I wonder whether you share my enthusiasm. A New Year is inevitable, just like things ending or changing is inevitable. If nothing else, the passage of time and the change it brings that come in the form of “New Year” will arrive when you predict it will, every year, without ceasing, on January 1. It makes me think: If I had the choice to start a New Year on January 1 or a different date, would I keep the current calendar as it is, or would I change it up? Maybe I’d start declaring February 1st as the first day of my New Year. If I’m not ready for the New Year on that day, perhaps I’d be inclined to choose April 1st as the day. It’s a silly idea, thinking I could have control over the passage of time? The Gregorian Calendar has been part of our lives for more than 400 years. I suppose I should get used to it. While you and I can’t control when a New Year starts, or control any aspect of time, we can choose to make every moment as good as it can possibly be. That’s the first choice we face as we watch the clock tick onward: how we will spend each moment. Life is full of choices, decisions, conundrums, and situations that look a lot like something stuck between a rock and a hard place. I get it! Sometimes I feel like I make 50 decisions an hour, traveling at such a speed that time seems to simultaneously stand still and fly by (as if that were possible). We don’t always have time to weigh every option and treat every decision like it was our most important. Sometimes we must simply decide. But, there are some decisions that need more time and deserve more time, and those include where we will live, thrive, enjoy our golden years. Perhaps you are living that reality as you read this, and are in the midst of making a senior care decision for a parent or loved one. In this issue of Aging Tree Illustrated, we’re outlining senior care options with the hope that this content prompts you to ask the right questions, get the answers you need, and helps you determine the most ideal scenario for the situation. It’s one of those big decisions in life, and I encourage you to make the most of the resources available to you. Also in this issue, we offer you tips and tricks to make the most of your time this month as you navigate the kitchen with new recipes by John Sobol, fitness tips for staying healthy in the New Year courtesy of Odetta Canady, and perspectives from our contributors that cover everything from caregivers’ experiences of senior care to ideas on getting out and about and enjoying another fresh start in 2017. We don’t have control over when January 1 comes around, and its timing may not always be ideal, but with it – and with the start of each new moment – we have a chance to begin, decide, step out in faith, and repeat it all over again. Embracing fresh starts, using your time wisely for decision making and moving forward with confidence are healthy ways to begin a New Year with hope and enthusiasm. When that decision is your first decision, amazing things can happen. Make your days great…you’re in charge of them.

Making a Plan for Your Senior Living Lifestyle
Part 1 Assisted Living Options

n the first of a three-part series on senior living options, we take a look at Assisted Living Facilities (ALFs)--what they offer, what they cost, and what types of funding resources are available. In the next two issues of Aging Tree magazine, we will cover memory and continuing care (February issue) and nursing homes (March issue). The information provided in this series is simply a starting place. As you plan for the future, discuss your options with trusted family members and seek the advice of professionals. What is Assisted Living?
Before we get into types of assisted living, it’s important to remember that the ultimate goal of this option is to enable people to remain as independent as possible as they also receive the best services and care for their budget. Even within the same state, definitions of assisted living can vary. When considering this senior living option, it’s important to know exactly what each facility offers and what the monthly costs include. In general, assisted living facilities range from single-family homes to large communities and offer different levels of assistance with everyday activities like cooking, cleaning, laundry, and medication.
What types of Assisted Living are available?
Many seniors live in their homes or in age-restricted peer communities where they enjoy the most autonomy. If needed, they may choose to hire at-home help from personal caregivers or home health care services. That scenario changes primarily for two reasons: 1) seniors and their families make a
reactive decision based on a change in health that requires a person to acquire a different living situation; 2) a person plans ahead for their senior years and chooses a specific option to fit their lifestyle wants and needs. Residents usually live in an age restricted apartment complex setting and pay monthly rent. Such facilities are licensed solely as independent living and offer no medical or personal care services. Independent Living Option-Assisted Living Facility For those who anticipate needing an increased level of assistance at some point, some assisted living facilities also offer independent living. If additional care is required, a resident can then easily transition within the same community and have access to assisted living services. For those individuals affected by memory challenges, a standard Assisted Living Community can be a good fit as long as the individual is not exit seeking or needing special care services. Usually a smaller type of home is a good fit for these individuals providing more interaction and the ability to watch over the resident in a smaller setting. Alzheimer’s and Memory Care Some assisted living communities provide special-

Continuing Care Retirement Communities (CCRCs) Large CCRC communities, composed of single-family homes, villas, apartments, and other facilities, provide a variety of senior care options in one setting. This allows residents to advance in treatment and care as their needs grow throughout the aging process. This premier option offers peace of mind but also comes at a higher price. (Stay tuned for more on this in the February issue of Aging Tree magazine.) Extended Congregate Care or Limited Nursing Services (ECC) (LNS) Acting as a bridge between assisted living and nursing home care, an ECC or LNS community provides a full-time nursing staff for residents and can provide additional care not permitted in a standard assisted living community. This type of facility can be a good solution for those individuals needing more care but not wanting full nursing home care.
How much does Assisted Living cost?
As emotionally charged as planning for your long-term care can be, those feelings can become compounded once you start to figure out the costs involved. Nationwide, the average cost of assisted living in 2016 was approximately $3,600 per month. However, costs vary widely by state and region and can be nearly twice that price, depending on features such as private rooms, cont. pg 10 |8|

What are possible funding sources for Assisted Living expenses?
For an in-depth look into this topic, read Aging Tree’s three-part series “How to Pay for Senior Care.” In addition, AARP provides a handy, online long-term care calculator. You can compare estimated yearly costs of assisted living, nursing homes, adult day care, and home health aid between up to two states and regions. In general, sources of funding include, but are not limited to: Personal resources About four out of five people pay for assisted living expenses out of their own pocket, according to AARP. Other personal means of payment may include savings, reverse mortgages, long term care policies, home equity lines of credit, and assisted living loans, among others. Medicare Many seniors are surprised to learn that Medicare DOES NOT cover the costs of assisted living or other forms of longterm care and services.
Medicaid In most states, Medicaid, funded by the federal government, offers some financial assistance for assisted living, usually via Medicaid HCBS Waivers (Home and Community Based Services). Assistance varies depending on the state’s definition of “assisted living,” an individual’s eligibility, and other factors. In addition, not all facilities accept Medicaid. The state of Florida has eliminated waivers and provides assistance through its Statewide Medicaid Managed Care Long-Term Care Program health plans: Health Maintenance Organizations (HMOs) and Provider Service Networks (PSNs). The Florida Department of Children & Families offers a cash assistance program to eligible, low income seniors called Optional State Supplementation. A monthly cash pay-
ment helps offset the room and board portion of the fees for assisted living. This benefit is not available to those already receiving Medicaid benefits for assisted living. An eye-opening example from AALTCI’s 2015 Long-Term Care Insurance Price Index reflects a difference in cost of 119% per year and illustrates the importance of getting cost comparisons before purchasing this type of insurance. Single Female, Age 55: $164,000 Benefit Company A: $890 per-year Company B: $1,829 per-year VA Aid If you are a veteran or survivor covered under a VA pension, you may be eligible for the Aid and Attendance Benefit, which provides an additional monetary payment. In general, this program works better for those choosing assisted living, while Medicaid is preferred for nursing home care. Although the two programs can’t be used concurrently by one person, a couple may be eligible to receive benefits under both programs, with one spouse covered by Medicaid, for example, and one by the VA benefit. Seek the counsel of a Aging Tree, Medicaid or VA planner who will explain factors such as the benefit’s long application processing time and varying asset limitations. When to Consider Assisted Living
From One Caregivers Perspective...Is It Time?
I
am one of those people that cherishes the holiday season though I am always eager to move forward with the new year. The celebrations and festivities are wonderful but there is much more to it than that as we know. There is also grief, hurt, pain and just plain “messiness” in our lives. I truly cherish the challenge of digging deeper in these circumstances to find the joy and hope for the future. For the more than
settling in can be present at other times of the day as well. This presents a real challenge to the caregiver’s ability in the areas of nutrition, safety, family routines and more. A particularly distressing challenge for caregivers occurs when verbal, physical or sexual aggression escalates. A symptom of the disease process, this can pose a very real threat to the emotional and physical state of the caregiver and family.
44 million Americans caring for a loved one with dementia or other debilitating condition it is very likely that the holidays presented an additional challenge emotionally and physically. With visits from family and friends there may (or may not) be new awareness, understanding, and appreciation for the scope of caregiving. As we cast a vision for 2017 we have the opportunity to evaluate what has changed with our loved one and family to design a plan for next steps. Let’s look at selected aspects of the decision-making process for insight and solutions. As you look at the care needs for your loved one consider where you are in these areas: As the stage of dementia increases so often does the wandering. This may be outside of the house or within and can happen in the time it takes for you to shower. The chance for falls and other injuries increases as your ability to keep them in a safe secure area is decreased. Home safety issues are an ever-present challenge and this provides a new dimension of complexity. “Sundowner Syndrome” is a highly agitated state of behavior that typically occurs later in the day. This agitation and difficulty
It is not uncommon for health care needs to increase over time. This includes health care needs of the caregiver as well. Nearly three quarters (72%) of family caregivers report not going to the doctor as often as they should and 55% say they skip appointments for themselves. Poor eating habits, decreased exercise, weakened immune system and greater risk for chronic disease has also been reported among caregivers. Sound familiar? There is much written about the psychological costs of caregiving and of making difficult care decisions. Some professionals find similarities between this and aspects of post-traumatic stress disorder symptoms where the brain is always on alert and there is routine sleep deprivation and exhaustion. It is no surprise that the emotional, mental and physical effects of caregiving can be particularly difficult for the spouses and senior children of those who need care. A dear colleague of mine who suffered a heart attack during caregiving is now weaning from most of her medications and has a level of health and vitality she never knew possible. Additional areas of caregiver stress include employment stress due to interruptions in meeting work expectations and financial

By Cheree M. Albert MSN, ARNP, CPNP stress. Family relationship stress is real. Last, but not least, is the subject of guilt. It has been said that during caregiving the guilt can be constant. Guilt for not doing enough, caring enough for your family’s needs, feeling overwhelmed then feeling guilty for having negative feelings. It is comforting to be reminded that this is a situation over which we have limited control. Alexis Abramson, a gerontologist and author of The Caregiver’s Survival Handbook adds that “we are in control of how we react to it” which is empowering. Barry J. Jacobs, a psychologist and author of The Emotional Survival Guide for Caregiver’s offers this simple tool to caregivers “to accept that having negative feelings about caregiving is normal”. VOTRAN GOLD SERVICE GETS YOU WHERE YOU NEED TO GO As we move forward in life, we make adjustments based on our mobility and lifestyle. Votran is here to help with those changes. Did you know that Votran’s award winning service operated on regular fixed routes also has a companion service called Gold Service? Gold Service offers transit mobility services for those who need a little extra help when it comes to their transit needs. Gold Service is Votran’s door to door, reserved ride service that requires an application process to determine eligibility. Once your eligibility is approved, Gold Service riders call a day, or several days, in advance of their trip to make a reservation. When calling in for a reservation, the call center agent will ask for the specific address of your pick up and drop off locations for your trip, and you should be sure to tell them if you will travel with any mobility assistance such as a service animal, walker or wheelchair. Gold Service can get riders to the same destinations that our fixed route buses can in Volusia County. Votran’s Gold Service operators are trained in great customer service, safety and awareness of the different mobility needs of riders. Since Gold Service is door to door, the operator will come to the door and escort the rider to the bus. After arriving at the destination, the operator will escort the rider to the entrance of the business or facility. This is a public shared ride service where other riders may be boarded and deboarded along the route to your destination. Getting you to your destination on time, comfortably and safely is paramount to Votran’s Gold Service mission. This year, Votran was recognized with two awards from the Florida Commission for the Transportation Disadvantaged. Votran received the 2016 Safety Award, and Arlene Thompson was honored as the 2016 Dispatcher/Scheduler of the Year. As you can see, Votran takes providing safe transportation and quality customer service very seriously. The Votran Gold eligibility application is available in a number of ways. Glaucoma Awareness Month
Get an Eye Exam for the Silent Thief of Sight Glaucoma is a disease in which damage to the optic nerve leads to progressive, irreversible vision loss. It is the second leading cause of blindness in the world – it is estimated that over 3 million Americans have glaucoma, but only half of those know they have it. With Glaucoma Awareness month upon us, it is so important to get an eye exam for this silent disease. Anyone can get glaucoma, but risk factors include having a family history, diabetics, and the elderly. African Americans have a higher risk as well. However, it is still important to get checked because the most common type of glaucoma, open-angle, has no symptoms early in the disease. Open-angle causes gradual loss of your peripheral vision, and can lead to blindness if left untreated. Your ophthalmologist makes the diagnosis of glaucoma during an eye exam and looking at your eye pressure. While it is more likely you will develop glaucoma if your eye pressures are high, some peo-
ple with high eye pressure never develop glaucoma, and some people with normal pressure can develop glaucoma. In addi-
tion to checking your eye pressure, your eye doctor will dilate your pupil to examine your optic nerve. Your doctor may also use a diagnostic machine such as the OCT to visualize and assess damage to your optic nerve. If damage is severe enough, peripheral vision changes can be detected on a Visual Field Test. Patients won’t notice any vision changes until late

in the disease when there is significant vision loss. Unfortunately, the vision loss is irreversible so it is important to get diagnosed as early as possible. The only treatments available are those that lower intraocular pressure. Lowering eye pressure can be accomplished using eye drops, laser, or surgery. The choice of treatment depends on many factors, which are unique to each patient and should be discussed with your doctor. Currently there is no cure for glaucoma, but with early diagnosis and treatment your ophthalmologist can help control the disease before vision loss or blindness occur. INSIDE ADVISOR The Holidays are over, and we’ve all survived another year circling the sun. Happy New Year!! Last month, you may have gotten together with loved ones you haven’t seen in a while. You may have discovered that it isn’t safe for them to live at home alone anymore. You should recommend that they consider moving to an “assisted living” community. With that suggestion comes a world of fears. The most common worries experienced are loss of economic security and independence. Those fears can be overcome. Anxiety is many times caused by a misunderstanding about what assisted living is. Many people don’t realize that there is a big difference between a nursing home and an assisted living community.
Legal - Affordable Eldercare or Assisted Living Some clients tell me that their Assisted Living Community is like living on a cruise ship. Others enjoy the home-like setting that their community offers. However, many of my clients are shocked by the price of Assisted Living. I’m always surprised by that because many forget to consider that living at home costs money and as importantly, comes with risk. Many, if not all, of these expenses disappear or are drastically reduced when living in an assisted living community. More importantly, the risks are all but eliminated. When living in an assisted living community, the client no longer needs to worry about the roof, fallen trees, the air conditioner, appliances, etc. The stress relief for the client and their loved ones is palpable. Nevertheless, there usually is a shortfall between one’s total income and the cost of an assisted living residence. That could put a strain on finances. However, there are solutions. The Government has programs that will make the cost of an assisted living community more affordable.

Options For Vision Loss Macular Degeneration (AMD) is the number one cause of vision loss, affecting more than 11 million Americans. More than glaucoma and cataracts combined. There are two basic forms of Macular Degeneration wet and dry. Around 90% of the cases are the “dry” (atrophic) type, This happens when yellow deposits called drusen accumulates under the retina, causing cell death resulting in gradual vision loss. Around 10% of cases are the “wet” type (exudative). Wet AMD occurs when abnormal blood vessels form behind the macula. These blood vessels are weak and have a tendency to leak fluid and blood resulting in scarring of the macula. Vision loss can happen rapidly with the wet type. If you experience distortion in your central vision, wavy lines etc. See your Ophthalmologist as soon as possible. There are treatments for the wet form that may slow the progression. There’s no cure for AMD at this time. However, there are precautions you can take to possibly slow the progression once you’ve been diagnosed, such as a healthy

These may allow you to watch TV, a sporting event etc. Some are small enough that they can be mounted on a regular pair of glasses.
4 - Reading devices. There’s several devices that can convert text into read-aloud speech 5 - Prismatic eyewear. These are manufactured to re-direct images to a useful area of the macula and can be used for one’s near as well as distance. These are some of many options. Results can vary from person to person.
