Mindfulness Meditation: Living Life With a Lot Less Stress It‘s easy to get caught up in our thoughts — whether it’s worrying about things that need to be done, past events, future possibilities or just a nagging feeling that we’ve forgotten something important. It’s not always easy to deal with and accept our thoughts. But learning mindfulness can be a big help.

Mindfulness meditation is a mental training practice that teaches us to slow down racing thoughts, let go of negativity and calm both mind and body.

Techniques can vary, but mindfulness meditation involves a breathing practice and awareness of body and mind. No props or preparation are required. All you need is a comfortable place to sit, a few minutes of free time and a judgment-free mindset.

SETTLE IN Find a quiet space. Using a cushion or chair.

Sit up straight but not stiffly. Allow your head and shoulders to rest comfortably. Place your hands on the tops of your legs with upper arms at your side.

STAY FOCUSED Thoughts will try to pull your attention away from the breath.

Notice them, but don‘t follow them.

Gently return your focus to your breath. Some people count their breaths as a way to stay focused.

BREATHE Close your eyes, take a deep breath and relax. Feel the fall and rise of your chest and the expansion and contraction of your belly. With each breath, notice the coolness as it enters and the warmth as it exits.

Don‘t control the breath but follow its natural flow.

TAKE 10 A daily practice will provide the most benefits. It can be 10 minutes per day. But 20 minutes twice a day is often recommended for maximum benefit.

Easy Eggroll in a Bowl Ingredients: 1 tablespoon avocado oil 1 red or yellow onion, chopped 1 pound lean ground beef, ground

chicken or ground turkey ½ teaspoon sea salt ¾ cup chopped shiitake

or cremini mushrooms ¹⁄₃ cup reduced sodium

tamari soy sauce ½ head of green cabbage, shredded

or 1 (16-ounce) bag coleslaw mix 2 carrots, peeled and shredded 1 stalk celery, chopped 2 teaspoons toasted sesame oil* Salt and pepper to taste Garnish: 2 to 3 green onions, thinly sliced

Directions: 1 Heat oil in a large skillet and add the onion, ground meat and salt.

Sauté over medium heat for 3 to 4 minutes, breaking up the meat with a wooden spatula. Next, add the mushrooms and continue to cook until meat is browned, and the mushrooms have softened.

*Cook’s note: For best flavor and results, make sure you use toasted sesame oil, not regular sesame oil.

4 Serve over a bowl of brown rice

2 Add the soy sauce, cabbage, carrots and celery. Stir to combine and sauté for 4 to 5 minutes allowing the vegetables to cook.

3 Remove from heat. Add in the sesame oil and some salt and pepper to taste. Adjust seasonings, as desired. Garnish with sliced green onions. and enjoy!

C E RV I C A L C A N C E R? RECENTLY, THE AMERICAN CANCER SOCIETY (ACS) UPDATED ITS GUIDELINES FOR CERVICAL CANCER SCREENING FOR WOMEN AGED 25 TO 65 YEARS TO GET A HUMAN PAPILLOMA VIRUS (HPV) TEST.

Brenda Shoup, MD Gynecologic Oncology AdventHealth Cancer Center Brenda Shoup, MD, is Board-certified in Gynecologic Oncology and Obstetrics and Gynecology. She received her medical training at The University of Kansas School of Medicine, and completed her residency at University of Florida-Shands Hospital and fellowship in Gynecologic Oncology from The University of South Florida Moffitt Cancer Center.

She is a member of the Society of Gynecologic Oncologists and the American College of Obstetrics and Gynecology. Shoup performs minimally invasive procedures with the da Vinci Surgical System and has special interest in treating pelvic masses and all gynecologic cancers, as well as patients with genetic predispositions to cancer.

What’s the difference between an HPV test, a Pap test and an HPV/Pap cotest?

3 A traditional pap smear is a collection of cells from the surface of the cervix that is fixed to a slide and interpreted by a cytopathology expert. They can see precancerous changes and the degree to which the cells are abnormal. This is purely a screening test and not diagnostic of cancer or precancer. Further testing – usually a biopsy – is required for diagnosis.

et’s start with some basics.

The cervix is part of the path that babies pass through during birth. Cancer that starts in the cells of the cervix is called “cervical cancer.”

Almost all cervical cancers are caused by an HPV infection.

Many people will never know they have HPV because the body can usually fight off the infection before any symptoms occur.

But high-risk types of HPV can cause serious precancers and cancers of the cervix.

The goal of cervical cancer screening is to find pre-cancers that are likely to progress to cancer and to remove or treat them before they do. Screening can also find cervical cancer at an early stage, when it is easier to treat.

“Cervical cancer screening is very effective. It has decreased the rates of cervical cancer by half between 1975 and 2014 due to widespread uptake of screening,” said Brenda Shoup, MD, Gynecologic Oncology, AdventHealth Cancer Center.

And now, the ACS has recommended getting the HPV test in place of receiving a Pap test or HPV/Pap cotest.* “HPV testing are generally more accurate and reliable than Pap smears – the mainstay of cervical cancer screening for decades. Pap smears may miss some precancerous lesions. Pap smears may also detect things unrelated to cancer and cause unnecessary further testing,” said Shoup.

The new guidelines also recommend a new age to begin screening: 25 instead of 21 The ACS has decided that the benefits of screening before age 25 do not outweigh the possible harms. This is a change that is directly related to the now widespread use of the HPV vaccine, which helps prevent six types of cancer, including cervical cancer.

Because most women in their early 20s have received the HPV vaccine, there has been a significant drop in HPV infections and cervical precancer in this age group.

Screening people in this age group often leads to unnecessary treatment, which can have negative side effects. That’s why the ACS recommends starting screening later – at age 25.

Of course, the HPV test doesn’t tell you whether you have cancer. Instead, the test detects the presence of HPV in your system.

What happens when someone receives an abnormal cervical screening test result?

Knowing whether you have a type of HPV that increases your cervical cancer risk means that you and your doctor can better decide on the next steps in your health care. Those steps might include follow-up monitoring, further testing or treatment of abnormal cells.

“Currently, this usually leads to

a colposcopy, which is an exam using a microscope to evaluate the cervix closely, and possibly a biopsy. After an abnormal cervical screening test, the patient may be at a prolonged elevated risk of developing cervical cancer and may not be able to return to screening every five years,” said Shoup.

Please keep in mind, some women will need exams or evaluations on a more frequent basis than just every five years.

Don’t wait to consult with a doctor if you have problems between screenings. See a doctor right away if you have symptoms of cervical cancer – don’t wait to be screened. These symptoms include unusual bleeding or discharge from the vagina or pain during sex. *Please note: While it is important to understand these new ACS screening guidelines, at this time, the Society of Gynecologic Oncologists, the United States Preventative Services Task Force (USPST), the American College of Obstetricians and Gynecologists and the practicing Gynecologic Oncologists at AdventHealth are not ready to fully commit to adopting them.

ENDOMETRIOSIS & YOUR HEART HEALTH How to help make sure things don’t go from bad to worse According to research, women who have endometriosis may have a greater longterm risk of developing cardiac disease than women who do not. According to experts, women with endometriosis were 62 percent more likely to have a heart attack, develop chest pain or need heart procedures to open blocked arteries.

However, women 40 or younger with endometriosis had more than three times greater risk for these complications than women without endometriosis.

ver the course of her lifetime, a woman will encounter a wide range of life and health events. During her reproductive years, those events can include pregnancy, menopause and the effects of endometriosis, a condition related to the menstrual cycle.

“Endometriosis is a disease where the inner lining of the uterus, the endometrium, is present in other places within the body. It is commonly located within the pelvis but can be present anywhere. Each month during menstruation, these endometrial implants grow and become inflamed, causing pain and scarring,” said Emily Clark, MD, OB/Gyn, AdventHealth Shawnee Mission.

Because most women with endometriosis have no symptoms, there isn’t an exact number, but about 10 percent of reproductiveaged women are thought to have endometriosis.

Symptoms can range from no symptoms at all to extremely heavy and painful periods, bowel movements and intercourse as well as infertility.

In addition to being incredibly painful, endometriosis is now thought to be linked to something else – an increased risk of heart

disease. Endometriosis can affect the heart due to the changes in estrogen levels that occur naturally and from endometriosis treatment.

“Endometriosis causes inflammation and is associated with high cholesterol levels, so it may be a risk factor for heart disease in and of itself. The treatment for endometriosis often involves NSAIDs (like ibuprofen or naproxen) and/or hormonal therapy (like birth control pills). If these and other treatments don’t work, removal of the ovaries (an oophorectomy) may be the last resort. This effectively causes surgical menopause,” said Clark.

The hormonal changes that occur during menopause can bring increased cardiovascular risk in the form of higher blood pressure and cholesterol levels.

If cardiovascular disease runs strongly in the family, it’s also important to see a cardiologist – in addition to your OB/Gyn – to further assess the likelihood of having cardiovascular disease and to optimize treatment.

“The increase in heart disease seen in women with endometriosis is most dramatic in women less than age 40, although endometriosis can occur at any reproductive age. Other risk factors for endometriosis include never having been pregnant or had children, having long periods lasting more than seven days, having menstrual cycles lasting less than 27 days, or having a family member with endometriosis,” said Clark.

Of course, most adults already have

one or more risk factors for heart disease, like high blood pressure, diabetes and obesity, even at a younger age. Since endometriosis may further increase cardiovascular risk in women, addressing risk factors is even more important for preventing heart disease and lifethreatening heart events.

Fortunately, there are several ways that women with endometriosis can decrease their risk. The most important thing is to stay as metabolically healthy as possible.

For example, just before menopause, women should understand that they’re about to start experiencing weight gain and a shift in their body fat distribution as they begin to carry more abdominal fat, which is considered dangerous in terms of heart disease risk.

“Trying to stay ‘heart healthy’ includes not smoking, exercising regularly, eating a healthy diet, controlling your blood pressure and maintaining a healthy weight. This is also very important in women approaching menopause,” said Clark.

Intermittent Fasting Can Provide Lasting Health Benefits Intermittent fasting (IF) is a catch-all term for many different eating patterns. Most popular involve an overnight fast, only consuming caloric foods between certain daytime hours — with 8, 10 or 12 hour eating windows being the most common.

Research shows that IF can promote some important health benefits including weight loss, reduced inflammation and improved blood glucose and insulin levels.

“I’m an advocate of intermittent fasting for my patients who would benefit from weight reduction,” said Megan Schlick, ND, AdventHealth Center for Whole Health. “I have them hold off on eating the first meal until after 10 am, and nothing but water after dinner. Typically, patients are not snacking on healthful items in the evening, so it’s a nice black-and-white boundary that many find helpful.”

“But I have found some challenges.

Fasting patients must be intentional about still getting in the daily 5+ servings of veggies and fruit, but in a tighter timeframe. All patients should talk with their primary care doctor before beginning an intermittent fasting routine,” said Schlick.

INTERMITTENT FASTING

HEALTH BENEFITS WEIGHT LOSS During periods of fasting, the body shifts to using stored body fat as an energy source. This can result in fat loss and a lower body mass index (BMI). Additionally, intermittent fasting reduces total caloric intake on fasted and non-fasted days, resulting in weight loss.

IMPROVED METABOLIC HEALTH Insulin resistance, a hallmark trait of type 2 diabetes, can improve in individuals following intermittent fasting routines. And metabolic syndrome – including high blood pressure, elevated blood sugar, large waist circumference and abnormal blood cholesterol levels – may be prevented or reversed.

REDUCED CARDIOVASCULAR DISEASE RISK Research suggests intermittent fasting improves blood pressure, triglyceride, LDL and total cholesterol levels. Improved cardiovascular risk factors associated with intermittent fasting appear to be related to weight loss and lowered insulin levels.

DECREASED INFLAMMATION Intermittent fasting may result in a reduced risk of various inflammatory conditions, including diabetes, cancer and heart disease. Intermittent fasting may also reduce proinflammatory factors that can lead to atherosclerosis.
