pullapart color-coded capsules for Dose Escalation and Up-Dosing, and in a sachet for maintenance treatment. The powder is emptied from the capsules or sachet and mixed with a small amount of semisolid food - such as applesauce, yogurt, or pudding - that the patient then consumes.

The effectiveness of Palforzia is supported by a randomized, double-blind, placebo-controlled study conducted in the U. S., Canada and Europe in approximately 500 peanut-allergic individuals. Effectiveness was assessed by evaluating the percentage of study participants tolerating an oral challenge with a single 600 mg dose of peanut protein (twice the daily maintenance dose of Palforzia) with no more than mild allergic symptoms after 6 months of maintenance treatment. The results showed that 67.2% of Palforzia recipients tolerated a 600 mg dose of peanut protein in the challenge, compared to 4.0% of placebo recipients. The safety of Palforzia was assessed in two double-blind, placebo-controlled studies in approximately 700 peanut-allergic individuals. The most commonly reported side effects of Palforzia were abdominal pain, vomiting, nausea, tingling in the mouth, itching (including in the mouth and ears), cough, runny nose, throat irritation and tightness, hives, wheezing and shortness of breath and anaphylaxis. Palforzia should not be administered to those with uncontrolled asthma. To mitigate the risk of anaphylaxis associated with Palforzia, the FDA is requiring a Risk Evaluation and Mitigation Strategy (REMS) with this approval, which includes elements to assure safe use. Palforzia will only be available through specially certified healthcare providers, health care settings, and pharmacies to patients who are enrolled in the REMS program. The FDA is requiring that healthcare providers who prescribe Palforzia - and healthcare settings that dispense and administer Palforzia - are educated on the risk of anaphylaxis associated with its use. In addition, the Initial Dose Escalation phase and first dose of each Up-Dosing level must only be administered to patients in a certified healthcare setting equipped to monitor patients and to identify and manage anaphylaxis. Patients or their parents or caregivers must also be counseled on the need for the patients to have injectable epinephrine available for immediate use at all times, the need for continued dietary peanut avoidance, and how to recognize the signs and symptoms of anaphylaxis. The FDA granted approval of Palforzia to A immune Therapeutics. The FDA, an agency within the U. S. Department of Health and Human Services, protects the public health by assuring the safety, effectiveness, and security of human and veterinary drugs, vaccines and other biological products for human use, and medical devices. The agency also is responsible for the safety and security of our nation’s food supply, cosmetics, dietary supplements, products that give off electronic radiation, and for regulating tobacco products.

Veterinary Services Services Veterinary Peninsula Veterinary Services is a full-service, AAHA-accredited hospital located in Millsboro, Delaware. Owner Dr. April Reid moved from Brooklyn, New York in 2015 to start a practice in this area after spending summers in Rehoboth as a child. Dr. Reid is a graduate of Ross University School of Veterinary Medicine. She previously owned a thoroughbred racetrack practice for 14 years before making the move to small animal practice. Dr. Reid works closely with several local rescue organizations and is currently serving as the vice president of the Delaware Veterinary Medical Association. In addition, she is working towards board certification in veterinary practice. In her spare time, Dr. Reid enjoys spending time with her two Great Danes, Ambrose and Amelia, and her three cats.

ALLERGIES IN PETS Pets presenting for allergies and the resulting skin issues is a common problem in this geographical area. Allergies in both human and pets are sensitivities to things in the environment or food that may not be harmful, but for some reason the individual’s body may see it as such and mount an immune response. Histamines are released and systemic signs such as inflammation, swelling and itchiness begin. Allergies are very common in pets and can develop over time. Allergies tend to present as respiratory, dermatological issues or both. They can be seasonal or non-seasonal and acute or chronic. Humans are most familiar with respiratory allergies during certain seasons of the year and our pets can also be affected in the same ways. Typically pollen, trees, weeds, grass, mold and fungi cause respiratory allergies. The pet may suddenly develop red, watery, itchy eyes and “gun-fire” sneezing. This is uncomfortable for the pet and worrisome for the owner. Acute cases are usually treated with allergy medication or the offending cause is removed and the clinical signs disappear. If the clinical signs do continue, the pet may develop secondary issues such as respiratory infections, which may need to be treated with antibiotics. Though we tend to think of respiratory allergies as developing to sources outside the home, they may also develop to indoor substances such as laundry detergents, home fragrances,

candles, carpets etc. Also remember that homes are not airtight and outdoor air can enter and cause symptoms even in a pet kept completely indoors. Allergies that present as skin issues are a bit more complicated to manage. The pet usually presents to the veterinarian for the first time with a skin infection. The pet may be licking and chewing skin and paws or scratching, shaking and rubbing the ears. As pets are exposed to a variety of things both indoor and outdoor, the pet is usually treated and the symptoms ease. However if the pet’s skin/ears do not improve or the owner notices a seasonal aspect to the skin problems it is possible that allergies are involved.

may not have an actual allergy, but intolerance to certain proteins and ingredients in food. The symptoms seen with intolerance are mainly gastrointestinal in nature. Most owners will switch foods, but a food trial for allergies actually takes 4-6 weeks on a limited ingredient diet with no treats. Ingredients are then slowly added back in the diet until the culprit is discovered. This is labor intensive for the owner and intradermal or serum allergy testing will give an answer much more

quickly with less expense in the long run as pets may be allergic to more than one food item. Uncommon things that pets may be allergic to are dogs to cats (and vice versa), feathers, cigarette smoke, drugs, perfume, cleaning products, rubber and plastic. If you feel your pet has allergies, the best course of action is to see a veterinarian and discuss your pet’s individual health concerns.

Areas of the body most affected by skin allergies are paws, ears, eyes, the rectal area and the abdominal and inguinal areas. Since the pet is itchy, he or she will lick and chew until the skin barrier is broken and that is when a secondary bacterial or yeast infections can begin to affect the skin. In order to find out what the allergen(s) is, allergy testing is available. The gold-standard allergy testing is the intradermal test and is most commonly available through boarded veterinary dermatologists. There is also serum allergy testing which is done from the animal’s blood. Allergy testing is available for both environmental factors and food. Food allergies are less common than owners think, as the animal

Profiles April Reid D.V.M. Owner/Veterinarian Dr. Reid founded Peninsula Veterinary Services in 2017. She owned a thoroughbred racetrack practice for 14 years before switching to small animal medicine. She moved to the area in 2015 from Brooklyn with the intention of moving back as soon as possible. But she soon settled into the beach lifestyle, started the practice, and now just visits the city and Brooklyn as much as possible. Associate Veterinarian Dr. Rochelle Day graduated from Ross University School of Veterinary Medicine in 2015 after earning her undergraduate degree at University of Delaware in Animal Science and Agriculture. She joined our practice in January 2020. She and Dr. Reid had previously worked together and when she decided to move back to Delaware to be closer to her family, they were happy to professionally reunite. She has special interests in surgery, disease pathogenesis and preventative care. Dr. Day shares her home with her rambunctious husky, Abbadon whom she enjoys riding her bike with tandem on a leash. In her spare time, Dr. Day enjoys going to car shows, the art of yoga, working out and spending time with family and friends.

Poor Sleep For Caregivers A particularly difficult phenomenon for caregivers is called “sundowning.” This can occur in Alzheimer’s and other dementia patients, particularly in the mid-stages of the disease. The patient behaves relatively normally during daylight hours; however when darkness comes, they become agitated, often throughout the night, and unable to sleep, compromising the caregiver’s sleep.

any of us find ourselves becoming caregivers for our loved ones, whether it be for an infant, sick child, ailing spouse, parent or parent-in-law. According to a 2015 study by AARP and the National Alliance for Caregiving, an estimated 16-18% of adults are caregivers. That study estimated 60% of the caregivers are female. Beyond the obvious strains of caregiving, is the detrimental impact it has on our sleep. Poor sleep has been found to negatively impact; physical health, cognitive skills, productivity, weight management, mood, coping ability and even depression. When we are sleep deprived, we make mistakes, become clumsy and have increased risk of auto and occupational accidents.

Parents of newborns are wellknown to be sleep deprived. Although infants need a lot of sleep, it takes a while for them to consolidate most of their sleep into a somewhat solid block of nighttime sleep, accompanied by daytime naps. How can you safeguard your sleep while caring for someone who needs you? There are some strategies everyone can use, while others will depend upon your particular circumstances.

Start with good “sleep hygiene.” That simply means to develop good sleep habits:

Sleep is frequently interrupted by needs of the loved ones. We wake up to care for babies that need feeding, a diaper change or simply won’t sleep through the night. Our ailing loved ones often need medication or other assistance during the night, or perhaps they have a sleep disorder. The caregiver can suffer from insomnia (inability to fall asleep or stay asleep) due to worry or stress. The many stresses of caregiving make it difficult to relax and fall asleep. These can include; the physical stress of caring for someone, the emotional stress of seeing someone you love face challenges or failures, and often financial stresses as you try to balance all of the additional costs needed for care on a limited income.

Avoid caffeine late in the day.

As much as possible keep regular bedtime and awakening-time.

Get bright light in the morning (the earlier, the better).

Avoid light in the evenings before bed – including light from screens (T.V., computers, phones and tablets).

Keep your bedroom cool, quiet and dark.

Eat a nutritious, balanced diet.

If you need a bedtime or night-time snack, try to choose something that is high protein.

Exercise regularly, but not too close to bedtime.

Good sleep hygiene for your loved one can also help. Particularly good light exposure in the mornings and limiting it in the evenings. Look for resources to help with caring for your loved one, such as

finding a relative or friend to relieve you for a few hours or hiring someone to assist with household chores. Physicians can help. A discussion with your loved one’s physician may connect you with an agency that can help give you a break or relief. Consider asking them to evaluate your loved one for symptoms of sleep disorders in addition to their other health issues. A medication evaluation might be worthwhile. If there are any medications that have unwanted side effects on the patient’s sleep, discuss pros and cons of alternative medications or treatments. Finally, consider contacting a local sleep disorder center for help. While some only perform diagnostic testing, look for one that has specialists on-staff to provide therapy for issues that are common in caregivers, like insomnia. Getting some help for yourself while you’re helping your loved one can be a great improvement for both of you!

Long, Esquire When my daughter was only 5 days old, I received the chilling phone call from her pediatrician that no new parent wants to get: “There’s something wrong” the doctor told me. Jordyn’s newborn bloodwork had come back abnormal and we had to act on it- fast! “Take her to the closest pediatric hospital, do not wait until morning and we’ll call and let them know she’s coming.” With tears and trembling hands, we rushed our tiny 5-day old baby to A.I. DuPont where, as promised, we were met with an ER staff that already had her bloodwork results in hand.

This ER visit began our journey of trying to understand Jordyn’s rare thyroid condition, which then transitioned into the daunting task of looking for a pediatric endocrinologist to treat her. In my line of work, I don’t typically work with endocrinologists, pediatric or otherwise, so this was a bit of uncharted water for me. Initially I consulted with a doctor and was less than impressed with the level of care we received. A few weeks later, I joined an online support group, and through that group, found a fantastic doctor that had recently moved to our area from Boston. We met with her, and knew right away that we finally found the right doctor for Jordyn. Every time we see this new doctor, I am so thankful that I didn’t settle for what I considered to be sub-par service for my little girl! People injured in car accidents and work injuries go through the same process of trying to find not just any doctor, but a good doctor to care for their injuries. Most people find themselves feeling like a “fish out of water” in that they need treatment right away but are not sure where to go for the best treatment. In Delaware, injured workers must generally see a Certified Healthcare Provider in order for the doctor’s bills to be paid by the workers’ compensation insurance company. On the other hand, car accident victims can see any doctor they choose and no additional certification is necessary. One of the benefits to working with my legal team is that you get a “friend in the business.” As a workers’ compensation and personal injury attorney, I am familiar with the local doctors who would be best to treat your injuries. Whole life insurance is permanent insurance. You pay a level premium each year which is applied to the cost of insurance and to cash value. This asset called whole life insurance with it’s “pool of money” is available for a life event (if necessary) and gives

you permission to spend down your retirement assets while still leaving a financial legacy to your family. Why a Long Term Care Rider? It’s the most cost effective way of having Long Term Care (LTC) coverage available if and when you need it. Long term care costs can financially ruin you and your family

or worse...you can’t afford the care. The cost of a pure LTC policy is high. Insurance companies can’t guarantee level premiums each year because they are paying claims at a high rate. So, even if you get a pure LTC policy, you don’t know how much or how often the premiums will increase, and if you can’t afford the increase, they can decrease your benefits.

Monthly long term care costs can be in excess of $5,000 a month. These costs vary from state to state. A pure LTC policy is a “use it or lose it” policy meaning that you can only use it if you have a claim. As a rider on a whole life policy, the premium is much less and you have a permanent insurance policy with the ability to have LTC coverage.

Helping Others Build and Lead Their Ideal Life You’ve read the books, attended the seminars and taken the classes and still, your life is not where you want it to be. The promise of “how-to”, “3 magic steps to success” and “5 secrets to getting everything you want” hasn’t produced the results you thought it would. I get it. My efforts left me wondering why my affirmations and positive thinking hadn’t gotten me very far. Why hustling and working smarter was not yielding the results I wanted. I could think of a million reasons why things were the way they were. But that didn’t make my life better. So I ended up settling – at least for a period of time. Settling for a life that appeared to be good enough. Maybe you’re like most people and you feel overworked, underpaid, and flat out exhausted. Or you’re dealing with money stress, health issues, kids off track or eating out of control. If I asked you “why”, you might say, because…and fill in the blank. Because your boss says to, because your spouse is a certain way, because you don’t have the money, because you don’t have the right education, or just because. In whatever way the situation, circumstance or condition is manifesting, you know somethings gotta give. I’m here to tell you that life doesn’t have to be that way. You don’t have to settle. Life doesn’t have to be hard, frustrating and overwhelming. The Bessell Warhol Project “I’m seeking out a new way to live, and if it’s surfing, that’s the way I’ll do it. Meeting my idol: My idea to combine the love for art and shaping art at San Diego State University, but also by my own personal experiences meeting Any and he was always someone I wanted to meet.

My early years: At a very young age, I was exposed to the infectious world of art by my mother. As a result, I gained an interest and appreciation for the sub-culture that would coincidentally shape my future. I began my professional shaping career working at Sunset Surfboards at age 13, and was working in the beautiful town of La Jolla, California. I was one of the youngest shapers in the industry, and made a name for myself as a surfer and shaper around my home break of Windansea Beach in La Jolla. Creating surfboards became my passion, and I moved to Hawaii at age 18…This is what you had to do in those days to prove you had what of Oahu. I got a job at Lightning Bolt, one of Hawaii's premier surfboard manufacturers, and later became one of the few shapers allowed to manufacture surfboards under the Lightning Bolt name. After legitimizing myself as a shaper, I returned to San Diego to open Bessell Surfboards. By 1987 we had grown into one of the largest surfboard shapers in Southern California. Today, I am still every bit as much an artist as a surfboard shaper, and am truly blessed to combine both of my passions into the Bessell Warhol Project!

Carl Ekstrom, one of my mentors and the inventor of the asymmetrical surfboard tail was a good friend of mine, and sold two surfboards to Andy in 1967. Andy loved surfboards and thought they were truly art, and Carl had cultivated a relationship with 1980’s, Carl told me to tell Andy hello if I ran into him. Gary’s older brother was a PR agent for the Playboy Club, and invited us to the grand opening of the Manhattan club. Andy Warhol ended up being there hanging out myself to Andy. I was so nervous, but I walked up and said we have a mutual friend Interview Magazine and we had two meetings with each other.

Slide on into 2021 with these fresh ideas for those hard to cover Sliding Doors!

Often while on an in-home consultation, the topic of handling the sliding door comes up. It’s always accompanied with some sort of joke, or look of bewilderment. I usually look forward to figuring out my customer’s way of articulating their concern. To paraphrase, it generally goes like this. “I need to cover this—I HATE VERTICAL BLINDS.” Of course, I need only remember back to my first apartment to understand why. Cheap vertical blinds are everywhere. Their top treatments are either bent up and trying to fall, or already placed in that closet, behind the water heater. The vertical louvers are seldom part of a complete set. They like to break and fall, and usually there is tape or unmatched colors mixed in for good measure. The good news- the custom blinds industry has you covered with more modern and beautiful options! Vertical Honeycomb ShadesHoneycomb shades are quite popular these days, so why not for sliders? Several manufacturers make honeycombs that are rotated on their side, and anchored to the stack side, and make a solid option for this application. Not only are these available in many colors and textures, but they are some of the most insulative coverings available. When you want to use the door, simply slide the shade into the closed position. You want it half open- no problem. Generally, magnets are used to help in the fully closed position. ShuttersShutters are one of the hottest products in the window covering industry, thanks in part to myriad of home improvement shows that seem to always drop in shutters as one of the final touches. Shutters are also a great insulator and are extremely easy to use. If you have kids or pets that are prone to a little rough housing, these are a nice hearty choice.

Sheer ShadesAnother trendy approach comprises multiple layers of sheer fabric, arrayed in a varying degree of light blocking and privacy choices. Essentially, when the vanes are opened, you can see right through the sheer backing. When you want that privacy, close the vanes and you’re all set. Natural Woven Wood DrapesIf you are a big fan of a natural look, several manufacturers market woven shades that are turned for use in vertical applications. Generally cordless and extremely light, these can be optioned with base bamboo materials that are cost competitive to custom vertical blinds. If you want to add darkening or higher levels of privacy, linings can be applied to the back. “Old school” Vertical BlindsYes, you’re reading this correctly. These are still made! As a provider of many different window treatments, I want to go on the record to say that vertical blinds are a great product. If you skip the dollar store version, you will find a smooth, effective, and long-lasting covering for a large glass door. No other treatment combines versatility, ease of use, and affordability quite like this venerable leader. Modern vertical blinds are available in high end vinyls and fabrics. For a contemporary look, we recommend outfitting your vertical blind with a stained wood valance, which can easily be matched to flooring, cabinetry, etc. As always, we recommend consulting with professionals to ensure your product is right for your application. This will also help improve the finished look and long-term reliability of your new treatment. Also- do make sure your seller offers you some sort of warranty for this or any window covering. Not only is this good insurance in case of damage or faulty components, but custom blinds retailers are not looking to warranty something that will fall apart.
