Perception vs Reality Part 3: Nursing Home vs Assisted Living Communities – Understanding the Differences FEATURE ARTICLE

F

or many years, when seniors could no longer live independently or families were unequipped to care for elderly loved ones, nursing homes were usually the sole option. Things have changed, and today, seniors have many more living options to choose from. One thing that hasn’t quite changed, unfortunately, is the misconception that all of those choices are synonymous with “nursing home.” This has been exacerbated by the negative connotations associated with nursing homes-often unfairly, sometimes understandably so. In Part 3 of our three-part series on senior living options, we are looking at the differences between assisted living and nursing homes to help clarify the commonly-held belief that they are one in the same. This misconception can contribute to families completely avoiding the topic of senior living and/or elder care until an unexpected life change forces them to make a reactive, rather than planned, decision. Not planning ahead for care in later years can result in emotional and financial stress when families are forced

to decide too quickly on a caregiving solution. As mentioned in last month’s feature (Part 2), making the best choice for senior living involves making the most informed choice--and sooner rather than later.

So, let’s clarify: Assisted Living Facilities (ALFs) are not nursing homes. Neither are Continuing Care Retirement Communities (CCRCs), although a nursing home--also known as a Skilled Nursing Facility (SNF)--may be an option offered within the setting of a CCRC. ALFs and nursing homes usually differ in 1) setting, 2) a resident’s level of independence and the level of care provided, and 3) costs.

Types of ALFs may include Independent Living, Continuing Care Retirement Communities, and Alzheimer’s and Memory Care. Aging Tree’s CEO Corey Shenk counsels families, couples, and individuals every day on living options and ways for seniors to maintain their independence. “Assisted living is meant to provide a vibrant atmosphere,” Shenk says. Depending on the facility and the services it offers, that care may include everything from meal preparation to housekeeping and other daily activities. ALF environments also provide social interaction and additional services such as transportation. They can even have a “country club feel”, offering amenities such as golf courses and spas, although these types of communities come with a heftier price tag. Nursing homes are often designed as single-level structures with rooms housing from one to three residents who share a bathroom. rehabilitation area, and spaces where residents gather for social activities. Rooms are traditionally arranged along long hallways, similar to a hospital setting, in order to facilitate ease of care. But design models are changing to encourage more communal living and interaction among smaller groups of residents.

Different Levels of Independence and Care

One of the biggest fears seniors face is loss of independence. In fact, one study found that seniors feared moving into a nursing home and losing their independence more than they feared death. Yet, maintaining independence is the main goal of assisted living communities. The level of assistance with daily tasks, services, amenities, and costs varies widely among ALFs.

In Florida, the law requires an ALF to use an assessment form that is filled out by a potential resident’s physician. This forms provides the information facility staff need to make the best recommendation for the type of help a resident may require, such as bathing or medication management, and the cost for those services. If a person is “bed bound,” an assisted living facility cannot accept him or her. The common criteria a person has to meet to be able to still reside in an ALF is to be able to “stand and pivot.” Again, the level of care and assistance varies among individual facilities. Nursing homes provide custodial care--feeding, bathing, dressing, etc.--as well as skilled nursing care and other medically related care 24 hours a day, seven days a week. Federal law requires that these facilities also provide

rehabilitative, pharmaceutical, dietary, emergency dental, activities, room maintenance, and personal hygiene services to the patient. Shenk describes nursing homes as consisting of a “Wing A” and “Wing B,” with A providing rehabilitation and short-term care and B providing long-term care. Shenk says many residents who enter nursing homes do so via the hospital following an illness or injury. Patients enter short-term care and/or rehabilitation in order to regain the level of health they enjoyed before being hospitalized. This helps ensure the best transition back into independent living. Unlike assisted living facilities or short-term care in a nursing home, long-term nursing home care becomes necessary when a person can no longer function independently due to a high acuity

level (i.e., the level of severity of a condition) or a person has no other place to go. In these cases, a nursing home can be the appropriate option. For individuals needing more care but not wanting full nursing home care, an Extended Congregate Care (ECC), or Limited Nursing Services (LNS), community can be a good solution. Serving as a bridge between assisted living and nursing home care, this type of facility provides a full-time nursing staff for residents and can provide additional care not permitted in a standard assisted living community.

The Differences in Costs and Funding for Care

According to Genworth Financial, Inc., the average monthly cost in 2016 for assisted living in Orlando, for example, was $3,250 while the monthly cost of a semi-private room in a nursing home was $7,604. Be aware, these costs vary depending on location, facility, and services. Another common misconception regarding senior care is that care is covered by Medicare. However, Medicare DOES NOT cover the costs of assisted living or other forms of long-term care and services. Normally, Medicare or a replacement plan does cover the costs of 30-100 days of short-term care or rehabilitation. Following

quires care may be transferred to the Medicaid-certified, long-term care (Wing B) of a nursing home. In this situation, the patient will pay out-of-pocket from personal resources, savings, or long term care insurance each month. If those assets are exhausted and the resident is eligible, Medicaid will then cover care. In most states, Medicaid offers some financial assistance for assisted living, depending on the state’s definition of assisted living, an individual’s eligibility, and other factors. In addition, not all facilities accept Medicaid. Even if a person hasn’t qualified for Medicaid in the past, a nursing home resident may be eligible for coverage. In addition, the state of Florida provides assistance through its Statewide Medicaid Managed Care Long-Term Care Program health plans: Health Maintenance

The elder care industry, like other industries, can’t help but change with the times and for the better. Starting a new medication regimen, moving from the family home, no longer driving or bringing in help can present a challenge even if you, your family and parent has been receptive to such discussions in the past. Ken Robbins, a geriatric psychiatrist at the University of Wisconsin-Madison shares that “there are two fundamentally different types of parents”. “Those with whom you have a relationship in which you can be straightforward and they welcome your

David Solie, author of How to Say it to Seniors writes that “parents want foremost to maintain a sense of control and dignity in a season marked by many losses”. Frequently checking our internal GPS to recalculate where we are in the journey is essential. Moving forward slowly, with care and caution will go far to balance differing needs and goals. Having served as an “assistant navigator” there are a few thoughts to consider before jumping in. Take some very focused time and do your homework. Engage close family members in the process at this point to the extent that they are willing and able. Perhaps the first subject to gain clarity about is the extent to which family and extended family will participate in this process and what form that will take (moral support, day-today hands on, financial?) Talk to the extremely knowledgeable staff at Aging Tree and get connected to a wealth of resources. Collect information and research a variety of possible strategies. forward gracefully when we try to dictate the result or convince someone to do something they do not want to do? I am thinking not very often. The most common sensitive issues relate to driving, health issues, in-home care and moving or relocating. These are complex and often emotionally charged topics. Gathering facts first provides a greater opportunity to problem solve together in a less stressful way. It may make the situation feel easier and more possible. At many mileposts along the journey of life we endeavor whenever possible to choose the “right” moments and this is no exception. Testing the waters to see if there is an openness for conversation with close attention to the positive is key. “How’s the car?’ and “What did the doctor say?” are two possible examples. Following and understanding your parent’s cues can be enhanced by using reflexive listening, an effective communication technique for us all especially for challenging conversations. Rephrasing what is heard, as a way of playing back what you understand, makes the

individual feel supported and moves the conversation forward. Here are a few examples: “What I hear you saying… It’s also worth thinking about this…” and “That sounds very upsetting for you… Have you thought about…?” After all the goal is to show that you want to help in a respectful non-judgmental way. It is not uncommon for one family member to have greater opportunity for observation and conversation. Having other family members who may not live nearby take the initiative to connect on a regular basis is foundational to good communication. In some situations, either among family members or with parents there can be a great deal of resistance that challenges the possibility of moving forward with the family conversation. In this case, a neutral third party can be called in to assist. This can be especially beneficial when the issue is critical and there are great risks. When your parent is of sound mind and making decisions that you disagree with (not endangering ones) consider the value of continuing the conversation in a

positive way even if you are worried or frustrated. In the presence of a sound mind the choices are ultimately his or hers. Take the opportunity to revisit the matter later. Have empathy and assure your family member that you will be there at each milepost along the journey. Frequently checking the internal and external GPS will keep you on the right path. INSIDE ADVISOR Legal News that you need nursing home care should be greeted with hope and optimism. The care and surroundings at some nursing homes is excellent. But you do need to prepare for the cost by exploring ways that it can be more affordable. First Step: Overcome Denial. Accept that there is an 80% (approximately) chance that a person people over age 65 will need some kind of long-term care, and that loved ones might not be able to handle your needs. Dry Eye Syndrome At the end of the day, after staring at a computer screen for hours, your eyes start to feel tired and sandy. You may be one of the 30 million Americans that suffer from dry eye disease, and not even know it. Dry eye is a condition in which a person doesn’t have enough quality tears to lubricate and nourish the eye. A healthy tear film is necessary for good vision and overall eye health. Chronic dryness can cause the surface of the eye to become inflamed, which may reduce tear production even more. This results in a continuous cycle of dryness. There are many causes of dry eye - age, environment, medications, medical conditions, a history of eye surgery, and contact lens use. Elderly people frequently experience dry eye, but it can occur at any age. Symptoms can vary from blurriness, burning, scratchiness, redness, and

discomfort from wearing contact lenses. While it may sound strange, people with dry eye often have tearing, because the eye is responding to the underlying irritation. It is diagnosed in an ophthalmologist’s office with a comprehensive exam and various tests. Depending on the cause of dry eye, your doctor may use various approaches to relieve the symptoms. It is important to know that this is a chronic condition and there is no “quick fix.” The first step is avoiding the environment factors that aggravate the condition such as smoke and fans. A humidifier can help add moisture to dry indoor air. Remembering to take breaks when working at a computer for long periods of time can go a long way. High Blood Pressure (HTN) High Blood Pressure (HTN) is a very common problem in the USA. There are NO symptoms of HTN. It will creep up on you and cause long term problems if left untreated. I see people every day who just had a blood pressure (BP) checked somewhere or even at home and rush to the ER thinking that it will kill them today. High BP is not the cause of sudden death but can contribute to other problems that will kill you over time. I have seen 250/150’s walk in to the ER for an unrelated problem and never knew their BP was that high or how long it was that high in the first place. If the pressure in your home pipes are high they will ultimately cause leaks and possible a broken pipe. The long-term problems are in the organs that pump (heart) and filter

W

hen I travel I love to learn about the legends and lure about a specific destination. Have you ever seen the movie “A Close Encounter of a Third Kind”? The Steven Spielberg movie about alien encounters just happens to be going into its 40th anniversary this year. Want to know why I brought it up? We will be going to have our own close encounter on our upcoming trip to Devils Tower this June. If you have never been to Devil’s Tower in Wyoming just outside of Yellowstone National Park you may find the legend very interesting. It comes with great Native American stories around this rock formation. The story goes something like this. One day two young Indian boys ran off and were lost. When they became tired they slept, hungry they would hunt, and soon enough four days had passed without their family coming to get them. They began feeling as if something or someone was following them. And something was… it was a big, huge, monstrous bear named Mato. The bear was hungry and was going to eat the boys. Of course, what

is any hungry bear to do? The boys only could do one thing in this moment, as they knew their bows weren’t strong enough to

kill the bear and their legs weren’t fast enough to outrun him- they prayed. It is Time to Plant a Spring Garden

W

ith a milder than usual winter this year now is the best time to get a jump start on your spring garden. With a diminishing chance of a freeze this winter we decided to play the odds and began planting some of our spring crops in early February. But it is definitely time to start with your spring planting. While it may be a bit late to start with cooler weather plants like broccoli, cauliflower, spinach or cabbage, there is still time for plenty of other choices. The cucurbit family offers several popular vegetables that do well in the warmer weather of our spring. This group of plants includes cucumbers, summer and winter squash, pumpkins, watermelon, muskmelons and gourds. This family of plants is characterized by their bright yellow flowers. They do well in well drained but fertile soil and prefer the warmer days of our Florida spring. Nutritionally they require only small amounts of nitrogen but higher amounts of potassium for fruit development. Generally they do well into the warmer months of May and even June. The challenge with them is trying to keep the green pickle worm from devouring the fruit. On the farm we plant early and harvest as much as we can before they arrive in the area usually in early April. Another excellent crop to plant right now is beans. Most beans love the warm days and spring up

quickly. You can of course plant bush beans or you can build a simple trellis and try your hand at pole beans. We prefer the pole beans at Common Ground Farm as they tend to out produce bush beans and there is less back breaking work to picking them. There is also the aesthetic beauty of a trellis covered in vines full of beans that I never get tired of seeing. For a really good bean we grow both Fortex (a green bean) and Monte Gusto (a yellow bean). You can find both of these online at Johnny’s Seeds. Of course the time is right for tomatoes as well. Whichever variety you choose you will want to look for a good disease resistant strain. Tomatoes originated in the Peruvian mountains and prefer a drier cooler climate. Florida’s warm humid climate is less then favorable for tomatoes. They are prone to fungus and other diseases. For that reason we stay with hybrids that have been grown to resist diseases common to them in our area. Charger is probably the easiest to obtain on a garden for a home garden. You can find them again on line at Johnny’s Seeds. Other vegetables to consider include peppers, eggplant, okra and some lines of heat tolerant lettuce. Space prevents me from going into more detail but you are more than welcome to attend a free tour at the farm any Saturday at 10am and ask all the questions you can think of. Festive Irish Leprechaun Stew

By John Sobol

Celebrate St. Patrick’s Day this March 17th with this festive stew. It is perfect for leftover meat. Save all your pork and beef roasts and prepare my Irish mother’s recipe. DIRECTIONS:

Place meat in plastic with flour and shake to coat. (if needed) Place 1 tbsp oil in frying pan add meat and brown (if needed) or place in large pot on med heat. Add bay leaf thyme rosemary salt & pepper add broth and wine and simmer.

Recipe: Leprechaun Stew From the Kitchen of: John Ingredients:

Serves:4-6

1lb of stew meat beef, pork or both 1 med. onion chopped 2-3 celery stalks chopped 2 carrots sliced 1 pkg mushrooms 14 oz can tomatoes 2 red skin potatoes chopped ½ cup peas & green beans 1 large can beef broth 1 bell pepper sliced

1½ tbsp Worcestershire 1 clove garlic ½ c flour 1 tsp thyme 1 tsp rosemary 1 bay leaf 1 tbsp salt or to taste ½ tsp pepper ½ c red wine 2 tbsp oil

Mean while, heat oil in frying pan and cook celery , bell pepper and carrots for about 5 mins, add onion and mushrooms cook another 5 mins add garlic the last 45 seconds. Add to pot peas, green beans, tomatoes and red potatoes let simmer and stir often Cook 2 - 3 hours or longer, remove bay leaf. Enjoy with biscuits!

How does the reception area look, smell, and feel? Request a tour of the facilities you are considering before selecting a facility. 2. Do the current residents appear happy and cared for well? Observe the temperament, condition, and care received by resident’s as you are touring. 3. What amenities are included in the care contract? What comes with a standard room? Are there private or semiprivate rooms available? If private rooms are available, are they standard or do they come at an extra fee? 4. Is there physical, occupational and speech therapy available on-site? Are there dedicated physical, occupational and speech therapists on staff? 5. What is the nurse staffing rating of the facility specific to RN to client ratio? Does the facility staff mostly RN’s? Do the RN’s have bachelor’s degrees? LPN’s? CNA’s? Is the facility adequately staffed to provide your loved one with the level of care they need? 6. Is the food served nutritious and appealing? Ask to observe a meal. What is the protocol for communicating concerns and care events to family members? 9. What type of insurances are accepted? Does this facility take your loved one’s health insurance?
