Menopause and the Pelvic Floor

The side effects of menopause are increasingly discussed and understood. As the taboos around the transition are broken down, society is becoming more comfortable with the symptoms, including hot flushes and night sweats.

But one menopausal side effect that is less well-recognised is the impact that it can have on the pelvic floor; Rowena Woods, Urology Clinical Specialist Nurse at King Edward VII’s hospital, discusses the impact of the menopause on the pelvic floor, and the steps women can take to prevent damage.

Why does the pelvic floor suffer during menopause?

Menopause is caused by a change in hormones, and in particular the female sex hormone, oestrogen. As women reach their 40s and 50s and begin to experience menopause, their oestrogen levels drop off, which can create issues for bladder control and the pelvic floor. Oestrogen is key for stimulating the muscles involved in supporting the pelvic floor, and so when this begins to decline, some women can experience incontinence and struggle controlling their bladder when they laugh or cough. This is particularly common amongst women who have had children in the past.

Those affected can feel like they’re less able to control their bladder, experiencing leaks or feeling like things are “dragging”, which can of course be unpleasant and distressing.

What other factors can affect the pelvic floor?

Staying healthy and active can also prevent incontinence and retain pelvic floor strength. If you’re overweight you should look to enjoy a healthy and balanced diet, particularly one rich in fibre to

prevent constipation and the need to strain, which can in turn weaken the pelvic floor.

Building strength and supporting muscles can also help. Smoking is bad for muscle development and quitting may help improve your bladder control. Core strength can also help, and menopausal women should try to do low impact exercises like swimming, walking pilates and abdominal work outs.

How can I build my pelvic floor strength?

Pelvic floor exercises can help to improve bladder control, and all women from the age of 12 should be regularly exercising their pelvic floor to prevent incontinence. Women who have had children will have likely been taught to do these in the past, but these should be kept up throughout life.

This consists of practising three times a day - doing 10 slow exercises, squeezing for 10 seconds as if you were trying to hold a wee in, and then resting for four seconds; this should be followed by 10 quick squeezes. You can do this yourself, or use a “trainer”.

However, if you’re struggling with incontinence, or have little or no strength in your pelvic floor, you may need some initial help getting started. At pelvic floor clinics, women can benefit from electrical stimulation, which can prompt contraction in the muscles and begin to build them up.

In addition, menopausal women may benefit from vaginal oestrogen, which can be prescribed by your GP, this can address some of the hormone imbalances caused by menopause without the need for hormones to enter the bloodstream, as it is applied as a pessary or cream.

How Exercise Can Improve Your Sleep Schedule

Establishing a regular sleeping pattern is particularly difficult, especially with the darker days and nights. Fortunately, simple lifestyle changes such as introducing exercise to your daily routine and increasing your activity levels can help to improve this.

We spoke to Dean Zweck, Product Development Manager at leading health club brand, Total Fitness, who shared his insight into how exercise can help regulate your sleep and the best exercises to help you drift off at night.

How does exercise aid sleep?

When you exercise, your body breaks down energy stores, works muscles to the point of fatigue, and takes our body outside its resting normal state. As you expend energy, your body will feel physically tired due to the stresses placed upon it through training. Your central nervous system also loses its ability to recruit and activate muscle which causes muscle fatigue, therefore making you feel tired.

Whilst it’s important not to over-exert yourself, it is normal to feel a sense of physical fatigue after exercising and this will help you to feel ready for sleep in the evening. Furthermore, an increase in your level of physical activity will also increase the amount of time spent in deep sleep, which is the most physically restorative phase of sleep. This restorative phase of sleep enhances regeneration & repair, both physical and hormonal.

There are also wellbeing benefits of exercise that can help you to get to sleep, as the endorphins released during physical activity can improve your overall mood and help combat low mood and stress, which could be interrupting your sleep.

When should I exercise to help sleep?

It’s important that you work out at a time that best suits your daily routine and you feel most motivated. Map out your day to ensure that some form of physical activity, whether that’s an intense HIIT work out or a leisurely walk, is incorporated into your day.

Slow paced exercises which place an emphasis on breathing, such as yoga, can be a great addition to your evening schedule and will help you to relax just before bed.

Whilst any form of physical activity will contribute towards a better night sleep, there are some exercises that are particularly effective in helping people fall asleep:

Meditative Yoga

Yoga does not only have a multitude of physical benefits, such as strengthening muscles and improving mobility to name a few, it can also aid our wellbeing. This popular practice relieves physical tension whilst calming your mind by encouraging relaxation, through muscle tension and relaxation, deep stretches and targeting breathing. There are plenty of free yoga classes available online including meditative yoga classes which incorporates elements of meditation into the physical practice and is a great option for the evenings.

HIIT

Built up of intense exercises and a fast pace, HIIT workouts are a fantastic way to tire out your muscles and help prepare you for the deep sleep your body needs. As many of us are spending the entire day at home and at our desks, HIIT workouts are a great way to get the blood pumping and aerobic system working, which is important for your overall heath.

Walking

A brisk walk is the perfect way to get some fresh air and work your muscles. A great form of exercise for people of all fitness levels and experience, the impact a brisk walk can have on your body is not to be overlooked - 30 minutes of brisk walking will burn between 100-300 calories. In addition to this, the change of scenery will provide stimulation and a much-needed break from screens, which can be disruptive to our sleep.

Swimming

Swimming is a high-intensity exercise that engages all the muscle groups in your body, including the large muscles in your shoulders, chest, quads and abdominals. Using all these muscles means that a lot of energy will be expended whilst swimming, so your body will need plenty of time (and sleep) to repair and recover. What’s more, our bodies also use up energy trying to maintain a warm body temperature whilst we swim - this leads to a greater fatigue than other high intensity forms of exercise.

Midlife Madness

It’s usually about now (mid-January) that the Christmas chaos has been cleared up – the presents that were opened with dubious joy or downright distaste have been stacked for inspection later and there are the beginnings of serious attempts to ‘get back to normal’ and by that, I mean stop midday drinking – I’m not pretending it will be a dry January but at least we won’t have days in a row when we start before lunch. Some sort of routine is usually been re-established – often we have work to thank for that and that’s when I undertake a sort of review of the presents that weren’t put to immediate use.

Over the years I realise there is often a theme, during the years of the pandemic presents seemed to be all about comfort, at home spa sets, lovely chocs and wine and some really good recipe books. But this year there seemed to be some differences and the combined ‘messages’ were very different.

An aunt bought me a gym membership!!! Only for a month mind you but when did I ever say that I wanted to join a gym!! It was clear certain sectors of the family were in cahoots as there were also presents of active wear, a yoga mat and a Pilates ball – no subtlety there then.

So, clearly, they want me to shape up – all I’m going to say is it is a darned good job none of the aforementioned articles were bought by hubby dear!!!! He may not have remained as hubby dear!!

Then there were the cook books – the Dr Mosely blood sugar diet – really! I mean what is wrong with carbs (don’t answer that it could become an argument) but I never said I wanted to forgo the food pleasures of this life! I did get some alcohol which sort of evened it out but of course that’s all gone now!!

Then there was a cornucopia of self-help ‘stuff’. I mean I’m

almost at a loss for words. Books, quotes calendars or fridge magnets. Some of the books were journals encouraging me to write down every day my thoughts and feelings. Well if I did that I think it would only be a matter of weeks before the police or those lovely folk in white coats turn up. There were books that encourage me to set daily and weekly goals. Honestly, some days my main goal is getting out of bed and not killing anyone before breakfast! There were books that encouraged reflections – well that’s a sure-fire way to a breakdown!! There was a set of happy cards you know with those positive words for each day – I did find there was a degree of help in arguing the toss about why I shouldn’t necessarily be happy on particular days!

There was also an assortment of ‘healing crystals’ and even a book of spells – now that contained some interesting reading.

The thing that really upset me the most was what they must think about me – presumably overweight and unfit – I can probably live with that although actually I wouldn’t class myself as overweight. I might not be particularly fit but I’m happy with myself isn’t that important too, isn’t todays rhetoric all about accepting ourselves as we are.

But it’s clearly not just about my body is it – they either think I need a personality transplant or that at the very least I could do with some ‘improvement’. This is my family and closest friends – and here I was thinking they liked me, no, more than that, they cared for me. Well it just goes to show how wrong you can be. So, my advice to you is that yes, it is the thought that counts –but not just the thought of the giver – what will the recipient think. What thoughts will your gifts give them?

Since then I have used the counselling voucher I got as I need therapy following all their gifts!!

Lessons in Life

Sometimes it can look as if some people have it easy in life – they have it all – wealth, happy relationships, successful careers, fulfilment and health, but the older I get the more I realise that most people do suffer from trial and tribulations of one sort or another – though they may not talk about it and indeed many others may not know.

But like anyone else they have the chance to choose how they react to those adversities and some people may become hardened and bitter while others seem to develop a softness and compassion from these tough experiences.

Many of us will have heard of Maya Angelou or even read one of her books such as ‘I know why the caged bird sings’ a book that chronicles her childhood from 3 – 16 recounting an unsettled and sometimes traumatic childhood that included rape and racism. But her story is one of recovery and triumph as Maya ends this part of her story giving birth to her son and begins to feel confident for the first time.

Keanu Reeves is a well know actor and celebrity who has known considerable adversity in his life, death and illness of loved ones and many other challenges but he too has managed not to let these adversities define him but rather he has risen above them and because he is in the public eye we can all learn a bit from his story and approach to life.

Your Difficulties don’t Define You

Some of the difficulties and challenges we face are tough but they needn’t define who we are unless we allow them to make us a victim. Many people have childhoods that are far from ideal – but they can’t be changed in retrospect so it is up to us what life we choose to create for ourselves in the present.

I’m not sure I can go as far as being thankful but there is no doubt that hard times can develop our character.

Money isn’t the answer to everything

It is easy to think that if we had more money life would be better and in lots of ways for many of us it would BUT prioritising money and making money often means we ignore other important aspects of life and sometimes they are the ones that matter a lot more than money.

Many of you will read this and feel like that’s okay to say when you are a multi-millionaire but we also

need to remember that he has given away many millions to charitable causes.

There is nothing wrong with money and financial security BUT when that becomes an overriding priority it can cause shifts in what we see as important. Money might support health but it can’t make you healthy or happy.

Life is in the moments

Over the last few years many of have changed perspective and realise that lots of things simply can’t be taken for granted. The only thing we can really be sure about is the here and now. The idea that we ‘save’ things for later in many ways can be seen as folly! Save them for when – maybe we won’t get the chance later so if we have the chance now –just do it!

Enjoy what you do

I’m a big believer in the fact that we create our life, or at least many aspects of it. Every day we make choices that have a bearing on how our future will be. I personally advocate doing a life audit periodically. Which areas of life are you happy with and which not so much. At that point many will say that the things they aren’t so happy with are things they can’t change but on closer inspection nearly always there are a few things you can change – if you want to - but often fear and being outside our comfort zone or fear of the unknown, prevents us.

Now I’m not saying we don’t all have to do some things we aren’t wild about BUT a lot of us need to get better at saying no and somehow squeezing into our busy schedules things that make our heart sing.

Be grateful and kind

The thing about gratitude is that it actually helps us to – it means we focus on the good which can change our perspective and it can help us in terms of our physiology. It seems to me the world needs a lot more kindness but when we are being kind to others we also need to be kind to ourselves after all we are all in this together.

We probably all have room for a few lessons in life – and often we can look to others to get those.

Firstly, Happy New Year!

I hope that you enjoyed the end of 2022 and are looking forward to what is in store in the New Year. Of course, a New Year brings the opening of the January transfer window and both at our level, in League One, and at the highest level, in the Premier League, clubs are looking at how they can strengthen their squads.

If you look at the Premier League, for me, Chelsea are lacking a bit of creativity, Manchester United could probably do with a striker, Arsenal potentially need a bit more depth within their squad and Liverpool are crying out for a central midfield player, but for all of those teams, identifying those areas is one thing, finding the solution to the issues is another.

Transfers are complicated. First it is about identifying the position that you need to fill and then you need to work out what ‘type’ of player you are after. For example, if Liverpool need a central midfield player – what attributes does he need? Is he a box-to-box player? Is he an attacking midfielder? Or is he someone that needs to strengthen their defensive solidity?

Once that has been identified, the next stage is finding players that fit that profile. It is not a computer game, you need to do your research, watch those players in action, work out their strength and weaknesses and then present a list. Often clubs will have a Plan A list, a Plan B list and a Plan C list. Those lists will be dependant on availability, wages, agent fees and competition.

Once you know the type of player you want and have worked out their identity, then you need to approach the club and see if they are available. I think outside of the Premier League there is no such thing as some who isn’t for sale because everyone has a price. A lot of clubs might say that as a first part of negotiation to drive the bid.

It is very rare that clubs will accept the first bid they receive. They might look to play teams off against each other, saying to their representatives that you are rejecting their offer because you have had a higher one from elsewhere. You might not even have received a higher bid, but it creates a bit of uncertainty in the rival club.

Once you have got to a point that the club are acceptable to the terms, you need to then agree personal terms and that involves speaking to the agent. Some negotiations can take an hour, some can take months. Often the agent will want a fee for facilitating the deal. It can take ages because they are very demanding!

A transfer is hard. It doesn’t matter what level you are doing business at. You have to remain decisive in your plans whether you are selling or buying. Know what you want from the outset, be strong in negotiations and don’t have regrets.

“There’s nowhere better to reset, relax and set those New Year intentions than in our beautiful British bolthole” said Albin Berglund, the Swedish-born cofounder of Bergman Design House. “The name ‘Lund Collection’ stems from my surname and means ‘green pastures’ in Swedish – which couldn’t be more apt for describing this countryside oasis. Marie and I wanted to channel the Swedish concept of ‘Hygge’ with a warming, natureinspired colour palette that brings the outside in, soft and tactile textures and of course, an open roaring fire. Here, guests will swap the sounds of city traffic for tweeting birds and tranquil nature, and feel the weight of the world lift from their shoulders as they cross the cottage threshold.”

New Beginnings at The Best Exotic Marigold Hotel

This month Milton Keynes Theatre welcomes the brand-new stage adaptation of The Best Exotic Marigold Hotel. Based on the Sunday Times bestseller which inspired one of this century’s most treasured films, it takes us on a journey to India with an eclectic group of British retirees as they embark on a new life. We caught up with Rula Lenska who plays Madge as she embarks on a journey of love, and second chances as she embraces the vibrancy of modern-day India, in charmed unexpected and life-changing ways.

What attracted you to The Best Exotic Marigold Hotel? I can’t imagine anybody saying no to it. It’s on everybody’s list of favourite books and films, and it’s particularly appealing for us as a bunch of older actors. The characters have such wonderful storylines and it’s a beautifully-adapted script by Deborah Moggach, who wrote the original book. It’s full of hope, everybody’s journey is different and it’s a feel-good, beautiful piece of theatre.

How would you describe the character of Madge? And have you avoided studying Celia Imrie’s performance in the film? She’s a rather brittle cougar who has built up her own business and is out looking for a rich maharajah with his own elephants. And no, I haven’t watched the film since landing the part. Likewise I haven’t gone back to the book. I was advised by the director [Lucy Bailey] not to because one can only portray so much with the lines one is given in the script. You want to bring your own interpretation to bear on it.

Can you relate to her in any way?

There’s always something of oneself in one’s characters. I’m an optimist by nature, as Madge is, and I’m a romantic. I’ve had a couple of husbands and she’s had three. She’s a person who never gives in or gives up, which is very much me as well. I think once you start giving in then it’s the beginning of the end. We’re at a time in our lives where this is a golden opportunity for the cast. My role isn’t particularly huge but it’s just a joy to be part of an ensemble piece that has all its sadnesses but is also, as I say, full of hope. With the world the way it is at the moment, hope is such an important thing to hang on to.

Do you feel enough stories are being told about the older generation?

No and in my opinion there are never enough. Time goes so fast as you start getting older and there are fewer good parts, particularly for older women - which is another reason why it is a joy to be offered something like this. The whole cast are just magnificent and delightful to be with. Hayley Mills is exactly the same as she was 40 years ago and the Indian contingent are just superb. They’re so helpful, so knowledgeable and so beautiful. They’re incredible.

Have you worked with any of your co-stars before?

I haven’t, no. I’ve known Hayley for many years but we haven’t worked together previously. Likewise, I haven’t worked with the rest of the cast but it’s a delight to watch each one of them find their way, develop their characters and develop our relationships with each other. And the set is magnificent; this dilapidated old Raj palace which is decaying but full of charm.

Why do you think this story resonates with people so much?

Like the book and the film, it’s about having a new lease of life. This story is what people need at the moment. There’s been so much sadness, so much fear, so much anxiety, and while the story is very true and very real it’s also escapism. It’s full of brightness, sunshine and discoveries. It’s magical.

You’ve had such a varied career. What have been your highlights?

There have been many, with the first real highlight being Rock Follies. I will never forget it. It was an extraordinary piece of television and it put me on the map. It had fantastic storylines, fantastic music and fantastic fantasy sequences. There have been many theatre pieces, doing Same Time, Next Year with my ex-husband [Dennis Waterman] and the play 84 Charing Cross Road - which up until now I think is probably my greatest triumph. Then there have been many, many television appearances, most recently in Coronation Street. I also absolutely loved working on Robin of Sherwood, Doctor Who… loads and loads of things.

What do you most enjoy about stage work?

I love the rehearsal process, where you get a bunch of people who you don’t know but some of whom you know about and you’ve seen their work. Then you all go on this journey together - not just the cast but the crew and production team - from the first day of rehearsal until you open. When you do television or film your definitive performance is not really your choice. You do something, then it’s cut together by directors and editors. On stage you are in charge and your performance continues to grow as you develop and discover new things. It’s a constant work in progress. And when you feel that you have the audience in the palm of your hands - whether it’s making them laugh, making them think, making them sad, making them happy - it’s a wonderful feeling.

When were you last on stage?

I was in a play earlier this year called Evelyn at Southwark Playhouse. I played a woman with dementia who’s an ageing hippie, still smokes dope and swears like a trooper. And the last time I toured was in Pygmalion with Alastair McGowan in 2014.

What are you most looking forward to about taking The Best Exotic Marigold Hotel around the country?

I’m looking forward to performing in enthusiastic venues, which I’m in no doubt will happen because everyone one mentions it to says ‘It’s my favourite film’ or ‘I love the book’. I think it appeals across the ages. And when you’re touring in a good show with a cast who all get on with each other it’s the greatest fun. Of course it’s also tiring, going from one end of the country to the other, but I do think this is going to be tremendous fun.

What do you hope audiences take away from seeing the show?

I hope they’ll be uplifted, enchanted, sometimes maybe a bit tearful but generally leaving the theatre with a good feeling in their hearts and their souls.

Is there anything you couldn’t be on tour without?

My pillow with a silk pillowcase. Because I’ve done a lot of touring, I tend to make wherever I go into a mini home. I don’t stay in hotels, I prefer to get self-contained accommodation or an attic room with an en suite not too far away from the theatre.

The Best Exotic marigold Hotel is appearing at Milton Keynes Theatre from Tuesday 24 - Saturday 28 January 2023. Tickets are available from www.atgtickets.com/miltonkeynes
