Summer is coming to a close and we are going into another beautiful fall season.

Our children are back to school and vacations are coming to an end. We now have the holidays to look forward to. With this in mind we start planning for all the events and gift giving. Lumbar Disc Herniation

By William L. Pfaff, MD Introduction A common cause of low back and leg pain is a ruptured or herniated disc.

Symptoms may include dull or sharp pain, muscle spasm or cramping, leg pain and leg weakness or loss of leg function.

Sneezing, coughing or bending usually intensifies the pain. Rarely bowel or bladder control is lost, and if this occurs, seek medical attention at once.

The disc, which is located between the bones of the spine (vertebrae), splits or ruptures. When this happens, the inner gel-like substance (nucleus pulposus) leaks out. This is called a herniation of the nucleus pulposus or a disc ‘herniation’.

Leg pain is a symptom frequently associated with a lumbar herniated disc.

Pressure on one or more nerves that causes pain, burning, tingling, numbness, and weakness that extends from the buttock into the leg and sometimes into the foot. Usually one side (left or right) is affected.

Anatomy In between each of the five lumbar vertebrae (bones) is a disc, a tough fibrous shock-absorbing pad. Endplates line the ends of each vertebra and help hold the individual discs in place.

Each disc contains a tire-like outer band (called the annulus fibrosus) that encases a gel-like substance (called the nucleus pulposus). Nerve roots exit the spinal canal through small passageways between the vertebrae and discs.

Pain and other symptoms can develop when the damaged disc pushes into the spinal canal or nerve roots. Disc herniation occurs when the annulus fibrosus breaks open or cracks allowing the nucleus pulposus to escape. This is called a Herniated Nucleus Pulposus or herniated disc.

Causes Many factors increase the risk for disc herniation: 1) Lifestyle choices such as

tobacco use, lack of regular exercise, and poor nutrition substantially contribute to poor disc health. 2) As the body ages, natural biochemical changes cause discs to gradually dry out affecting disc strength and resiliency. 3) Poor posture combined with the habitual use of incorrect body mechanics stresses the lumbar spine, affecting its normal ability to carry the bulk of the body’s weight. Combine these factors with the affects from daily wear and tear, injury, incorrect lifting, or twisting and it is easy to understand why a disc may herniate.

Lifting something incorrectly can cause disc pressure to rise to several hundred pounds per square inch.

A herniation may develop suddenly or gradually over weeks or months.

Interestingly, not every herniated disc causes symptoms. Some people discover they have a bulging or herniated disc after an MRI (magnetic resonance imaging) for an unrelated reason.

Most of the time, the symptoms prompt the patient to seek medical care. The visit with the doctor usually includes a review of medical history, symptom evaluation and the history of treatments and medications the patient has tried and a physical with a neurological examination.

An x-ray may be needed to rule out other causes of back pain. An MRI scan verifies the extent and location of the disc herniation.

non-steroidal anti-inflammatory drugs (NSAIDS). These work by relieving both the swelling and the pain. Discuss NSAID use with your doctor first.

Physical therapy may be recommended and prescribed. Specifically, a special type of physical therapy referred to as ‘McKenzie style’ has been shown to significantly reduce symptoms and help remodel the disc and get the pressure off of the nerves. Ice and heat therapy, gentle massage, stretching and lumbar traction are other helpful modalities.

If leg pain is severe, or leg weakness is developing, an epidural steroid injection may be indicated. An epidural steroid injection is an injection of anti-inflammatory medication (steroid) directly to the area of the herniated disc and affected nerves. This can be performed more than once, at least 6 weeks apart and up to three or four times per year.

In four to six weeks, the majority of patients find that their symptoms are relieved without surgery. Be optimistic if you find yourself with a disc herniation and remember that only a small percentage of patients go on to require surgery.

Surgical Treatment Surgery is considered if non-surgical

treatment fails. Constant pain, leg weakness, or loss of function requires further evaluation. Rarely does a lumbar disc herniation cause bowel/bladder incontinence or groin/genital numbness, which requires immediate medical attention. If surgery is recommended, be involved and ask the purpose of the operation and what results you can expect. Never be afraid to seek a second opinion.

To relieve nerve pressure and leg pain, surgery usually involves a partial disc removal called a discectomy. In addition, the surgeon may need to access the herniated disc by removing a portion of the bone covering the nerve. This procedure is called a laminotomy.

Fortunately, these procedures can often be done utilizing minimally invasive techniques. Minimally invasive surgery does not require large incisions, but instead uses small cuts and tiny specialized instruments and devices.

Prevention Aging is inevitable, but lifestyle changes can help to prevent lumbar disc problems. Risk factors include poor posture and body mechanics, weak abdominal muscles, smoking, and obesity. Start now to adopt habits that will help preserve your spine for the future.

Non-surgical treatment Most patients with a lumbar herniated disc do not need surgery. Initially, the doctor may recommend some physical therapy and medications, and/or an epidural steroid injection.

Medications may include an antiinflammatory to reduce swelling , a muscle relaxant to calm spasms, and a pain reliever to alleviate intense but short-lived pain (acute pain). Mild to moderate pain may be treated with

With September around the corner and kids preparing to return to school buying clothes, backpacks, paper and pens, parents should consider vision as part of this preparation.

Eye exams are routine for many children whose visual problems were caught early and corrected with eyeglasses or contact lenses depending on their age. Early in children’s eyecare frames are designed with safety, comfort and durability in mind. Preschool, kindergarten and grade school ages often find frames that are colorful, flexible and in shapes that fit the typically narrow pupillary distances as well having pads for the bridge of noses that are less developed.

Today’s children are more aware of style and brands they are exposed to at an early age such as Fisher Price, Adidas, LinDberg Kids, Solo Bambini, Rec Specs, and dilli dalli to name a few. The prescription is always the main consideration when choosing a child’s eyewear. When selecting frames the optician should be able to read the doctor’s prescription and have suggestions on frame styles that best fit the child but also lenses that provide safe, clear vision.

Lenses, depending on the prescription, can be thicker either in the center (plus lenses) or the outer edge (minus lenses). Frame selection can cosmetically help children feel more comfortable in these cases.

Frames with wider temples can hide some of the edge thickness on a minus prescription. Those with the plus lenses can benefit from having a frame where the pupil fits as close to the geometric center of the frame as possible. The lens with less decentration (moving optical center) creates less thickness and better peripheral vision.

Do I Need A Night Guard?

By Richard J. McCann, D.M.D., P.A. If you frequently wake up with a sore jaw, a dull headache or tooth pain, you may want to ask a qualified dental professional if you are suffering from a condition called bruxism.

Bruxism, or teeth grinding, affects about 10 to 15 percent of adults, according to the American Dental Association.

There are several ways to alleviate the effects of bruxism. One of the most common, which your dentist may recommend, is a night guard.

This is an appliance typically worn while sleeping through the night, which is available without a prescription at most pharmacies. To use the appliance, you simply boil it, let it cool for a few seconds and then gently bite into it to shape it to your own teeth.

Night guards are also available through your dentist. Guards of this type tend to be of a higher quality and superior fit, as they are made in a special laboratory from an impression of your mouth carefully taken by a dental professional. This is generally a more expensive device than one purchased over-thecounter, but can be more comfortable to wear.

Regardless of where a night guard is purchased, its main function is to prevent the surfaces of your teeth from grinding together, causing you to chip and crack your teeth and strain your jaw muscles.

Other Treatments for Teeth Grinding If a night guard is uncomfortable, there are other possible treatments listed below that you can ask your dentist about.

Teeth straightening: Teeth that are out of alignment--what dentists call malocclusion-can contribute to teeth grinding. This can be remedied by corrective measures such as braces.

Stress prevention: One of the main causes of bruxism is anxiety, so your dentist might advise you to utilize one or more at-home methods of relaxation, like meditation, or in some cases professional counseling, to get you to relax and stop clenching your jaw.

He completed his post-graduate medical education at the University of Pennsylvania Graduate Hospital in Philadelphia with a residency in internal medicine (1991-1994), chief medical resident (19941995), and cardiology fellowship (1995-1998).

He has served as medical director of the Cardiac Catheterization Laboratory and as chief of the Department of Cardiology at Beebe Medical Center in Lewes, Del. Dr. Rosa is trained in non-invasive cardiology, nuclear transesophageal echocardiograms, as well as invasive diagnostic procedures and pacemaker implantations. Dr. Rosa is board certified by the American Boards of Internal Medicine and Cardiology. He is also a Certified Aviation Medical Examiner and is board certified in nuclear cardiology.

Is an Aspirin a day still a valuable tool to prevent cardiovascular events?

There has been a considerable amount of interest in recent months over the efficacy of using aspirin as a preventive measure in reducing cardiovascular events. In the fall of 2018, the publication of 3 major clinical studies offered strong evidence to the contrary. Here is the issue, however that has led to major confusion in a great segment of the general public: We are talking about “Primary Prevention” that is, avoidance of a first cardiovascular event such as a heart attack, stroke, or sudden cardiac death. For patients that have already survived any of those events, or have had cardiac surgery such as coronary bypass or intervention (angioplasty or stents), vascular surgery or intervention, mini strokes (transient ischemic attacks), or peripheral vascular disease, the use of aspirin remains very important, as “Secondary Prevention”. In other words, to prevent yet another event or need for further surgery or other procedures.

The recent studies involved a variety of patients ranging from type 2 diabetics to non-diabetics and ages mid to late 50’s to over 70 years of age. The use of aspirin to prevent cardiovascular events in patients that have

not yet suffered from such a problem, even by the age of 70 years, showed to reduce the probabilities by 12% (in the best-case scenario) but at a significant cost of a 29 % increase risk of bleeding. Some of the bleedings were serious or even fatal in a small percentage of cases. It is important to note that the dose of aspirin utilized in these trials was 100 mgs against a placebo.

So, what have changed over the years to make the use of aspirin seemingly not that helpful or quite frankly, potentially dangerous? Several factors have been found to play a major role in this paradigm shift. First of all, the old primary prevention trials (late 80’s-early 90”) included a significantly higher number of smokers, poorly controlled hypertensive patients and diabetics likewise with less strict control of their blood sugar levels. Second, and perhaps more important, there was a relatively low use of cholesterol lowering drugs, particularly statins. In the recent trials, which were named ASPREE, ARRIVE and ASCEND, the use of statins among participants reached 34 %, 43% and 75 %, respectively. Other influential factors nowadays, are better dietary habits and more regular

physical activity in the general population, as a whole.

To summarize the practical implications of the most current medical evidence about the use of aspirin for cardiovascular disease, here are the take home points: 1.-Aspirin remains a very important medication to be used in all cases (unless specific contraindications are present) for patients with a history of heart attack, stroke, or coronary surgery, stents/angioplasty, carotid artery disease/ surgery

or peripheral vascular disease, particularly in diabetics and/ or smokers. This is the Secondary Prevention scenario.

2.- For patients whom have not yet suffered any cardiovascular events, specifically heart attacks, angina, strokes or “ministrokes”, or cardiac surgery or intervention (stents or angioplasty) or peripheral vascular disease, the use of aspirin to prevent a first event (“Primary Prevention”), is still beneficial but it comes at a considerable higher risk for bleeding, some of which could

be serious or even fatal.

It is important that, in all cases, before starting or stopping the use of aspirin patients consult with their cardiologists or health care provider responsible for their care. It needs to be done for the right reasons.

Aspirin have been around for over 150 years in the medical field and have been a lifesaving drug for millions. Recent evidence brings to light the need for a more careful and thoughtful use of this old and trusted friend. Let’s have that conversation!

Do you try to hide your legs in the warmer months because of bulging or unsightly veins?

Often, it is too late to try to prevent bulging veins, however, walking regularly, drinking plenty of water, and not crossing your legs can help improve the health of your veins.

Varicose veins bulge in some areas or can be red and spidery in other areas.

This condition is very common; it can be painful or hard to deal with, and in some cases, it may indicate a larger health issue.

Spider veins and varicose veins often start when we are younger. They frequently appear at the time of pregnancy, but may appear spontaneously at any time throughout our lives.

Varicose veins may occur in many parts of the body, however, the leg veins are most often affected because these veins must work against gravity. The valves within your veins may weaken or break over time, causing some of the blood to pool in the legs instead of going to the heart. This back-up of blood results in bulging veins.

Chances are if you have varicose veins, then your mother or grandmother did as well. Women are at an increased risk for varicose veins compared to men.

I have smoker’s lines but I don’t smoke!

Are you frustrated about having “smoker’s” lines around your lips and you’ve never smoked?

You are not alone! This is a common complaint of a lot of people, having those pesky vertical creases around your lips that appear when we pucker, use a straw or just speak! And with time and continued motion of our mouths, these creases can become permanently etched in our skin.

You don’t have to smoke to have smokers lip lines Of course smoking makes these creases worse, but it is not the only cause of “smoker’s” lip lines. The cause of these lip lines is due to thinning of the skin around our mouths and losing the strength of the natural white roll of the lip border, where the lip transitions from the pink color of the lip to the normal facial skin. It is this natural white roll that forms the transition or border between the actual lip and the skin surrounding the lip, where lip liner would be drawn.

This white roll border is what usually prevents applied lipstick from bleeding or smudging away from the lip itself onto the surround skin. If you were to observe a child’s lip, this natural white roll of the lip border is typically present and can be felt as a soft firmness that defines the border of the lip.

Lip lines and creases are also caused by the daily use of the mouth in everyday speaking. The facial expressions with using everyday speech causes the mouth to crease in the same areas repeatedly and over time, the creases become permanent.

So what can we do to address this pesky concern of lip lines?

Thankfully, we have several options! The first problem issue I like to address when treating these lines, is to restore the strength of the natural white roll that has been lost. This is done by adding a hyaluronic acid filler directly into the white roll. By rebuilding and strengthening this natural border between the lip and the surrounding facial skin, the lips cannot collapse as easily like an accordion and form lip lines and creases, when we are speaking, eating or making other mouth motions. Hyaluronic acids that are typically used in this area of the lip include brand name fillers like Restylane and Juvederm. https://www.renovemedspa.org/lipfillers Lip fillers can strengthen lip borders These fillers are specifically designed to match natural lip softness and flexibility while providing moisture and strength. Hyaluronic acid in our skin binds water in the skin. With more hyaluronidase, that break down hyaluronic acid, we lose moisture in our skin and lips with age.

Repair collagen damage with Bellafill Another way to more permanently improve lips creases is to injected a collagen stimulator, such as Bellafill, to the skin around the lips, exactly where the lip creases are. With injection of Bellafill, you will see a smoothing of the lip lines right away, but also continued improvement over the next 8 weeks as new collagen is laid down and damaged collagen around the lips is remodeled, softening lips creases, for years to come afterwards. Healing Your Inner Child Oprah Winfrey on a recent Super Soul Conversations OWN podcast titled “When You Know Better, You Do Better,” shared her testimony.

Oprah said when she was 14 years old, she became pregnant and hid the pregnancy for seven months until the baby was born. Success Hinges on Dedicated Volunteers Janis Hanwell, ITN Executive Director

Non-driving seniors need reliable transportation With the last of the baby boomers reaching their senior years, more than one-third of individuals age 70 and over no longer drive. One of the biggest challenges non-driving seniors encounter, especially in Sussex County, is finding safe, reliable transportation that can accommodate their unique needs. While public transit providers work to expand options in lower Delaware, many older adults either have limited or no access to these services or cannot find a local service to meet their special transportation needs.

It is these and various other circumstances that keep some older adults driving much longer than they might if they had convenient access to other more personalized transportation alternatives.

ITN Volunteer drivers and dispatchers train new volunteers and share their own experiences.

Serving 55+ and adults with visual impairments ITN Southern Delaware is a nonprofit, communitysupported, membershipbased transportation cooperative that is addressing this issue “full speed” by providing arm-through-arm, door-

through-door ride services within the membership area with no limitations to ride purpose. As an all-volunteer community-based network, ITN provides reliable transportation for seniors 55+ and adults with visual impairments throughout Sussex and Lower Kent Counties. By providing rides any day, any time to doctor appointments, worship services, consumer and business related locations, to social and recreational events and activities, and much more, the nonprofit is supporting lifelong safety, increasing older nondriving adults’ mobility and independence, and enhancing their ability to age-in-place and take pleasure in an improved Quality of Life.

ITN volunteers make riders feel safe The increasing success of ITN Southern Delaware hinges on the support of volunteers. ITN drivers, dispatchers, and support staff are all dedicated, caring volunteers who make riders feel safe, comfortable, and cared for. They are committed to helping others and giving back to the community. All receive a free ITN membership, and immediately begin banking credits for their own future transportation needs. Volunteers can select their own hours and drivers drive their own cars around their own neighborhoods, pick up their own neighbors, and give them rides to places in and around their own communities. ITN is neighbor helping neighbor; the very core idea of “community”.

Volunteerism benefits self and others There is no prerequisite skill or special license necessary to volunteer with ITN. It only requires time around your own schedule and a desire to do something meaningful in your community. More than twothirds of ITN volunteers are women, many of whom define success in later life in terms of helping others rather than accumulating wealth. Over 85% of all ITN volunteers say they have developed new friendships through their volunteer activities. They say that it is a great way to make new social connections in Southern Delaware, and that donating their time rewards them with a greater purpose in life. They feel that giving back to society means they make a difference in the lives of others. Not only is it a fulfilling way to keep active and maintain physical health,

volunteers say being generous provides a significant source of happiness by giving them a sense of belonging and higher self-esteem. Furthermore, research supports the notion that volunteering improves an individual’s longevity and quality of life.

Volunteers Come in All Shapes and Sizes During our almost 36 years of service to the community, Delaware Hospice has been touched by the tremendous support of thousands of volunteers. These fun-loving, individuals have donated their time and talents throughout the years in a variety of ways. Within the past year alone, 407 volunteers have donated over 24,995 hours! The quality of life for patients and their families improve dramatically through the healthcare services provided and the 1,109 patient visits our volunteers fulfilled. Many of our volunteers come to us and offer up their skills for very unusual projects. While others are able to meet, our daily needs.

“I have been very blessed and love to give back in some small way,” commented Coles Marsh of Milton when asked why he volunteers.

Cole supports Delaware Hospice by visiting with patients specifically Veterans and volunteers at the reception desk at the Delaware Hospice Center.

Based on skill sets and wishes, volunteers help in a variety of areas throughout Delaware Hospice. Running errands with or for patients is always a needed service and is a wonderful experience for the volunteer and patient. Visiting with patients in one of our other eight programs such as Delaware Transitions, Delaware Palliative, or New Hope are other rewarding volunteer opportunities. Many volunteers choose to talk, read, and play cards with a patient and their family. Other volunteers enjoy helping in the Delaware Hospice offices and contribute administratively by updating databases, completing informational packets, or even taking pictures at our fundraising events. Volunteers are always able to see how their dedication positively affects our patients’ lives.

She has worked in many clinical areas (critical care, ER, cardiac rehabilitation, and orthopedics). Most recently, she became certified as a healing touch practitioner and is currently completing advanced training as a family constellation facilitator. Catherine is the founder and director of the Lewes Wellness Center in Lewes.

Dr. Jose A. Pando is an integrative rheumatologist practicing in Delaware for the last 20 years. He completed his internship and residency at the Yale University Primary Care Program, fellowships at Georgetown and NIH. Most recently, he completed an integrative medicine fellowship with Andrew Weil at the University of Arizona.

Let’s talk about Back pain

What is new at Delaware Arthritis Most of us have experienced back pain at one time or another. It is one of the most frequent causes for doctor office appointments as well as Emergency Department visits.

The approximate annual cost most recently published is estimated at 100 billion dollars ( 75% of the number is due to lost wages and productivity). Among the most common causes of back pain can be due to injury, muscle or ligament strains, bulging or ruptured discs, Arthritis as in Osteoarthritis or inflammation (Ankylosing Spondylitis or Psoriasis), Scoliosis or Osteoporosis (when you experience a compression fracture).

Should I be concerned if I have Back pain?

In some cases, back pain may be indicative of other medical problems and you should call your doctor if your back pain is accompanied with a fever, unexplained weight loss, shortness of breath. You should also seek medical attention when pain is severe and does not improve with rest. If the pain radiates to the legs or you have weakness or numbness of the legs also may be symptoms that require evaluation.

How do you manage Back pain?

Most patients that experience acute back pain recover completely in a few weeks. In some cases, the pain persists for months and becomes chronic.

To keep your back healthy, it is important to develop a program that focuses on strengthening and flexibility.

Medications can be used to decrease muscle spasms and decrease pain.

We use muscle relaxants and nonsteroidal medications as first line medical treatment. At Delaware Arthritis, we DO NOT usually use narcotics with the management of back pain. We prefer the use of CBD oil or integrative modalities such as like Yoga, Tai Chi, Pilates, massage therapy and energy work (Healing Touch).

We believe that a healthy back program starts with stretching exercises and activities that will increase the strength of your core muscles (muscles in the abdomen and low back).

How do your diagnosis Following the examination, a series of X rays are the first step. In cases of severe pain in which you need a deeper view of the spine a CT Scan or an MRI may be requested.

When the pain is more debilitating and causes functional limitations, we suggest physical therapy, massage therapy, acupuncture and chiropractic care can be valuable in decreasing the intensity of pain.

What is a Skilled Nursing Community?

A skilled nursing community—or “nursing home,” as they are more commonly called—is a community created for those who need regular medical attention as well as assistance completing many of the tasks found in their daily lives.

While some other long-term care options offer similar services, what truly sets skilled nursing homes apart is the presence of licensed medical professionals on premises. As the name suggests, a 24/7 staff of skilled nurses and caregivers, as well as physicians are always available. Because of this, skilled nursing communities are an ideal option for older adults requiring round-the-clock care as well as those recovering from a life-changing accident, injury or illness.

In most cases, the individualized care and attention make skilled nursing communities a perfect fit for those suffering or recovering from traumatic illnesses. Children and Chiropractic Care Dr. Tracy Hudson Dr. Hudson is a ’95 Cape Henlopen High School graduate and continued her educational career at the University of North Carolina at Wilmington where she completed her Bachelor’s degree in Physical Education. While attending UNCW, Dr. Hudson suffered an injury playing softball and she sought treatment with a local chiropractor. After a few adjustments, Dr. Hudson was back on the field. Now being a true believer, she decided to further her education in the natural healing art. She attended Life University in Atlanta, Ga. for 3 years, and then transferred to Texas Chiropractic College in Houston. After graduating in 2004, she moved to Clearwater, Fla., where she was in private practice for 8 years. During that time, she also became a Fellow of the International Academy of Medical Acupuncture. She has now moved back to her home town to share her love of chiropractic with our community.

Children and Chiropractic Care: Success Stories There are been significant studies that show health improvements for children using chiropractic treatments. For example, in 2008 the International Chiropractic Pediatric Association published a case study showing the successful chiropractic care of pediatric patients with chronic constipation.

Likewise, the Journal of the Canadian Chiropractic Association published an article in 2009 indicating the effective treatment of Gastroesophageal Reflux Disease of a 3-month old baby girl. Similar achievements have been made for children with colic, asthma, and chronic ear infections.

Why Should My Child See a Pediatric Chiropractor?

Little ones are constantly enduring “micro-traumas”. The word “microtraumas” may sound scary, but we’re talking about natural incidents like going through the birth canal, falling while learning to walk, bumping heads, being thrown in the air by crazy Uncle Mike, etc. These adventures enhance our children’s understanding of the world, and yet can be the source of subluxations that may adversely affect their little bodies. Seven ways to prevent osteoporosis When it comes to osteoporosis, Bayhealth OB-GYN Melisa Edler, MD, says it’s important to recognize prevention starts at any age. “Even young girls and their parents need to be aware,” she said. With that in mind, here are seven ways Dr. Edler says you can prevent osteoporosis.

Eat a balanced diet to make sure you’re getting enough calcium and vitamin D. “Milk is a good source of both, since it’s often fortified with vitamin D,” said Dr. Edler. “You can also get calcium from any dairy product, and green, leafy vegetables, orange juice, and some cereals also contain calcium. Check food labels to find the ones that do have calcium.”

Spend time in the sun. “For vitamin D, you should also spend 10 to 15 minutes out in the sun each day. Sun exposure is how your body produces vitamin D,” explained Dr. Edler.

For those who can’t get outside or have trouble with the foods that contain calcium and vitamin D, supplements are an option. “While it’s best to get your calcium and vitamin D from diet and lifestyle, taking them in supplement form is better than nothing,” said Dr. Edler.

“But it’s very important you stick to the recommended doses because you can overdo it with vitamin D and calcium.”

The dosage recommendations for calcium are 1,300 mg per day for those ages 9–18 years old; 1,000 mg per day for those ages 19–50 years old; and 1,200 mg per day for those age 51 years old or older. For vitamin D, the recommended doses are 600 IU per day for those under age 70 and 800 IU per day for anyone age 70 and older.

Quit smoking. Dr. Edler says smoking, and even secondhand smoke, can lower bone density and increase risk of osteoporosis.

Watch your alcohol consumption.

Dr. Edler says while alcohol intake alone doesn’t increase risk of osteoporosis, if you consume three or more alcoholic drinks per day, it can increase your risk.

Prevent falls. “A couple of suggestions that will help you prevent falls are to use a night light and remove clutter from the floor,” said Dr. Edler.

Dr. Edler says women typically need to be screened for osteoporosis beginning at age 65. “The main osteoporosis screening tool is called a DEXA scan. The results of the initial scan will determine the frequency and need for any future scans, along with the presence of any risk factors,” she explained. She also says if you’re a postmenopausal woman and any of the following apply then you should talk to your doctor about considering earlier screening: • You weigh less than 127 pounds • You have a history of a low trauma fracture, which is when you break a bone in a situation you wouldn’t expect to; such as while opening a window • You take certain medications including steroids • One of your parents had a history of a hip fracture • You’re a current smoker • You suffer with alcoholism • You have rheumatoid arthritis

“To prevent osteoporosis, you need to do weight-bearing exercise, which puts weight resistance on your bones,” explained Dr. Edler. “Something as simple as walking can help. Another suggestion is to walk with a light set of weights.”

Breast Cancer Is Not a Choice - But the Final Results Are!

Unfortunately, for some women, after their mastectomy surgery, chemo, radiation, hair loss or reconstruction, they are left having to accept new body changes. After breast reconstruction, nipple and areola enhancement are typically the final step. This treatment is a form of micropigmentation also known as Permanent Cosmetics or Cosmetic Tattooing. This can often create a realistic result restoring a natural appearance to the breast.

Meridith L. Rothstein I was born in Easton MD, and raised in Newark DE. I graduated from St Marks in New Castle County, and attended Widener University in Chester, Pa. I met my husband while working for Marriott in Virginia Beach, we were engaged 3 months after we met, love at first sight. We have traveled the world for his US Air Force career. I have two kids, a son who is in college in Philadelphia, and a daughter who attends Cape Henlopen High School.

In April of 2012, I was diagnosed with Stage 2a Invasive Lobular Carcinoma, I was 39 year old. I was stunned. The first mammogram I had diagnosed me with breast cancer, the second mapped the double mastectomy I had 3 weeks later. I decided that I had been through too much in my life, and got angry at the cancer, angry and ready to fight. I did any and everything I could for the healthiest long-term outcome, to include- a double mastectomy with reconstruction, 6 rounds of chemotherapy and a complete hysterectomy(this was due to the Tamoxifen I’d be takiing and the studies showed the possibility of uterine cancer with Tamoxifen use).

My husband was my rock throughout all of my treatment. He supported all decisions, and it strengthened our relationship. We had a great deal of support from our USAF family, as well as from our local friends and family. I also had a great Circle of Women who were my health care providers and have become great friends who still support me in my Survival to this day.

As of April 17, 2019, I am 7 years cancer Free.

My husband retired from the USAF in July of 2016, and we settled in nicely to retirement in Lewes DE. He is in school to begin his next adventure as a Real Estate Broker, and I am thrilled and blessed to be working for the Delaware Breast Cancer Coalition helping newly diagnosed as well as my sister survivors continue on their journey toward good health. I am also working with and advocating for the National Breast Cancer Coalition to End Breast Cancer by the year 2020.

My name is Meghan Morgan and I am a 48-yearold wife, mother, and grandmother. I have been married to my husband Tony for 23 years. We have a daughter Autumn and son Nick. My joy and motivation to go thru this process has been my family – especially my grandson Lincoln!

I was diagnosed with breast cancer during the late summer of 2013, when I was 59. I always had my annual mammogram before school started because both my husband and I were busy teachers and drama directors at our respective schools in West Central Pennsylvania. When the lab called to ask me to come in for a second mammogram, I wasn’t too concerned. I had dense breasts, and periodically this had happened. When they called me to schedule the biopsy, I started to worry. I remember that I was standing in front of the toilet paper display in the grocery store when the doctor called to confirm the diagnosis. I put back the 4 pack of toilet paper and, instead picked up the 36 roll pack. I remember thinking, “Now, I must live long enough to use up all of this toilet paper!” I subsequently had a bilateral mastectomy with reconstruction. I was fortunate to have not needed chemo or radiation. The support of my husband, family members, colleagues, students, and friends kept me going throughout my healing.

I am generally an upbeat person, and I also credit my sense of humor for helping me cope.

I discovered that if I could find the humor in the situation, that it kept me from getting depressed or feeling sorry for myself. I can still make others laugh by telling my funny hospital stories!

I retired from the State of Delaware, Division of Developmental Disabilities Services after 27 years in August 2017. In the summer of 2016, we purchased part of Ryan’s Blueberry Farm in, Frankford, DE with plans for that to be my second career. We were successful with the new business Kingsley Orchards until my annual mammogram October 2018.

The results were triple negative breast cancer – This type of breast cancer is VERY aggressive and has a high potential for reoccurrence. With this information, we decided to sell the farm to reduce my stress and allow me to spend as much time with family as possible.

After a 2nd opinion at John Hopkins, it was very reassuring to know that Tunnell Cancer Center in Rehoboth offers identical treatment. Lumpectomy, chemo and radiation are now considered equal to mastectomy. I followed the plan and I was fortunate my cancer was stage 1 with no lymph node involvement. Over the last 8 months, I had lumpectomy, 16 rounds of chemo, 4 Neulasta injections, and 20 radiation treatments.

The treatment team, volunteers and pre chemo medications at Tunnell Cancer Center were FANTASTIC— I praise God for my family, friends, church, facebook support groups, Delaware Breast Cancer Coalition – Meredith, and Tunnell Center Breast Cancer Navigator- Kathy. There are many resources available: including Cancer Support Community, and Women Supporting Women.

These programs offer free wigs, hats, 1:1 support and counseling.

My recommendation: be positive, pray, live each day to the fullest, enjoy your family and friends, and get your annual mammogram!

One lesson that breast cancer taught me is that tomorrow is not guaranteed. Consequently, in 2016, my husband and I decided to retire early and move to Rehoboth Beach. I do miss my school and students, but stay quite busy in retirement. I volunteer at CAMP Rehoboth and with Immanuel Shelter. I sing with CAMP Chorus, usher at Clearspace Theater and have helped with theater productions at CAMP Rehoboth. I am an active member of my community HOA Board. I am also a Mentor with DBCC. While I am grateful for the skilled surgeons and wonderful medical care I received in Pennsylvania, there was not the support system in place in PA as there is here in Delaware. The programs offered by Delaware Breast Cancer Coalition are indispensable to women during all stages of the breast cancer journey.

Eileen, age 54, resides in Rehoboth Beach with husband Barry and their dog Lucy Bear. Prior to her initial breast cancer diagnosis (stage 2b) in 2016 she was an avid triathlete competing in over 50 MultiSport events in her forties. After recovering from a double mastectomy she returned to teaching throughout chemotherapy treatments.

Vincenza Fabber was born in NYC (Manhattan), in an area known as “Little Italy”. Her grandmother, mother and aunt were cancer survivors. Vinnie was frequently ill as a child. By age 5, she was diagnosed with leukemia. Much of her youth was spent in hospitals.

Vinnie didn’t know she had leukemia, only that she was very ill. One evening while in the hospital, her uncle asked what she wanted to be when she grew up.

Her response: “A psychologist”…

She connected with Lois at the Delaware Breast Cancer Coalition and received a mentor. The mentor helped her with her journey through chemo.

She retired early to enjoy life at the beach. She has joined in numerous Delaware Breast Cancer Coalition activities and is interested in a yoga retreat.

Upon graduation from high school, she got married and soon after had the first (of 6) children. Her working career started on Wall Street, while attending nursing school. Then, she purchased a take-out Chinese restaurant. This turned out to be a very profitable investment, leading her to develop and sell several more before leaving New York and moving to Pennsylvania.

Mike Baker is the founder of C.M.Baker Photography with over 25 years experience. His photography ventures have been quite diverse.

Throughout the year you may find him shooting a wedding, a Quinceañera, in the pits of a NASCAR race at Dover Downs and now at front stage at the Firefly Festival or flying over doing aerials. Giving back to the community has always been an important part of it all. Mike was awarded “Volunteer of the Year” from CDCC (Central De Chamber of Comm.) in 2005, ”Volunteer of the Year” from Kent County Tourism 2010,”Chairman’s Outstanding Partnership Award from CDCC in 2010 along with “Business Person of the Year” from CDCC in 2008. Since 1997, Mike has been the “official” photographer for the KC Heart Ball and for the “Go Red” in Kent County since it’s onset. Some of the past and present organizations have been: MD Kidney Foundation, De Breast Cancer Coalition, Various De Fire Companies, Bayhealth Foundation, Beebe Hospital, Dover Air Force Base, Autism DE, Relay for Life, MS Bike to the Bay and the Amish Bike Tour , to name a few.

At the end of that school year in 2017 she was diagnosed with stage 4 bone mets. Since then, thankfully, the cancer has remained stable.

As time went by, Vinnie’s health improved. There were many experimental treatments for Leukemia back then.

Not sure what treatment regimen was used but by age 9 she was in remission and eventually leukemia free.

Vinnie enrolled in a community college and began taking early childhood development and psychology courses. Several colleges and universities later, she had acquired multiple Associates, Bachelors, Masters and her PhD in Clinical Psychology.

Dr. Fabber’s degrees and certifications include: Early Childhood Education, Human Services, Counseling Psychology, Clinical Psychology, Certified Clinical Correctional Health Care Provider, Certified Hypnotherapist, Registered Play Therapist, Filial Therapist, EAGALA Equine Assisted Therapy certified, Licensed Mental Health Professional, Certified CPR Trainer, Certified Trauma Informed Care Specialist, and a National Institute of Correction Crisis and Hostage Negotiator.

• In 2002, Vinnie relocated to Delaware and became the Statewide Director of Behavioral Health Services for the Department of Correction (DOC).

• Post DOC retirement, she continues to provide behavioral health services in private practice in Georgetown, Delaware and oversight of treatment services for the Joseph Patrick Fabber Memorial Foundation, a 501c3 non-profit she founded in 2001 with a primary focus on suicide prevention.

Veterinary Services Peninsula Veterinary Services is a full-service, AAHA-accredited hospital located in Long Neck, Delaware. Owner Dr. April Reid moved from Brooklyn, New York in 2015 to start a practice in this area after spending summers in Rehoboth growing up. Dr. Reid is a graduate of Ross University School of Veterinary Medicine. She previously owned a thoroughbred racetrack practice for 14 years before making the move to small animal practice. Dr. Reid works closely with several local rescue organizations. She is currently serving as the vice president of the Delaware Veterinary Medical Association and pursuing board certification in veterinary practice. In her spare time, Dr. Reid enjoys spending time with her two Great Danes, Ambrose and Amelia, and her three cats.

What Should I Feed My Pet??

That is the number one question a veterinarian is asked, especially lately with numerous recalls and studies that have implicated certain foods with heart disease. Unfortunately, there is no easy answer to this as there is no one-size-fits-all food for our pets. Here are several areas to consider when choosing a food.

Life Stage This one would seem to be obvious but it can be confusing depending on the breed of your dog. Puppy foods tend to be higher in calories because growing requires energy.

The age at which your puppy should be switched to an adult food is when they have reached 80% of their adult size. This occurs earlier in small dogs than in large dogs.

Large breed puppies are more prone to developmental bone diseases so they should normally eat large breed puppy food due to the calcium to phosphorus ratio. Senior dog foods usually contain increased protein and fiber but fewer calories, as older dogs tend to eat less but also are not as active, so obesity can become an issue. In addition, there are “All Life Stages” foods, which most dogs can eat throughout their lives no matter their age. This is a good choice when there are multiple dogs in the household and it is impossible to separate them for feeding.

Grain-Free?

While grain-free dog food sounds healthier, the truth is that only a tiny percentage of dogs (less than 1%) are actually allergic to grains. Grains serve as a source of carbohydrates for dogs, which they need for energy. Grains also provide a source of fiber, which is

important for gastrointestinal health.

Grain-free food therefore must use other ingredients as a carbohydrate source and these are usually potatoes.

Chickpeas, lentils, or peas. These alternative carbohydrates can be higher in calories and lead to weight gain. In the recent FDA investigation into the increased rate of heart disease in dogs eating grain-free diets, 93 percent contained peas or lentils and 42 percent contained potatoes or sweet potatoes as the carbohydrate source.

Food Allergies Food allergies or food sensitivity in pets can manifest as digestive problems, skin problems and even behavioral problems. The most common allergies are to chicken, beef or dairy products. Elimination

diets or allergy testing may be needed to obtain an absolute diagnosis to determine the culprit. If the sensitivity is to a certain protein, the options are to switch to another protein source or to a hydrolyzed protein. Hydrolyzed proteins are those that have been broken down chemically so the immune system does not recognize them as a protein to which it is sensitive. These foods are often prescription foods.

By-Products and Marketing By-products are not evil. By-products are just a secondary product derived when a primary product is produced.

Chickens are prepared for human consumption and by-products are everything that isn’t in the package sold in the grocery store. The byproducts are high in essential amino acids and have high bioavailability for our pets. These by-products are now being sold in pet food stores as dehydrated treats or cleverly marketed as exotic ingredients in boutique-brand foods.

AAFCO The Association of American Feed Control does not regulate pet food.

Its mission is to protect the consumer through labeling guidelines. In order for a food to be labeled as “complete and balanced” it must meet nutritional standards established by AAFCO in one of two ways-feeding trials or nutrient content analysis.

The current FDA investigation is exposing the fact that the nutrient content analysis may not be enough to ensure a food is safe for your dog. If a food has not met AAFCO standards it must state on the

Favorite Tips from the Professional Staff at Oceanova, the Spa at Ocean View

Oceanova, the Spa at Ocean View, would like to share advice and tips from our staff, on how the services we offer can help improve your wellbeing and how you can continue to maintain these benefits between your appointments.

Skin Care Featuring Image and A Natural Difference professional Skin Care Oceanova Spa’s staff of aestheticians including Michelle Bossard, Stacey Ruberti, Lorena Trinidad and Amanda Vosselman offer their advice and recommends for an effective skin care regiment.

Michelle, an aesthetician since 2007, believes that there are many simple things one can do to maximize their skincare regiment.

First, give a new product at least 6 weeks to effectively work, since your skin regenerates itself every 6 weeks. Second, if your skin is feeling extra dry, layer a thicker moisturizer on at night, since your skins’ natural oils come out at night. This process will remoisturize your skin. Third, keep your eye cream in the fridge. The coolness of the cream will help with any puffiness or bags under the eyes when applied in the morning.

Finally, keep skin care products in the shower, like your masque or exfoliant. The steam will open your pores. This is also a time saver, allowing you to do this while washing your hair or shaving.

Stacey recommends adding Dermaplaning to your facial. Dermaplaning is a gentle way to exfoliate, remove peach fuzz and improve the skins overall texture. It will leave your skin feeling like glass and will enable makeup go on nice and smooth. Stacey’s favorite tip is that a “serum” always goes onto the skin first after cleansing and toning, in order to deliver the active ingredients to the skin most efficiently. Then apply eye cream, which can also be used on the lips. Finish with a nice moisturizer.

Amanda, a PSU graduate in kinesiology, health and wellness, has 14 years of experience as an aesthetician. Amanda’s favorite quote is “You can’t treat your skin

like a flip flop and expect it to look like a Louis Vuitton!”. The magic number for scheduling a professional facial treatment is monthly. Michele Bartley LMT, has over 8 years of massage experience working with area chiropractors and at Oceanova. What Michele loves most about massage therapy is the ability to make people feel better, alleviating their pain and helping them to relax. “Seeing

people feel physically better though massage therapy is incredibly rewarding”. Michele’s advice to clients is to use their body evenly.

Whitney came into the field of Massage Therapy in 2009 because she has always wanted to help people. Something simple she recommends to her clients, that they can do at home, is to soak in Epsom salts (magnesium sulfate). For many people, sites like Craigslist and eBay offer a chance to save on everyday items and luxury products, both new and used—all from the comfort of their own homes.

Yet, while online shopping may be a convenient way to find deals and oneof-a-kind items, it’s important to protect your identity and financial information, particularly when dealing with individual sellers. Before you purchase anything listed on an online classified ad, auction, or marketplace site, keep the following precautions in mind.

Online classifieds On classified sites run by newspapers and popular venues like Craigslist, sellers tend to offer used items and may be open to negotiation. Since most transactions are done in cash, there’s little buyer protection.

To stay safe: • Never wire funds. If a seller asks you to wire payment using Western Union or MoneyGram, you’re likely dealing with a scammer.

• Safeguard your personal information. Sellers on Craigslist and similar sites don’t need your personal financial information, such as credit card numbers. To keep your information safe, it’s best to pay with cash.

What still amazes us to this day is how so many of our clients struggled with the thought of calling a lawyer after being injured. Despite the fact they were injured as a result of someone else’s negligence or reckless behavior, they still struggled with making the call.

Many of our clients do not want to be viewed as litigious or “sue happy.”

We are thankful for the opportunity to have guided many through the challenges of being injured (medical bills, wage loss, vehicle damage, struggling with the activities of daily living, impact on relationships with family and friends). We are certain they would advise any “potential client” that contacting a lawyer is the right call. Let us share with you some of our observations as to the reasons behind the hesitation, in hopes of making the decision to contact an attorney a “no-brainer.”

Sometimes, the insurance company’s quick offer of a cash settlement seems like the answer to a prayer for someone struggling financially. The minor aches and pains she may be experiencing at the time are viewed as temporary – and certainly no reason to pass up the “opportunity” of making this month’s rent or mortgage payment.

In exchange for settlement cash, an injured person must sign a document known as a “Release.” By signing this document, an injured person gives up any and all claims she may have against the at-fault party, now or in the future. Even if the injuries become serious and disabling over time, an injured party is typically stuck with the short-sighted deal she made with the insurance company. Since this person ultimately never contacts an attorney, it is difficult to estimate how many individuals succumb to this “deal.”

You love your home. But your mortgage? That might be a different story.

In a growing industry, any business wanting to stay on top must develop a skilled and cohesive team. This entails hiring the best talent available and cultivating that talent to provide the best service possible. In certain industries, a pre-defined notion of what this type of talent looks like is endemic.

In traditionally male-dominated fields such as pest control, only 8.4 percent of the workforce is made up of females, according to the Bureau of Labor Statistics. But this number is on the rise, up from only 3.7 percent in 2014. With the number of women entering the pest control field more than doubling in just under five years, it is important to understand the vital role this type of diversity plays in this historically male-dominated field.

At Bennett Termite and Pest Solutions, 29 percent of the staff is female, more than triple the industry average. Four women total are on staff serving in roles including bookkeeping, customer service, and field technician.

Belinda Bennett, owner, along with her husband Dean, said, “I strive to recognize greatness when I see it, and my goal was to put these ladies in the spotlight. They give our customers their undivided attention and go above and beyond to exemplify our mission at Bennett Termite.”

there are today. At that point, her quest to be the best at getting rid of pests was a balancing act to ensure she had plenty of time for her kids. After thirteen years in the industry in Florida and taking some time off to be with her kids, Lisa is now going on three years with Bennett Termite and Pest Solutions as a highly trained professional with state certifications for termite and general pests and a leading asset among their roster of experts.

“There have been plenty of times when I’ve gone to the home of a woman or a senior citizen who’s been glad to see a woman at her door,” she said on an early spring morning outside the home of a Bennett customer in Milton. “Sometimes people feel more comfortable bringing a woman into their house.”

“I’m also one of those people who likes to move,” she added. “And that’s good because in a typical day I could be working a hammer drill in a crawl space, digging a trench for termite work, or dealing with 350 feet of pressurized hose.”

But even then you have to look out for other things. Some houses have the most beautiful decorations, like those glass bulbs that light up at night time.

Today, gender norms are being busted in the field and beyond. According to a survey done by Insight Pest Control, 83 percent of the women surveyed “feel that the industry at large has become more welcoming toward women since they began their career.” From out in the field to back at the office, woman are having a greater impact in the world of pest control. The effect of women in pest control is not lost on those at Bennett Termite and Pest Solutions.

None of this praise surprises Dean Bennett, who, after 15 years of managing pest control companies, founded Bennett Termite and Pest Solutions with his wife Belinda in 2011. In fact, it’s the kind of quality service Belinda and Dean have always expected from the highly-trained and certified technicians who provide pest services to residential, commercial, and hospitality properties. I was born and raised in Port Chester, NY; a small town just outside of NYC. I graduated from Port Chester Senior high School in 1988. Directly after graduation I enlisted in the Armed Forces as a Seabee builder in the United States Navy. In the nine years I served as a Navy Seabee I learned most of the skills I have used for the past 25 plus years in the painting industry. Including drywall repair, finish trim, and light carpentry.
