ara's words above are words we should all listen to in our daily lives. Sometimes we don't say what we mean but are trying to convey how we feel. Sometimes things hurt and sometimes we don't understand. Sometimes we say things we don't mean and sometimes we hear things the wrong way. The good news is that we have each new day to start fresh, learn from yesterday and make a better life for ourselves. Individuals with Alzheimer’s and dementia don't get to do that. The memories that we all build over a lifetime are slowly erased for them. Learning to understand how someone feels when they are afflicted with this terrible disease can help us understand how to better help them. The same goes for our daily lives. November is National Alzheimer's awareness month. The number of people with the disease has soared to nearly 5.4 million. Join me in helping to raise awareness this month by learning about Alzheimer's and dementia so we can work together to find a cure. In this month’s Legacy magazine, Evelyn Stanley Dedmon writes a beautiful article to try and help us to better understand the disease. November 11th is the day we celebrate our Veterans. Thank you to our country's veterans for all you have done for us. We would not be where we are without your sacrifice. Thank you also to our active military who continue to keep us safe. This month, Cheree Albert helps us learn more about the Florida Medicaid Long Term Care Program to get an update on the statewide financial assistance program to help our elderly stay in their homes or move to assisted living. Travel Smart: Travel with a Group for an Incredible Travel Experience

ast week, myself and 20 others returned from an amazing group trip to Spain. It was such an incredible experience! It seems like such a whirlwind just to think back to day one already. Everything about this trip was unbelievable—Spain is probably my favorite place in Europe—but if I were to focus this trip down to one word, I would say that word would be communication. Yup, I know, not the word you were expecting, right? Maybe it’s an odd word to describe a trip? But in Spain, everything, day by day, is all surrounded by the way people communicate with one another. One of the first things you learn about the Spanish culture when visiting Spain is how social the people are. There is no “to go” food or drink because they actually take time to sit or stand to have an espresso, a glass of wine, a snack, or a meal. They’ll enjoy a conversation with their friends and family instead of being on the go all the time. Their apartments are sparsely decorated and rarely visited because the events and visiting are all done in the streets, restaurants, and public venues. All the arts, history, and culture surround communication. From the art of the bull fight to the passion of the flamenco dance,
the story told in the performance is truly and traditionally Spain! We were provided a handful of helpful tutorials while on the trip, but one of the best tidbits I could offer is to do some research before you go. Learn how to order your favorite coffee, a glass of wine, or ask for a restroom in the local language. It will really help you a lot when traveling!

“You’ve Never Met all Your Fans” Answering the phone with a drawn out, “Rooo te maaaan”, Emil “Buzzie” Reutimann made me smile from the first moment I heard his voice! Living an authentic and full life doing exactly what he loves, Buzzie was busy doing his work for the day, “jacking the car up, throwing on a battery charger, and loadin' it on the trailer”. He jovially added, “I don’t have a job, I just wake up, walk outside and work on cars”. In the race car business since he was 13 years old, now 77-yearold Buzzie is probably one of the youngest spirits one could know. Buzzie raced up in the northern part of the U.S. and Canada and won the Syracuse race in 1971 and
ie retired in Florida and opened his speed shop business where he still builds cars. He is his son
David’s biggest fan. David had a very successful multi-million dollar NASCAR career, winning in Charlotte and Chicago and even being once named Rookie of the year. Family is very important to Buzzie. Family has been the cornerstone of the legacy surrounding the Reutimann family business since the 1920’s. Last year, the Reutimann family was celebrated for their 100 years in the community at Zephyrhills Founder’s Day. It is a name rich in family tradition. he could realize his dream to race cars, rather than sell them. Known for his “Double O” race car number, Buzzie laughs as he explains that his dad, also a race car driver walked by young Buzzie one day as he was working on his car. His dad remarked, “That’s the nearest to nothing I’ve ever seen. In fact, it’s double nothing.” Buzzie has proudly represented that number ever since. Another Reason to Celebrate
Help for Seniors Through FL State Medicaid: Where is the Florida Medicaid Long-Term Care program headed?
I
am grateful that both my undergraduate and graduate nursing programs were laser focused on the needs of the community. After nearly 40 years of looking through this lens, it is no secret that the landscape is changing in our neighborhoods and communities. Repeatedly we have heard the growing number of people who are turning 65 every day in this country, reportedly 10,000! How will cities and neighborhoods need to adapt to this aging population? What do we know and what can we do to manage this shift? These needs and changes in a service area are monitored each year by the Area Agency on Aging (AAA) as required by the State Department of Elder Affairs. In our region the designated AAA is ElderSource. ElderSource also serves as the Aging and Disability Resource Center (ARDC) for the region. Specific goals for this annual report include: where our seniors are living, which resources are needed and what gaps need to be filled. To learn more about the process of accessing the Florida Medicaid Long-Term Care Program specifically and ElderSource in general I had the opportunity to speak with Linda Levin M.S.G, who is the Chief Executive Officer of ElderSource, serving Baker,
Clay, Duval, Flagler, Nassau, St. Johns and Volusia County. What was most striking about our conversation and the resources provided was the sheer number of people who need help. The 2016 Profile of Older Americans noted that nearly 24% of the population or 515,200 seniors (60+) live in this service area. Of these 9,717 seniors are over the age of 80. Living below 125% of the pov-
erty level are 71,040 seniors. More than 123,171 seniors live alone. The number of seniors having at least one type of disability, such as hearing or vision loss or just too frail to adequately care for themselves, totaled 160,063. This 2016 assessment noted that 2,125 people were on the wait list to receive home delivered meals and 4,667 were on the wait list to secure Medicaid managed longterm care help. The most recent assessment for 2017 noted that there were 46,038 reported cases of Alzheimer’s disease in this service area with over 7,100 people on the waiting lists for ElderSource managed and contract programs. This number included those waiting for Medicaid Managed Care

By Cheree M. Albert MSN, ARNP, CPNP Long Term Care. By 2020 it is expected that the number of seniors age 60+ in the region will grow to 573,880 and to 729,702 by 2030. It was no surprise given these statistics that the number of calls to this agency alone is 5,000 per month according to Levin. Calls to the Help Line first reach the Customer Service Specialist. Often these specialists can provide the needed information and help to connect them to programs in the community. If more is required, then an Intake Specialist will perform a screening to help identify next steps. This screening includes an assessment of both income and “frailty requirements” which includes information regarding the person’s ability to perform Activities of Daily Living as well as their environment, mental health, caregiver needs and other information. Levin shared that it is important to know that the wait list is based on frailty, not on the length of time on the list. The Medicaid Long Term Care Program serves those who are income eligible and those who are most frail first, though it is possible to be eligible and receive some services through other programs even while waiting on the list for this program. | 14 |
Each month the state provides the names of those who are released from the wait list for the program. ElderSource staff help them through their eligibility determination. After financial and in-home assessments have been completed a Choice Counselor provides information regarding the various plans that are available in the region. Levin shared that “Families can choose their plan, if not then they are automatically enrolled in one. The plan then takes over, does the care plan and contracts with vendors who provide the services”. Clients not in the program, not income eligible or not released from the list will be referred to other programs the ARDC contracts with in the region. Where is the Florida Medicaid Long-Term Care program head-
ed? Levin shared her insights, first acknowledging the extent to which this is largely a legislative issue with funding from both State and Federal sources. She added, “Where I would like it to go – all of our programs need to grow. [That] More people are aging and living longer is good news. One issue is the supply and demand on the budget. Keeping [people] in the home and their community is much more cost effective…advocacy takes place every year to grow these programs”. At this point you have perhaps caught a glimpse of the immense scope of this issue. You may even be challenged by it in some way. It is no small undertaking, this mission that Corey Shenk, Aging Tree’s own Chief Executive Officer, Linda Levin and countless others have embraced with both
passion and commitment to serve this growing number of seniors in our community to the fullest extent possible. I asked Linda how long she had been working at ElderSource, she replied “In January it will be 15 years…I love our work”. As a caregiver who needed information and received help from Aging Tree for the sweet 93-year-old living in our home, I am forever grateful for this incredible network of information, encouragement and support. I am thankful for all those who have answered the call to serve in this area, in whatever capacity that is, for we know the need is great. Veterans and Their Widow(ers)…Are you Aware of This Valuable Financial Benefit? Are you a wartime Veteran or a widow of a Veteran? If so, first of all, thank you for your service to our country. Let me tell you about a veteran’s benefit that you and your family may be unaware of. It is called the Veteran Aid and Attendance Pension. It is an a benefit that will help you pay for homecare, Assisted Living, and/or Nursing Home expenses. To qualify, you must be: a “wartime veteran or surviving spouse;” must need the assistance of another person to perform daily tasks, such as eating, dressing, undressing, taking care of the needs of nature, etc.; and
meet the meet a complicated income and asset test. Blind individuals, patients in a nursing home for mental or physical incapacity, or residents in an assisted living facility may also qualify. The benefit amount is between about $1,100 – over $2,000 per month. Those payments are usually the difference between the independence of a veteran or surviving spouse by allowing them to stay at home with care, living in a high quality assisted living facility, or living in a nursing home. Be proactive and apply for the benefit now, as it can take over a year to complete the process. But the VA will pay the benefit retroactively to the date of application. Memory Care Behavior Management in Dementia Care: Behaviors that Cannot be Changed INSIDE ADVISOR
The following are behaviors caused by the disease process of dementia and cannot be changed. The good news is you can learn and implement strategies to cope with them and prevent the progression into more difficult behaviors. Listed below are the most common behaviors that cannot be changed and tips to cope with these behaviors: • Thought blocking is an interruption of thought processes with delayed or no response. This can be frustrating for both the person talking and the one listening. Sometimes the caregiver wants to help by guessing what they want to say or interrupting their attempt to verbalize their thoughts by changing the subject. It may be beneficial to wait quietly and allow the person time to verbalize their thoughts or feelings. The
most appropriate response will be realized by their response whether positive or negative. • Repetition noted by verbalizing the same words or sentences over and over can be a challenge. • The most important thing to remember is they don’t know they are repeating themselves. As a family member or caregiver practice hearing them for the first time every time. This will help you answer the first time and lessen the frustration of hearing and saying the same thing over and over. • Loss of short term memory may be gradual, intermittent or by a rapid progression. A caregiver may have to wait and see how their memory impairment is manifesting itself before responding. It’s always better to wait than it is to push

forward before you have the whole picture. Inability to carry out tasks that have always been simple to perform is especially disturbing to the person with dementia. They may feel like a failure or be afraid of what is happening to their ability to function. By giving them gentle reminders and assistance with the task allows you to be part of the solution and not on the outside telling them what to do. This can be a very touchy area depending on the stage of dementia. Resistance to try or learn new tasks. This is a challenge for family members as they want to help them get better or at least maintain their cognitive functioning. Watch for clues that indicate you should wait until another time or change the task to a simpler one such as frustration, anxiety, or depression. Time of day/night becomes unimportant or forgotten. Depending on the stage of dementia, the caregiver may be able to gently lead them to understand the time of day or night. But having the person experience less fear and/or anxiety is more important than staying on a schedule they do not understand. The caregiver may have to conform to their understanding of day and time. Difficult to participate in conversations may cause the per| 22 |
son to isolate themselves. The caregiver can gently assist them in conversation helping them to respond in an appropriate manner. Saying, “don’t you remember” is another way of reminding them that they don’t remember. They may not remember the names of friends or family members. You can use the person’s name in conversation which will inform or remind the person without embarrassment. • Poor or shortened attention span can be solved by keeping conversations short with “reminding words” placed within your conversation. Example: Bill, your uncle who lives in California, called yesterday. Keep the area of conversation simple to avoid distraction and/ or confusion. This can help the
person stay in the moment and avoid confusion. Example: Too many pictures on the walls, more than one person talking near the person, and music playing during the conversation. • Difficulty in finding the correct words or speaking them correctly. You can bring them gently to remembrance by saying, “I was just thinking about…” (if you know, insert what they were trying to say or give them a clue) • Unable to or has difficulty recognizing family and friends. Make a page for each family member and close friend with
their picture and a few important facts that could come up in conversation. Before and/or during the visit assist the person to review this information. Disorientation, complete or partial to person, place, time, or situation. This can be very scary for the person with dementia and should be handled in such a way that would not cause embarrassment but allow them to comfortably remain in the visit.

Spray cooking spray on squeaky hinges to quiet them. 4. Is a bulb is broken in your lamp and you can't remove it, unplug the lamp first the

Every year on Veterans Day, we honor the people who risk their lives to protect our freedom. Social Security honors veterans and active duty members of the military every day by giving them the respect they deserve. Social Security’s disability program is an important part of our obligation to wounded warriors and their families. For military members who return home with injuries, Social Security is a resource they can turn to. If you know any wounded veterans, please let them know about Social Security’s Wounded Warriors website. You can find it at www.socialsecurity.gov/ woundedwarriors. The Wounded Warriors website answers many commonly asked questions, and shares other useful information about disability benefits, including how veterans can receive expedited processing of disability claims. Benefits available through Social Security are different than those from the Department of Veterans Affairs and require a separate application. The expedited process is used for military service members who become disabled while on active
military service on or after October 1, 2001, regardless of where the disability occurs. Even active duty military who continue to receive pay while in a hospital or on medical leave
should consider applying for disability benefits if they’re unable to work due to a disabling condition. Active duty status and receipt of military pay doesn’t necessarily prevent payment of Social Security disability benefits. Although a person can’t receive Social Security disability benefits while engaging in substantial work for pay or profit, receipt of

Nana's Comforting Creamed Onions
Prep time 30 minutes Serves 10-12
3-4 large sweet or yellow onions ½ stick butter ¼ cup Wondra flour or regular flour 1 cup milk or cream ½ tsp salt ¼ tsp pepper
My Nana made this side dish every year for our holiday feasts. It was one of my Brother Tom’s favorites. DIRECTIONS: � Peel onions and slice ¼” slices, cutting out hard center at the ends � Cover onions in saucepan with water and parboil for 15 minutes Drain onions and place in cooling spray coated casserole dish Melt butter in saucepan Stir flour into cold milk and add to melted butter and heat until thickened. Add more flour or milk depending on consistency desired. Stir in salt and pepper. Once heated to the consistency of a thick gravy, remove from heat and add to the onions and stir together. Onions will have a little water left in them so that will thin the sauce a little. Bread crumbs may be added but we like ours plain. Cover with foil and leave an air vent and bake. Garnish with favorite herb. Serve hot. ALTERNATIVE: Use carrots prepared the same way, slicing them ½” thick. Parboil for 25 minutes then prepare the sauce the same as the onions. My non-onion family members like these as well. Omit bread crumbs
