our success in life requires an enormous threshold of discomfort and inconvenience because of the many sacrifices, disappointments and frustrations you will incur. Every person who has ever been hugely successful has disciplined themselves to do things that other people won’t do. I have a great story about embracing discomfort that happened while I was finishing my dissertation for my PhD and getting ready to defend it. After three years of classes and another two years of research, I was so ready to be done. Little did I know I was about to experience the ultimate in inconvenience and discomfort. In the middle of my defense, one of my committee members, a professor from the Statistics Department discovered a major flaw in one of the statistical designs that I had used to determine my results. He would not sign off on the defense. My options at that point were to go back and rerun the entire statistics for the dissertation and defend again or I could walk away and be ABD (known in academia as “all but dissertation” for those individuals who have yet to defend or never do)! I have to admit I had to seriously think about it. I was so frustrated knowing that it was probably going to take me at least another 6 months to a year to do the stats, which more than likely would change the results and require an entire rewrite of that part of the dissertation. And then have to defend again! After a few weeks of struggling with a decision, I kept asking myself, “do you want this degree or not?” It was as simple as that. If I wanted PhD after my name and the title of doctor, then I had to do whatever it took to complete it. Not to mention the fact that I really didn’t think I could live with myself if I had given up then after all those years, even though it was frustrating, inconvenient and definitely UNCOMFORTABLE! So, I did it – reran the stats, which interestingly enough did not change the results, rewrote the section and defended again, successfully. But the most rewarding thing was that this statistics professor wrote me an email after it was over commending me for having the discipline and perseverance to do what I did. Here is what he said to me: Dr. Diane: I just wanted to comment on your dissertation defense. I have served on 300+ graduate committees at Maryland. You were not the only one that was offered the opportunity to defend their work a second time. It has probably happened 20 to 25 times since I have been at Maryland. I cannot recall ever writing a letter like this one before. Most of the time I go away from these second defenses wondering if I had lowered my expectation, or if the student had really performed enough better to justify having passed the student. Yours was different. I left your exam feeling very good about your defense. You really did a GREAT job and should feel very satisfied with what you accomplished!!! Congratulations and best wishes in your future pursuits. Larry I still have that email as a reminder that in the face of defeat, I didn’t give up. Of course it was inconvenient, of course it was uncomfortable, of course I struggled almost daily with what I had to do, but if you ask me today – it was so worth it. Sometimes it’s hard to see the light at the end of the tunnel, or the reward for your efforts, but know that it’s there – just waiting to see if you really want it, if you are really willing to do whatever it takes. And when you do, believe me, it’s so worth it! In health and happiness,

What Does Every Woman Want?

M

any years ago a childhood friend of mine, due to a gambling addiction, got into a difficult situation involving some very significant debts. One day he shared with me that the organized crime group involved had threatened to break every bone in his body—or worse—if he did not pay up. He said when he told them he had no funds available and asked if he could repay the debt in some other way he was told that if he married the crime lord’s daughter they would let him off. He said the offer was made because “she was an ugly witch you could smell a mile away”. The problem was that, unbeknownst to the underworld characters involved, my friend was already married. He said he felt there was no hope for him, and his addiction would cost him his life. At this point I volunteered to marry the witch and save him. He argued, but I persisted. To make a long story short, the marriage was arranged and a few weeks later the witch and I were wed at a big blast in a well known New York Hotel. I did the best I could to put on a pleasant face for the evening but as it grew late I told my bride I needed to go to our hotel room and get some rest and she agreed to come with me. When we got to the room I went into the bathroom to catch my breath and try to figure out how I was going to manage to just get through the night—let alone the years that lay ahead of us. After spending as much time as I could in hiding I stepped out of the bathroom to find a beautiful woman sitting on the edge of the bed. “Where’s my wife?” “It’s me. Now that you are my husband I can reveal to you that a spell was cast upon me. I am a beautiful woman for twelve hours and a witch for twelve hours. Now that you are my husband my schedule is your choice. I can be beautiful during the day and impress the neighbors or at night for your pleasure. Tell me what you would prefer.” I knew from my mother’s wisdom that there was only one

correct answer and I shared it with her, “Do what will make you happy”. She answered, “You have freed me from the spell. I can be beautiful twenty four hours a day now.” And she has for over fifty years. What does every woman want? Yes, a life of her own! Several years ago my wife and I were speaking in a southern city, and we interacted as we would at home. My wife, Bobbie, is free to correct me and has her time on stage. I could feel the hostility in the audience towards my wife and me. Later when talking to one of the women I knew, she said the hostility had to do with the relationship they saw my wife and me display. They were confusing the place of politeness versus submissiveness. And because they had become submissive in their marriages they had internalized their anger and were expressing it towards me. It is vital for women who want to remain healthy to do what feels right so that it is not an intellectual decision as to whether you have a career or become a housewife. I have a drawing by a woman entitled, “Will the real me please stand up”. She drew a picture of herself split down the middle with half being the professional me holding a black purse and the other half, the mommy me, holding her daughter’s hand. It doesn’t take an art therapist to see who is smiling and who is frowning and which one her heart desires to be. So do not wait for a life threatening illness to give you permission to go home and be with your child. I will also add that if the desire of others imposes the life of a homemaker upon you and you want to be a lawyer or opera singer then again, do what makes you happy. If you have twin sisters—one of whom always strives to please her parents, make everyone happy by doing whatever they want and be a good girl who internalizes anger—while her sister is a little devil who drives everyone nuts and expresses her feelings, guess who is more likely to develop breast cancer, autoimmune


diseases and more. Of course everyone answers the good girl, and studies show you are correct. Relationships help women to live longer than men, and married men longer than single men, with the same cancers but relationships are not meant to make you submissive or lead to an attitude of, “I’ll make this marriage or job work if it kills me”. Think of a relationship as an ordeal or a struggle. My wife does because she knows 1+1=3. That is when two people get together they create a third entity, the marriage or relationship of any type. Relationships are work because they relate to creating a third entity and are not about personal benefits. How to decide what to do relates to your health. If the relationship is affecting your health then I would say to eliminate it and love from a distance. However, if your health is not at risk then ask yourself how could love resolve this dilemma? Love is a weapon I recommend. You can kill with kindness, torment with tenderness and become blind to faults, with love. I learned the power of love years ago after an accident. I fell off our roof when a ladder broke and hit my head on the pavement. I developed amnesia due to the injury and it improved our marriage dramatically. When my memory returned I sought marriage counseling. The therapist handed me a piece of paper and said, “Read this

and do what it says and save yourself a lot of time and money.” What she handed me was Corinthians 1:13. I have been working at it ever since and it has provided me with thirty five wonderful years of married life and thirty five out of fifty two isn’t too bad. So construct your life out of the bricks of love but never forget to use humor as the cement that helps to hold the bricks together. I will also prescribe a dog or cat for you and do not ever let the child in you die. They will create healing relationships and teach you to live in the moment. Do You Really Know What Hormones Do?

A

s Sue thought about her upcoming anniversary celebration, it was with mixed emotions. She was really looking forward to an evening out with her husband alone but was dreading what she knew would come after – SEX! It was hard for her to believe how her once robust libido had taken a sharp nose dive shortly after the birth of her children. Funny how nature works, but no it wasn’t funny at all especially now that her sex drive was virtually nonexistent. Sexual intimacy was something she had once enjoyed immensely in the early years of her marriage but now it had become a chore, a responsibility, an obligation, something she felt she had to do to keep her marriage alive. Could Mother Nature really be that cruel? Could she ever rediscover the passion and even a glimmer of those physical sensations she once felt? Later that day, while at the Apple Store picking out a new IPod phone to give her husband for their anniversary celebration, she ran into a friend she hadn’t seen in quite a while. “Wow, Mary! You look great! What have you been doing with yourself?” asked Sue. Mary informed her that she had been to see a Functional Medicine Doctor in West Hartford, who had put her on a new dietary program, addressed some of her nutritional deficiencies with nutritional supplementation and started her on Bioidentical Hormone Replacement Therapy (BHRT) and it had helped her immensely. “Can you tell me a little more about it?” asked Sue. “Truthfully, I do not even really know what hormones do and why they are so important to us.” Mary began by explaining that hormones are chemical messengers that carry information between cells or groups of cells via the bloodstream. They work to regulate the function and metabolic activity of the target tissue. Our bodies have hundreds of receptors for hormones located on various tissues throughout the body so hormones are not just related to sex and reproduction. They have much more far-reaching effects and are important in the proper functioning of the brain, nerves, muscles (including the heart muscle) as well as bone and connective tissue. The bottom line is all of our bodies’ systems are interrelated and an imbalance in one system can create an imbalance in other systems. When our hormones are in balance we look and feel great, have lots of energy, sleep like a baby, have a strong sex drive, are better able to 10

manage our stress and all of the other systems in our body function well such as the digestive and immune systems. “When we speak of BHRT, “bio” means body and “identical” means the same, so BHRT simply means that you are using hormones that have the same exact biochemical structure to the hormones that are produced in and that circulate naturally in our bodies,” Mary explained. “Because of this biochemical synergy, the body knows exactly what to do with the hormone when it is presented with it. Any biochemistry student will tell you that if you begin with the same chemical substrate in a given chemical reaction you will always end up with the same end product. When synthetic hormones, which do not have the identical biochemical structure to our bodies, are added to that same chemical reaction the end products cannot possibly be the same. This is one reason why we see side effects with the synthetic hormones.” “So, where do these Bioidentical hormones come from?” asked Sue. Mary informed her that they are derived from plants, yams and soybeans to be exact, but that they are synthesized in labs. “So how can they be natural if they are synthesized in labs?” asked Sue. “Do you consider the supplements you take to be natural?” asked Mary. “Well, I guess so,” said Sue. “I hadn’t really thought about it. I guess they are synthesized in labs as well. Is that what you’re getting at?” “Precisely,” said Mary. “I still feel pretty confused,” said Sue. “Everything I have heard says that hormones are dangerous and we should stay away from them. Aren’t you afraid that you could be harming your body in some way? “Let me explain a few things to you Sue,” said Mary. “The way my functional medicine physician explained it made so much sense. These synthetic hormones are just that, synthetic, which means that they are artificial, fake, copied, unreal and imitation. Do you really want to put something synthetic in your mouth? This leads me to another point the doctor made. These synthetic hormones are given orally and last I heard our hormones don’t normally go into our GI tracts. They are secreted from the sex glands or adrenal glands and circulate in the blood until they


reach their target tissue where they then exert their effects. Also, because they are given orally a much larger dose of the hormone has to be given. The reason being that a sizeable proportion of that hormone is delivered to the liver where it is broken down leaving only a small percentage left to get into circulation. So, there you have it, the 3 major reasons why synthetic hormones cause all sorts of problems in the body. They are not the same biochemical structure as what is in our bodies, they are given orally where our hormones never go and lastly they are given in large doses because of the first pass effect through the liver. Mary continued to explain to Sue that these bioidentical hormones are formulated to be given transdermally, via the skin, to mimic, as best we can, what the body does naturally. Our hormones are naturally released from glands and then circulate in blood to the target tissues where they exert their effects. The bioidentical hormones are alternately applied to the non-fatty tissue of the wrist, inside the elbow and behind the knee so that they are absorbed directly into circulation where they travel to the target tissues. Hormones are lipophilic (fat loving) substances so if we were to apply them to fatty tissue they would go directly into the fat where it is stored and then released into circulation much more haphazardly. As Mary implied above, the other advantage to giving the hormones via the skin is that we are often able to give substantially lower doses than those given orally when we bypass the liver. “All I can tell you Sue is that since I started taking the bioidentical hormones I have noticed amazing results. My energy has improved along with my moods which used to be quite up and down and for my kids and husband to have noticed is definitely saying something. I am much better able to handle my stress much like when I was in my twenties when I could deal with a lot of things coming at me at once. I just don’t get as overwhelmed by life anymore. I feel I am able to stay on task and stay more focused when doing things and my memory for things has absolutely improved. I don’t feel like I am chasing my tail looking for things and trying to remember things. It is just incredible. toxins that are present in our world today didn’t even exist thirty years ago. As we continue to produce more man-made synthetic chemicals to “improve” our way of living we are actually damaging our bodies and our planet in the process. Many of the personal care products we use every day such as cosmetics, sunscreens, lotions, soaps, shampoos and perfumes have not been thoroughly tested to determine their overall safety with regard to our health and the health of our environment. Not to mention the food additives, preservatives and dyes, lawn chemicals, household cleaners, pharmaceuticals, over the counter medications, sealants, adhesives, paints, solvents, plastics and pesticides. Frankly, the list is endless and when you think about it pretty scary. Another frightening point to consider is that the combination of one or more of these chemicals can have a cumulative effect in the body greatly increasing the toxicity many times above the toxicity levels seen when exposed to these chemicals individually. Like the hormones that they mimic, environmental toxins are lipophilic (fat loving) substances, which mean that they have a propensity to accumulate in fatty tissue. This fact along with the sheer number of chemicals we are exposed to each day makes it almost impossible for the body to effectively detoxify these chemicals. Lose Cellulite and Inches In our experience, clients who have had 16 treatments, while watching their diet and drinking the appropriate amount of water, have lost a full pant size. One study reported that 36 women, treated twice a week over an eight week period, showed a loss of inches in 100% of the participants and visible reduction in the appearance of cellulite in 91% of the participants. In some individuals, almost three inches were lost after treatment.

Relaxing and Detoxifying So, what does a treatment feel like? Imagine a deep, invigorating massage. Clients find it quite relaxing and some have reported that they’ve nearly fallen asleep. Are You Having Trouble Conceiving? By Myriah Hinchey, ND

W

hen it comes to conception, many women find it surprising and often frustrating that the thing they spent most of their lives trying to prevent is not as easy and automatic as they thought. Although it is probably the most natural process in the human body, conceiving a child is a complicated event that requires multiple systems in the body to be functioning synergistically. Surprisingly, it is normal for conception to take several months to occur. Often, the emotional stress and disappointment felt when it doesn’t happen right away and the resultant physical hormonal ramifications of the stress response feed into a vicious cycle that further complicates the process. It is important to note that it is not always the woman who has an issue. In today’s world of toxins and unhealthy food, many men have metabolic disorders that can lead to unhealthy sperm or low sperm counts. There have been studies that show a steady annual decrease in sperm count, which is thought to be linked to common toxins in our environment, and to multiple nutrient deficiencies. Newly emerging data shows that men have an issue with infertility 40% of the time and often it is both parties whose underlying imbalances and deficiencies lead to trouble conceiving.

Create Healthy Environments Inside and Out For both men and women, creating a healthy internal and external environmental is the most important step to take before trying to conceive. This includes surrounding yourselves with a strong support system of family and friends, eliminating toxins from your home, detoxifying your body of harmful toxins, identifying any type of genetic disorder that can hinder the ability to conceive or carry a pregnancy to term (i.e., MTHFR SNP), and providing proper nourishment to your body so that your cells function at an optimal level, therefore leading to optimal functioning of all of the body’s systems, including your reproductive system. Most people know the common things they should avoid after conception (stress, caffeine, alcohol, nicotine, sugar, etc.) and things they should do to have a healthy pregnancy (take prenatal vitamins, exercise, limit stress, limit sugary processed foods, eat healthy, etc.) but few people understand how these things impact the ability to actually conceive. More so, many times it is the lack of particular nutrients that causes imbalance in hormones which results in an inability to conceive.

Nourishment – Beyond the Basics Proper nourishment goes beyond the basics of providing adequate levels of folate to prevent neural tube defects. Proper nourishment includes getting the right amount of phytonutrients (found in brightly pigmented foods) that act medicinally to our cells, some of which actually balance hormones. Additionally, getting the right amount of essential fatty acids is important for brain and nervous-system development of the fetus, but even before conception, it is crucial for the production of hormones in both mother and father. Essential amino acids (found in certain proteins) are equally important for detoxification, development of neurotransmitters, and production of some hormones. Multiple vitamins and minerals are needed to support all of the aforementioned processes and all biochemical process in the body, including conception.

Is the Thyroid to Blame? Often to blame is the underlying problem of a low-functioning thyroid. If you are not getting the proper amount of iron, selenium, iodine, and zinc in your diet, your body cannot make adequate thyroid hormone. Additionally, cortisol (the stress hormone) inhibits the conversion of T4 to T3, which decreases the thyroid’s ability to function properly, which can cause an imbalance in your sex hormones, and affect ovulation or sperm production. There are numerous other health issues that can hinder one’s ability to get pregnant, including endometriosis, irregular ovulation, immune issues, and scarring of fallopian tubes, to name a few.

Timing When trying to conceive, it is important to understand how the reproductive process works, starting with your menstrual cycle and ovulation. If you recognize the signs of ovulation, you can better time intercourse to increase the likelihood of fertilization of the egg and its subsequent implantation. A woman gets her period 14 days after ovulation. The beginning part of the cycle, known as the follicular phase, is variable in length. The later part, which is fixed in length, is called the luteal phase: If your cycles are the typical 28 days long, you ovulate on day 14; if your cycles are 30 days, you ovulate on day 16; and if your cycles are 23 days, you ovulate on day 9. This is important to understand because if someone with a longer or shorter cycle assumes they ovulate on day 14, because it is the average day for someone with a normal (28 day) cycle, they can often miss it. There are many ways to determine ovulation and the window of fertility such as monitoring your basal body temperature, checking both salivary and cervical mucus, and calculating your day of


ovulation based on cycle length. If your cycle is constantly fluctuating in length it can make all methods of tracking very tricky. There are certain herbs you can use to help with the production and synchronization of FSH and LH which can help to normalize periods to 28 days consistently. Additionally, the cervical mucus changes to a thinner egg white consistency, called spinnbarkeit, a few days before ovulation. This provides nourishment and decreased viscosity for the sperm, enhancing its ability to reach the egg and achieve fertilization. The appearance of spinnbarkeit is a simple and easy marker that signifies the fertile window. Even if the cycles are irregular, you can usually tell ovulation is about to occur by this physical phenomenon. If you are having a hard time conceiving or if you are just starting to think about having a baby, providing the body with the nutrients it needs to function properly and eliminating exposure to toxins is crucial. It will improve the outcome of not only the ability to conceive, but also that of your pregnancy and the health of you and your baby. Understanding what is happening in your body and doing your best to provide the nourishment that you both need is the first and most important step. Stressed and Fatigued?

S

tress and fatigue are, by far, the most common health complaints of women over 50. There may be a myriad of other issues on the complaint list, however, stress and fatigue are almost always somewhere in the mix. Stress has been a daily buzz word in our lives ever since Dr. Hans Selye’s published his “General Adaptation Syndrome” and coined the term “stress.” Stress has become something we think we all should be free of, something to avoid at all costs, the boogey man of modern life, lurking in the shadows at home, on the job, in the car, and affecting everything from relationships to career success, finances, and health. Indeed, it affects all areas of our lives. “I was a little excited but mostly blorft. “Blorft” is an adjective I just made up that means ‘Completely overwhelmed but proceeding as if everything is fine and reacting to the stress with the torpor of a possum.’ I have been blorft every day for the past seven years.” ~ Tina Fey, Bossypants The healthy human stress response has always been necessary for survival. Thankfully, our species has always had the ability to respond appropriately to a perceived danger of a life or death situation. Going back to the caveman days, if you were being chased by a buck-toothed giant rodent that had visions of you as its main meal, your primary means of survival may have been your body’s ability to initiate the fight-or-flight stress response. Most of today’s stresses come from the physical and emotional traumas of everyday life. Instead of being chased by a wild animal, stress can come from our jobs, businesses, finances, traffic, spouses, parents, and kids. Stress has many negative health consequences, and for women they include chronic fatigue, hypothyroidism, depression, lowered sex drive, sleep loss, weight gain, diabetes, early menopause, panic attacks, heart palpitations, cardiovascular events, and digestive issues. “Pain is a relatively objective, physical phenomenon; suffering is our psychological resistance to what happens. Events may create physical pain, but they do not in themselves create suffering. Resistance creates suffering. Stress happens when your mind resists what is... The only problem in your life is your mind’s resistance to life as it unfolds. ” ~ Dan Millman The brain has different types of receptors for the various female hormones making women much more exposed to the many dangers of the stress response. Women are much more likely to develop auto-immune conditions as they age, also due to their unique biology. The stress response involves the adrenal glands, along with the hypothalamus and pituitary glands, collectively known as the 18

HPA axis. When a stressor is perceived, whether real or imagined, the hypothalamus releases a hormone, (corticotrophin), which tells the pituitary to release another hormone (adrenocorticotrophic hormone ) which then signals the adrenal gland to produce cortisol (and DHEA). The cortisol then sends a message back to the pituitary and hypothalamus to inform the HPA axis that the cortisol has been made and there is no need to keep making more of it. That is the simplified version. However, when one is continuously and chronically stressed, that natural feedback system can be altered and more cortisol keeps being produced. Unchecked, the cortisol load can then lead to the many health consequences listed above. Living in the chronic state of exhaustion, along with other stress related symptoms, is not what women had in mind for life after 50. This is the time meant for enjoying the fruits of a lifetime of work and accomplishments, time for travel, family, grandchildren, culture, new interests, and personal growth. “Each moment of worry, anxiety or stress represents lack of faith in miracles, for they never cease.” ~ T.F. Fire and Ice at Saybrook Point Inn

O

n an icy cold day in January, I was lucky enough to be seated at the Fire Bar at the Saybrook Point Inn for their 25th anniversary “Fire and Ice” celebration. On the waterfront patio, they built an actual ice bar with a giant martini ice sculpture for complementary vodka tastings. There were two other ice bars with sushi demonstrations and chilled seafood. And I got to experience the restaurant’s chefs and staff competing in an oyster shucking contest! When it got too cold for even the Fire Bar to warm me up, I took shelter inside the restaurant, Fresh Salt. The beautiful new dining room and bar has near perfect water views from every seat in the house. But let me get started about the food! The amazing Chef Leslie Tripp’s menu is centered around seafood, but also offers a wide variety of updated classics as well as some new creations and a couple of serious steaks. He is dedicated to sourcing as many local products as possible, using regional producers, and gets his fish and shellfish directly off the day boats that work out of Narragansett. Boat captains call him directly before they even dock to tell him what they’ve caught, and he plans his weekly seafood offerings around what looks the best. Selections from the raw bar include only native oysters, clams and squid. The calamari is sourced from Point Judith and is prepared a number of ways, and the Nantucket Bay Scallops Carbonara is mouth- watering on top of tagliatelle, guanciale, truffled grana padana and topped with an organic poached egg. For you lobster lovers, you can order lobster rolls by the Connecticut version (hot buttered lobster) or the Maine version (chilled lobster salad). The bar area is cozy and warm and offers small plates including burgers, fried calamari, steamed PEI mussels and some of the best clam chowder I’ve had. Not into seafood, not to worry…they have a

What’s a Doula? Your Professional Labor Coach!

Women who had support from a Doula were: • 28% less likely to have a cesarean section • 31% less likely to use synthetic oxytocin to speed up labor • 9% less likely to use any pain medication • 34% less likely to rate their childbirth experience negatively. According to another study in 2002, women were also more likely to report having a happy view of their birth later, when they had a Doula present as well. The most important factors in a woman’s satisfaction with labor and birth were: “

W

hat’s a Doula?” It’s one of the most common questions I get asked as I talk with someone about what I do. Whether it’s chatting with the girl behind me on line at the grocery store, or my doctor or an interested parent who sees me at a Baby Fair or Parenting Expo. So, what IS a Doula? A Doula is a non-medical, women’s health professional that specializes in the support of women through pregnancy, labor and postpartum. Some Doulas will also offer support for women though abortion, miscarriage, obstetrical procedures and/or as a support for a dying person as well. Mainly, people tend to see our role as a professional Labor Coach, assisting the woman’s partner in supporting her through her labor journey, or in the case of single women, offering total support during labor. While that’s true, and it is a very big part of the job, Doulas are really educators, observers, and advocates for patients during their time in the childbearing year. We help patients to understand all of their options, the pros and cons of each option and then honor and respect their choices. We educate parents on the normal things that go along with pregnancy, labor and postpartum. Knowing why something is happening, or understanding what to expect from the process reduces fear and anxiety, two things that severely hamper the labor process. We know from many studies in the last several years, that hiring a Doula to help care for you prenatally and during birth leads to lower rates of cesarean delivery, epidural use, and the use of instrumental delivery (vacuum, forceps etc.). So, why not just ask mom, or my best friend to come with me to help out in labor? Well, while mom or best friend might be of some help, they are also not trained in all of the various techniques for helping women through labor both physically and psychologically. The Cochrane review discovered that outcomes were best for women who had support from someone who was neither a friend nor family member, nor who worked for the hospital, basically, from a non-biased professional advocate.

• Having good support from caregivers • Having a high-quality relationship with caregivers • Being involved in decision-making about care • Having better-than-expected experiences, or having high expectations. Having a Doula helps to facilitate all of these factors. A Doula is a caregiver that you select. Sometimes we don’t have the option of who our Doctor is, or choosing our birth place either due to insurance regulations, or physical limitations/availability of other caregivers in an area. But your Doula works for you, and only you. We don’t work for insurance companies, or hospitals, or your doctor. Working for you means that we are going to advocate for what is best for you, your family and your baby, not what’s best for us, or the hospital. Here is what the husband of a former client of ours said about the care that he and his wife received with us: “From my point of view as a husband, having Jessica alongside during the birth was a game-changer. We were anxious to get to the hospital after Caitlin had been in labor for several hours, but because of Jessica’s expertise, she encouraged Caitlin to continue to labor at home. I’m glad we did because it helped Caitlin remain calm and focused. Our doctor said she would welcome Jessica at every one of her deliveries. We were blessed to have Jessica with us. She prayed with us, supported us, and loved us. Also, Dana went out of her way to visit us at home to support Caitlin as she attempted to breast-feed. Dana & Jessica went above and beyond. Simply, all we can say is thank you!” ~ Hakem The continuous support that Hakem and his wife Caitlin received allowed them to remain calm and focused during their labor, and when they needed help at home a few days later, late at night, they were able to have me come out and help with breastfeeding and some anxiety that they were both having. When parents don’t have to struggle to find answers, or live with unanswered questions, fears or doubts, they feel better about their situation and their ability to parent. They feel empowered.


Having said all that, many people have the same questions for myself, and my partner Jessica Brown about what we do, what we charge, and how to find a good Doula. You now know what we do, so the next question is often, “Well, how much does it cost?” And that is a good question. Doula care costs between $450.00 and $1200.00, and covers prenatal care, birth and postpartum as well. Doula care is not always covered by insurance, although you can occasionally get reimbursed for your care. Most Doulas include in their care package, one to two prenatal visits, on-call birth care from 38 weeks, and one postpartum visit in this fee, along with unlimited e-mail and phone support. Jessica and I do practice a bit differently (and several other Doulas we know do as well), because in our experience, women and families needed a bit more than that to develop a really close relationship with us, and a deep comfort level and sense of trust. We also felt we needed more time with clients to educate them and answer questions. We do monthly prenatal visits from the time you are a client, or at least four visits, even if that means seeing you weekly if you hire us at 36 weeks. If you are high risk, we will see you twice a month. The visits are between 1-2 hours and are relaxed and informal. We

are also on call for you from the time you hire us, so whether you go into labor early at 30 weeks, or suffer a miscarriage or need a scheduled C-section, we will be there for all of it. Postpartum we usually do one visit the day after you come home from the hospital, or two days after the birth if you gave birth at home. If you had a C-section, we’d visit you twice to be sure you weren’t suffering any complications or depression. The comprehensive support you receive from a Doula also lessens your likelihood for depression postpartum, simply due to the reassurance provided by a caring knowledgeable person, and observance for early signs of Postpartum Depression by someone who knows just what to look for. It seems like a lot of money to pay out of pocket, but the savings can be enormous in other ways. If you didn’t have an epidural, for instance, you would save nearly $500 on the co-pay on most insurance plans that nearly pays for the whole Doula service! Americans spend an average of $10005,000 on baby items and getting ready for the birth of their child, most of which isn’t going to help assure the safety and happiness of mother and baby the way that a Doula would. It’s an investment, but a good one! The other big question tends to be “How do I find a good Doula?” Firstly, ask

people you know who may have had a Doula with their births, did they like her, and was she what they needed? If you don’t have that, sites like www.doulamatch.net or www.doulas.com are great ways to find Doulas in your area, and then ask for interviews with likely candidates. At the interviews, ask your prospective Doula these key questions: 1. “Where did you train?” A Doula should have been through a certification program or quality apprenticeship program before practicing.

2. “What are your policies?” Think to yourself; am I comfortable with her method of practice? 3. “How many births have you attended?” Women in a Certification process may have only been to a few births, while Doulas practicing for years may have been to hundreds. 4. “Do you have references?” A good Doula should give you several references, both from clients, and personal ones as well, if you request them. 5. “Do you have experience with…?” After the “with” insert any special concern you may have. Does she have experience with Multiples, with VBAC? Cultural or religious things, such as the quiet needed at a Muslim birth, or the need to respect a lack of all intervention for a Christian Scientist.

hree years ago, I was in NYC waiting for my date, Andrew, to arrive from New Jersey for dinner. That afternoon my phone rang; Andrew had been diagnosed with cancer and needed surgery right away. What Is Colloidal Silver? Living tissues and organs are masses of billions of cells whose energy comes from minerals (e.g., calcium, magnesium) and trace minerals (e.g., silver, iron, copper, zinc, selenium, manganese). Natural colloidal chemistry in the body’s most important fluids (e.g., blood) breaks down minerals into particles small enough for cells to use them, and scientists have learned to produce colloids in the laboratory. Colloidal silver is a suspension of a high content of ultra-fine, positively charged silver ions in water that can be applied topically or ingested orally/ sublingually, or through the airways from a nebulizer. Since colloidal silver attacks only single-celled organisms, it is safe for all multicelled living matter (see Safety discussion below). Dr. Harry Margraf, a biochemist and pioneering silver researcher who worked in the 1970s with the late Carl Moyer, M.D., chairman of Washington University’s Department of Surgery, in fact stated that “silver is the best all-around germ fighter we have.” More recently, the researchers of one study proved that silver can enhance the action of existing antibiotics against certain bacteria, “thus strengthening the antibiotic arsenal for fighting bacterial infections.” (Science Translational Medicine, June 2013).

History & the FDA Silver has been used since ancient Greek and Roman times medicinally, as an antimicrobial, and for preserving foods and liquids. Physicians commonly used colloidal silver as a mainstream antibiotic until 1938. Unfortunately, due to high production costs, allopathic medicine began favoring fast-working, financially lucrative, synthetic pharmaceuticals. Although the FDA ruled in 1999 that manufacturers of OTC colloidal silver products may not promote their healing properties, and may only refer to them as “mineral supplements,” some of the manufacturing methods used before 1938 are still used today.

Uses & Dosage

Reported uses of colloidal silver include: (1) eye infections (e.g., pink eye; conjunctivitis); (2) ear, nasal, and gum infections; (3) mouth ulcers; (4) colds and flu; (5) sore/strep throat; (6) cuts, scrapes, rashes, and mild burns; (7) yeast and urinary tract infections; and (8) respiratory infections. Each use calls for a different dose. In 1940, the researcher RA Kehoe reported that the average daily intake of fruits

and vegetables provided 50-100 mcg of silver as a trace element. Since that time, the commercial farm soils of this country have become extremely deficient in trace minerals. Since there are very few plant sources of trace silver available today, colloidal silver is probably the best substitute, even though electro-colloidal silver does not necessarily have the identical metabolic effect. One teaspoon of 10 ppm colloidal silver, taken daily, provides 50 mcg of silver and could be considered a ‘nutritional’ amount appropriate for extended periods. Any amount above 100 mcg/day, or two teaspoons, can be considered a ‘therapeutic’ amount. One company, however, maintains that even according to the EPA daily Oral Silver Reference Dose, seven teaspoons/day of its 10ppm colloidal silver can be used safely for short-term immune support.

Evaluating Product Quality Highest quality colloidal silver has these characteristics: (1) a high percentage of positively charged, silver particles [but higher parts per million (ppm) alone does not make a product better]; (2) no contamination by salts, proteins, silver compounds, stabilizers, or oxidation; (3) the smallest particle size to maximize surface area and efficiency [measured in micrometers (microns) or nanometers (nm), with the ideal particle size being below 0.1 microns (100 nm)]; (4) confirmation of particle size by transmission electron microscopy (TEM); and (5) suspension in pharmaceutical-grade purified water.

Safety

As previously noted, high-quality colloidal silver is perfectly safe. The only potential side effect of concern is reduction of probiotic bacteria in the gut, which can be countered through daily, separate ingestion of quality probiotics. To minimize this effect, also try swishing the silver in your mouth


for 30 seconds before swallowing to promote non-intestinal absorption, or take smaller doses multiple times per day or with meals. As for toxicity, silver can act as a heavy metal poison or as a trace mineral nutrient. The difference is in the particle size, NOT the concentration. The smaller the particle, the less likely it will behave as a toxin. Also, colloidal silver contaminants degrade and diminish the bio-activity of colloidal silver and render it potentially toxic. The worst toxic reaction is a condition called argyria, usually resulting from the massive intake of silver salts, such as silver nitrate, silver sulfate, and silver chloride. This is primarily a benign cosmetic condition characterized by a permanent, bluish discoloration of the skin (American Family Physician, Dec 1987). Argyria is undesirable but causes no physical discomfort and does not have any other known side effects. Whether serious, long-term risk of kidney damage can arise from high-quality, uncontaminated colloidal silver is currently being debated. It also is argued that this natural remedy interferes with the absorption of certain prescription medications, including certain antibiotics (Quinolone, Tetracycline), Levothyroxine, and Penicillamine.

uintessential New England…that was my first thought as I drove through the picturesque, winding, maple and oak tree-lined roads into Kennebunkport. Is Stress Making You Fat?

T

he answer is YES. Stress can be exactly what is causing you to keep putting on weight even though you have been making healthy dietary choices and exercising regularly. Stress is anything that impacts your body whether real or perceived. It can be emotional in the case of dealing with family, financial and work issues, chemical from the foods, medications, alcohol or hormonal imbalances, or physical from type of job, illnesses, and surgeries to environmental allergies and exposure to pollutants. People experience and perceive stress differently; some people eat when stressed while others can’t eat at all. Either way, weight gain can happen. When your body is under stress its primitive response is to conserve energy and store fat.

Your adrenal glands are small glands that sit on top of the kidneys that control stress and fluid balance. They release the stress hormones adrenaline and cortisol into your bloodstream and cause a chain reaction to occur. This response prepares your body for increased energy needs which elevate your insulin, sugar, and fat. The adrenal gland helps maintain blood glucose levels throughout the day, which is your fuel. Cortisol is your natural steroid and is responsible for helping the body cope with these stressors, it is the cease fire on the front lines and puts out your fires all day long. This stress response, “fight or flight”, was the way we would protect ourselves from danger. For most of us, emotional stress can trigger this same response (which occurs way too often) causing insulin resistance or metabolic syndrome (high blood sugar, high blood pressure, and high cholesterol). If you are not eating often enough or eating enough calories, your body goes into starvation mode which is a stressor. Our natural tendency in this mode is to store food for fuel. This affects your metabolism from burning calories at a regular rate throughout the day because if your body does not know when it will get more fuel in, it will turn down the rate of burning that fuel. Also if you are not eating at regular intervals throughout the day, your body goes into this starvation mode. Your pancreas releases insulin, signaling the body that it is hungry and so when we don’t eat, the circulating glucose in our bloodstream gets used up. This causes your blood sugar levels to drop causing hypoglycemia where you feel like you better eat something quick before you pass out. If you are eating enough calories for your body to make enough energy, the adrenal gland then needs to tell the body to make its own fuel called glycogen. Glyco-

gen is made from our body breaking down muscle mass, not our fat. With too much circulating insulin and low blood glucose levels (hypoglycemia), we naturally need to eat something, like a simple carbohydrate. So having a small glass of juice or a piece of fruit is a healthy choice that would help with our immediate symptoms, followed by having something else more sustaining like protein and vegetables. But most of us reach for more sugary treats and when doing so tend overeat. This carbohydrate overload then causes our body to release more insulin which then leads to a slowing of metabolism and feeling fatigued, like your energy is crashing. This then triggers you to feel hungry again and the cycle continues. Meanwhile, every time you let your sugar levels go too low or too high, the food we eat goes right to storage. We then notice weight gain around the middle and muscle wasting in our extremities. Being overweight is a huge stressor to your system causing chronic elevation of stress hormones and can lead to insulin resistance, diabetes, heart disease, depression, memory loss, bone loss, joint deterioration, cancer, and immune diseases. You cannot lose weight if you do not eat. When we limit our calorie intake our body does not have enough fuel which slows our metabolism and affects our ability to deal with our life stressors. Exercise is a great natural stress reliever, but if we exercise excessively and don’t consume enough calories, our body will feel stressed leading to muscle fatigue and low stamina. Working out at the gym with weights and walking on the treadmill is great for cardiovascular health and muscle conditioning, which may make you fit, but still fat. This is why you can train for a marathon and not lose a pound (just ask my husband). Stress happens; it is a part of everyday life. But it is how an individual copes with a particular stress that makes the difference in how it impacts our bodies. Everyone needs to find a sense of balance by reducing their emotional stressors, exercise by just moving their bodies, eat a healthy balanced diet, sleep well, and supplement with vitamins to support our metabolism. All this when balanced equals your wellness. Ways to reduce your stress, keep energy up, metabolism going, and promote weight loss: 1. Deep breath, 3 sets of cleansing breaths in through nose and out your mouth. 2. REST lying down 20-40 min, no external stimulation, a time to digital detox. 3. Eat every 3-4 hours, 5-6 x a day – keeps metabolism going 4. Eat within 1-2 hours of waking –you


have not eaten in a long time and your body needs fuel to start your day. 5. Do not eat within 2-3 hours of going to sleep –anything that has not been fully digested will go right to storage as fat. 6. 100 (good) calories = 1 hour of energy, 100 calorie cookie pack will dump quickly, but 100 calorie almond pack will keep you satiated and provide energy. 7. All snacks and meals should contain a protein and plant based fiber source 8. Stay hydrated –drink plenty of water, you should drink about 50% of your body weight in ounces. 9. Keep a food and activity log –when you are accountable for what you are eating, you will make better food choices. 10. Regular activity –walking, muscle conditioning, 20-40 min, 4-5 x a week. 11. Eat REAL foods –check food labels, if you can’t read it, don’t eat it. 12. Avoid white carbs and refined sugars –Sugar is just as addictive as any illicit drugs. The more you eat it, the more you crave it. 13. Healthy fats in moderation –olive oil, coconut oil, avocado, tree nuts, butter, grape seed oil, and sunflower oil. 14. Beyond Calcium Supplementation

steoporosis is a serious disease that affects the strength and density of the bones. Osteoporosis is one of the most predominant bone-density diseases in Western society.

Who is at risk? Rates of low bone density are most commonly found in postmenopausal women, specifically Asian-American women. However, osteoporosis is not strictly a women’s disorder. Men over 70 also have a high risk of decreased bone density. Other risk factors for osteoporosis include early menopause, long-term use of certain medications, alcohol abuse, smoking history, sedentary lifestyle, and low body weight.

Diagnosis Osteoporosis is diagnosed through a bone density scan, called a DEXA scan. This is a fancy term for a type of x-ray that can assess the density of specific bones to gauge fracture risk. The scan will deem bone density normal, osteopenic or osteoporotic. Normal bone density is considered the appropriate bone density. Osteopenia is the term for bone that is losing some density, however, is not significant enough to be classified as osteoporosis. To reach osteoporosis means you have a significant amount of bone loss. The spine and hips are the typical locations for osteopenia and osteoporosis to be found.

Skeleton Physiology 101 The skeleton is continually breaking down and rebuilding. This balance is critical for strong bones. It is said the entire skeleton is regenerated every 7-10 years! This makes it important to provide the essential nutrients needed to make bones strong and less likely to fracture or break. The breakdown and building up of bone is contingent on parathyroid hormone, the hormone that is secreted from the parathyroid glands. There are four parathyroid glands located on the back of the thyroid that function to balance the amount of calcium circulation in the blood. If there is too little, bone is broken down, if there is too much, bone is built up. Ensuring that the parathyroid glands are managing calcium balance is essential in preventing and treating osteoporosis.

Cortical and Trabecular Bone The skeleton is made of cortical bone and trabecular bone. Trabecular bone is located inside the bone where the bone marrow is stored. The protective hard outside of the bone is the cortical bone. The bones that make the spinal column are 90% trabecular and 10% cortical, making them an easy target for compression fracture, when the spine begins to compress onto itself. The hip is made up of about 50% trabecular and cortical. The bones of the arms and legs are about 90% cortical and 10% trabecular. Fracture is much less likely, comparatively, to occur in a limb.

Hormonal Influences: Estrogen encourages bone to mineralize, or strengthen. In postmenopausal women, estrogen levels decrease, which affects bone density as the breakdown of bone is less inhibited. This does not mean estrogen replacement therapy is a requirement to keep bones healthy. Other hormones that lead to estrogen formation include testosterone and DHEA, both of which help women maintain muscle mass and bone density.

Heredity: Genetic influences as to when bone mass will be at peak mineralization are a large predicting factor in the development of osteoporosis. Any woman whose mother experienced a fracture due to osteoporosis has a 30% greater risk of developing osteoporosis. Other Predicting Factors: Smoking and alcohol use have been linked to a decrease in bone density. Studies show that women who smoke or consume more than seven ounces of alcoholic beverage a week reduce their bone density and increase the risk of fractures. Weight Bearing Exercise: Sedentary behavior increases the risk of osteoporosis exponentially. Bones need weight bearing movements to maintain their strength. Consider walking, stair climbing, aerobics or weight training to maintain bone strength. Consult your physician as to what type of exercise would be appropriate for you. Nutrition: Nutrients, including calcium, magnesium, vitamin D, zinc, boron and vitamin K2, work synergistically to build bone strength. Each nutrient plays an essential role in the structure and strength of the skeletal system, therefore making the practice of only supplementing with calcium for bone health a thing of the past. As for supplementation amounts, it is recommended you have an individualized plan based on your risk. Consult your physician as to the amount of nutrients needed to keep your bones healthy. A healthy diet during pregnancy is very similar to a generally healthy diet. The best way to reduce stress in our body is to reduce inflammation. Through diet, we can be very effective in reducing inflammation. The typical U.S. diet is very inflammatory, which means that when we eat our typical foods, we increase swelling inside our body. Inflammation is an underlying factor to every disease and illness today. By reducing inflammation in our body through a healthy diet, we are able to prevent illness and promote healing from disease. Maintaining a healthy diet during pregnancy supports optimal development of the baby as well as supporting the mother through pregnancy. So often, women feel depleted during pregnancy. This depletion can present as morning sickness, fatigue, and pain. The amount of food a pregnant woman eats is not the only important factor; the quality of food is vital as well. A healthy diet during pregnancy can support a strong healthy mother and a strong healthy baby.

Anti-Inflammatory Diet What is an anti-inflammatory diet? Here are the basics: • Gluten-free • Avoid refined sugars and processed foods • Eat a lot of vegetables and some fruit • Limit meat to natural meats that are free of growth hormones and antibiotics

A gluten-free diet is becoming more and more common today. Gluten is a protein that is found in certain grains, including wheat, rye, barley, and some oats. When ingested, gluten can contribute to many body issues such as pain, digestive problems, bloating, learning and behavioral challenges, and much more. Add Healthy Protein As we all know, protein is very important for growth. There are some women that feel better on a vegetarian diet, but it takes a lot of preparation to eat healthy on a


vegetarian regimen. For a pregnant woman, eating animal protein is often the better choice. While eating meat can be a great source of protein, it is important to limit meat intake to natural sources such as grass-fed beef, natural and organic poultry, and wild seafood that is low in mercury. Today, natural meats are more available in all grocery chains. In addition to a healthy diet, there are other strategies for reducing stress in the body and improving body health.

Integrative Manual Therapy for a Healthy Pregnancy Most pregnant women experience considerable back and foot pain. In addition, they often have urinary and bowel issues such as bladder incontinence and constipation. Integrative Manual Therapy can be helpful in reducing these symptoms. Integrative Manual Therapy (IMT) was developed by Dr. Sharon Giammatteo, PhD, IMT,C, over the last thirty-five years. IMT is a gentle, hands-on therapy that focuses on alleviating tension patterns in the body that affect the spine and soft tissues of the body. IMT specializes in identifying and treating underlying contributors to pain and dysfunction. IMT incorporates the whole body. With a pregnant woman, assessment of the spine and possible underlying contributors to spinal dysfunction is integral to this comprehensive approach. The IMT therapist looks at the ability of the joints to move, for example, the lumbar spine and the sacroiliac joints. The therapist also assesses the alignment of the joints, such as the individual spinal vertebrae and the pelvis. Gentle techniques are used to improve mobility and alignment of these joints. IMT Therapists assess the tension of the fascial tissues in the lumbar and pelvic region to determine areas of compression and possible ‘protection.’ Assessment goes beyond the joints and includes soft tissues such as the pelvic organs, nerves, arteries and veins, lymphatic drainage vessels, and more. Pregnant women often suffer from poor alignment of the spine and pelvis. Treating the alignment of the spine with IMT can result in significant reductions in pain as well as increased mobility and ease of movement with position changes, walking, and more.

Healthy Elimination It is common to see a pregnant woman running to the bathroom. Urinating more frequently during pregnancy is appropriate to some degree because of the increased pressures on the bladder as the uterus grows. But for many women, there is inflammation and

scar tissue in the pelvis from prior dysfunction that can contribute to increased pressures in the area. IMT can be helpful to reduce this pelvic inflammation and scar tissue and support healthy bladder function. Constipation and bowel urgency can become a real challenge for some pregnant women. These issues can be caused by a variety of reasons, including dehydration and poor diet. Changing one’s diet is always beneficial and can create a healthier pregnancy and a healthier baby. But often, there is structural dysfunction at the bowel region which can be improved with manual therapy. This bowel dysfunction can contribute to left hip pain and stiffness as well. Performing Integrative Manual Therapy for the bowel, focusing on improving circulation and drainage in the region as well as promoting mobility of the hip and pelvis, can create optimal changes in bowel and hip function.

The Optimal Pregnancy and Delivery Ultimately, having an optimal pregnancy and delivering a vital, healthy baby can be promoted by eating an anti-inflammatory diet and maintaining a structurally healthy body. Using Integrative Manual Therapy while maintaining a healthy diet can help to reduce stress in the body which will lead to the healthiest pregnancy. As a pregnant woman, it can be very helpful to introduce stress-relieving practices into the day such as yoga and meditation. Introducing these strategies into a daily routine can reduce the stress caused by life.
