B.E. Smith Family Center Opening Spring 2019 Thanks to a lead gift from Doug and Nan Smith, Kansas City will soon be home to a world-class facility that will serve as a welcoming, safe place for children of many diverse abilities.

Currently under construction on the AdventHealth Shawnee Mission campus, the B.E. Smith Family Center will house the renowned Early Learning and Britain Development programs, with the primary goal of promoting understanding and inclusion of children with special needs. Opening in spring 2019, the B.E. Smith Family Center will feature an integrated harness system in multiple classrooms and therapy areas, which is the first of its kind in the world. Holmes and Watson. And yes, diabetes and heart disease.

It’s true. The risk of heart disease death and stroke is twice as high in people with diabetes. And right now, an estimated 29 million Americans have this disease. Even worse, another 86 million adults have prediabetes, a condition of elevated blood sugar just below the threshold for diabetes.

First things first – what exactly is diabetes anyway?

“Diabetes (technically, diabetes mellitus) is not just one disease but a conjunction of diseases that are related to abnormal metabolism of sugar in the blood,” said Andrew Waters, MD, Cardiologist, AdventHealth Shawnee Mission. “There are two primary types of diabetes: Type 1, caused by an insulin deficiency, and Type 2, a condition characterized by insulin resistance, meaning cells fail to respond to insulin.”

Dealing with diabetes is already a challenge. What’s even more serious is knowing it can increase the risk of heart disease.

What’s the biggest heart health risk for people with diabetes? It’s called atherosclerotic heart disease, also known as hardening of the arteries.

“This is the formation of plaque within the arteries, which can lead to obstruction of blood flow and possibly heart attacks,” said Waters.

“Heart failure is also significantly more prevalent in patients with diabetes.”

Fortunately, if you already have diabetes, there are several things you can do to help prevent heart disease, including: kkKeep your blood sugar as normal as possible. kkControl your blood pressure, with medication if necessary. The target for people with diabetes is under 130/80. kkGet your cholesterol numbers under control. You may need to take medication to do this. kkLose pounds to lower your risk if you are overweight. kkAsk your doctor if you should take an aspirin a day. kkExercise regularly. kkEat a heart-healthy diet such as the Mediterranean diet or DASH diet. kkQuit smoking. kkWork to reduce daily stress.

“Basically, it means sticking to your medical regimen, controlling your diet and managing your risk factors both for diabetes and for atherosclerotic heart disease,” said Waters.

The good news is type 2 diabetes is largely preventable.

While the majority of new cases of type 2 diabetes occur between the ages of 45 and 65, there’s plenty you can do right now to reduce your risk of diabetes in the future.

Of course, a healthy lifestyle is important for everyone. “Diet is such an important component of having a healthy lifestyle,” said Waters.

“Dietary choices such as the use of sugar-sweetened beverages, and the use of the Western diet has been associated with higher incidence of type 2 diabetes.”

On the other hand, following a Mediterranean style diet, and consuming nuts and whole grains can measurably reduce the risk for type 2 diabetes.

“A sedentary lifestyle is also a major risk,” said Waters. “Being sedentary increases the risk of obesity, and can increase your risk of type 2 diabetes overall.”

Tips to help reduce your risk of diabetes: Lose excess body fat.

Being overweight is a big risk factor for diabetes. kkFollow a plant-based, low-calorie diet.

Eating a variety of fruits and vegetables reduces diabetes risk.

Foods to avoid are those rich in trans fats, saturated fat and sugar. kkDrink water.

Cut sugar-sweetened beverages out of your diet and the risk of both obesity and diabetes falls. kkMove your body.

A simple regimen of brisk walking can do the trick. kkStress less.

The stress response triggers the release of several hormones that increase blood sugar.

Physical activity and social support help relieve stress, along with mindfulness meditation. kkSleep well.

Chronic sleep deprivation and poor quality sleep increase the risk for diabetes and obesity. kkKeep medical appointments.

The warning signs of type 2 diabetes are less dramatic than those of type 1 diabetes. That’s why it’s important to see your doctor regularly. kk

That’s why it’s so important to put that same effort into finding the right OB/GYN. “Building a relationship with your OB/GYN is crucial,” said Meghan Vaglio, MD, MPH, OB/GYN. “She or he will be your prime source of information, reassurance and guidance throughout your reproductive years.”

The fact is, whether you’re just starting out on your pregnancy journey, or are simply looking for the right OB/GYN for you, this will be one of your most meaningful relationships for years to come.

B/GYN “There are many ways to find an OB/GYN – talking to friends and families, doing research on your own or talking with your primary care providers,” said Vaglio.

You should call the offices of two or three doctors who interest you. The staff can tell you about the doctor's education, training and board certification, as well as provide information about insurance plans, lab procedures and hospital affiliation.

Because choosing your OB/GYN is such a personal choice, nothing will substitute for a faceto-face meeting. “During these meetings you can assess whether you feel like the provider will also be a good personality match for you and your family,” said Vaglio.

You will find that comfort and rapport are just as important as your health care provider's experience or knowledge. After all, a doctor needs to be aware of all of your health issues — even ones that are sensitive in nature. If you

don't feel like you can talk about all of your health concerns, you may need to find another doctor.

Having a Primary Care Physician Is Still A Primary Need.

Pay attention to the staff working in the OB/GYN’s office.

Your Primary Care Physician (PCP) plays a separate – but vital – role from your OB/GYN. “Your PCP will help organize your comprehensive care throughout your life, and at times will assist during your pregnancy with managing medications, chronic conditions or referrals,” said Vaglio.

After your pregnancy, your PCP can work with your OB/GYN and also assist in routine screening exams that can include: kkBreast exam (for women) kkDermatological exam kkFamily history kkHead and neck exam kkHeart exam kkLung exam kkNeurological exam kkReview of overall appearance kkVital signs assessment (blood pressure, temperature, heart rate, respiration rate)

“After all, you’ll be seeing and working with them for years to come,” said Vaglio. “If at any time you have concerns, it’s important to bring them up early, so you can get the most from your relationship with your OB/GYN.” Be sure to ask providers about their call coverage and who you would contact if you have a question or an emergency.

It is important to know how providers work together to provide you the best care.

Remember, your obstetric care does not need to be separate from your gynecology care.

“One of the best parts of our profession is that we can provide care for women throughout their life, from puberty till after menopause,” said Vaglio.

There are lots of OB/GYNs out there. Make sure you find one who’s best for you.

Your PCP also has expertise in helping you manage conditions like high blood pressure, diabetes, asthma and high cholesterol, and will often coordinate care among the specialists you may need to see.

Your PCP and your OB/ GYN should be partners with you in your health journey. After all, the better informed you are about your health status, the more motivated you will be to lead a positive and healthy lifestyle.
