Public Assistance Programs Medicaid and other Health Programs The State of Wisconsin offers a number of health benefits programs for eligible children, individuals, and families. For eligibility information, call Medicaid Enrollment Services at (800) 291-2002. BadgerCare Plus www.dhs.wisconsin.gov/badgercareplus For pregnant women, children, and families who meet eligibility requirements. This plan has two tiers, depending on income. Family Planning Only Services www.dhs.wisconsin.gov Family planning and reproductive health care coverage for individuals age 15 and older. Elderly, Blind or Disabled Plans (EBD) www.dhs.wisconsin.gov/medicaid/member-info.htm Medicaid for people determined blind or disabled by the Disability Determination Bureau (DDB) or who are at least 65 years old. Medicaid Purchase Plan (MAPP) www.dhs.wisconsin.gov/medicaid/medicaid-purchase-plan.htm If you are disabled and have a Medicaid deductible, you may qualify for MAPP, which provides the standard Medicaid coverage without the deductible. You must work, be at least 18 years old, reside in Wisconsin, have countable assets less than $15,000, have a determined disability, be employed in a paid position or enrolled in a certified health and employment counseling program. Have an adjusted family income of less than 25% of the Federal Poverty Level.
