A MESSAGE FROM SUNNYVALE RECREATION Our staff continues to work hard providing a variety of safe classes and activities to keep you and your family healthy and engaged! We are thrilled to be offering our full schedule of recreation classes. Our Sunnyvale Library and Recreation Services team is coming back strong with new, unique recreation classes to challenge and excite our community. We’ve had an incredible summer, bringing our Sunnyvale youth the Summer camps they’ve been craving. As a county, we are still making progress with vaccinations to mitigate the impact of COVID-19, so we encourage you to continue diligently following the strictest County/State Health Orders by wearing a mask even if you are vaccinated. It’s back to school time and we have something for everyone. We’re thrilled to welcome back Super Safari Preschool and recreation classes for preschoolers. Our pottery studio is spinning back to life with all the classes you’ve been waiting to get your hands on. The brand-new Washington Community Swim Center opens Aug. 2, 2021 for swim lessons and Splash n’ Dash Camp, so you can join us in finishing off the summer with a splash. Dedication and ribbon cutting scheduled for Aug. 6, 2021 will be livestreamed on Facebook. Find details on City of Sunnyvale or Sunnyvale Rec Facebook channels. As always, we have big plans for the future! We anticipate the opening of the brand-new Fair Oaks Park, including Magical Bridge Inclusive Playground, February 2022. A new design concept has been approved for Plaza Del Sol to better meet the needs of Sunnyvale residents which features a play area, spray misters, an enhanced labyrinth and walls with seating planters. The Lakewood Branch Library and Learning Center conceptual design is finished and a final design is being developed. There’s something for everyone in our safe facilities and programs, so come join us and ease back into active life! What We’ve Done

Health Screening Questions

As always, the City will follow both the State and County Health Orders, and where there is a conflict between the two, the stricter rules must be followed. Check Sunnyvale.ca.gov/virus for possible activity changes.

Please answer the following questions, before arriving to every class, camp day or program.

Masks – When to Wear Them? Masks are REQUIRED FOR ALL, regardless of vaccination status in some specific cases: • Inside all City facilities and park buildings • Outdoor Youth Programs • Aquatic facilities Masks may be REMOVED when participants are: • Inside the swimming pool • Actively using the showers inside locker rooms • Actively eating/drinking No Valve Masks No Gaiter Masks

What Is Considered a Close Contact? Close contact is someone who was 0 to 6 feet of an infected person for at least 15 minutes or more than within a 24 hour period regardless of whether both parties were wearing masks. The ≥15 minutes could be continuous contact with the positive case or someone who had repeated shortduration interactions with the infected person within the 24 hour period.

Close Contacts and COVID-19 Positive Test Notifications: If there is a reported close contact or positive COVID-19 antigen/PCR test, staff will notify participants via email and may give additional instructions on isolation/quarantine as directed by the Santa Clara County of Public Health. For youth programs, Sunnyvale Recreation is MANDATED to report positive test cases to the Santa Clara County of Public Health within 24 hours of notification.

GYMNASTICS ParentTots Built around imaginative themes and gymnastics activities young children begin to learn how to hop on two feet and hang and support from low gymnastic equipment in this structured basic movement class. Children get comfortable being upside down by rolling, and learn to perform basic gymnastic positions, including tuck, straddle, pike and sticking ta-da landings. One participating child per parent. Super Safari PRESCHOOL

Super Safari is a play-based recreation program designed to provide children their first experience in a structured environment. The program runs September through May and follows the Sunnyvale School District academic calendar. Children must be toilet trained and have the most current vaccinations by first day of class.

SUPER SAFARI COVID-19 PROTOCOLS Each Super Safari class is designed with a modification to include a stable cohort of children every month. Children and staff are required to wear a face mask while indoors. Social distancing and other safety protocols are designed to align with the most current State or Santa Clara County health order (whichever is stricter). Hand sanitizer will be available onsite. Parent/guardians are required to wear a facial covering for outdoor check in/out. Only participants and staff are allowed inside the classroom.

Level 1B – Beginner Improver

Level 1C – Beginner Intermediate

Designed to teach swimmers how to float, on their own, on their front and back as well as front and back glides off the wall independently. Additionally, freestyle (front crawl) with side breathing is introduced along with backstroke to swim independently. Swimmers are support while learning breaststroke frog kick. Note: A parent/guardian is required to be in the water for each swimmer in the lesson. Level 2B – Intermediate Improver Designed for swimmers to refine the three competitive strokes (freestyle, backstroke and breaststroke) and are introduced to open turns and flip turns. The butterfly arm stroke is introduced and combined with dolphin kick. Note: A parent/guardian is required to be on-site for each swimmer to be in the water during the lesson. Open Painting Studio

Focus on the fundamentals of painting with acrylics while unleashing your inner artist. Each participant is given their own table top easel station to create a 16 x 20 canvas that captures several artistic techniques. Supplies are provided. Rich Bland, Professional Golfers’ Association of America (PGA) Director of Instruction A professional golfer for over 40 years, Rich Bland has won numerous events since beginning competitive golf as a junior player. In addition to Sunnyvale’s courses, he has worked at Sunol Valley Golf Course, Sea Ranch and taught at John Jacobs Golf Schools. As a player, he has qualified for the PGA National Senior Championship. As an instructor, he has been awarded the PGA’s Junior Golf Leader Award, with 17 of his juniors earning golf scholarships at major universities.

ACTIVITIES REGISTRATION, POLICIES AND PROCEDURES How to Register Online Sunnyvale.ca.gov, search “Registration” Note: Not available for some classes Phone 408-730-7350 Walk In Recreation Center (see inside front cover for hours) 550 E. Remington Drive, Sunnyvale • Age level listed in course descriptions – participant must be that age by class start date.

Cancellation Policy • Programs may be canceled when the minimum enrollment is not met. Please register early. • If a program is canceled, participants are notified by phone and given the option to transfer to another class or receive a full refund. Please note refunds, paid by credit card may take one to two week to process, while cash/check payments may take up to six to eight weeks to process. • Camp: Notice must be received 10 business days from the start of the program to be eligible for a refund. See refund/transfer policy for additional information. • Classes: Notice must be received before the secondclass meeting.

• Camp and class registration is accepted (space permitting) until the second class meeting unless otherwise listed.

• Super Safari Preschool: Submit a written notice of intent to withdraw, including the last day your child will attend and submit a refund request form with the original receipt at least two (2) weeks before your child’s last day.

• Super Safari Preschool: Children must be toilet trained and have the most current vaccination by the first day of class.

• Swim Lessons: Notice must be received two business days from the start of the class session to be eligible for a refund.

• For tennis registration, see pages 50-51.

Waiting Lists • A waiting list is established if a program is full. Mail-in registrations that are received after program is full are automatically placed on the waiting list. • If a space becomes available, staff will notify parent/ guardian by telephone. • Payment is not charged until an opening becomes available. (Checks sent with mail-in registrations will be returned if space does not become available.) • If you are paying for multiple classes with one check, you will receive a refund check from the City within approximately 4 – 6 weeks for a camp/swim lesson when no space is available.

COVID-19 Medical Illness Policy: During a session, if the participant is identified as a close contact or receives a positive test notification and is unable to attend class/camp due to isolation/quarantine instructions, the participant may request a prorated refund. Please note refund request requirements below: • Camp/Classes: provide a signed doctors note (and if applicable the positive test result/close contact letter notification) either during the session or no later than 1 week of the class/camp end date for a prorated refund.

• SYBL Basketball: See program description for policy. • See refund/transfer policy for additional information.

Refund/Transfer Policy • Camp refund requests received less than 10 business days prior to first day of the program receive a credit on customer’s Sunnyvale account that must be used within 6 months. If credits are not used within the 6-month time period, they are removed from the customer’s account. Note: refunds are not issued for unused credits. • Class refund and transfer requests must be received prior to the second class meeting for most classes. • Swim Lesson class refund and transfer requests must be received two business days before the first day of the class session. • A $10 processing fee per transaction applies for all refunds. A $5 processing fee per transaction applies for all transfers. • Refunds and transfers will not be issued for one- and two-day camps or classes • Super Safari Preschool: Tuition refund requests must be received within required minimum notice (see cancellation policy above), we will pro-rate and refund any days remaining in the month you are unable to attend. If written notifications is not submitted before the two (2) week period, your refund will be forfeited. • SYBL Basketball: See program description for policy. • Anyone who does not meet the minimum requirements of the Lifeguard Training skills test given the first day of class will receive a full refund.

How Do I Request a Refund/Transfer?

Emergency Card Requirement

• Refunds/Transfers may be requested by phone (408-730-7350) or in person at the Community Center, Recreation Center front desk. If submitting in person, be prepared to present a receipt. If submitting by phone; be prepared to share info from your receipt. Receipts can be printed online.

For Camp or Super Safari Preschool Participants

• Refunds will be issued in the form of a check or credit card, depending on the original method of payment. Cash payments will be refunded by check.

Health Screening/Temperature Check Policy For in-person classes, camps and activities, staff and participants are required to answer health screening questions and conduct a temperate check upon arrival. If staff/participants answer “yes” to any of the questions, STAY HOME, seek medical care/testing. and notify staff at recreation@sunnyvale.ca.gov or 408-730-7350.

Missed or Make-Up Classes If a camp/class meeting is cancelled by the City, staff will make every effort to notify you as soon as possible by phone or email. If a make-up class cannot be scheduled, staff will issue a pro-rated refund or credit. Please make sure your contact information is current so we can reach you. The City does not offer make-up classes or issue credits when individual customers miss a class for illness or other personal reasons.

Non-Participants The City of Sunnyvale does not allow unregistered individuals in the program space, except when family and friends are invited to attend youth performances or demonstrations.

To participate in any City of Sunnyvale camp or Super Safari Preschool, all children are required to have an online emergency card on file. The City of Sunnyvale has partnered with ePACT Network, to offer an easy-to-use safe and secure system for families.

How Do I Fill Out an Emergency Card Online? After registration, the parent/guardian will receive an email from ePACT Network (member.support@epactnetwork.com) to fill out the emergency card online. This must be completed by the first day of the program.

Fee Waiver Assistance & Subsidized Program for Youth Sunnyvale residents may apply to cover registration fees for youth (age 17 and under) with up to $234 in scholarship funds per approved child, per annual funding cycle. Subsidized rate is at normally 70% off and includes all youth swim lessons, select camps, and other activities. Fee Waiver funds and subsidized pricing availability are limited and subject to the availability of funds. Any funds not used are forfeited and cannot be carried over to the next funding cycle, nor used for another child. Subsidized Program: If preapproved, families can pay a subsidized rate on select programs.

How to Apply Applications are available at the Sunnyvale Community Center, Columbia Neighborhood Center or online at Sunnyvale.ca.gov. Please allow two to four weeks to process your application.

RELEASE, WAIVER AND HOLD HARMLESS AGREEMENT + COVID-19 INFORMED CONSENT In consideration of participation in a class or activity (whether in-person or online) offered by the City of Sunnyvale, Department of Library and Recreation Services, I, the undersigned for myself and/or as the parent/guardian of the Minor enrolled in the class or activity, agree to indemnify and hold the City of Sunnyvale harmless and hereby waive, release and discharge any and all claims for damage, for death, personal injury, bodily injury (including illness and communicable disease) or property damage which I and/or the Minor may have or which hereinafter may accrue to me and/or the Minor against the City of Sunnyvale, its City Council, employees, agents, and volunteers from and against any liability arising out of or connected in any way with my and/or the Minor’s participation in this class or activity, even though that liability may arise out of active or passive negligence or carelessness on the part of the person or entities mentioned above. It is further understood and agreed that this waiver, release, and assumption of risks has been freely entered into and is to be binding on my/our heirs and assigns. Additionally, I fully understand that that my participation in the above-referenced activity exposes me to the risk of personal injury, death, communicable diseases, illnesses, viruses, and/or property damage. Knowing the risks, nevertheless, I hereby acknowledge that I and/or the named Minor am voluntarily participating in this activity agree to assume those risks on behalf of me and/or the above named Minor and to release and to hold harmless all of the persons or entities mentioned above who (through negligence or carelessness) might otherwise be liable to me and/or the above named Minor (or my/our heirs or assignees) for damages. PHOTOGRAPHIC RELEASE: I understand that photographs, recordings, or videos may be taken during this activity, and hereby grant the City permission use my image and/or that of the named minor, which may be captured through video, photo, digital camera or other media during participation in online or in-person classes, for City of Sunnyvale promotional materials and publications. COVID-19 AND OTHER COMMUNICABLE DISEASES: I agree, understand, and acknowledge, that an inherent risk of exposure to the disease COVID-19 (as defined by the World Health Organization and any strains, variants, or mutations thereof) and SARS-CoV-2 (the virus that can cause COVID-19) (collectively, “COVID-19”), and any other communicable or infectious disease, exists in any public place where people are present. “Communicable disease” means any disease or illness caused by microorganisms such as bacteria, viruses, parasites, or fungi that can be spread, directly or indirectly, from one person to another. “Infectious disease” means any disease or illness caused by microorganisms such as bacteria, viruses, parasites, or fungi that enter the body, multiply, and can cause an infection. COVID-19 is an extremely contagious communicable disease that can lead to severe illness and death. No precautions can eliminate the risk of exposure to COVID-19, and the risk of exposure applies to everyone. According to the Centers for Disease Control and Prevention (“CDC”), older adults (people 65 years and older) and people of any age who have underlying medical conditions might be at higher risk for severe illness and death from COVID-19. I also acknowledge that the risk of exposure to COVID-19 and any other communicable or infectious disease includes the risk that you will expose others that you later encounter, even if you are not experiencing or displaying any symptoms of illness. I acknowledge and agree to voluntarily assume any and all risks in any way related to exposure to COVID-19 and any other communicable or infectious disease, including illness, injury, or death of yourself or others, and including without limitation, all risks based on the sole, joint, active or passive negligence of any City of Sunnyvale or any of its officers, employees, agents, and volunteers. You acknowledge that your participation in a class, camp, activity, or event is entirely voluntary. By participating in a an class, camp, activity, or event you attest you are knowledgeable about your individual risk of developing severe illness if you are infected with COVID-19 or other communicable or infectious disease; you have made an informed decision about participating based on your individual risk; and you have decided whether to consult with a health care provider based on your individual risk. I further acknowledge and agree to follow all COVID-19 rules, regulations, mandates and safety protocols issued by City of Sunnyvale, County of Santa Clara, State of California and/or the CDC, including but not limited to: (a) mandatory COVID-19 testing at such times and frequency as required by the CDC, State of California, County of Santa Clara or other relevant agency, which may include testing before and after your activity, event, class or camp; (b) daily health screening (including temperature checks) for signs and symptoms of COVID-19 and known exposure to COVID-19; (c) denial of participation due to signs and symptoms of a possible COVID-19 infection or known exposure to COVID-19; (d) participation in contact tracing and data collection for COVID-19 surveillance; (e) mandatory isolation and/or quarantine as ordered by a public health authority; (f) closure of an activity, class, camp or event due to a positive or suspected COVID-19 infection in a participant (g) mandatory use of face coverings/face masks; and (h) following social distancing requirements of the City or any health authority.
