Opioid epidemic-Nashville Hospitals to restrict supply?

Minty Fudge Brownie Bites – perfect for Valentine’s day A recent NPR story by Blake Farmer looked at how local hospitals are, belatedly, trying to curb and control the Opioids they have in the past dispensed like free candy to patients. “We really do have a lot of responsibility and culpability and this burden, and so we have to make sure we do whatever we can to stem this tide and turn the ship in the other direction,” said John Young, national medical director of cardiovascular services for LifePoint Hospitals, adding, “You don’t want to portray the fact that you’re not going to treat people appropriately.” The Brentwood-based hospital chain is now putting special emphasis on how it handles people coming into the ER looking for pain medicine.

The story adds: “The big hospital chains based in Nashville are accepting some of the blame for the country’s opioid crisis, which grows more deadly by the year. They admit they were going overboard with opioids to make people as pain-free as possible. So in an effort to be part of the cure, they’re issuing an uncomfortable warning to patients — you’re going to feel some pain”. Of equal interest might be the outpouring of complaints from readers commenting on the story, many of whom seemed to have managed without pain reduction drugs, but an equal number believing they are the only option. Serves: 16-18 Total time: 30 min.

Ingredients: • 2 cups raw walnuts • 4-5 tablespoons cocoa powder (I use 5) • 1½ cups Medjool dates pitted (make sure they are juicy!) • 1/2 teaspoon peppermint extract • 1 tablespoon coconut oil • 1/4 teaspoon sea salt (plus more for garnish) • 1 teaspoon vanilla extract

Directions: In a food processor, pulse together walnuts and 4 tablespoons cocoa powder until they form the consistency of crumbled chocolate cookies. Add remaining ingredients and pulse until mixture is sticky and holds together; this should only take 10-15 seconds. Turn dough out onto a baking sheet and roll into whatever size balls you like. Mine are approximately 1 ½” in diameter. Sprinkle with extra sea salt, if using. Freeze, gently covered with plastic wrap, for 20 minutes OR refrigerate for 1 hour before enjoying. Keep leftovers tightly sealed in the refrigerator up to 1 week.

Cold weather help for Nashville homeless During the recent very cold weather, The Nashville Office of Emergency Management “expended more than 480 man hours doing cold patrols to check on the homeless and other vulnerable people in the community”. OEM activates cold patrols when overnight lows dip below freezing. Volunteers from OEM go to areas where the homeless are known to camp or gather. The volunteers offer people a ride to a shelter or provide them with a blanket if they decide not to go to a shelter. One of the shelters, Room In The Inn, is run in partnership with Metro Social Services is open for any person

Nashville Hospitals and doctors swamped by Flu patients According to a Jan 12th article in the Knox Times, (www.knoxnews.com) the flu season has hit Tennessee hard this year-and still has a few weeks to run. “We’re up to our proverbial ears in influenza,” said Dr. William Schaffner, an infectious disease specialist with Vanderbilt University Medical Center in Nashville. “Over the last two weeks, our hospitalization numbers in Middle Tennessee have gone through the roof. Every hospital, every emergency room, every Minute Clinic and, I think, virtually every doctor’s office … is full of flu, and influenza-like illness”, reported the Knox Times. The elderly and children younger than five are at highest risk for hospitalization and death, along with people who have immune systems compromised or who have underlying serious chronic health conditions. The Tennessee Health Department offers guidelines on how to deal with-or try and avoid, getting the flu. “Staying home and calling your health care provider is usually the right move,” said TDH Chief Medical Officer David Reagan, MD, PhD. “Most people

Local Nashville author writes book for ‘grumpy kids!’ Local professional writer Judith Wolf Mandell has recently written Sammy’s Broken Leg (Oh, No!) and the Amazing Cast That Fixed It. It’s a whimsical book for glum, grumpy kids in a clunky cast...for children curious about life in a cast...and for kiddos who aren’t real happy about a prolonged recovery from any condition. The book was inspired by her granddaughter’s mostly miserable month in a chest-to-ankle cast to heal a broken femur when she was two years old. Mandell “vowed to create a book that’s entertaining, encouraging and educational”. So she did and it’s been a best seller. robably the number one complaint I hear from couples when they arrive in my office is “We just don’t know how to communicate.” As we go through our daily lives we observe all sorts of people and objects. We use our past experiences to filter and organize our observations and information we take in. Sometimes we encounter new information that changes the way we think about a person, group, or object. Taking our observations and selecting, organizing, and interpreting the information make up our perceptions. These perceptions are how we see the way things “are.” Our perceptions are not necessarily based on “facts” — but are based on our personal realities that are informed by past experience. Our perception of people and objects around us impacts how we communicate and act toward others1.

Perception Disconnection in action Suzie asks Bob if they can talk about the budget after work. Bob comes home and Suzie says bluntly: “We are out of money. There is too much debt on credit cards. You need to take out a 401K loan so we can get everything paid off.” Bob leaves the exchange thinking, “She is clueless. She doesn’t understand or care about where the debt came from – she thinks her way to solve this is the only way. I needed to get some clarity to some important financial questions but not now.” In this example, the Perception Disconnection is clear: Suzie simply intended to communicate urgency and actually called on Bob to execute an action to get the job done. Bob actually interpreted Suzie’s communication as hostile and blaming. Good communication didn’t occur and the chances of the proposed debt elimination strategy being implemented are now slim to none.

Communication frameworks and our perceptions Selecting and organizing incoming information happens very quickly — many times without much thought. But the meaning, or framework, that we give to our experiences is called a schemata. Schemata are like databases of stored, related information that we use to interpret new experiences. Over time we take in information and combine it to make more complex and meaningful thought. These schemata are the “filter” our mind uses to categorize our perceptions before, during, and after communication and interactions with others. This is much like an app on a smartphone — these schemata help us interpret our world. Just like apps must be regularly updated to improve their functioning we humans must also update and adapt our schemata with new experiences. These updated schemata will change our perceptions of situations. In couple’s communication I find that communication issues are not necessarily around the subject being communicated. Instead there is a disconnection between the information one person is trying to convey and what the other person is actually hearing. I call this a Perception Disconnection.

How to manage a Perception Disconnection When it comes to a Perception Disconnection clarity the key to management is to try and avoid the disconnection. It’s easy to make the other person “wrong”, saying something like, “What’s wrong with you? Why don’t you understand what I’m saying?”2 Approach your partner with openness and find out where the communication disconnection occurred. Asking something like “It seems like you are angry about what I said. What did you hear me say?” will help clarify what the other person’s reaction is based upon. Old patterns of thought create problems, especially when we try to interpret new information through old incompatible frameworks. Sometimes it is difficult to revise these patterns and doing so takes effort and usually involves some mistakes, disappointments, and frustrations. Dental Care should start before the first tooth appears

Even babies can develop tooth decay if good feeding habits aren’t practiced. Putting a baby to sleep with a bottle might be convenient, but can harm the baby’s teeth. When the sugars from juice or milk remain on a baby’s teeth for hours, they can eat away at the enamel, creating a condition known as bottle mouth. Pocked, pitted, or discolored front teeth are signs of bottle mouth. Kids with severe cases might develop cavities and need all of their front teeth pulled (permanent teeth will grow in later).

When should Kids see a Dentist?

The ADA recommends that children see a dentist by their first birthday. If a child seems to be at risk for cavities or other problems, the dentist may start applying topical fluoride even before all teeth come in (this also can be done in the pediatrician’s office). Fluoride hardens the tooth enamel, helping to ward off the most common childhood oral disease — dental cavities (also called dental caries).

Many parents have a tough time judging how much dental care their kids need. They know they want to prevent cavities, but they don’t always know the best way to do so. Here are some tips and guidelines courtesy of KidsHealth.org

How can you prevent Cavities?

Cavities happen when bacteria and food left on the teeth after eating are not brushed away. Acid collects on a tooth, softening its enamel until a hole — or cavity — forms. Here’s how to keep cavities away: • Start good oral habits early. Teach kids to brush at least twice a day with fluoride toothpaste and to floss regularly. • Get enough fluoride. Regular use of fluoride toughens the enamel, making it harder for acid to penetrate. Although many towns require tap water to be fluoridated, others don’t. If your water supply is not fluoridated or if your family uses purified water, ask your dentist for fluoride supplements. Most toothpastes contain fluoride but toothpaste alone will not fully protect a child’s teeth. Be careful, however, since too much fluoride can cause tooth discoloration. Check with your dentist before supplementing. • Limit or avoid certain foods. (see sidebar) • As your child’s permanent teeth grow in, the dentist can help prevent decay by applying a thin wash of resin (called a sealant) to the back teeth, where most chewing is done.

When should Kids start brushing their Teeth?

Good dental care begins before a baby’s first tooth appears. Just because you can’t see the teeth doesn’t mean they aren’t there. Teeth actually begin to form in the second trimester of pregnancy. At birth, your baby has 20 primary teeth, some of which are fully developed in the jaw. Here’s when and how to care for those little choppers: • Even before your baby starts teething, run a clean, damp washcloth over the gums to clear away harmful bacteria. • Once your baby gets teeth, brush them with an infant toothbrush. Use water and a tiny bit of fluoride toothpaste (about the size of a grain of rice). Use fluoride toothpaste that carries the American Dental Association’s (ADA) seal of acceptance. (If you are using baby toothpaste without the fluoride, keep it to the same amount because you still want to minimize any toothpaste that is swallowed.) • Once your baby’s teeth touch, you can begin flossing in between them. • Around age 2, your child should learn to spit while brushing. Avoid giving your child water to swish and spit because this might make swallowing toothpaste more likely. • Kids ages 3 and up should use only a pea-sized amount of fluoride toothpaste. • Always supervise kids younger than 6 while brushing, as they are more likely to swallow toothpaste.

Looking ahead

As kids grow, plan on routine dental checkups anywhere from once every 3 months to once a year, depending on your dentist’s recommendations. Keeping sugary foods in check, encouraging regular brushing and flossing, and working with your dentist will lead good dental health. The American Academy of Pediatrics says sports drinks can be helpful for young athletes engaged in prolonged, vigorous physical activities, but unnecessary in most cases. Before your next sip, check the label to make sure your drink of choice is low in sugar or drink water.

Nuts contain protein and minerals important for overall health. In addition, nuts that are low in carbohydrates don’t add to your risk of cavities. Why? The Basics of understanding Body Language Experts say some 90% of communication is non-verbal, so how you read someone’s body language (and how they read yours) is important in understanding people you meet for the first time. Makenzie Jones (pictured) provides some basic guidelines.

“W

hat did I say wrong?” you may ask yourself after an encounter with someone that did not go as expected. However, it may not have been anything that you said, but how you said it. The adage “actions speak louder than words” is key to remember in our daily interactions. While we can say whatever words we want to put out there, much of our non-verbal communication is innate and shows meaning we do not say. Patti Wood, author of Success Signals: A Guide to Reading Body Language, says “Almost 90% of what we say comes from nonverbal cues.” It is important to observe not only the body language of the people around you but your own as well. Men and women differ in both expressions and interpretations of body language, beginning with approaches to communication in general. Men communicate mostly to transmit information and solve problems while women aim to express feelings and achieve emotional intimacy. Therefore, women use more nonverbal communication than men

do and are better at interpreting others’ messages. It takes an average of three eye-gaze attempts by a woman before a man picks up on the signal. Men are less subtle than women, often using their hands, while women rely more heavily on facial expressions. Face-to-face encounters are preferred by men, but women are also comfortable interacting side-by-side. Proximity is viewed by men as a sign of aggression or confrontation and they associate touch with sexual intimacy, but women are more tolerant of others in their space and use touch to express friendship and sympathy. Because body language can often contradict the spoken word, sometimes it may be helpful to use perception checking to be sure what the other person means. This is first describing the behavior you noticed, providing two possible interpretations, then asking for clarification. For example, I could say to my mom, “You seem to be irritated since you came home. Did you have a frustrating day at work, or did 14

I do something? Can you help me understand what’s up so maybe I can help?” Sometimes it may be useful to tune out someone’s words and look solely at their body language, especially the eyes, which have been called “the windows to the soul.” Eye contact shows confidence, interest, and attention to the person who is speaking. The eyebrows and lids can show a plethora of expression such as fear, frustration, sadness, or excitement. First impressions can make or break many relationships and job opportunities with body language as a major factor. Posture, tone, and eye contact give insight into a person’s confidence, attitude, and attention to others. Standing up straight, open chest, feet facing forward, eye contact, and a firm handshake are powerful to making a positive impression. These all show confidence, attentiveness, and are things people find desirable in someone they meet. On the other hand, slumping, crossing your arms, and having a weak handshake express insecurity and can indicate to another that you are not willing or able to meet their wants or needs. Synchrony in body language shows that the two people are on the same page. Tone and pitch of voice and rate of speech are also considered key factors in nonverbal communication, as they are not actual words, but give insight beyond them. How many times have you heard from your child, “Yeah, Mom, that’s cool,” with a sigh and lack of interest? Sarcasm runs rampant in our society. A high-pitched, fast-paced list of things to do can show someone’s anxiety even when they tell you that they “have it all under control.” “It’s fine, I’m fine,” has become a cliché when things are obviously not okay. Here are some body language actions that you may need to know in decoding others’ messages as well as when you are observing your own: Arms crossed on chest

When you live in this world, there are various types of complex interactions happening. As your field of play increases, the complexity of interaction also goes on increasing. person is totally bereft of understanding. With your understanding you can create situations where the other person would be able to understand you better. If you are expecting the other to understand and comply with you all the time while you don’t understand the limitations, possibilities, needs and capabilities of that person, conflict is all that will happen. It is bound to happen. If you move your understanding beyond theirs, their understanding also becomes a part of your understanding. You will be able to embrace their limitations and capabilities. In everyone, there are some positive things and some negative things. If you embrace all this in your understanding, you can make the relationship the way you want it. If you leave it to their understanding, it will become accidental. If they are very magnanimous, things will happen well for you. If not, the relationship will break up. All I am asking is: do you want to be the one who decides what happens to your life? Whether 22

they are close relationships, professional, political, global or whatever, don’t you want to be the person who decides what happens in your life? If you do, you better include everything and everyone into your understanding. You should enhance your understanding to such a point that you can look beyond people’s madness also. What to do when injured by a drunk driver By David E. High, Attorney-At-Law

Our regular legal advice contributor, David High, of High Law Office, PLLC offers some advice if you have an accident with, and are injured by, a drunk driver.

U

nfortunately, thousands of people are arrested every year in our community for driving under the influence. It is a crime in Tennessee to drive under the influence of alcohol or drugs, (even legally prescribed drugs). It is also against the law to drive in Tennessee with a blood alcohol level of .08% or higher. Unfortunately, many personal injury accidents result from individuals driving under the influence of alcohol and/or drugs. In addition to the State of Tennessee prosecuting and punishing the offenders in Criminal Court, if they cause injuries to others, they are liable for civil damages in tort. Liability can include both compensatory and punitive damages. If you, or a family member, are injured due to the negligence of a drunk driver, it is important that you promptly hire counsel to pursue your civil claims. While the District Attorney will prosecute the criminal charges, you need your

own attorney in order to prosecute your claim for civil damages. The liability insurance carrier for the owner and/or operator of the offending vehicle will be liable for your personal injury damages and your property damages. There also can be some liability for punitive damages for “reckless conduct” for driving a vehicle while under the influence of alcohol or drugs, or knowingly allowing someone to drive your vehicle when under the influence of alcohol or drugs. The owner of a vehicle that knowingly allows it to be operated by an intoxicated or impaired driver is potentially as guilty of driving under the influence as the driver. Both the vehicle owner and the driver can be prosecuted criminally and sued civilly for money damages. Unfortunately, many of the drunk driver offenders are very irresponsible people who have no liability insurance or are underinsured. If you are injured by a drunk driver and you have an 26

auto insurance policy in Tennessee, it generally contains uninsured or underinsured motorist coverage. Thus, if you are injured by an uninsured drunk driver and uninsured auto owner, then you have a right to file a claim with your uninsured motorist coverage and your insurance carrier will be liable for any judgment for compensatory damages against these uninsured entities up to the amount of your coverage. It is very important that you promptly notify your insurance carrier of the accident and that you timely file suit. If the drunk driver has insufficient coverage, additional underinsured coverage is also likely available under your insurance policy. An experienced attorney can obtain these benefits for you. If you, or a loved one, are injured due to the negligence of a drunk driver or an individual who allows their vehicle to be operated by a person under the influence of alcohol or drugs, it is very important to promptly hire an attorney who is experienced in prosecuting and handling these types of cases. avender oil, in my opinion, is one that I truly wouldn’t be without due to its many beneficial uses. Its scientific name is Lavandula and the essential oil is steam distilled from the buds. Do not get confused with Spike Lavender which has more camphor in it. I want to list the top 5 uses for Lavender essential oil (yes there are many more uses, but these are my favorite). • Calming. Not only is it useful to help relax your mind before bed but it’s also useful for those who suffer mild anxiety. There have

been several studies where lavender has been used to help reduce depression as well as PTSD symptoms. • Healing. It can help to relieve burns, cuts, and scrapes due to the antimicrobial compound and best if used with a carrier oil. • Beauty. It’s been used for many years for help with skin and even acne. In combination with Rosemary essential oil it can increase the growth of hair for those who deal with thinning hair. • Headaches. Just inhaling it for about 10-15 min can reduce headache pain and possibly even migraines. • Pain. Whether it’s a stomach ache, muscle pain, or even PMS, massaging in the area of pain calms that part of the body. It’s even being studied for the pain, depression, and nausea for those who are dealing with cancer.

OK I know I said five but here’s one I can’t leave out: Perfumes, air fresheners, and other items that are “fragrant” contain lots of chemicals that are not good for us to breathe in. Just add a few drops to vodka, or witch hazel and spritz around the house, or even put it in a diffuser. Lavender is a pretty safe oil but it does come with precautions. Do not use if you are pregnant because it is a natural relaxer, many pregnant women have used but I must URGE you to always check with your doctor first. For the most part, it is safe to use on children; always dilute and use only 1-2 drops. With babies use up to two tablespoons of a carrier oil and only 1 drop of oil, put on bottoms of feet.

Q. Are you board certified? In the United States, there are 24 medical specialty boards. To become certified, physicians must spend several years after their residency receiving inpractice training, as well as pass a written and oral exam.

“SMILE” Vision Surgery Debuted Statewide in Nashville New and Minimally-invasive Surgery for Myopia (Nearsightedness) is First Major Advance in LASIK Technology in 25 Years, Reducing Dependence on Glasses and Contacts

The first major advance in LASIK technology in 25 years, the SMILE procedure, was performed in Nashville recently at Wang Vision 3D Cataract & LASIK Center by its director, internationally renowned ophthalmologist Dr. Ming Wang, Harvard & MIT (MD, magna cum laude); PhD (laser physics).
