HE call from the Society of St. Vincent de Paul to the government to act now to prevent people slipping in to poverty this winter must be heeded. As we face into the second winter since the pandemic struck, we are now contending with a cost-of-living crisis on multiple fronts. Energy prices are soaring, we are seeing rents rise well beyond the control of Rent Pressure Zones, particularly in larger cities and towns, and increasing transport costs are also pushing inflation up. The Budget may have brought some relief with a more generous Fuel Allowance to begin immediately, but this lifeline was unfortunately not extended to all those who will need it in the coming months. The financial impacts of the pandemic remain with the households supported by the Society which notes: “many families will have used up savings or be carrying the burden of debt from the extra costs and lower incomes of the last 18 months. “Worrying levels of people in research we commissioned at the beginning of the year reported cutting back on heating or electricity due to cost, including 42 percent of people unable to work due to illness or disability; almost one in three renters; and 36 percent of one-parent families. “Throughout winter, those in receipt of the Pandemic Unemployment Payment will also see their incomes reduce as the payment is gradually withdrawn. he West of Ireland, and Mayo in particular, has become the preferred location for many prominent film promoters/producers in recent years and visitors to Achill over last week-end could have shared some of the experience. Award-winning Finnish director Klaus Härö’s English-language feature debut “ Sailor, My Love” completed shooting on the Mayo island on Friday. The film is produced by Kai Nordberg and Kaarle Aho for Making Movies Oy (Finland) and by David Collins for Samson Films (Ireland) with support from the Finnish Film Foundation, Fís Éireann/Screen Ireland and the WRAP Fund (an initiative of the Western Development Commission) - among others. The original screenplay is written by Jimmy Karlsson and Kirsi Vikman. My Sailor, My Love tells the story of Howard, an aging widower who falls in love with his home aide despite his daughter, Grace, not approving of the relationship. Post-production is taking place in Belgium and Finland during the first half of 2022 with delivery scheduled for mid-summer 2022. The cast includes James Cosmo Braveheart, Game of Thrones), Bríd Brennan (Dancing at Lughnasa, Brooklyn), Catherine Walker (A Dark Song, The Delinquent Season) and Nora-Jane Noone (The Magdalene sisters, Wildfire). Pre-production in Mayo took place over eight weeks from mid-July to midseptember with principal photography,
mainly on Achill Island, for a further six weeks until the 23rd of October. Just over half of the cast and crew, totaling 85, are based in the West of Ireland. In addition, 150 local extras were engaged during the shoot. Approximately 3,500 bed nights were generated in Achill over the course of the production. Producer Kai Nordberg says: “It is important that small European countries can join their resources to create high-end content for the world market. My Sailor, My Love is very much taking the best out of
both countries to make a great film.” Producer David Collins commented: "Through collaborating with Making Movies Oy, we are very proud and happy at the creative and financial contribution we have been able to make to help Klaus realise his veryspecial vision for this film. Sligo woman’s online success continues
Sligo-born Larissa Feeney is achieving continued success with her business, Accountant Online, currently Ireland’s largest online accountancy firm, which is aiming to triple its workforce with 100 new jobs over the next four years. The skilled positions will be in the accountancy and technology fields with ancillary staff also being hired to bring
the total workforce to 150 by early 2026. The company, which was one of the first in the country to use a remote working model prior to the pandemic, offers all staff the opportunity to work remotely. The business, which was established by current CEO Larissa in 2015, now has 51 employees at its offices in Dublin, Derry, Letterkenny as
well as remotely across the Ireland and overseas. It has over 2,000 clients and has won numerous awards including Practice of the Year 2020 in the Irish Accountancy Awards. Larissa is originally from Kintogher, Sligo and lives in Letterkenny now. The company is exceeding its growth predictions achieving a 45pc increase in turnover during the Covid-19 pandemic as companies realised the benefits of using online accountants. It showed turnover of €2.1m in its latest financial year (to its year end in March 2021) and is on track to achieve turnover of €3m this year (to March 2022). Accountant Online is predicting turnover of €10m by its year end in 2026. Larissa Feeney says: “Accountant Online has been growing steadily and we are delighted to be announcing the next phase of our development with 100 jobs over the next few years. When I set up the company, the accountancy industry was still at a very traditional stage and, while there has been a lot of digital transformation in the industry, there is still a lot more room for clients to benefit from further innovation and digitisation. “Technology has always been core to our business and that has allowed us to capitalise on the move by customers towards digital professional services, the pace of which has been accelerated by the Covid-19 pandemic. Dramatic advances in CRM and cloud technologies are also allowing us to service our clients really well and more efficiently than traditional accountancy practice methods.”

Another factor which may eventually come into play, particularly in light of the uncertainty around the pandemic, is the plight of employees of companies currently benefiting from the EWSS, when the tapering of wage subsidy rates take effect over the coming months. Many of those companies are in the beleaguered hospitality sector where volatility still remains. Greater numbers could be pushed onto welfare. Data from the Vincentian Partnership for Social Justice shows that in 2021 the gap between social welfare for a single adult and the cost of a basic but decent standard of living is €49 every week. For a lone parent with two children, the weekly gap is €82. “During the coming months, we are deeply concerned that this crisis will come to a head as households are unable to find extra room in the budget for escalating energy costs. “For those at risk of disconnection – or on a
prepayment meter who cannot afford to top up – this could mean being unable to afford to stay warm and keep the lights on. This has serious health implications and is a situation that should be avoided at all costs. The society says it spends between €4.5 and €5 million per year supporting people in energy poverty. Resulting from a combination of very low incomes, hard-to-heat houses, and energy costs, thousands of households cannot afford to keep their homes warm enough or must cut back on other essentials, including food, to meet their energy needs. To add to many consumers’ concerns over energy issues there was the revelation last week that there was the distinct possibility of power shortages for households this winter. This was alleviated by the news that some plants forced to close during the pandemic would now be generating again. However, it all serves to show that nothing is guaranteed in such a climate of uncertainty.

West applicants sought for Eden suicide prevention programme
A
ground-breaking suicide prevention programme will be delivered in Mayo, Roscommon, and Galway in the coming months, offering hope to those at risk of taking their own lives. Run by the HSE’s National Counselling Service (NCS) in partnership with Suicide or Survive, Eden is an educational programme that also incorporates a therapeutic element and is geared at supporting people to move away from suicide as an option in times of crisis. The programme has been operating since 2007 and has been externally evaluated by Dublin City University. In 2016, the Eden programme was delivered for the first time outside of the east coast of Ireland in the west. Since that time, following ongoing funding from the HSE National Office for Suicide Prevention, it has been run on a face-to-face basis and, more recently, online during the Covid-19 pandemic. Recruitment is now underway for new participants in Mayo, Galway and Roscommon. Open to people over the age of 18 years who have attempted or contemplated suicide, Eden provides a safe space where participants can explore their experiences, learn skills to help them monitor and manage their mental health and build a range of supports appropriate to their needs. This programme runs one morning (three hours) a week for 26 weeks. A maximum of 16 participants are accepted per programme and it is hoped to run this latest round on both a face-to-face and online basis subject to demand. Please note this is NOT an emergency service. Participants can self-refer. Screening and interviews will begin in November 2021 to identify participant needs, including whether they require additional support before commencing the programme. Across the state, when combined with diagnostics waiting lists, there are more than 1.1 million people waiting on healthcare in this state and that is a disgrace. “That is why Sinn Féin has proposed funding for 600 more hospital beds next year above pre-committed levels and €150m investment in theatre capacity and diagnostic equipment. “The government has failed to produce a credible plan for tackling waiting lists and they have failed to put the investment in. “This is failing patients and staff in Mayo and throughout the state.”

Emergency action needed on MUH waiting times Mayo Sinn Féin TD Rose Conway-Walsh has called on the government to take emergency action to address the spiralling waiting lists and ease the pressure on Mayo University Hospital. In a recent statement Deputy Conway-Walsh said: “22 people have now been waiting more than 4 years to access inpatient treatment at MUH. At the end of 2020, there was one person waiting. “Even taking into account Covid restrictions and the HSE malware attack, it is shocking to think that anyone would have to wait this long to be seen. “In 2021, we have seen the number of people waiting over 2 years rise from 172 to 270. While overall, the waiting lists for inpatient care have gone from 1,009 to 1,279. That is more than a 25% increase. “There are currently

Budget will reduce the cost of living – Deputy Ring
M
ichael Ring T.D. said last week that Fine Gael is working to reduce the cost of living and Budget 2022 contains the measures to do it. The following ae just some of the issues he covered in his statement: “Budget 2022 will help to recover our economy from the pandemic, repair our public finances and restore our public services. Fine Gael in Government has prioritised reducing the cost of living in order to provide support for people at a time of rising prices. Budget 2022 contains measures to do just that. “Every person paying income tax will receive a tax benefit thanks to a total budget package of €520 million. The standard rate band is increasing by €1,500 and we are increasing each of the personal tax, employee tax and earned income credits by €50. “For several Budgets now, Fine Gael has prioritised supports for small businesses and entrepreneurs; we recognise they are crucial to job creation and the development of our local economy in Mayo. Supporting entrepreneurs and the wider business community will be central to our broader national recovery from the pandemic, and so there is a significant package for them in Budget 2022. “The Employment Investment Incentive (EII) scheme is being extended for a further three years and will also be opened up to a wider range of investment funds. There is a further €30m of investment in the Innovation Equity Fund. There will be an extension of corporation tax relief to certain start-up companies and a new tax credit for the digital gaming sector. “Welfare payments are increasing and there will be a 100% Christmas bonus in 2021.
The Old Age Pension will increase by €5 per week and the Living Alone Allowance will increase by €3 per week. The duration of Parent’s Benefit will increase by 2 weeks to 7 weeks from July next year, and from June next year, the Back to School Clothing and Footwear Allowance will increase by €10, equalising the income thresholds for the allowance for one and two parent households and increasing the income threshold for the Working Family Payment by €10 for all families. We are increasing the income disregard for Carer's Allowance from €332.50 to €350 for a single person and from €665 to €750 for a couple. “The minimum wage will increase by 30c to €10.50 in January, the eighth consecutive increase to the minimum wage under Fine Gael in Government. In tandem with this, we will increase the ceiling of the second USC band to ensure the increase in the minimum wage won’t push people into higher rates of USC. “Fine Gael is prioritising reducing childcare costs and we recently produced a report proposing a major reprioritisation of early childhood in our national ambitions, with a need for increased focus on well-being in early childhood. Budget 2022 reflects this with a new funding stream for up to 4,700 early years and childcare providers which will be in place from September 2022 onwards, at an estimated cost of €69 million next year. This will support improvements in the quality of childcare provision, and will be linked to a commitment of no increases in fees to parents. “Next year, we will spend in excess of €2 billion, or over 25% of the Education budget on providing additional teaching and care supports for children with special educational needs. This represents an increase of

We are also adding 980 new teachers in special education in mainstream schools, special classes and special schools, and we will open over 1,700 new special class places next year. “Addressing medical waiting lists is a huge priority for Government and €250 million has been allocated to address this challenge, further to the Acute Waiting List Action Plan. We are extending free GP care to children aged 6 and 7 next year, with the intention of reaching all children under 12 with this Fine Gael led policy. “The government is acutely aware of the impact on households of increasing energy costs. To directly address this, we are increasing the weekly rate of the Fuel Allowance by €5 and increasing to €120 the amount of means allowed above the maximum State Pension (Contributory) rate for the Fuel Allowance means test. The qualifying period on jobseekers allowance and supplementary welfare allowance will be reduced from 15 to 12 months. “There is a comprehensive €1.85 billion agriculture package, an increase of €32 million on last year. This will maintain critical supports across the sector while also providing
for a number of new measures. “One positive legacy of the pandemic is that remote and flexible working has becoming commonplace, something Fine Gael has been campaigning for quite some time. Budget 2022 provides an income tax deduction amounting to 30 per cent of the cost of vouched expenses for heat, electricity, and broadband in respect of those incurred while working from home. “As part of a range of housing measures announced in Budget 2022, we are extending the Help to Buy Scheme, which has already helped over 28,300 people to buy their first home. “Budget 2022 will fund the recruitment of 800 new gardaí, 400 garda civilian staff to free up gardaí for front line policing, and a 20% increase in the Garda Mountain Bike Unit. This will help to protect communities in Mayo and keep them safe. “As part of a comprehensive Climate Action package in Budget 2022, we are extending the €5,000 relief for Battery Electric Vehicles to the end of 2023, to continue to incentivise the uptake of electric vehicles. Every single euro raised by the increase in Carbon Tax will be returned to the people of Ireland in a progressive manner. There is an additional €174 million for energy efficiency and to protect the most vulnerable in society.
Council approves Carrick Destination Plan Local councillors got an opportunity to discuss Failte Ireland’s Destination Plan project for Carrick last week, which has significant enhancement proposals for the county town. After some debate on the issue the area councillors gave approval to the plan. The Part 8 planning application was proposed by Cllr Sean McGowan and it was seconded by Cllr Enda Stenson. But there were mixed opinions among councillors. Cllr Des Guckian said there were some good ideas and some “ridiculous ideas” set out in the plan. He noted the maps were not very clear and he was concerned about narrowing streets and widening footpaths. “Designers want Irish towns to look like French towns”, he said. He queried the cost of the design fees and he was informed they cost 10-12% of the total funding. Cllr Guckian estimated this to be approx.. €50,000. Carrick-on-Shannon has been allocated the maximum level of funding by Fáilte Ire-

land - €500,000 to develop its potential as a tourism ‘destination town.’ The Destination Towns project will develop an integrated approach to making Carrick-on-Shannon accessible to visitors. Plans include new interpretative signage, improved lighting and coach parking to compliment the current public realm plans
to encourage visitors to stay in the area for longer. Plans include street enhancement from Cryan's Hotel to Carrick Suite Plaza, new public lighting, street furniture and road signs. A new covered area and landscaped feature will replace the bus shelter on the Dublin road.

allina Comedy Club returns to Ballina Arts Centre on Friday 29th October, after the long Covid hiatus, with a double-bill of two of Ireland’s leading comedy talents: John Colleary and Danny Ryan. Apart from being one of the most in demand comics around, John Colleary has also co-written and starred in IFTA nominated shows, The Savage Eye and Irish Pictorial Weekly. Sligo fares badly in IBAL Litter Survey IT wasn’t good news for Sligo in first post-lockdown survey by Irish Business Against Litter. It has resulted in Sligo losing its Clean status and falling to 28th position in the ranking of 40 areas nationwide. The survey found that while the majority of towns have cleaned up over the past 12 months, litter in main cities has deteriorated to levels not seen in ten years The An Taisce report for Sligo stated three heavily littered sites resulted in Sligo losing its Clean status. Three heavily littered sites resulted in Sligo losing its Clean status. Two of the approach roads (N15 Donegal and R286 Dromahair) were both very much deserving of the top litter grade, as was Glenview Football Club / O’Boyle Park and O’Connell Street – the latter was particularly well presented and maintained with all aspects in very good condition e.g., paving, street bins, hanging baskets, ornamental trees etc. Two sites were brought down not by casual litter but dumping of rubbish – Recycle Centre, Connaughton Road and the residential area of City View. By far the most heavily littered site surveyed in Sligo was the University Hospital grounds. It didn’t get into this state overnight – it was flagged in IBAL 2020. Heavy levels of all manner of food related litter, used nappies and other miscellaneous items were found throughout the grounds, a mix of recent

or years it has been described as the most dangerous stretch of road in the country. Now it is relegated to the status of a local road and a cycle/walkway. The new N4 between Collooney and Castlebaldwin was officially opened by An Taoiseach, Micheál Martin who, almost simultaneously, unveiled a monument to those who lost their lives on the old road. The 15km stretch of dual carriageway between Collooney and Castlebaldwin
replaces the existing road, which has seen dozens of fatalities and accidents over the past number of decades. The new road comes after 15 years of campaigning by a local action group, which saw the erection of 31 crosses on the old road to symbolise the number of lives lost on this section of the N4. Michéal Martin said this project will significantly improve road safety for local communities and everyone travelling between Sligo and Dublin.

"A project like today's N4 Collooney to Castlebaldwin is a prime example of the upgrades needed to the national roads network to deliver on TII's remit to enhance safety and improve efficiency under the Government's Project Ireland 2040 plan," Mr O'Rourke said. The old N4 will become a Local Road with a new cycleway, enabling people to walk or cycle for short local journeys.

New EP release from The Flies R OSCOMMON rock and roll band, The Flies, have just released a new EP, ‘Back in the Garage’ on all streaming platforms, along with a music video for the track ‘Ignorance is Bliss’. "We've been playing together for four years. We started off playing shows in local secondary schools and at small festivals and played headline shows in Sligo and Carrick-onShannon in the period before the Covid-19 pandemic. After our first EP release we also played lots of shows including one in Dublin supporting the inimitable Jinx Lennon. Our final gig before the lockdown hit was a support slot with The Stunning. We are dying for things to open up again fully so we can get back to doing what we have come to love so much - playing live (and having lives more generally)," said the band. "Our new EP is our third to date and is titled ‘Back in the Garage’. It is available on all digital platforms. We recorded the basic tracks at The Blue Room Studio in Sligo and completed the overdubs and vocals at home in late summer, in a similar manner to how we recorded our last EP - ‘Looser’. We tried new things during the writing of this EP - we will consciously avoid the word “experiment” but there are slower, more intimate moments on the record which we don’t think were there in previous releases." The band say that their latest EP is influenced by Queens of the Stone Age, Teenage Fanclub, Bob Dylan, Nirvana, Sparklehorse and Sonic Youth amongst others.

Extent of damage to pub trade revealed New figures from the Vintners Federation of Ireland and the Licensed Vintners Association have revealed the extent of the damage caused by the pandemic to the pub trade in the North-West and across the country. In Leitrim alone, nearly 5% of pubs (5 in number also) have closed down since January of 2020, 12 in Mayo and 4 Sligo. However, the figures reveal that across the region Galway and Roscommon appear to be the worst hit, with 16 closures in each county. The nationwide figures from the VFI and LVA show that for the country as a whole 349 pubs have closed since the beginning of the pandemic, which vintners’ representatives say signals the extent of the distress faced by pubs as restrictions continue to impact the sector. The unknown factor, of course, is the impact the latest surge in infections will have on the sector, particularly if the full “reopening” should be delayed further. According to the two bodies the revelation of a 5% drop in pub licence renewals is a dramatic escalation in the crisis faced by pubs, coming just before Government is pondering on whether to remove all remaining Covid restrictions currently operated by the trade.

The figures, which were taken from an analysis of licence renewals published by the Revenue Commissioners, shows pubs have closed across the country, with Cork losing 37 pubs while Dublin has lost 33 public houses. Donegal lost the same number, 33, while Kerry has seen 22 pubs not renew their licences. In light of these new figures, both the VFI and LVA say it’s critically important restrictions on trading are removed at the scheduled date. By October 22, nightclubs and late bars will have remained closed for 585 consecutive days since the outset of the pandemic. Padraig Cribben, Chief Executive of the VFI, says,
“These new figures reveal Covid’s dramatic impact on the trade over the past 19 months. To lose 5% of pubs in such a short timeframe is unprecedented and underlines how the pub trade bore the brunt of pandemic restrictions. “In light of recent speculation about delaying the ending of restrictions, the news that pubs are closing across the country will only heighten anxiety in the trade. Uncertainty is a key factor in why so many pubs have closed so Government must put an end to the crisis by announcing the easing of restrictions will proceed as planned.” Donal O’Keeffe, Chief Executive of the LVA, says: “Losing 349 pubs across the
country indicates the magnitude of what’s happened to the pub trade since March 2020. These pubs closed even with the Government supports in place. We think the reopening of late-night hospitality, the return of the bar counter, the end of mandatory table service and a resumption of normal trading hours should proceed next Friday. If access is limited to those who have been vaccinated there is no justification for not proceeding the easing of the rest of the restrictions. With more than 90% of the adult population vaccinated and vaccine passes likely to remain in place, if we can’t allow hospitality to trade relatively normally now then when will we ever be able to?

Bundoran’s Conor Nominated for Three Top Global Awards

ed Bull Athlete and Bundoran-based surfer, Conor Maguire, is certainly making some waves. He has been nominated in three out of the four categories at the 2021 Global Red Bull Big Wave Awards which honour the athletes worldwide that are pushing the boundaries of what’s possible and celebrate the most death-defying surfing of the season with life-changing rewards.
A seasoned Big Wave surfer, Maguire achieved deserved global recognition last October, surfing what is believed to be the biggest wave to hit the West-Coast of Ireland reaching 60 feet, earning himself nominations for the Biggest Tow, Ride of the Year and Performer of the Year. The only Male European nominated in three award categories, from Bundoran County Donegal, Conor honed his skills off
the North-West Coast of Ireland, taming the giant Atlantic swells of Mullaghmore since the tender age of 11. Maguire commented; “This is without doubt the biggest moment of my surfing career to date and one I won’t forget. I have an incredibly supportive network and team around me, and without them I wouldn’t be able to do what I love at this level, so I am really happy to see our hard work recognised on a global scale like this. To be named up there with the likes of Kai Lenny in the Performer of the Year category, that means a huge amount to me and my team. It is definitely motivation for the Big Wave season ahead.” $350,000 in prize money will be awarded across four categories for men’s and women’s divisions: Ride of the Year, Biggest Paddle, Biggest Tow, and Performer of the Year. Winning photographers and videographers will also take home prize money. Conor, in his quest for glory, is sharing the stage with the best performers in the world.This year’s nominees spanned the globe with surfers representing countries including Ireland, Germany, Brazil, South Africa, the
United States, and France. The majority of the nominated rides come from the infamous big wave breaks of Nazaré in Portugal and Pe’ahi (a.k.a. “Jaws”) in Hawaii, USA, but this year’s shortlist also sees rides from Mavericks in Half
Moon Bay, California, USA, Todos Santos in Mexico, and of course Mullaghmore here in Ireland. The winners will be announced during the 2021 Red Bull Big Wave Awards Show on October 29, broadcast on Red Bull TV.

I
t will come as no surprise to hear that the all-electric vehicle (EV) market in Ireland is growing increasingly competitive, with more and more manufacturer’s jumping on the rapidly-accelerating bandwagon, so to speak. One major player on the EV field is Opel, whose all-new Mokka-e has just been launched.
Reinventing Opel The future strategy for the Iconic German vehicle manufacturer can be summed up with the simple phrase “We’re Reinventing Opel”, and this is truly evident in recent offerings from Opel – namely the new Corsa supermini, new Crossland small SUV/Crossover, and now the new Mokka. Coming soon will be the launch of the new Opel Grandland, with a new Astra and Manta in the not too distant future. The new Opel Mokka-e features Opel’s new design language, with the ‘Vizor’ face the centrepiece. There is no need for a traditional grille in electric cars like the Mokka-e, so the ‘Vizor’ creates a seamless integration of future lighting and other technologies, all within a high-tech protective surface. The vizor of the new Mokka extends in one single sweep across the front of the vehicle underneath the bonnet, visually stretching its width and organising the fascia with a reduced amount of elements. No superfluous decoration disturbs the focus on a pure, flush surface that perfectly integrates the headlights. The legendary Opel lightning-flash emblem proudly dominates the centre. It looks pure, precise and technical, a perfect match to the new Mokka that will only have highquality LED lamps as well as the latest generation of IntelliLux LED® matrix headlights – a true highlight in this market segment. The Opel Vizor will become a hallmark of all
Opel models in the course of the 2020s with the idea of further clustering top technologies. Head-Turning Design Possessing a magical blend of elegant styling, a sumptuous cabin and advanced safety systems, the new Opel Mokka-e is a real headturning car in every respect. The Mokka-e uses a 100kW electric motor which produces 134bhp and 260Nm of torque, and is linked to a 50kWh battery, which you can charge at up to 100kW at a public rapid charger in as little as 30 minutes (15-80%). Test Car My Opel Mokka-e test car was a range-topping Elite model, which looked amazing in vibrant Matcha Green Metallic paintwork with a contrasting black roof, mirror casings, and body mouldings for a rugged SUV look. STRESS TEST VS ECHOCARDIOGRAM: THE DIFFERENCE I
t’s no secret that heart disease remains the leading cause of death in the United States. According to the Centers for Disease Control and Prevention, it accounts for approximately one out of every four deaths. And while there are plenty of risk factors that you can control, such as diet, exercise, and smoking, there are other factors that are out of your hands, such as heredity, aging, and even gender. The American Heart Association warns that although women are typically at lower risk for heart attacks than men, their chances of dying from a heart attack are actually greater, and the risk of death from heart disease increases once a woman reaches her menopausal years. One reason may be that the symptoms of heart disease in women aren’t always obvious. “It can be tricky because women don’t always have the same symptoms that we typically relate to heart disease like chest pain,” explains Dr. Jorge Alvarez, a cardiologist in San Antonio, Texas. Things like increased fatigue, shortness of breath, or difficulty performing tasks that were once easy can all be early warning signs. “A lot of times, we shrug these things off as age, but it is important to mention them to your physician so an appropriate screening and
evaluation can take place,” he adds. Depending on symptoms, family history, and risk factors, that screening will usually begin with a traditional resting echocardiogram or “Echo” to get a clear picture of the heart’s structure. In some cases, this may be followed by a stress echocardiogram, or “stress test,” to determine how the heart responds to workload. Each of these tests has a specific function when it comes to the diagnosis of any cardiovascular issues.
Understanding the Echo To understand how an Echo works, it helps to have a basic understanding of the intricate functions of the heart. Dr. Alvarez explains it by comparing the heart to a house. The heart muscle is the walls, and they can be either thick or thin. The four valves of the heart are the doors, opening, and closing to allow blood to flow in or out of the ventricles. The arteries serve as the plumbing, transporting the blood throughout the body, and the conduction system is the electricity that causes your heart to beat. All of these areas of the heart work together, and each one can affect or be affected by another. Your doctor can identify structural cardiac issues like valve disorders or function issues like heart failure through an echocardiogram. The
Echo can also pick up many diseases that can affect the heart either from the lungs or from specific types of cancers. It can also be used to determine the cardiovascular effects of certain medications, such as chemotherapeutics. This common, painless procedure uses high-frequency sound waves to produce images of the chambers and valves of the heart. Performed by attaching adhesive electrodes to the chest that are hooked up to a monitor, it allows the doctor to observe how the heart is functioning. It can identify abnormalities with the size of the heart and any issues associated with the opening and closing of the valves. A wand with a gel-coated tip may be run across the chest to pick up the sound waves (Doppler signals) to determine both the speed and the direction of the blood moving through the heart. “Echocardiograms are used to look for anatomical issues or to evaluate the heart function,” explains Dr. Alvarez. “It should always be the first test done, and then you progress from there.”
Don’t Stress the Stress Test. If more information is needed, your doctor may ask for a cardiac “stress test.” Also called an exercise stress echocardiogram, the stress test is
designed to monitor the heart’s response to physical activity. Unlike the traditional echocardiogram, this procedure typically requires you to be in a fasted state and refrain from taking certain medications on the day of the test. It is usually performed on a treadmill that gradually increases in both speeds and inclines until a target heart rate is reached or you experience symptoms that require you to stop. A baseline Echo is taken before the start of the stress test and again at the completion for comparison. “The stress test is done to
evaluate for symptoms of possible coronary artery disease, or to evaluate the functional capacity and ability to raise your heart rate or rhythm,” explains Dr. Alvarez, adding that there are alternatives to the treadmill test for those people who are unable to physically exercise. “There is a pharmacological stress test, where we use
When To Test Unlike mammograms or colonoscopies, Alvarez says there is no recommended age for a screening stress test. Instead, a screening depends on your cardiovascular risk factors. Things like diabetes, hypertension, tobacco abuse, family history, and high cholesterol are all risk factors that should be considered. However, just because you don’t have high-risk factors doesn’t mean that you should ignore any symptoms. As Dr. Alverez mentioned earlier, this is especially true for women. “The cornerstone symptom would be chest pain, but, as I said earlier, that isn’t always the case for women,” he says. “A simple change in functional capacity can often be the earliest symptom.” It is never too late to develop heart-healthy habits. Control what you can in terms of your diet, activity level, and lifestyle, and keep your doctor informed of any risk factors. Consult your doctor if you begin to notice things like extreme fatigue or shortness of breath while going about your daily activities or if you experience a decrease in your exercise tolerance. Being in touch with your body and advocating for yourself when you suspect something isn’t “right” may just save your life. SIMPLE STRATEGIES TO UNRAVEL BACK PAIN M
ost of us have experienced back pain at some point. It’s one of the most common frustrations as we age. Back pain – like many things – is unique to your own body and should always be evaluated by your physician to rule out any more serious issues. Quite often however, unraveling why it is happening is the key to resolving acute pain as well as ensuring it doesn’t recur. Movement can be part of the solution.
Genetic vs. Cultural Posture We have unique and miraculous bodies. Back pain, once you have ruled out other issues, can be diminished once you understand your own body and the elements that play a role in causing the pain. But while you try to figure those elements out, it can also be frustratingly mysterious. I have had to do this for myself, and the path was not a direct one. First, I had to work to understand why my back hurt. For me, it was because my pelvic bones had shifted and were pulling on my low back. Then I had to figure out what I was doing to cause the pain. Over time I learned that I was putting additional torque on my pelvis by continuing to pull it in the direction it had already moved. I’m oversimplifying a bit but often, back pain is a perfect storm of forces influenced by a variety of factors. Here are two factors that are particularly helpful to consider as you play detective with your unique situation. Genetic Posture I got it from my mama! Or dad, or grandma… you get the idea. Our genetic posture is formed in us from our parents. It is our basic body structure and includes any issues we might inherit. It can be things like leg length difference, scoliosis or
long toes. It’s not good or bad. It is just the way we’re made and important to understand as part of the big picture of how the forces of movement transfer through our body.
Cultural Posture What do you do all day? Your lifestyle is your cultural posture and has two elements of its own. What you have to do, and what you love to do. These are the forces applied to your body (or lack thereof if you sit or stand still for long periods) based on what you’re doing with your time. Do you sit at a desk? Are you on your feet all day? Maybe you love to go for a run after work. Or maybe you work a lot in your garden. However this shows up in your life, think about the impact of these activities on your spine and body. Understanding the influences on your body can help you balance it out. The goal is to achieve balanced mobility through the whole body and keep all the tissues of the body stimulated and moving the best they can in your unique body. That will keep ease in movement and all of the systems healthy. Everything Is Connected to Everything Else Our amazing bodies are a collection of systems that have to work together. They are all important. Our musculoskeletal system is made up of bones and muscles and quite often we focus on this a lot. There is also an interconnected system made up of tissue called fascia that is interwoven in and around our muscles, organs and bones. It’s so much more complicated than simply muscles moving bones. What you do (or don’t do) all day informs the communication of the system. Back pain can sometimes be a

symptom of the place in the system where the communication isn’t happening. Or a place that is strained because of tension coming from somewhere else in the body. When pain flares, our instincts can lead us to try stretching that one area or worse, to stop moving altogether. But movement is typically a key part of the solution. Depending on the cause of the pain, certain movements can help relieve back pain. In many cases, the right movement program can make pain a much less frequent concern. Taking time to notice the ways your systems interact with your posture can inform your choices and how to care for your body. Restoring Balance in the Body First and foremost, if you have pain, please seek support from a medical professional and only exercise with their clearance. I’m not a doctor and this is not medical advice. As a Pilates instructor and movement professional with 25 years of experience, I have some useful concepts and practical suggestions to share with you. But they should always be pursued with permission from your own doctor. There is no one simple fix, but here are some basic movements to help you move more and feel better.
Move Your Spine in All Planes of Motion Moving your spine in all of the planes of motion regularly at a pace that is comfortable for your unique body can be part of the solution. Usually this feels wonderful. Choosing exercises in all planes of motion help us maintain balance in our body. All planes of motion include exercises that bring your spine into length and: • Extension – This looks and feels like arching your back, especially your upper back. • Flexion – The opposite of extension – when your spine curves forward. • Side Bend or Lateral Flexion – When you tilt your head toward your shoulder and bend your body sideways. • Rotation or Spiral – Your shoulders and upper body turn one direction while your hips and lower body head the other way to create a gentle twist through your spine. Practice Mindful Breathing Breathing well can help a lot. By learning diaphragmatic breathing you can organically organize the musculature of your trunk (i.e., your core). As an added benefit, this mindful breathing practice can also release tension, calm the nervous system and help you feel energized. Visit a Specialist If you are moving your body and have areas of your spine that actively hurt, in addition to getting help moving your body in a balanced way, getting support from a manual
therapist will help you understand and treat the symptoms of your pain. Movement support and manual therapy go hand in hand. Ligaments can remodel (mine did) but you need the movement expertise to help strengthen your body. Support in both areas is key so if possible, find a trainer who understands back pain who can work with you in person or online.
Build Strength Finally, do your exercises. I like to call them Movement Vitamins. If you have a home exercise program from a medical professional, doing it every day should be like brushing your teeth or taking your vitamins. This is the best way to build the stability and balance to your system to ensure that pain doesn’t recur. Here are a few favorites we like to share with clients at our studio.
Cat Stretch Begin kneeling on all fours. Your arms should be underneath your shoulders and knees directly underneath the hips, slightly apart. Feel your spine reaching long from tail to head. Inhale to grow longer and find length through the spine. As you exhale, use your abdominals to round your spine like a Halloween cat. Start from your tailbone and leave the head for last. Hold this c-curve position while you inhale to breathe into the sides of your ribcage rounding further. Finish with an exhale and beginning with the tail end of the spine extend one vertebra at a time until you return to your starting position. Repeat 6-8 times. Spinal Rotation Begin kneeling with hands under shoulders and knees under hips. Your spine should be in a neutral position and feel as though it’s reaching long from tail to head. Your shoulder blades are stable against your rib cage. Take an inhale and extend one arm out to the side. As you exhale,
lead with your fingertips to thread your arm under your body. On your next inhale, return your arm out to the side. Exhale to open your chest and allow your spine to rotate as you then reach your arm to the ceiling. Inhale to return to your starting position and repeat with your other arm. Repeat 4 times on each side.
Hip Rolls Start lying on your back with your knees bent and feet hip distance apart. Your arms are by your sides. Take a deep breath into the sides of your rib cage and feel yourself grow long through your spine. As you exhale, recruit your abdominals into an imprinted position by feeling your hip bones and ribs pull closer together and there is little or no gap between your spine and the floor. Once imprinted, articulate your spine starting from the tail to roll up into a bridge position where wide shoulders support your weight. Your hip joints are extended, and your knees should feel they are energetically reaching long over your toes. Take an inhale to stay here at the top, then roll down as you exhale to return to your starting position. Imagine your sit bones reaching toward your heels as you articulate down through your spine. Repeat 8 times.
Do More of What You Love Unraveling back pain is like being a detective for yourself. The trick is to think about your unique genetic and cultural postures and bring balanced movement into the body all day long. There are so many systems working together to keep us moving and pain free. Noticing and understanding the systems will inform your choices and help you care for your body. Moving your body in a conscious and enjoyable way is part of the process. Train for your unique life. Move your spine in all planes of motion. Breathe well. Bring movement to all the joints of the body and build up enough strength to do the things that you love.
