tโ€™s a New Year. Itโ€™s a new beginning. Itโ€™s an opportunity for us to begin writing the first pages of a brand-new, 365-page book. How will your 2016 story begin? New beginnings are full of hope. We get to be in control of what we write. We can choose to begin with a full smile โ€“ and a full pen โ€“ and create our own happiness. Itโ€™s my sincere hope your 2016 book is full of everything meaningful to you. This year also begins a new chapter for Aging Tree; another year we will have the privilege of helping people make decisions about senior care for their loved ones. Each year, we counsel thousands of individuals on this topic, and in almost all cases, it gets down to one thing: How will they pay for senior care and services? So, we thought it appropriate to begin 2016 with the first of three articles on the subject, beginning in this edition, and help answer questions and share tools to organize your planning. We are often asked, โ€œDoes Medicare pay for assisted living or home care?โ€ The short answer is, โ€œNo.โ€ Medicare covers acute or short-term care and services, but not long-term care. We will discuss different funding options in depth over the next three months. In addition, the New Year is bringing changes to our magazine, too. Weโ€™re collaborating with new partners and writers to inform you each month about various subjects. How to Pay For Senior Care

Part 1 of 3: Assets (Cash) and Insurance

t may not be the most pleasant experience to think about, but itโ€™s one of the most important financial issues youโ€™ll face during your retirement: How will you pay for senior care if your family canโ€™t take care of you? Several funding methods exist today to pay for senior care. The most common are assets (cash), insurance (long-term care insurance or life insurance), VA benefits or Medicaid, reverse mortgages, and trusts. In this first of article of Aging Tree Illustratedโ€™s three-part series, โ€œHow to Pay for Senior Care,โ€ we are focusing on paying for senior care with assets (cash) or insurance. (Check back in February for background and information on paying for senior care with Veterans Administration bene๏ฌts and Medicaid, and in March for funding sources that include reverse mortgages and trusts.) The information presented in these articles should be used as a navigational tool, and should not replace professional counsel by your trusted attorney, financial planner, insurance agent, and/or accountant. Understand the de๏ฌnition of โ€œsenior care.โ€ Senior care, as referenced in this article, includes independent living, assisted living, continuing care retirement communities, memory care facilities, nursing homes, hospice care, and in-home care.

The Navigational Tools

Get a feel for the costs. According to the U.S. Department of Health and Human Services (per 2010 statistics, the most recent available), senior care costs range from about $3,300 per month to $7,000 per month: โ€ข $205 per day or $6,235 per month for a semi-private room in a nursing home โ€ข $229 per day or $6,965 per month for a private room in a nursing home โ€ข $3,293 per month for care in an assisted living facility (for a one-bedroom unit)

Letโ€™s start with the basics.

Know your starting point. There are several important steps that experts recommend you attend to before exploring funding options for senior care. These include ensuring you are 1) getting the most from your Social Security benefits; 2) checking your veteransโ€™ aid and attendance benefits, if applicable; 3) and assuring your financial records are in order. These financial records may include your sources of income, assets, Social Security and Medicare benefits, insurance policy details, bank information, tax returns, wills, mortgages and debts, just to name a few.

Home healthcare aides, homemaker services, and services in adult day health care centers cost less. Know the primary funding categories. In general, there are four primary funding sources to assist in paying for senior care: 1) government programs, including Medicaid and veterans benefits; 2) insurance coverage, including life insurance and long-term care insurance; 3) private assistance from foundations or pharmaceutical companies; and 4) personal property and

A word about Medicare. Several experts we interviewed for this article mentioned Medicare and the confusion that surrounds it for many of their clients when it comes to paying for senior care. Bottom line: Medicare does not pay for senior care. Barbara Coenson, an Elder Law attorney based in Lake Mary, has counseled clients regarding Medicare and has witnessed the confusion regarding what it covers. โ€œAs people are getting toward retirement, they often use Medicare and only think in terms of Medicare for covering healthcare,โ€ she says, โ€œBut Medicare is a health insurance program thatโ€™s in place for people to get better. โ€œSometimes, you need healthcare, but youโ€™re not getting better,โ€ adds Coenson. โ€œThatโ€™s where Medicare coverage ends.โ€ Coenson gives an example. โ€œLetโ€™s say someone is in rehabilitation, and Medicare pays for 100 days of rehab, but they only used 30 days,โ€ explains Cocont. pg 9 |8|


enson. โ€œBut, after 30 days, they still require someone to help them with their care, so they transitioned into long-term care. Thatโ€™s the point at which Medicare stops paying. โ€œThis catches a lot of families off guard because, all of a sudden, they have to reach into their pockets and transition to home health care, or an assisted living facility or nursing home,โ€ she adds. โ€œThis is when families say, โ€˜What are we going to do now?โ€™โ€ Coenson says long-term care insurance policies can help seniors transition from Medicare use into assisted living or other senior care, and provide a funding source as families plan the next steps.

LONG-TERM CARE INSURANCE

According to the 2012 long-term care insurance Price Index, as available via The American Association for Long-Term Care Insurance, the costs for long-term care insurance can vary significantly (each below represents a daily benefit of $150 for 3 years): Aging Tree half page Ad_Layout 1 2/19/14 10:45 AM Page 1

Like other insurance products, many factors are considered when companies calculate long-term care insurance premiums. For example, your age, gender, current health, past health issues, and your family history of health issues play key roles. โ€œWhen I counsel people, we review their medical history, we examine the likelihood they will be looking for some senior care placement, and we talk about the alterna-

tives,โ€ says DeLand-based attorney Astrid de Parry. โ€œIn past decades, there was an opportunity to look for help in the family (for senior care) when extended family models were more common,โ€ she adds. โ€œPeople could reach out to family members. Sometimes, that is still an option, but not as often. The best plan is to have a plan and not leave it to chance.โ€ Barney Lane, Certified Financial Planner โ„ข in DeLand, agrees. โ€œMany clients do not picture themselves going into a nursing home; they picture themselves being cared for in their homes,โ€ he says, adding that in-home care often is covered in long-term care policies. โ€œI like to counsel my clients through these options,โ€ says Lane. โ€œFor example, I may want to get taken care of in my home when Iโ€™m older, but my wife might not be able to. Thatโ€™s why folks need to plan around it.โ€ Looking at all the alternatives and understanding the costs and benefits of every policy are keys to selecting long-term care insurance. If you anticipate you may prefer in-home care, look into that option when you are selecting a long-term care insurance provider. If instead you anticipate

youโ€™ll prefer or need a facility, get the details. Ask yourself: What are my goals and how does the product fit them? Your trusted consultants should steer you toward the best solution. โ€œSome of my clients have been fortunate to have purchased long-term care policies, and there are some good products out there that help them quite a bit,โ€ says de Parry. โ€œWhat is best and right for any single individual depends on that personโ€™s circumstances.โ€

LIFE INSURANCE

Experts say cash-value life insurance policies are not designed for long-term care coverage; they are designed as a death benefit. They do have a cash value, but that cash value comes to fruition upon the policyholderโ€™s death, oftentimes to be used for the needs of family members who survive him or her. However, several universal life policies (โ€œhybridโ€ policies) on the market are designed to pull from a death benefit over several years. These policies not only provide a death benefit, but also an opportunity to pay for long-term care via a policy rider. ANNUITIES (CASH)

Some annuity policies have long-term care riders that can be added to those contracts, too. These vary in type, costs, and benefits. Lane says the biggest risk when it comes to using assets to fund senior care is a longevity risk: people can outlive their assets. He says this comes into play when we spend more than we have to fund the number of years we live, or we experience a health event in that timeframe and itโ€™s not covered by insurance. Says Lane, โ€œYou donโ€™t want to be in a situation where youโ€™re home alone and your assets are running out. โ€œThe past days of people retiring with a pension are coming to an end, and there are few guaranteed lifetime benefits,โ€ says Lane. โ€œOur challenge is to determine how to make a clientโ€™s assets last a lifetime.โ€ Thomas B. Fleishel, Certified Financial Planner โ„ข in DeLand, also has insight regarding using annuities to fund senior care. โ€œIf a client doesnโ€™t need every bit of their assets for retirement income, they may be able to afford to carve off a portion for long-term care,โ€ says Fleishel. โ€œAsset-based long-term care policies use the assets to leverage the money for long-term care ben-

efits. That money can be removed for use in long-term care without being taxed.โ€ According to Fleishel, approximately 40 percent of Americans will need long-term care after the age of 65. So, he urges clients to look into the future as clearly as possible when it comes to how theyโ€™ll fund care in their final retirement years. Says Fleishel: โ€œIf you donโ€™t have coverage for senior care, and youโ€™re living on a fixed income, what will you do financially? Invest in Your Health

Because Saving Time and Money Isnโ€™t Enough

D

uring a recent conversation with my client, A.C., we were engaged in a wonderful discussion about his plans for the new year and his new found time freedom after having just retired a few days earlier. He was excited because he was receiving expert counsel about his financial portfolio. He was optimistic that he would have what he needed to move forward with this new season of life and long term care. When thinking about how to pay for senior care our minds naturally gravitate toward the financial issues involved, such as investing, Social Security, mortgages, trusts, and insurance including long term care. We examine the value of our assets and diligently plan with the help of experts in these areas for whatever will happen next, hoping we will have what we need. It warms my heart that Aging Tree has a wealth of resources in this extremely important area of life. Re๏ฌ‚ecting upon my conversation with A.C., there was one area of his portfolio that he had not mentioned. There was an investment strategy he had not included in his analysis. Dr. William Sears, author of Prime Time Health: A Scientifically Proven Plan for Feeling Younger and Living Longer identifies four key components of planning for prime time health: Save enough money, Save enough time, Nurture your relationships and Maintain your health. Why is health the essential ingredient? The fact of the matter is that not everyone is able to save enough money or have the time freedom they would like, but we all can make deposits into our IRAH at any age. Deposits to your IRAH will give you the opportunity to add years to your life and life to your years. So what is this extraordinary investment strategy? Your IRAH is your Individual Retirement Account for Health and it is not something you have, it is who you are. Did you know that by getting healthier, you might save a lot of money over the course of your lifetime by eliminating

costly medical bills? In a recent article in Moneywatch it noted that a couple retiring at age 65 could potentially save $100,000 or more. Books such as Discover Wellness: How Staying Healthy Can Make You Rich by Dr. Bob Hoffmann and Dr. Jason A. Deitch and several other resources speak to the extraordinary untapped resource that lies within us all at whatever age, stage or condition of life we may find ourselves. So are you ready to start the complete overhaul to position yourself for success? Of course not! Changing oneโ€™s habits, especially those acquired over time will take motivation, in-

spiration, perhaps some perspiration and time. For many of us that still sounds like work. How about making health your hobby? Find a buddy and support each other in your efforts to improve your health and vitality. Start learning your numbers. For example, what is your waist circumference and body-mass index (BMI)? What are your blood pressure, heart rate and cholesterol counts? What are healthy ranges for these numbers? What other numbers do you need to know? Read a book and surround yourself with people that inspire you in the area of health and wellness. Several employers and insurance companies sponsor wellness programs. Some programs even pay small financial incentives for participation. Take time to learn

By Cheree M. Albert MSN, ARNP, CPNP about wellness programs you may be eligible for; grab your buddy and GO! Taking a short brisk walk daily. If you already are, add a little more to your current exercise routine by adding 10 more minutes or carrying small hand weights with you. Improve one or two of your meals this week by increasing your intake of fruits, vegetables, grains and legumes. Try to reduce your consumption of processed foods, meat, sugar, sodium, fat and soda. Eliminate seconds for one or two meals. Before you start eating take just one serving, put the rest in a leftover container and into the fridge. Find five to 10 minutes each day to relax and get away from the things that stress you. Focus on your breathing -- inhale and exhale slowly. Plan for at least seven hours of restorative sleep each night. Seek help if this is a challenge. Schedule an appointment for your annual physical. Review what screening tests you are eligible for with your insurance carrier. Schedule your dental checkup. Unhealthy teeth can cause physical illness and shorten your lifespan. If you have an unhealthy habit such as smoking or consuming too much alcohol, reduce it by a little bit each week. Think one or two fewer cigarettes, one or two fewer drinks. Be kind to yourself. Remember that a new habit can takes months to acquire. These are just a few of the deposits into your IRAH that will make a difference in your health and wealth. Choose one simple change to do this week. Next week, choose another. At the end of each week, take a few minutes to re๏ฌ‚ect on your progress. Do you feel better, even just a little bit? Enjoy the results of your investment. Share with a friend and celebrate!

You wonder whether youโ€™ll end up losing your lifeโ€™s savings โ€“ or much of it โ€“ to senior care (which can include assisted living care, nursing home care, inhome care, etc.). You donโ€™t know what to do. You think itโ€™s too late to plan. As youโ€™ve aged and your physical or mental health has declined, you and your loved ones have started noticing itโ€™s getting harder for you to get in and out of the car, and getting up from the couch has become a struggle. Youโ€™ve started to forget things, too. You need help, but you donโ€™t want to be a burden on your children. So, you do research and learn senior care is expensive. In fact, the average annual cost of Senior care in Volusia County is about $42,500 for homecare, about

$45,000 for an assisted living residence, and about $95,500 for a nursing home facility, according to the 2015 Genworth Cost of Care Survey for Florida. https:// www.genworth.com/dam/Americas/ US/PDFs/Consumer/corporate/cost-ofcare/118928FL_040115_gnw.pdf. Maybe you thought Medicare or your Medicare supplement would pay for senior care, but they wonโ€™t, except in the rarest of circumstances and only for a short time. You suddenly realize that all or most of your life savings might be lost to senior care expenses, instead of going to your loved ones. Your savings wonโ€™t last long. You think you have too much money to receive government benefits that your tax dollars support, and itโ€™s too late to plan. But, take heart: itโ€™s rarely too late to plan. Although better results are nearly always obtained when we plan before a crisis arises, there are planning methods available in a crisis, too. For example, sometimes, the โ€œwellโ€ spouse of a married couple can keep much or all of the coupleโ€™s income, and have up to $119,220 of countable assets. Other times, the well spouse can retain an

Itโ€™s also critical to plan for the possibility that the well spouse could die first. You usually canโ€™t disinherit the ill spouse, who can only have $2,000 of countable assets. During a crisis, unmarried people usually preserve a decent amount of their assets, too. The most common methods include purchasing an automobile, purchasing a pre-paid funeral, establishing a โ€œburial account,โ€ and paying off a mortgage. Other planning methods are available, too. As many as 28% to 45% seniors fall per year. As we age we begin to loose our balance for various reasons. It could be from how the medications may effect our bodies, various medical conditions such as inner ear conditions, low blood pressure, neurological problems or from surgeries knee & hip replacements, foot conditions and impaired vision. Also environmental harasses (having scattered rugs, poor lighting in the home) and or lack of physical activity. The effect of drugs on the elderly, and the difficulties surrounding medication compliance, are also believed to be the root cause for many of fall incidents. Yoga is a wonderful exercise that will help you to not only keep your joints limber but it will increase your flexibility, coordination, range of motion, strength, balance as well as relaxation. โ€ข Resistance and weight training. With resistance and weight training is a slow and controlled movement which will helps the muscles to work against tension. Start with the lowest possible resistance band or weight slow increase. For example start with a 2 lb weight increase 2.5 lbs weight after 4 weeks or pending on what your physician says. Using resistance bands and weights will help you to increase your flexibility, coordination, range of motion, strength and of course your balance.

Strong Roots

Itโ€™s Beginning to Look a Little Like Winter

A

s I sit here just before Christmas with the beginning of winter just around the corner it seems like I should be saying BRRRR and be snuggled up next to my fireplace with a warm cup of tea as I write this monthโ€™s issue of Strong Roots. But the Florida truth is I am sitting at my desk in shorts with doors open drinking a glass of ice tea. It is the time of year that makes living in Florida worth all the months of heat and humidity. At least that is what I keep telling myself. Never the less we are prone to frost and freezing in our area that comes and goes just to remind us it is winter. With all the heat loving/ cold sensitive plants that grow around here we need to make plans to break out the blankets and towels and whatever is big enough to cover our sensitive plants. But what do you do when you need to protect something larger like a bed of vegetables? After all we do live in an area where gardening is not just a spring/summer event. On the farm we have a variety of ways to protect our crops from cold even down into the twenty degree range. Some of these methods may fit your needs and some will not be practical, but I thought I would go over some ways that we protect our sensitive crops. Our first method is to plant mostly cold tolerant crops. Many vegetables can tolerate our Florida winters with little to know extra protection. Most of the brassica family including cabbage, kale, broccoli, cauli๏ฌ‚ower and kohlrabi do well in cool weather and suffer little is any damage in a mild freeze. Spinach is a cold warrior and laughs in the face of a freeze. Most root crops will be fine safely snuggled in the warm earth. When choosing your winter seed you also want to look for the more cold tolerant ones. Most seed companies will give a comparison of cold and heat tolerances on their websites. I recommend Johnny Select Seeds. They do

a good job in this area. But what do we do when we still want to grow cold sensitive crops in the fall/winter season such as beans and tomatoes. After all it would not be Florida without fresh tomatoes in the winter. So what do we do then? First we plant so we can get the harvest in before the first freezes. Beans love heat and even a frost can kill them off.

So we plant early and then harvest until the first frost or freeze. Then we take then out and wait until the last frost in early march and replant. In other words we donโ€™t fight Mother Nature. The second thing we do is we shelter them in greenhouses. But this can be an expensive proposition even for a small greenhouse in a home garden. So the next best thing is what is called a low tunnel. This is a cheap and easy way for any home gardener to shelter their veggies of sensitive plants in the winter. To build a small low tunnel you can use anchor pins such as 2 foot long rebar rods or half inch electrical conduit and ยฝ inch

๏ข๏น๏€ ๏Š๏ฏ๏จ๏ฎ๏€ ๏Š๏ฏ๏ณ๏ฌ๏ฉ๏ฎ PVC pipe for the frame. You drive the anchors about 1 foot into the ground at the edges of the garden bed about every 6 to 8 feet. Then you fit the PVC over the anchors. The ยฝ PVC pipe is ๏ฌ‚exible enough that you can bend it easily. If it is a narrow bed you may need to cut the pipe in length. You only need to make it high enough to leave about a foot of space above the plants. On a long tunnel of more than 10 feet you may want to run a pipe along the top to steady the ribs and tie it together. We cover our tunnels with commercial frost cloth that is readily available from greenhouse suppliers in Apopka but it comes in 300 feet rolls. So if you donโ€™t need that much I recommend using sheet plastic or tarps. You can anchor the covering in a variety of ways such as long nails or ground cloth staples. But if you use plastic you need to remove it each morning to prevent the plants from cooking in the sun. Most of the supplies I have mentioned can be bought at your local box store like Lowes or Home Depot. Finally, if we face a really cold night below 30 degrees I recommend throwing a couple of 60 watt lights or a string of Christmas lights under the cover for heat. Remember this is the one time you donโ€™t want to be green wise and use LED lights. They donโ€™t generate enough heat to protect the plants. 1 whole chicken cooked 1 14 oz. can Veg All 1 can Campbells Cream of Potato Soup 1 can Campbells Cream of Celery Soup 1 can milk 1/2 tsp pepper 1 tsp. dried parsley 1 9โ€ deep dish frozen pie crust, defrosted but not baked 1 box refrigerated pie crust 1 egg slightly beaten DIRECTIONS: Preheat oven to 375 degrees. Tear meat off the bones into smaller pieces. Place in large mixing bowl and add additional ingredients, mixing well. Pour into pie crust. Cover top with other pie crust and brush with egg. Bake for approximately one hour.
