Every city has its problems - thereʼs no getting around it

‘AROUND THE CITY OF CHOICE’

HALLANDALE BEACH - Over the past three years the economic climate throughout our nation has impacted every citizen, business and government agency. Due to many years of strong fiscal leadership our city has continued to weather the storm. Over the course of Hallandale Beachʼs history there have been various levels of planning. There have been land-use plans, infrastructure planning and fiscal strategies implemented to point our city in the right direction. As our city has grown, strategic and financial planning has continued to be more complex and even more critical to sustainability. On the financial side of the equation, our conservative budgeting approach has continued to pay off. We have struck a balance over the past years of keeping taxes low while continuing to proactively address current and future needs. As of October 1 we are working in the new budget year. In reality we have already begun planning and looking towards next year. The City Commission has already discussed the next budget process. Proper planning involves short term and long term visioning. We are already in 2011-2012 and will continue to work towards completing the

pension funds .The city has three major pension funds; Police and Fire, Professional/ Management and General Employees. On September 15 we conducted a special workshop to evalu-

ate our investment strategies. Over the past years the Professional and General Pensions have become closed to new employees. Changes in employee benefits to

401 contributions have helped stabilize long term contribution costs for new hires. However, the existing plans are still in place until the last beneficiary is paid. This means the city must

continue to fund both these plans. This is where strategic planning and investment strategies will be critical to our future budgets. ––––––––––––––––––––– COOPER, Page 4A

established goals. The complexity arises with projecting future costs and revenues. Some cities have missed their projections and are now paying sorely for them. Many cities, including Hallandale Beach, are still paying their way out of severe losses that occurred in the stock market. There is plenty of finger pointing on who is to blame for the pension issues. I contend that it is easy to spread blame around like leaking oil and focusing on hindsight is not productive. The most important issue now is how to address long term costs. I have written many times about pension reform. It is important, but it is not the only piece of the puzzle. Pension trusts also need to be evaluated. Before summer break I requested that we conduct a special workshop meeting to address strategic financial planning for both the Professional\Management and General Employees Family Owned & Operated Since 1986

The City Commission serves as the trustees for both these plans. We meet quarterly to review investment performance with our pension managers from Principal Financial Group. Even with continued market fluctuations, Principal has been performing well compared to their benchmarks. They were asked to provide a review of their internal controls for monitoring investment managers as one goal of the workshop. They reviewed their extensive Due Diligence Program that evaluates each manager based on investment style, performance

and asset class in comparison to their peers. The next part of their presentation was a review of our underlying investment strategies and asset allocations to address long term liabilities. In our current asset allocation we have strong diversification to offset fluctuations but we are still subject to risks inherent to the market. The ways pension plans are funded are based on numerous factors. These factors include the benefit guaranteed, contributions and return on investment. Traditionally this ROI is 8%. In these funds it is 7.75%. Projections are made based on an actuarial formula and spread out over a 30 year horizon. Employee contribu-

tions are based on negotiations. Depending on how the fund makes or loses money impacts the city's contribution. If the fund does not make the projected ROI the city must contribute. This is called the unfunded liability and it is spread out over 3 to 5 years depending on the pension. When pension funds suffer a major loss like they did in 2008 their funding ratios also drop. In 2009 99% of the plans (77 plans evaluated in our peer group) were under funded. Funding levels are calculated by comparing market value of assets in the fund compared to the liabilities. The average funding ratio is around 72% in the peer group. As of 2010, the General Employee Plan

was 62% and the Management Professional Plan was at 71%. This means we still are still making up for the loss. In an open fund like the Police and Fire Pension these numbers continue to evolve depending on benefits and number of employees. Since there will be no new employees added to these two pensions, the city has the ability to project unfunded liability more accurately. By doing a long term plan the city could have the ability to level off contributions while making the fund stable and whole. The City Commission asked Principal to work with Staff to provide various funding scenarios. During the meeting we were informed that our research and proactive approach is mirroring private industry. Many agencies have moved to Asset Liability Modeling. The goal is to have a better understanding of all the liabilities and match assets and asset allocations to limit risk. The movement to Liability Driven Investing must be thoroughly evaluated as far as timing and how and when to move from the traditional structure to LDI due to extreme losses in 2008. In moving to a more conservative fund there is less risk but the inability to leverage some of the equity market returns when they are bullish. The Commission directed City Manager Antonio to work with Principal in doing multi illustrations and bring them back when ready. The Police and Fire Pension Fund are also evaluating different actuarial assumptions to

make sure we are proactively addressing our unfunded liabilities. Commissioner Ross and I serve as trustees on the Police and Fire Pension Board. The board is working to perform a study of the accuracy of assumptions to actual real life experiences within the fund. The fund also recently performed to required change by state statute from the 8% ROI to 7.75%. The City Commission will continue to work along with Staff on all facets of pension reform. There will be no quick silver bullet, but with continued due diligence, strategic planning and a fiscally conservative approach to our budgeting we should remain ahead of this critical issue. This Saturday from 10 AM to 3 PM the Palms Community Action Coalition in partnership with the city, will be hosting a “Historical Day and Health Fair” at OB John-

son Gymnasium located at 900 NW 8th Avenue. This event is part of the planning for the dedication of the new Foster Park Community Center which is currently under construction. One of the goals of the Palms Community and our city is to support historical preservation. Children are often called the “hidden victims” of domestic violence as they see, hear, and observe domestic violence in their homes whether adults know it or not. They can be tremendously impacted by exposure to abuse but also often display incredible resiliency. It is important to remember that the abuser is responsible for the violence and for the effects of the violence on the family. Studies suggest that between 3.3 and 10 million children are exposed to domestic violence each year. Children may be physically injured directly during a violent incident, they may be physically injured when trying to intervene in a violent incident and there is a major impact on children from living within a family that is exposed to the battererʼs constant power, control and manipulation. Batterers often use children as pawns to gain and maintain power and control over the parent who is being abused. They may use children to spy on the victim, undermine the victimʼs parenting, encourage children to participate in the abuse, etc. Frequently, children will believe that the domestic violence is their fault or that there is something that they can do to stop or reduce the violence in their home. It is important that we as a community support children who have been exposed to domestic violence. Question: I had a lot of cavities when I was a kid and my dentist back then filled them with silver amalgam fillings. My dentist tells me that my fillings are leaking and that teeth are getting cracks in them. Iʼve never had a problem before except a few years ago one tooth broke and I had to get a crown. The teeth heʼs talking about donʼt hurt and never bother me. Why should I do what he recommends and put in new fillings or crowns? Shouldnʼt I just wait until they break or start bothering me? Answer: A hundred years ago the horse and buggy was one of the best modes of transportation. But today we have much better ways of getting around. The automobile and airplanes certainly have many advantages over horseback, right? The “silver” amalgam fillings were developed around the time of the Civil War and certainly have served their purpose in dentistry for over a hundred years, but today they are not the best way to restore teeth and unquestionably have their own share of problems. Many dentists like myself havenʼt even used them in our dental practice for over fifteen years. First, you should know that “silver” is actually a misnomer. They really arenʼt “silver”. When first placed in the tooth the amalgam fillings are “silver colored” because the amalgam fillingʼs main component is mercury, up to 50%, which is silver colored. It is the mercury that imparts that color to the amalgam. Later that silver color often corrodes and discolors and the filling becomes black. The other 50% of the amalgam ingredients are lead, tin, copper, zinc and a small amount of silver. From a health standpoint alone, many researchers and physicians would argue that having mercury and

lead in the mouth is not healthy. When we remove the mercury filling material it is legally categorized as “biohazard” and dentists are not allowed to throw it into the trash or down the drain. By law we are required to have a bio-hazard removal service come to our office to take away our “bio-hazard” materials and dispose of them in a way that does not contaminate the environment. It is amazing that the government considers mercury amalgam filling waste biohazard and still has not completely outlawed the usage of the material as they have done in many other countries. When you were younger do you remember what was in the thermometers used to take your temperature? It was mercury, right? But, itʼs not longer used today, is it? There are health reasons for that. Mercury is toxic. The reason it was used in thermometers was that mercury is actually a metal that is a liquid. And the reason it worked as a thermometer is that mercury expands or contracts based on the temperature. Now imagine a filling in the middle of your tooth that expands and contracts each time you eat or drink something hot or cold. When it contracts it allows bacteria to go into the tooth in between the interface of the tooth and filling. When it expands, it places internal pressure on the remaining tooth. This is what commonly produces the cracks in the enamel of the teeth around the filling. Now, most dentists are all in agreement that when a tooth has a cavity and the decay is removed that if it was a small cavity the tooth will be 10-25% weaker, if a medium cavity 25-40% weaker, if a large cavity 4060% or more weaker.

So imagine a tooth that is weaker to begin with, and then having a filling that expands and contracts thousands upon thousands of times over a period of years. At some point something has to give, and the tooth often develops cracks, and these cracks often lead to fractured teeth. You might say, doesnʼt the mercury amalgam filling strengthen the tooth? The answer is a resounding no! It just fills the void of the cavity and actually relies on the strength of the remaining tooth structure. Today when dentists see teeth with leakage, which leads to decay, or visible fractures in the tooth around the filling most dentists will recommend being proactive. This means removing the old mercury amalgam filling and replacing it with a restoration that will strengthen the tooth and help keep it from breaking. The choices the dentist will typically recommend are a bonded composite filling, an onlay (usually porcelain, but many dentists still use gold on back teeth) or a

crown (again, usually porcelain but sometimes gold). Obviously the choice of which of these types of restoration will depend on the size of the original filling, the amount of decay or tooth that is cracked or fractured. Your dentist should be able to explain to you why he/she makes a particular recommendation. What will happen if you donʼt follow your dentistʼs recommendations. If itʼs a small filling and you have a

light bite and donʼt grind or clench, and if youʼre lucky, maybe nothing will happen. Or on the other hand, maybe nothing will happen for a while. If your mercury amalgam filling is medium or large in size or if you have a heavy bite and grind or clench your teeth, then at some point your tooth will undoubtedly break. Broken teeth are the most common emergency I have in my practice. Sometimes when that happens there is a lot

of pain involved. Sometimes even a root canal might be necessary if the fracture goes into the tooth nerve. Probably the worst-case scenario is that so much of the tooth breaks off that the tooth cannot be restored and has to be removed (extracted). That would then necessitate needing to replace the missing tooth with a much more involved treatment like a 3-unit porcelain bridge or implant. John, I canʼt tell you much more about your particular situation without seeing you as a patient. But in general you will be best served by proactive dentistry geared to prevent larger dental problems from occurring. Certainly replacing old mercury amalgam fillings that are leaking or exhibit cracks in the surrounding tooth structure with state-ofthe-art dental restorations (whether composite fillings, porcelain onlays or crowns) is usually the best way to preserve teeth and prevent pain and more expensive dental treatment down the line. Weight-loss surgery communication lacking in doctor's offices

(ARA) - An estimated 72 million adults currently are affected by obesity in the U.S. Yet, significant barriers are keeping adults affected by obesity and their primary care physicians from talking openly about bariatric, or weight loss, surgery. Bariatric surgery is the most effective treatment available for morbid obesity and can result in the improvement or complete resolution of obesity-related health conditions, such as type 2 diabetes and sleep apnea. Yet, only one in 10 who meet the National Institutes of Health guidelines for bariatric surgery have had their doctor recommend it, according to a national survey sponsored by the Obesity Action Coalition and Ethicon Endo-Surgery. The survey also found that health care professionals tend to underestimate patients' willingness to discuss their weight and their receptiveness to discuss bariatric surgery. Six in 10 physicians believe most individuals affected by obesity are too embarrassed to discuss their weight with a health care professional. However, more than four out of

Family caregivers face pain challenges

(NewsUSA) - Every day, many Americans find themselves in an unexpected new role. They become a family caregiver for a loved one suffering from chronic pain. Experts estimate pain affects 76 million Americans, more people than diabetes, coronary heart disease, and cancer combined. Pain can interfere with daily activities, and those affected may need help from family and friends. Providing care for a loved one experiencing chronic pain presents a unique set of challenges, such as making sure your loved one reports pain to his or her healthcare provider and that your loved one's pain is taken seriously by those who provide care. Unfortunately, there has been little information and few resources available to help family caregivers cope with these problems. The National Family Caregivers Association has teamed up with the national pain management education program Partners Against Pain to create Caregiver Cornerstones, a new program that provides information, encouragement, and tools to family caregivers who are caring for loved ones suffering from chronic pain. Accelerated Resolution Therapy (ART) is under the radar in mental health therapy

Flying under the radar is a brief model mental health therapy, Accelerated Resolution Therapy (ART), currently being studied in Florida at the University of South Florida for its use in treating PTSD.
