Paul is looking after his elderly mother Nonie who was physically challenged but now is becoming slightly forgetful and regressing into her childhood. He is worried about what if this develops further what should she do. Also, he has noticed unsavoury characters around the house and are afraid that they will persuade his mother to part with his limited funds.
The new assisted decision making legislation is on its way. This legislation provides that where a person is getting older and more vulnerable or even if it is a different situation like brain injury/stroke or other form of cognitive disorder that next of kin can arrange to put in place a plan to ensure the person with the disability or reduced mental capacity is looked after and not abused. It links into the HSE safeguarding policies. Moreover as next of kin have no legal right to act on behalf of a person to follow up on financial, property and other matters it is important to gain legal status to do this if required for vulnerable relatives.
Firstly, in the act is the ‘Functional Test’ of capacity. A person will be considered to have the capacity to make a decision if they can:
• Understand the information relevant to the decision
• Remember the information long enough to make a choice
• Use or weigh up the information to make a decision
• Communicate their decision (this may be with assistance)
Once a person’s capacity is called into question the legislation provides for three separate tiers of assisted decision making depending on the extent of lack of mental capacity of the person involved.
1. Assisted Decision Making where they appoint a person to help them with their decision making.
2. Co Decision Maker where they appoint a person to co decide on major decisions with them relating to for example property finances and personal welfare decisions.
3. A Decision-making representative who acts for them in relation to important decisions e.g. property finances and personal welfare decisions.
These helpers will do reports to the decision support service who acts as an overseer in relation to in particular 2 and 3 above. Paul could arrange with Nonie to become an assistant decision maker or co decision maker thereby ensuring that Nonie is protected. In all three tiers above the wishes, will and preference of Nonie must be respected. See www.decisionsupportservice.ie for more information. The update on the site for commencement of the legislation is now 2023 once an amending act to the legislation is passed.

Am I too old to straighten my teeth?
All too frequently I hear my patients ask if they are too old to straighten their teeth.The reality is age is not a factor when looking to straighten your teeth.
Younger patients i.e teenagers or those that are growing may have a faster response to orthodontic treatment but age is not a limiting factor.
I would like to straighten my teeth but am afraid of what the result would look like.
Nowdays most forms of orthodontic treatment utilise digital technology to treatment plan your orthodontic case. Treatments such as Invisalign and six month smiles can actually show you what the final result would look like before you commit to treatment.
Are all orthodontic systems the same?
No, unfortunately not all systems are the same. It’s a case of buyer beware. Its important to research the treatment options prior to committing to treatment. Some systems tip teeth without moving the root of the teeth. Therefore these are more prone to relapse.
If it sounds too good to be true it usually is!!
You do not need to see a dentist/ orthodontist to straighten your teeth. It can all be completed online without the hassle of attending a clinic.
Unfortunately, I see too many patients who have fallen into this trap of buying orthodontic systems online. When they present to the clinic it is too late and the damage is already done.Most will require extractions due to inadequately/ poorly planned treatment.
When planning tooth movements , here are some of the things that need to be considered : jaw joint discrepancies i.e is it the position of the teeth or the jaws or both that is causing the problem?
Is there an issue with the jaw joint? Does the patient have strong muscle/soft tissue activity which might increase the tendency to relapse?
Does the patient have a habit which is contributing to the bite ie thumb sucking, pen biting, tongue thrust ? Have the x rays been viewed to consider the shape/position of the roots and whether orthodontic
treatment is possible?
Is there any underlying disease such as cavities or gum disease which might prevent treatment?
None of the above factors can be assessed without preforming a clinical exam. The most difficult part of any treatment is the planning. If you undertake treatment which is poorly planned or even worse not planned at all then the results can be catastrophic. Very often patients are stuck with these online teeth straightening systems and have very few options as the treatment wasn’t
provided by a dental professional. Caveat Emptor!

Obesity and chronic health issues, such as heart disease, Type 2 diabetes and high blood pressure can go hand in hand. With a recent study by researchers at the Imperial College London showing that there are more adults in the world obese than underweight, we really need to look at prevention. However, until we recognise that there is more to weight gain than “calories in and calories out” it will be difficult to change obesity patterns. Yes, weight management is about healthy eating (even calorie restriction) and movement, but it is also about the quality of those calories and their impact on blood sugar balance and the hormonal effects of eating.Blood sugar is the amount of sugar (glucose) in your blood. Your body has to keep blood sugar within a very narrow range. We need sugar in our blood to keep our body working properly. Sugar is the main source of energy for all our cells. When we eat our blood sugar goes up, this is normal. All food even healthy food contains sugar. As our blood sugar goes up our body releases a hormone called insulin. Insulin tells our body to send sugar to our muscle cells if we need energy, or store it as fat. When we eat processed foods our blood sugar goes up faster and higher than with non processed natural foods, so we produce more insulin. The higher our insulin the less our body listens to it. This is called insulin resistance – a precursor to type 2 diabetes. It can be absolutely silent with few symptoms (gaining weight around the middle and fatigue may be associated). When we don’t exercise enough, we lose muscle mass. Muscle cells have more receptors for insulin, because muscle cells need more sugar. If we lose muscle mass we have fewer receptors so our blood sugar stays higher.
Feeling stressed and not sleeping well increases stress hormone levels. Stress hormones increase blood sugar to give us extra energy to get
through stressful situations. When we don’t use that extra sugar up, we have to release insulin to bring sugar back to normal which contributes further to insulin resistance and weight gain.

Mitchels played Keirns O Rahillys today in the town league final in Connolly Park under harsh weather conditions. Mitchels were up against it from the start down players due to injuries and not having a competitive game in a number of weeks. Mitchels competed for the first half against the wind they conceded and easy goal just before the break, Strand road were a lot sharper in possession and Deserved their victory. JOHN LYNCH TOURNAMENT: Saturday 29th last saw the 6th renewal of the John Lynch Memorial U10 Tournament. Now a staple fixture in the calendars of clubs throughout the land, the 2022 renewal was again a fantastic event and saw 494 young players leaving Connolly Park with wide smiles on their faces....iad chomh sásta le Diúic! Enjoyment and smiles on young faces has always been the motto of the event and this was again achieved in spades with the help of a huge crew of volunteers, referees, stewards and committee members, everyone did The Rock proud. FINAL FAOI 15: The Tralee/St Brendan District Under 15 Championship final, sponsored by Suits Select, took place on Sunday morning last. The boys of Ballymacelligott, our close rivals who had defeated Na Mistéalaigh, were our opponents. The game was played in the new pitch in Ardfert (behind the Community Centre). Conditions were difficult for both teams with a strong wind dictating teams’ tactics. Our boys played into the wind in the first half and showed great teamwork and athleticism on numerous occasions as they worked the ball into scoring positions.Ballymac never gave up the fight and our goalie and defence worked really hard to repel several dangerous attacks. We ran out worthy winners on a final score of 3-7 to 1-7. Really well done, boys!

Last Sunday saw us travel to Connolly Park to take on John Mitchels in this years town league final. Taking the field with mainly a second string team, we still managed to control the game from start to finish. With the elements against both sides and conditions particularly bad, we opened the brighter with midfielder Diarmuid O Sullivan converting two early placed balls, indeed O’Sullivan would continue to kick expertly throughout the contest with some superb frees from the ground.
As the half progressed we were completely dominant and this was rewarded on the scoreboard with points from Gearoid Hartnett, Ben Hanafan, Conor O’Sullivan (2) and Diarmuid O’Sullivan (3).
With a few minutes to go before the break , we were awarded a ‘45. This was taken by Diarmuid O’Sullivan, and for once he didn’t make a good connection, however his effort was dropping short and with a number of opposing players flapping at the incoming ball, it flew into the Mitchels net. This left us ahead at half time on a score line of 1-9 to 0-2. The second half saw us make a number of substitutions ,with Gearoid Savage,Ryan OCarroll and Shane McElligott being introduced.Shane McElligott made an immediate impact with two quick scores from play.

Conor O Sullivan, who cut through the John Mitchels rearguard and blasted an unstoppable shot to the roof of the net from twenty meters out, this brought the score to 2-11 to -02.
With the game now over as a contest , we added further scores from the busy Danial Bowler, a Gearoid Savage mark, a free from Diarmuid O Sullivan and another point from Shane McElligott. John Mitchels did manage to get a consolation goal near the end , but in truth the team in blue were far superior and dominated throughout the field. A good outing for the Strand Road men, particularly pleasing to give the fringe players an opportunity to get some well deserved game time, and to their credit they did not disappoint, playing some intelligent football from start to finish.

KDL Premier Camp 3 - 2 Dynamos. We suffered an unexpected defeat at the hands of Camp Utd last Friday night at Mounthawk Park. Having gone two goals down, Dynamos recovered and restored parity with a brace of goals from Darragh Lowth but Camp restored their lead with a breakaway goal towards the death.
