hanks to recent improvements in prevention, early detection and treatment, more than a million people in the U.S. now count themselves as survivors of colon or rectum cancer (also called colorectal cancer).

One of the most recent survivors of rectal cancer is Lawrence resident, Geri Lamer, whose primary care doctor diagnosed her with an abscess near her rectum. He sent Lamer to a surgeon to have it removed, telling her that if it returned, a biopsy would be needed. Sure enough, the abscess came back, and the biopsy indicated that it was a tumor.

That finding meant Lamer would need to undergo additional testing, including a CT (computed tomography), ultrasound and a colonoscopy.

These tests helped her medical team understand more precisely the size, location and most importantly, the extent of the tumor.

After meeting with several surgeons, Lamer decided to move forward with Dr. Cravens and Shawnee Mission Health (SMH) for her surgical care.

“Not only is Dr. Cravens very professional, but he was also able to explain every detail to me and my family without talking over our heads with medical jargon,” said Lamer. “He cared enough to take all the time necessary to make absolutely sure we understood the situation and the procedure.”

Cravens, a Fulton, MO native, received his medical degree from the University of Missouri and did a fellowship in colorectal surgery at the Colon & Rectal Clinic of Orlando. He began working as a Colon and Rectal Surgeon with SMH in 2013.

Cravens works closely with the oncologists at the Shawnee Mission Cancer Center to discuss each patient’s treatment plan. “We have a multi-disciplinary team — surgery, chemotherapy, radiology — that meets weekly to discuss all our patient plans,” said Cravens.

It was at one of these meetings that a treatment plan was developed for Lamer, which began with six weeks of preoperative chemotherapy and radiation treatments, followed by surgery.

“The cancer treatments shrunk the tumor considerably,” said Cravens, “which made the surgery less invasive, and shortened Geri’s recovery.”

Even before the surgery, Lamer appreciated the treatment she received at SMH. “I was really impressed with the pre-registration process at Shawnee Mission Health,” said Lamer. “They were very welcoming, and walked me through everything, so on the day of my surgery, all I had to do was show up and get my patient ID bracelets. It helped eliminate a lot of nervousness, and

made me a lot more confident going in.”

Cravens performs 12-15 colorectal surgeries each month, using the da Vinci® robotic technology in most cases. Robotic surgery allows for a better chance of completing the procedure in a minimally invasive fashion, resulting in smaller incisions, shorter recovery time and a shorter stay in the hospital.

“In my opinion, the overall effectiveness of the surgery is better with robotics because the visualization is better,” said Cravens. “It’s as if I’m looking at everything through a magnifying glass, so I can be much more precise, which means fewer complications from surgery.”

Lamer spent five days in the hospital following her surgery in June, and then was able to return home, and back to her work at the University of Kansas.

“Dr. Cravens was able to get clean edges all around the tumor, and it had not spread to any of my lymph nodes,” said Lamer. “The surgery was as successful as I could have hoped for!”

What Are The Risk Factors For Colorectal Cancer?

Colorectal cancer usually develops over several years, first growing as a precancerous growth called a polyp.

The following are the most common risk factors for developing colorectal cancer: » Family history of colon or rectal cancer » Personal or family history of polyps or colorectal cancer » Inflammatory bowel disease such as Crohn’s or ulcerative colitis » Smoking » Obesity and sedentary lifestyle » Lack of screening colonoscopy One of the most important risk factors is the lack of screening for rectal cancer. Routine cancer screening of the colon and rectum starting at age 50 for anyone with average risk is the best way to prevent colorectal cancer or to find it in its early stages. Talk to your health care provider about when you should begin screening.

ational Breast Cancer Awareness Month began in 1985, and now, 30 years later, it has become a major event across the country each October.

That’s why most physicians, like Anderson, recommend annual breast screening exams, such as mammograms, starting at age 40, in order to find breast cancers when they are still small and before they start to spread or cause symptoms.

However, a group of independent health experts called the U.S. Preventive Services Task Force (USPSTF), has created some controversy by recommending that routine screenings of average-risk women should not begin until age 50. The USPSTF also recommends that women should get screenings mammograms every two years, instead of annually.

On the other hand, critics of the USPSTF recommendations point out that these

Thankfully, the effort has helped millions of women lead longer, healthier lives; but it is estimated that in 2015, nearly a quarter million new cases of invasive breast cancer will be diagnosed in women, and more than 40,000 women will die from breast cancer.

That’s why Shawnee Mission Health wants to make sure, more than ever, that area women understand the importance of the early detection of breast cancer.

The numbers are clear. Women who undergo breast cancer screening mammograms have demonstrated significantly reduced deaths from the disease.

“In fact, early detection of breast cancer can lower the cancer mortality rate by 20-40 percent,” said Andy Anderson, MD, Shawnee Mission Health radiologist with fellowship training in breast imaging.

“In the past, self-exams were recommended, but since most women are not medically trained, there were problems,” said Anderson. “Some tumors were not detected and some non-cancerous lumps were mistaken to be tumors, which can cause a great deal of worry and fear.”

guidelines seem more focused on cutting costs than on saving women’s lives.

“Of course, mammography isn’t perfect,” said Anderson, “but it has undeniably saved many, many lives – including women aged 40-49.”

“The American Cancer Society will continue to recommend annual mammography screening to all healthy women starting at age 40,” said Anderson. “After all, the earlier cancer is caught, the more years of life are saved.”

The Breast Center's multidisciplinary team approach includes experienced techs and nurse navigators who are available to answer questions and help navigate women through the health care system.

The Breast Center at Shawnee Mission Medical Center is dedicated to exceptional breast care from the time of a breast cancer diagnosis through survivorship. And in the next few weeks, with the installation of 3D mammography technology, our ability to detect breast cancer will increase significantly.

“This technology is an important addition for the women we serve throughout the entire metro community,” said Anderson.

“3D mammography can raise breast cancer detection rates by more than 40 percent.”

Conventional mammography takes images of the breasts from two angles. In contrast, the 3D scanner moves in an arc over the breasts, taking images from various angles. The goal is to improve doctors' ability to spot small tumors and reduce false scares.

"We found 3D mammography really does help doctors find more invasive breast cancers, while cutting down on call backs," said Anderson.

A "call back" happens when a mammogram picks up something suspicious, and the doctor wants to do additional imaging or a biopsy. For most women, it turns out to be nothing; according to the American Cancer Society, fewer than 10 percent of women called back for more testing are diagnosed with breast cancer.

“Additional tests can cause a great deal of anxiety for women, so reducing call backs is a very big deal,” said Anderson. “And with 3D mammography, call backs can be reduced by 15 percent or more.”

What is 3D Mammography?

The Shawnee Mission Health’s Breast Center is dedicated to exceptional breast care from the time of a breast cancer diagnosis through survivorship. As part of our efforts to provide the very best diagnostic services possible, we now offer 3D mammography, also referred to as tomosynthesis (toh-moh-SIN-thah-sis), which significantly improves our ability to detect cancer early — when the chances for a cure are highest.

The point is to improve your doctor’s ability to identify small tumors. In fact, 3D mammography, used along with standard digital mammograms, raises breast cancer detection rates by more than 40 percent.

In addition, this advanced technology will reduce false scares and result in fewer “call backs” for additional imaging. Currently, about 10 percent of the 40 million women who get screening mammograms every year receive a “call back,” which can create a lot of anxiety and sometimes require additional types of tests.

Who could benefit most from 3D mammography?

Women with: » prior mammograms that had inconclusive results » dense breast tissue » a history of call backs for repeat breast scans » past biopsies for suspicious masses or lesions » breast cancer (now or previously), or with

a family history of cancer 3D mammography has been available in the U.S. since 2011. It was originally approved by the Food and Drug Administration to be used along with standard, twodimensional digital mammograms. But in 2014, the agency approved a 3D system that can be used alone.

“Shawnee Mission Health and Centra Care are both part of the Adventist Health System, and we’ve come together for the first time to bring enhanced urgent care and occupational medicine services to the Kansas City community,” said Derek Ortner, Administrative Director Network Development for Shawnee Mission Health (SMH).

SMH is one of the area’s longest providers of urgent care services, having served the community for more than 30 years. Now, Centra Care will build upon the same great care SMH has always provided from Board-certified doctors, with the added convenience of being able to make no-wait reservations online, as well as have a dedicated children’s waiting area and pediatric exam room.

“Our providers offer treatment to both adults and children for a variety of non life-threatening illnesses and injuries, including colds, flu, sore throats, ear aches, stomach aches, cuts, broken bones and

sprains,” said Ortner. “Centra Care is also able to fill most prescriptions for patients on site, so families don’t have to make one more stop when someone isn’t feeling well.”

In addition to caring for individuals of all ages when their regular doctor isn’t available, Centra Care also offers comprehensive business health and workers’ compensation services, including drug testing, health risk appraisals, occupational health examinations, on-site lab screenings, Department of Transportation examinations and other employment physicals.
