MESSAGE from the President

A

s we look back at 2016, we can be proud of the many accomplishments the Phelps Regional Health Care Foundation (Foundation) has made to provide philanthropic support to Phelps County Regional Medical Center (PCRMC). It was another year for all of us associated with the Foundation to work together toward the goal of improving the health of those who live in PCRMC’s seven-county service region.

We started the year with the 31st Annual Cancer Gala, which set another record of almost $161,000 raised for the Delbert Day Cancer Institute. Our annual golf tournament held in June raised another new record of over $122,000 for the Delbert Day Cancer Institute. In 2016, 376 employee donors pledged and raised more than $118,000 for the various funds that contribute to the lives of those in our community. In total, our third-party fundraisers, such as the Cancer Gala, Heart-2-Heart Luncheon, and Cupcakes for Cancer raised more than $247,000 for the Foundation. Thank you to our sponsors, donors, volunteers, and staff who made this another successful year. At the end of the year, we celebrated the Delbert Day Cancer Institute dedication after the community came together to build a cancer institute that would bring world-class cancer care to one single, accessible location. We extend our deepest gratitude for making this campaign a success. The annual report is our commitment to the community to showcase our mission to provide world-class healthcare close to home and provide fiscal responsibility for our friends and donors. Another record year for golf! ANNUAL GOLF TOURNAMENT RAISED OVER $122,000 Golfers love playing a round of golf and thrive on a little competition. The 18th Annual Phelps Regional Health Care Foundation Golf Tournament was no exception and set another new record of over $122,000 raised for the Delbert Day Cancer Institute (DDCI). On June 6th, 2016, the Phelps Regional Health Care Foundation Golf Tournament, held at the Oak Meadow Country Club, was nearly full as 224 golfers participated in the morning and afternoon sessions. McCarthy Building Companies, Inc. and iMedX were title sponsors for the 2016 event. In addition, many other businesses sponsored key areas such as Master Sponsor High Tech Roofing Company, LLC and Ace Sponsors Guarantee Electrical Company, Mo-Sci Corporation, and Murphy Group Holdings, Inc. In total, 75 businesses gave their support to the Phelps Regional Health Care Foundation and made it another successful year on the green. A new addition to the event included the popular “Gettin’ Wiggy with It” event. Participants donated $25 and in return received the opportunity to tee off at the ladies tee box on Hole #8. In order to make the event fun and entertaining, participants were required to wear a wig and costume while teeing off. The event was a fun twist and raised $2,450 for the Brenna’s Boutique in the Delbert Day Cancer Institute. After a round of golf, players enjoyed a meal in the Oak Meadow Country Club dining room. An awards ceremony followed, which recognized winners of various flights and top-tier sponsors. The proceeds from the Phelps Regional Health Care Foundation Golf Tournament benefited the new Delbert Day Cancer Institute. The DDCI aims to provide coordinated, patient-centered cancer care services to patients of Phelps County Regional Medical Center.

Phelps County Regional Medical Center Employees Generously Give NUMBERS CONTINUED TO RISE FOR EMPLOYEE GIVING IN 2016

Organizations and nonprofits alike have giving programs that allow employees to designate a percentage of their pay toward a charity of their choice. The employees of Phelps County Regional Medical Center (PCRMC) continued to generously support the Phelps Regional Health Care Foundation (Foundation), and they once again set a new level of giving in 2016! The employee giving campaign, themed “We’re All In,” took place during the month of October. Employees at PCRMC were challenged to make a donation to the Foundation. An astounding 376 PCRMC employees contributed during the 2016 employee giving campaign, resulting in over $118,700 raised. With 12 funds to choose from, employees could designate which Foundation Board Members (left to right) Representative Steven fund to support. We are grateful for our employees every day and their generous support of the employee giving campaign shows how much compassion we have at Phelps County Regional Medical Center.

We all need a Guardian Angel in our life THE EXTRA MILE FOR PATIENTS AT PCRMC

Thousands of patients come through PCRMC for hundreds of different reasons, and it can be at any moment a guardian angel steps into their life. It can be a kind word or a gentle touch that makes the difference in their care. They can be a doctor, nurse, or someone who is willing to go the extra mile for a patient to make them feel special and cared for at PCRMC. These guardian angels do not ask for anything in return. Created in 2010, the Guardian Angel Program gives an avenue for those patients, family members, friends, and colleagues to recognize their caregiver and thank them for the extraordinary care they received. When a patient makes a donation to the Foundation through the Guardian Angel Program, they can direct their donation to the fund of their choice and help another patient in need. It truly is a circle of giving. Our Guardian Angels could fill several pages in this report, but instead we will share a few of them, along with the comments shared by the grateful patients and their families.

The DAISY Award CELEBRATING OUR NURSES We know that all Phelps County Regional Medical Center (PCRMC) nurses change lives daily, work long hours, and provide patients with a smile and a hug, all while ensuring their patients receive the compassionate care they deserve. We also know that some nurses go the extra mile for a patient, and patients and staff remember those exceptional nurses for the extraordinary care they give. These are our DAISY Award winners, and in 2016 we honored eight of them. The DAISY Award is an international program that rewards and recognizes the extraordinary clinical skill and compassionate care given by nurses every day at PCRMC. Recipients are nominated by their patients or peers, and PCRMC honors those extraordinary nurses with this special award. The DAISY Award is made possible by funding to the Phelps Regional Health Care Foundation (Foundation) from the family of Vickie B. Allen, a beloved former RN at PCRMC who served the nursing profession for 40 years, and donations to the Foundation’s Enhancing Nursing Excellence Fund.

GET TO KNOW THE FOUNDATION

Lorrie is the Executive Director of the Phelps Regional Health Care Foundation and is dedicated to raising philanthropic dollars for PCRMC. Lorrie has over 15 years of experience in senior-level advancement at social service and health institutions. She started her career at the Phelps Regional Health Care Foundation in 2007, first as the Director of the Foundation and now serves as the Executive Director of the Foundation. In 2012, Lorrie achieved her Certified Fund Raising Executive (CFRE) certification. She is married to Robert Hartley, a middle school history teacher. Together they have three daughters; Brittany, Maria and Faith.

Donia started with the Phelps Regional Health Care Foundation in 2008, first as the Annual Giving Coordinator and now serves as the Assistant Director. She has over 14 years of philanthropic fundraising experience and earned her Certified Fund Raising Executive (CFRE) certification in 2015. As the Assistant Director, Donia oversees Foundation operations and collaborates with philanthropic community members to ensure quality healthcare is available to all patients close to home. Before joining the Phelps Regional Health Care Foundation, she worked with University Advancement at Missouri S&T. In her free time, she enjoys traveling and spending time with her family, especially her three grandchildren, Gage, Lanie and Scarlett. PHELPS REGIONAL HEALTH CARE FOUNDATION

Sneak peek after our community comes together DELBERT DAY CANCER INSTITUTE PRE-GRAND OPENING AND DONOR APPRECIATION EVENT Over 500 of the Phelps Regional Health Care Foundation’s (Foundation) donors, supporters, and friends gathered for a night of celebration and everyone had a chance to have a “sneak peek” into the state-of-the-art Delbert Day Cancer Institute (DDCI). This event, held on October 13, 2016, was the final celebration after our community came together to build a cancer institute where patients receive all-inclusive, patient-centered cancer services in a single, accessible location. The night included thanks and gratitude from Foundation Board President Ted Day, PCRMC CEO Edward Clayton, namesake Dr. Delbert Day, PCRMC Medical Director of the DDCI and Radiation Oncology Department and Foundation Board Member Dr. Christopher Spencer, Foundation Board Member and cancer survivor Cindy Beger, and Foundation Board Member and Representative Steven Lynch who emceed the event. Attendees received guided tours through the DDCI and listened to clinical staff at six different stations highlighting the ways the cancer institute would better-serve patients. After the tours were completed, attendees enjoyed an evening of hors d’oeuvres, refreshments and music. The event left donors with an up close and personal view of how their support achieved the DDCI, and provide a sense of fulfillment that the new cancer institute will grant a way for PCRMC physicians to deliver world-class care to cancer patients in one cohesive location. GET TO KNOW THE FOUNDATION

Marsha joined the Phelps Regional Health Care Foundation in August 2016. As a Major Gift Officer, she supports the Foundation in its initiatives and mission to ensure that health care delivery in Phelps County and the surrounding communities is transformational. She is a Rolla native and a graduate of Missouri S&T, recently returning after nearly two decades away serving both the private and public sectors. She is a lifelong philanthropist and has served as an officer and committee chair for various organizations throughout her entire career. In 2015, she earned her Certified Fund Raising Executive (CFRE) certification and her free time is spent enjoying the outdoors, traveling, and caring for her menagerie of rescue pets.

Cameron relocated back to Rolla from Springfield and joined the Foundation team in March 2016 and served as the Annual Giving Coordinator until May 2017. As the Annual Giving Coordinator he provided overall coordination and project management of Foundation-sponsored events and was responsible for the oversight of the annual giving program. As of June 2017, he has taken on a new role as a Major Gift Officer. As an avid runner, he spends his spare time training and running half marathons. Cameron was born and raised on a farm, where he has part ownership in his family’s beef cattle operation. On the weekends, he can be found spending time with his family and assisting with the management of the cattle herd. PHELPS REGIONAL HEALTH CARE FOUNDATION

Missouri Foundation for Health achieves Pillar Society As the philanthropic support for Phelps County Regional Medical Center (PCRMC), the Phelps Regional Health Care Foundation (Foundation) continues to seek support for patients to have better access to primary care. A multi-year grant of $499,252 from Missouri Foundation for Health (MFH) is helping us achieve that and increase patient access to health resources, reduce emergency department visits, enhance continuity of care, and strengthen communication among our area’s safety net providers. Missouri Foundation for Health is a resource for the region, working with communities and nonprofits to generate and accelerate positive changes in health. As a catalyst for change, MFH improves the health of Missourians through a combination of partnership, experience, knowledge, and funding.

Shelby O’Keefe Shelby O’Keefe moved back to the Midwest with her husband after living on the East Coast for several years. Shelby joined the Foundation team in November 2016 and serves as the Digital and Technical Writer. As the Digital and Technical Writer, Shelby is in charge of all aspects of marketing for the Foundation through digital and print media. Shelby is a health and fitness advocate and in her spare time enjoys teaching fitness classes, getting lost in nature with her husband Sam and dog Luna, traveling to new and exciting places, and spending time with their fur family.

Brittany serves as the Database Manager for the Phelps Regional Health Care Foundation. Brittany has been with the Foundation since April 2014 and is responsible for providing project and technical support to the Foundation staff through data maintenance, research, and analytic reporting. Brittany is a native of Rolla, MO and is proud to serve her hometown community. She is married to Ryan and together they have two daughters, Kinley and Sutton. In her spare time, Brittany enjoys attending live theatre, golfing, and spending time with her family.

Rolla Cancer Gala sets a new record! The Annual Cancer Gala is the Phelps Regional Health Care Foundation’s largest third-party fundraiser and has been incredibly successful thanks to donors like YOU! The 31st Annual Cancer Gala held on February 20, 2016 was another recordsetting year of over $160,000 raised for the Delbert Day Cancer Institute.

The theme for the event of the 31st Annual Cancer Gala was “Ticket to Ride” and everyone in attendance did just that on behalf of raising money for the Phelps Regional Health Care Foundation. The event was sold out with more than 500 in attendance. Participants enjoyed the silent auction, 50/50 raffle, and toasted to more than 30 years of fundraising for cancer patients!

Proceeds from the event were directed to the Delbert Day Cancer Institute, which provides all-inclusive, patient-centered services in a single, accessible location on the PCRMC campus. 10,000+ Cupcakes and $10,000+ raised for Cupcakes for cancer! ANOTHER YEAR OF SUCCESSFUL BAKING Brenna Heavin rocked the baking world again in 2016 and baked more than 3,000 cupcakes and raised $4,515 for the Joy of Caring Cancer Fund! Collectively, Brenna has raised more than $10,000 and baked more than 10,000 cupcakes since she started Cupcakes for Cancer in 2011. Cupcakes for Cancer is now a collaborative effort of Brenna’s family and friends. Many of Brenna’s family take vacation time during the month of July to help out with baking, frosting, and delivering the cupcakes. The family operates out of the First Baptist Church of Rolla and has expanded to six different, flavored cupcakes and various flavors of icing. The Joy of Caring Cancer Fund helps with expenses for local cancer patients in need and was originally started by Renee Joy Heavin, who battled breast cancer in the 1990s. Joy had a vision to provide resources for cancer patients in order to offset the worry and stress about how to pay for medical, household, and family expenses during a difficult time. Brenna continues to help cancer patients fight the battle against cancer by baking and selling cupcakes and raising money for our local community and those fighting cancer. Every year, Brenna and her family continue to break new records for cupcakes baked and dollars raised. With the generous donations of time and effort, Brenna raised over $10,000 and now has a room named in her honor at the Delbert Day Cancer Institute (DDCI). Brenna’s Boutique is a room where cancer patients can select a wig to wear while receiving treatment at the DDCI. Thank you Brenna and the Heavin Family for donating your time and effort toward the fight against cancer! Brenna is an example of how hard work and doing what you love can help those in need! Heart-2-Heart Luncheon ANNUAL LUNCHEON EXCEEDS EXPECTATIONS ANOTHER YEAR IN A ROW! Thanks to the Heart-2-Heart Committee for exceeding expectations and making it another successful luncheon! Attendees gathered Friday, December 2, 2016 at Matt’s Steakhouse to enjoy a heart-healthy lunch, a silent auction, and raffle. Keynote speaker Timothy J. Martin, MD, FACC, CCDS, Director of the Cardiac Catheterization Lab, discussed coronary atherosclerosis. The hard work of the Heart-2-Heart Committee raised $32,532 for the Phelps Regional Health Care Foundation's Heart2-Heart Fund. The 2016 luncheon directed on funds raised toward the Cardiac Catheterization Lab Initiative, which is focused on raising $500,000 toward the $1.5 million purchase of replacement equipment and customization of our Cardiac Catheterization Lab.

4th Annual Superhero 5K BOOM! KAPOW! RACING FOR A GOOD CAUSE! A superhero is a heroic character who possesses supernatural powers and is dedicated to fighting crime, protecting those in need, and battling supervillains. The event included superheroes and villains along with everyday heroes in uniform from military servicemen and women, firefighters, police officers, and emergency responders to cheer runners on during the race. After the race, participants enjoyed O’Doggy’s hot dogs and freshly-squeezed lemonade. A total of 278 participants joined in for the event, which included those from the Rolla community and surrounding counties. In addition to ensuring the event was a victory and enjoyable time for participants, Phelps County Regional Medical Center raised over $9,900 for the Joy of Caring Cancer Fund, which provides for cancer patients who have difficulty managing daily activities and challenges associated with their cancer diagnosis.

Hospice Matters 2RD ANNUAL BUTTERFLY RELEASE CELEBRATES THE END-OF-LIFE WITH AS MUCH RESPECT AS THE BEGINNING The end of every life deserves as much beauty, care, and respect as the beginning, and that is what Phelps Regional Homecare and Hospice brings to their patients. Their belief is that hospice matters. Everyone matters. The 2nd Annual Hospice Butterfly Release Celebration brought family and friends together at Huffman’s Flowers of the Field Gardening Center on May 21, 2016 in St. James, MO. Phelps Regional Homecare and Hospice serves those patients who have life-limiting illnesses, and provides the care and treatment they deserve during this difficult time period. Guest enjoyed beverages and light refreshments before the gathering to release butterflies in honor of their loved ones. The event raised $6,513 for the Phelps Regional Health Care Foundation’s Hospice Fund, which provides hospice patients with food, gas cards, utility bill payments, and other living expenses that otherwise the patients could not afford. In 2016, the Hospice Fund helped 88 hospice patients and assisted them and their families with needed support during a challenging time.

It assists with everything from nutritional supplements, wigs, mastectomy bras, lymphedema therapy and more. Provides transportation to patients within a 30-mile radius to necessary healthcare appointments and treatments. Funds are used to purchase vans, provide ongoing maintenance and obtain other needed equipment. Funds dental care to area youth from age one to 18 who meet financial qualifications.

All contributions received and managed by the Phelps Regional Health Care Foundation from corporate sponsorships and individual donors are used locally. All contributions are tax exempt.

HOW YOUR SUPPORT BUILDS A BETTER US Delbert Day Cancer Institute Fund “Cancer” is a frightening word and often patients and their families, when faced with this intimidating battle, are left at a loss on how to manage this difficult time. The Delbert Day Cancer Institute Fund created a vision to provide seamless patient-centered care with emotional support for patients throughout the duration of their treatments. The Delbert Day Cancer Institute (DDCI) became a reality as 2016 saw the completion of the construction. The hard-working and devoted cancer care team cares for each patient every step of the way, from screening and diagnosis, to treatment and survivorship.

The DDCI has a diverse team to provide each patient with a personalized plan for treatment and the highest quality of care. Patients have access to cutting-edge cancer treatment and state-of-the-art equipment as the standard for treatment. In 2016, the Fund purchased a new linear accelerator, PET/CT scanner and x-ray units, which helps provide patients with better screenings, diagnoses, and radiation treatments. In addition, the DDCI has a new state-of-the-art pharmacy and infusion area, which provides private areas to patients while receiving chemotherapy. During such a stressful time, patients should be able to be as comfortable as possible knowing they are receiving the best care, and the DDCI provides just that. “The Delbert Day Cancer Institute Fund has enabled us to start providing quality cancer care here locally. Besides the building and technology, it has provided us the framework to help people through their cancer journey in all aspects of their life,” says Dr. Christopher Spencer, Medical Director of the Delbert Day Cancer Institute and Radiation Oncology Department. “Our dedicated staff can now provide better physical, emotional, and financial support during this difficult time in their life.”

The DDCI has provided the community quality cancer care close to home, while providing emotional support every step of the way. The opening of the DDCI is a symbol of Phelps County Regional Medical Center’s commitment to providing incredible care and services to the people in our community. A fund for all

SUPPORTING THOSE WITH NOWHERE TO TURN In the summer of 2016, a family traveling from Oklahoma was the victim of a severe car accident on I-44 after their car hydroplaned and flipped off the road into a water-filled ditch. The mother was severely injured and transported to Phelps County Regional Medical Center (PCRMC). Because of the extent of her injuries, the mother was airlifted to Springfield for additional care. In this time of confusion, trauma, and need, the Greatest Needs Fund, through the Phelps Regional Health Care Foundation (Foundation), was able to support this family after their accident. This family had lost everything; the mother was in critical condition, they had lost all their possessions in the crash when their vehicle was submerged in water, and they anxiously awaited word of their mother’s health as she was fighting for her life. Because the family was left with little more than the clothes on their backs after the accident, caring and thoughtful PCRMC employees helped provide clothing, food, and other basic needs for the family with funds from the Greatest Needs Fund. To ease the anxiety of the family, it was critical that they be together to support each other during this uncertain and difficult time. To

make this happen, caring members of the Outside Patient Transportation Department came to work on their day off and drove the family to Springfield, where the family was provided Visa gift cards to aid them for several days, as well as a hotel room to stay in while their mother received treatment. In addition, the Greatest Needs Fund purchased a prepaid phone to provide the family immediate communication with their extended family. “One of the hardest things about the Greatest Needs Fund is knowing how many out there need assistance. One of the greatest feelings is being able to assist in that need, but the best thing of all is knowing the compassion that exists for others as it’s exhibited through the donors to the Foundation,” Cindy Butler, fund administrator. The Greatest Needs Fund is intended to help in a variety of ways at PCRMC. Thanks to you and your donations to the Greatest Needs Fund, PCRMC is able to help with patient needs such as prescription medications, transportation, medical co-pays, or in this case, helping a family who had nowhere to turn.

Breast Center Mammography Fund can be a saving grace PROVIDING A SERVICE MANY COULD NOT AFFORD

Many of the patients that walk through the Comprehensive Breast Center’s doors at Phelps County Regional Medical Center (PCRMC) have concerns about their breast health and want to find the source of the problems they experience. The Phelps Regional Health Care Foundation’s (Foundation) Breast Center Mammography Fund provides these women with the hope and access to services they otherwise might not be able to obtain on their own. Some women find themselves in a difficult situation with no insurance and financial constrains, but they still desperately need the mammograms and breast ultrasounds that the Breast Center provides. The Breast Center Mammography Fund is a crucial part of giving patients the treatment and care they deserve. The Fund helps provide these women with the peace of mind that they can access these vital preventative services without worrying about the cost. During the month of October, the Breast Center Mammography Fund provided up to 50 uninsured women in our community with the opportunity to come in throughout the month and obtain a free mammogram. Early detection is key to reducing the risk of cancer and a yearly mammogram helps these women detect any abnormalities as early as possible. “The opportunity for women to obtain a yearly mammogram has been well received and is often the only way many of the women screened are able to obtain a yearly mammogram,” says Tiffany Henry, fund administrator.

very year, thousands of people from our area contribute to the Phelps Regional Health Care Foundation in our direct support of Phelps County Regional Medical Center. We are overwhelmed with gratitude from the support we receive and our donors are the foundation on which we are able to pursue our mission every day. Your gifts are important to our vision to be the best hospital foundation dedicated to sustaining and advancing PCRMC. Without your gifts, we could not provide for the many needs of our population in order to continue building a healthier, happier community, and we are incredibly grateful for the support we receive from within our communities. Our support comes in all levels, from those who are able to give substantially to those with little. It takes every gift, at every level, to build us up and to be a pillar of support for our community. Buildings need the support of each individual brick to make it bigger and stronger. We are all stronger when we come together and our donors provide that support. The Foundation’s support comes from businesses of all sizes, private donors, grants, and charitable organization across our seven service counties and beyond. From Crawford, Dent, Gasconade, and Maries to Phelps, Pulaski, and Texas counties, we all come together to help one another. You are here for us because you understand the need and realize that it takes every gift, no matter the size, when building a better us.
