A
s father time continues to lead us – or push us – into life’s next moments, do we sometimes take for granted the present moment? Do we treat this moment as what it really is…the most important moment of all?
No matter your mood, emotions, or the events of the day, can you look within to appreciate this moment? Or, is your mind usually focused on the future, hoping for something different, something better? Martha, an 82-year-old retired factory worker, was experiencing severe memory loss. But, even in the throes of dementia, her spirits could not be dampened. She’d laugh at the silliness of life and find the joy. Those around her, those who loved her, did not understand her reactions to things. Wasn’t she sad? Upset? What was there to be cheerful about? They were filled with frustration and helplessness, while Martha remained joyful! COREY SHENK, EDITOR IN CHIEF

From Martha, we all can learn a valuable lesson: Seek joy, enjoy the good times, and savor the moment. When we’re caring for someone stricken with a degenerative disease – including Alzheimer’s or another form of dementia – it’s difficult to see hardly any moment as “good.” My late grandmother lived with dementia for years, and I was plagued with a feeling of helplessness and saddened by my lack of control in the situation. I had no way of assisting her in ways that I thought would be most valuable.

But, one day, I recognized that I was inviting these feelings of frustration into every interaction I had with my grandmother, and it affected how supportive of her I was able to be. I intentionally shifted my focus, and I soon recognized the good moments, brought laughter and joy into our relationship when I could, and learned to let the rest go. In truth, those were the only things I really could control.

Alzheimer’s & Dementia: Part 1 of 3
Symptoms and Stages
M
ost of us know someone who’s living with dementia, or whose life has been affected by Alzheimer’s disease. Few of us remain un-
touched by it. In fact, as many as 5 million Americans were living with Alzheimer’s in 2013. And, experts project that by 2050 as many as 14 million of us will be living with the progressive brain disease. In this, the first in a three-article series on Alzheimer’s disease and dementia, Aging Tree Illustrated is exploring the definitions
of dementia, symptoms of Alzheimer’s disease, stages of Alzheimer’s and more. In upcoming issues, we’ll examine how to cope with a diagnosis, how stroke as a risk factor for brain disease, as well as the latest medical findings, research and support systems in place to help families coping with dementia. Today, let’s start at the beginning.
What is dementia?
In general terms, dementia is a decline in mental ability that begins with memory loss, and gradually progresses enough to inhibit a person’s ability to engage in daily living activities. Alzheimer’s is the most common form of dementia, and accounts for upwards of 80 percent of dementia cases in seniors. However, other diseases are characterized by similar cognitive (brain) impairment, including Huntington’s disease and Lewy body dementia. Most dementias are progressive diseases, that is, they worsen over time. These include Alzheimer’s disease, vascular dementia, Lewy body dementia, and frontotemporal dementia. Other illnesses and diseases are linked to dementia as well, and include Huntington’s disease, traumatic brain injury, Creutzfeldt-Jakob disease, and Parkinson’s disease. Some dementia can be alleviated or reversed, such as dementias caused by infections and immune disorders, thyroid problems, nutritional deficiencies, side effects to medications, bleeding on the brain, poisoning, brain tumors, anoxia (lack of oxygen to tissues), and hydrocephalus (enlarged ventricles in the brain). In this series of articles, we’ll focus on dementia as a result of progressive diseases, with specific attention on Alzheimer’s.
Symptoms of Alzheimer’s disease
Alzheimer’s disease and dementia have many more symptoms than age-related memory loss, as you may suspect. But if you’re worried about yourself or a loved

Some symptoms of dementia are not related to disease, but if symptoms are related, early diagnosis is the best prescription for a most manageable journey ahead. Symptoms of Alzheimer’s and many progressive dementias can show themselves in memory, communication, motor activity, focus, judgment, visual perception, and the ability to plan events or organize ideas. Dementias damage brain cells enough to keep them from working properly. Different brain cells operate various areas of the brain. In Alzheimer’s patients, for example, the hippocampus of the brain is affected by the disease first. The hippocampus is the brain’s memory storage system and learning center. Medical professionals, notably neurologists, use several criteria to diagnose dementia. They may include examining the patient’s medical history and lab test results, and reviewing a patient’s changes in day-to-day functioning. Age is the best known risk factor for diagnosing many dementias, including Alzheimer’s disease. Most progressive dementias are suffered by older adults ages 60 and older. According to the Centers for Disease Control, as of age 65 a person’s risk for developing the disease doubles every five years.
Stages of Alzheimer’s disease
The Alzheimer’s Association states that though the symptoms of the disease can vary from person to person, its progression follows a relatively consistent pattern. • Stage 1 – In this stage, no impairment is experienced, nor are any symptoms presenting themselves. • Stage 2 – In this stage, a person may exhibit normal age-related memory loss or changes, or these may be early symptoms of Alzheimer’s. cont. pg 9

• Stage 3- A person may be experiencing a moderate decline in an ability to remember, think, plan or organize, but it may not be diagnosed as Alzheimer’s yet. This is the stage in which friends and family start getting concerned or notice a change in their loved one’s ability to remember names, for example. • Stage 4 – In this stage, symptoms of Alzheimer’s are more noticeable and can be detected easily. These symptoms may include forgetting a very recent event, forgetting stories from the past, or becoming socially withdrawn. • Stage 5 – This is what the medical community recognizes as “moderate severe cognitive decline.” In this stage, persons with the disease may forget their address, the name of their high school, or other significant facts about their life. A patient may not know where she is, or what day it is, and seemingly simple mathematical problems, like counting backwards from 30 by 3s, are difficult. • Stage 6 – This is where personality changes usually take place. Memory continues to decline, and the loved one may have major changes in sleeping

Though some medications on the market today can alleviate some symptoms of progressive dementias, they promise only modest, short-term results. let, and may begin to wander. • Stage 7 – Unable to respond to their environment, these patients also may not be able to engage in dialogue, or control their movements. Alzheimer’s disease, and all progressive dementias, are fatal; there are no known cures.
When are symptoms NOT indicative of dementia?
Memory loss not associated with dementia also is possible, of course, and these symptoms often can be alleviated with medical treatment. Depression, anxiety, a side-effect of another medication, and vitamin deficiency are a few of the causes of memory loss.
Diagnosing dementia
There’s no singular, definitive test to diagnose progressive dementia, though medical professionals are typically able to diagnose it with great certainty through various examinations. Alzheimer’s disease is diagnosed with specific criteria. Alzheimer’s makes its presence known when high levels of specific proteins inhibit cells of the hippocampus to communicate effectively with one another. Early detection of dementia can allow patients to get the best treatments possible during the disease’s progression, as well as allow family members and loved ones to make a plan for care in which the patient can be involved.

How Alzheimer’s changes the brain
Essentially, Alzheimer’s disease kills nerve cells and creates tissue loss in the brain, shrinking the size of the brain and affecting all its areas. The size of the hippocampus is greatly affected by Alzheimer’s, and on a cellular level, brain tissue with the disease has significantly fewer nerve cells and connections (synapses) between them. In addition, plaques (protein) build up between the cells in abnormally shaped clusters when the disease is present. These plaques are formed from protein pieces called beta-amyloid. Tangles are present in brains afflicted with the disease, too. Tangles are twisted groupings of dead or dying nerve cells entangled with another protein.
Diagnosing Alzheimer’s through the eye
Dr. Thomas J. Lewis of RealHealth (Alzheimer’s) Clinics, Lake Mary, says eye examinations can be used to detect Alzheimer’s. With standard eye instruments, doctors can see Alzheimer’s developing before the patient exhibits even mild cognitive impairment, he says. Beta-amyloid associated with Alzheimer’s can be seen in the eye, as can a loss of brain cells. These protein pieces are not always associated
with Alzheimer’s, but often they are, says Dr. Lewis. Today, a simple yet sophisticated Alzheimer’s measuring tool is available that scans the eye for loss of nervous system tissue. “The eye provides a very simple and non-invasive way to measure beta-amyloid formation,” he says. “Patients who show these formations are very often dementia sufferers, and those who do not have symptoms of Alzheimer’s but show beta-amyloid in their eye, either have some other illness or have blood markers of disease indicating that they are predisposed to Alzheimer’s or another neurodegenerative disease.” (Check back for more about medications and research in our upcoming articles in this threepart series.)
How can we lessen our chances of acquiring dementia?
Just as with most diseases that afflict our bodies, cardiovascular health, diet, exercise and family history can influence our chances of acquiring dementia. • The proper and healthy workings of our circulatory system, including our heart, are imperative for brain health. Some studies suggest that the prevention of diseases and health conditions that damage blood vessels, such as stroke or
diabetes, can lessen the risk of Alzheimer’s disease. • A diet rich in fruits, vegetables, fish/ shellfish, healthy fats, and whole grains lend to heart health, and heart health is directly related to brain health. Some researchers are investigating the benefits of vitamins E and C in reducing the risk of dementia, too. • Through exercise, we increase the flow of blood (and thus oxygen) to the brain, helping it continue peak functioning. • Experts also underscore the importance of seniors to maintain social engagement and continue with brain-stimulating activities, too. *** Check back for the next issues of Aging Tree Illustrated, when we’ll examine how to cope with a dementia/Alzheimer’s disease diagnosis, and the effect of stroke as a risk for brain disease, as well as the latest medical findings, research and support systems in place to help families coping with dementia. eniors living with dementia, Alzheimer’s disease and significant memory loss need special care. Additional attention and adapted living accommodations become essential when individuals no longer can remember to take medication, attend to their own basic living needs, or are prone to wander. When safety is at risk, it’s time to explore options. The decision to find memory care for a loved one can be a difficult one, but once you reach the sound determination that a change is needed, you’ll likely find several options from which to choose. In Central Florida, many senior-care communities offer memory care. They can come by different names, and offer varying degrees of assistance. Let’s explore some of the terminology you may encounter, and give you food for thought as you search for the best solution.

Communities that offer secure memory care often offer respite care, too. Short-term respite-care solutions can be lifesavers for caregivers who need a break for even a few days, or whose life circumstances dictate someone else care for their loved one for a limited time.
Secure Adult Day Care
Alternatively, to allow your loved one to experience social, yet secure, time away from home and his or her caregiver, consider researching a secure adult day care centers. Adult day care centers (often called “Adult Day Health Care” facilities), typically operate during daytime hours, Monday through Friday, and provide social activities and meals. Some adult day care centers also offer secure settings that are necessary for dementia patients, and most employ medical staff that can assist with clients’ daily living and support, including administering medication.
Staying Home: The Mother-In-Law Suite
Caring for your loved one or parent on your own property may be the perfect solution for you if you have the space and resources. Today’s families have more options than years ago, too. Loved ones with early Alzheimer’s or dementia may live in a family member’s home, of course, or
several hundred square feet that run off the main home’s utilities. A mother-in-law cottage or suite may not be ideal for your loved one if he or she requires ongoing supervision, assistance with medical needs, or is prone to wander. But, for many, it’s a solution worth exploring.
Home care
Keeping loved ones with dementia in the familiar surroundings at home can benefit them in several ways. So, if in-home care is the best option for your situation, and professional/medical assistance is needed to accomplish the goal, be sure to research providers’ credentials thoroughly. Legal Matters to Consider with Memory Loss: It’s always a difficult situation: A client is living with Alzheimer’s or dementia, and I’m not able to provide essential services because he or she doesn’t understand the details of the document that needs to be signed. I won’t allow clients to sign any estate-planning documents, such as power of attorney, when they don’t understand all they entail. Even if they can physically sign them, sometimes it’s just too late. But when is it really, “too late” to get these documents in order, to plan? It’s a seemingly simple question, but a complicated answer. The progression of dementia usually follows a tell-tale pattern. It starts when clients notice their short-term memory is not what it used to be. They think, “It’s just part of getting older,” so they don’t tell their adult children. But, when the child visits the parent, they find out that the parent’s short-term
Legal memory is much more affected than they were led to believe. Sometimes this memory loss presents itself as the parent asking the same question time and again within a few minutes. Maybe it becomes apparent when the unopened mail stacks up, or unexplainable cash withdrawals appear on the checking statement. Soon thereafter, the adult child gets nervous about these behaviors, and takes the parent to a lawyer to get a power of attorney. I hear stories like this weekly in my practice. However, a lawyer should not allow a client to sign documents when her or his mental capacity is so low that s/he is not able to understand what’s being signed. When that happens, I’m more concerned about determining whether I can provide services than determining what services I can provide. So, I investigate. Does the client have sufficient mental capacity to understand the nature of what they will sign? The law sets out a test: 1. 2.
Does the client know who their family is? Does the client have the ability to understand the nature and value of assets? For example, can the client identify the difference between a house and cash, whether a typical home is more or less valuable than $100 cash, and whether they can use
By Scott A. Selis, Esq. an end table to pay for groceries at the supermarket? 3. Does the client have the ability to understand the nature of what they are being asked to sign? If they can’t answer “yes” to the above, it may be too late to plan. In my experience, clients with moderate dementia can answer these questions, unless their dementia is far advanced. If the lawyer you consult with doesn’t explore these three questions in a manner that shows that they are experienced, don’t hire her/him. Seek and experienced attorney whose practices are ethical and who takes into serious consideration the client’s health. You, and your loved ones, deserve such service.

Financial Assistance for Seniors Did you know that there are programs available here in Florida that can provide financial assistance to seniors over 60 years of age? I am told by many of you that I meet, how important it is to you to maintain your sense of independence for as long as you desire. One great resource to help accomplish this is The Florida Department of Elder Affairs. They are a great resource that assists seniors in taking care of some basic needs. Many have difficulty paying electric bills, buying food and prescriptions, maintaining housing, having transportation to doctor appointments. The Elder Assistance program offers access to a Senior
Health Wise Farmers Market Nutrition Program and home-delivered meals. They will even help with providing food for your dog or cat. Housing assistance may be available for mortgage payments or property tax payments. Prescription medication assistance and savings is available through a free discount card. The card offers up to 40% savings on prescription medications. The great thing about it is that the card is free to use and there are no income requirements. Just remember that there are many of us in the community that care about our seniors and your well-being. There are agencies right here ready and willing to help. If you or your loved one needs assistance, contact Elder Assistance at 1-800-96-ELDER (1-800963-5337). They will provide you with information on these programs.

Nuggets of Knowledge Solving Problems with Computers and One on One Training COMPUTER TRAINING BRANCH
O
ne of the biggest motivators in learning anything is ‘the need to know something’! If you want to do something that takes a specific tool, you look into that tool, ask a friend or even do some reading on its use. Realize that you don’t have to know the entire tool, just how to use it to accomplish what you want. That is the same way using a computers should be taught. You have a problem, this tool (the computer) can help with that problem so you learn how to use it to solve that problem. You come to understand that the tool may also solve many, many other problems, but you’re not really interested in that at this time, just solving the problem you have at hand. It’s that simple!!
Now let’s take a look at the way ‘One on One Training’ works. We sit down and go over what you are wanting to do, then WE walk through the steps together, slowly, explaining each step in plain English as we go and you are interacting with your computer, therefore, you are no longer just a ‘viewer’, you are a ‘participant’. If you get lost, we backtrack and make sure you understand what you are doing as well as what you have done. Then I ask you to explain
Taking computer classes is a great idea, but realize, you will be getting a lot of information you most likely you will never use. I also realize that there are very basic computer classes out there, but even those can be more than you need. You have one specific problem, you want help with that issue and that issue only.
Most seniors are well aware of what they want to do with a computer, the problem is that most ‘folks in the know’ use words (and terms) that are very confusing to the average user. You DON’T need to ‘connect all the dots’ to accomplish your goal, just the ones you need too. Do you really care what DHCP, CPU Clock speed and Free RAM is all about? How about the ‘Windows Home’ or ‘Windows Central’ key, do you know where they are and what they do? Then why do you need to learn all about them? Sure, would be nice to know (if your into that), but they may not be necessary for what you want to do. Let’s take a look at just how a simple problem is explained in ‘computer talk’; Some of the terms like; directory, attachment icon, zipped files and clouds are only explained if they need to be and explained in a way that makes sense to you. Most of the time, once you see what we are doing and hear those terms you’ll get the idea of what we are talking about and maybe even pick up a few more new words to add to your ‘techie vocabulary’!

Make sure the diet includes a good amount of fresh vegetables and fruits to provide the necessary vitamins, minerals, and fiber to insure the immune system is working properly. Sometimes it is wise to add a natural food –based multivitamin/mineral supplement to ensure all needs are being met.

Strong Roots Spring is the Season of Fresh Starts and New Dreams GARDENING BRANCH

ow that spring has been officially ushered last month, you can almost feel it in the air. Even in Florida where our winters are brief and the landscape is seldom devoid of green things spring is the season that has always brought to me a sense of a fresh start. My mother was an artist and I remember her reusing some of her painting surfaces by giving them a fresh coat of white paint to cover over an unsatisfactory picture she had created. The fresh white board gave her a chance at a new start at creating something beautiful. To me spring offers us a chance to metaphorically put a fresh coat of white paint on the farm. It is a time when we begin to clear away the old winter crops and revitalize the soil. Then we bring out the young seedlings of zucchini, squash, peppers, tomatoes and plant them. We also sow the beans and cucumbers and watch as the first small leaves peak out from under the soil. In the matter of a few weeks the tired
winter plants are replaced with dark green leafy plants sporting beautiful yellow, purple and white flowers. The field is a buzz
with honey bees going about their work. Literally the fresh start has brought out a beautiful new creation. It never fails to thrill me as I watch nature put on the big show. never been one to follow social convention. I want to tell you that approaching sixty for me just means now I can do the things I want to do. All my life I have wanted to start my own business and so 7 years ago my wife, Pat and I started the farm. It offers us a reason to get up every morning and look forward to what the new day has to offer. While it may not be your passion we love to work with nature and teach people about good food. I can’t imagine myself just sitting down in front of a TV every day or hanging out each morning at the local fast food establishment. I have a passion that I am planning to live for as long as the good Lord allows me to. I am saying this because I know for many of you retirement may have been a harder transition then you might have imagined. Try to rethink retirement not as the end stage but just another stage of life. You have gained a lifetime of experience, wisdom and maturity. You have much to offer. Discover a dream or passion and begin following it. I think you will be surprised at how creative you can be and how much energy it will breathe into your life. I know as a nurse that as we get older the individuals who fair best are the ones who don’t quit dreaming and believing they still have something to offer. DIRECTIONS:
Preheat oven to 350 degrees. Place chicken breasts between wax paper and flatten to 1/4 inch using a meat mallet. In saucepan, melt one tablespoon butter and saute onions. Add spinach, cheese and nutmeg, mixing well. Divide into halves and shape into mounds in small greased baking dish. Place chicken over spinach mixture, tucking ends under slightly. Bake for 20 minutes or until done. While chicken is baking, melt one tablespoon butter in sauce pan and saute mushrooms. Add remaining ingredients and bring to a boil. Reduce heat and stir until sauce reduces and slightly thickens, about six minutes. Serve over cooked chicken.
VARIATIONS:
-Canned mushrooms can be used instead of fresh -I prefer a little thicker sauce so I usually add one tablespoon of cornstarch to a quarter cup of water. I add this slowly to the sauce during the last minute of cooking, stopping when it reaches a thicker consistency.
