Seven out of 10 adults take prescription medications, according to Mayo Clinic. That probably means YOU. You are the target audience for this campaign! Awareness leads to education, and education leads to informed decision-making. Be the person in the doctor’s office who knows the most about your medications; don’t leave it to the MDs. Remember that to be educated is to be empowered, and to be empowered is to have more control…even over your health. Join me for this daily “Know Your Meds” awareness campaign, and show your pride in understanding how important it is that all of us know our meds. It’s a life or death health issue that’s as important as any other observance on the calendar. Make your days great…you’re in charge of them.

What's in Your Medicine Cabinet?

Part 2: Over-the-Counter Medication In Part 2 of our three-part series What’s in Your Medicine Cabinet? We’re talking about over-the-counter (OTC) medication. Check out October’s issue for Part 1: 6 Concerns for Your Prescribed Meds, and be sure to look for Part 3: Supplements/Vitamins & Herbals in December’s issue. Last month, we discussed the aspects of prescription medications that can pertain to everyone, including cost, potential interactions, and basic storage and expiration dates. Much of that also applies to OTCs.

ou can feel it coming on…that scratchy feeling at the back of your throat, the sniffly-stuffy nose, and sneezes that scare the dog. Even with the millions of dollars poured into research over the decades, no one has discovered a cure for the common cold. Before putting a pot of chicken soup on to simmer and brewing that cup of tea--the old standbys-you head to the corner drugstore for an over-the-counter “cure.” The choices make you dizzier than your head cold. In fact, cold medicine is one of the most commonly used OTCs, says Jack Lay, R.Ph., of Medicine Shoppe DeLand. Others include those used to treat pain, allergy/sinus conditions, heartburn, cough/flu, constipation/diarrhea, and skin (dermatologic) issues. After years of trying various types of OTCs, we all know these medications may have limited effects on symptoms; some may work for each individual better than others work. Some people swear by common aspirin to soothe ache-y joints, while others find more relief from ibuprofen or similar non-steroidal anti-inflam-

matories. For some, effectiveness may depend on the brand, and often to save costs, people will use store brands of all these OTCs. Effectiveness can also depend on how OTCs have been stored in your home. As with prescription medications, OTCs become less effective over time or when stored improperly. All medication should be stored in a dry, cool place (NOT the bathroom medicine cabinet). In addition, be sure to check the expiration date of your medication before taking it. No matter the malady, OTCs can be a first-line of defense for consumers, because of both convenience and cost. In fact, 8 in 10 consumers use OTC medicines to relieve the symptoms of a range of illnesses.¹ While OTCs may seem safe due to their easy access and availability to the consumer, they can be harmful when used without the guidance of a healthcare professional. Care and caution still need to be used when taking nonprescription medication, which is the most safe and effective when you have sought the advice of a healthcare professional and follow the directions on the label. This can be especially true for older

According to the U.S. Food and Drug Administration: • “Increased access to OTC medicines is especially important for our maturing population. Two out of three older Americans rate their health as excellent to good, but four out of five report at least one chronic condition. • Fact is, today's OTC medicines offer greater opportunity to treat more of the aches and illnesses most likely to appear in our later years. As we live longer, work longer, and take a more active role in our own health care, the need grows to become better informed about self-care. • The best way to become better informed—for young and old alike—is to read and understand the information on OTC labels. Next to the medicine itself, label comprehension is the most important part of self-care with OTC medicines.”² The side effects of prescription medications are well-known; we’ve all heard the speedy narration on television advertisements of the long list of potential side effects. Lay says it is important to use caution when using multiple-ingredient combination products-such as a cold medicine that offers relief from congestion, coughing, and fever--because these medications may increase the risk of therapeutic duplication and/or unnecessary drug use. The latter proved true for Mary C. who ended up in the emergency room after taking a 12-hour sinus medication. Mary had been to the doctor days before and was treated for allergies but continued to experience worsening symptoms. Rather than return to the doctor, she bought a popular OTC in hopes of clearing up her severe congestion. “An hour after going to bed, I got up feeling terribly sick to my stomach, so I went into the bathroom,” she says. “I felt dizzy and sat on the floor. Then everything went black.” Mary said her husband and daughter called the ambulance after Mary seemed unable to stand. The ER doctor said her blood pressure was low and also diagnosed her with a severe sinus infection. “I didn’t read the label or research the medication before taking it, otherwise I would have known that two of the side effects are nausea and dizziness,” she says. “I also realize now that trying to self-medicate without talking to my doctor first wasn’t the best decision!” As with any medication, you should talk with your doctor or the pharmacist, even if you have taken a certain OTC in the past. If since then you’ve started a new prescription medication, for instance, you could experience a reaction you hadn’t experienced in the past.

This is why it’s important before choosing an OTC to check for potential interactions. Lay suggests visiting the website www. drugs.com, which offers a free and reliable drug interaction checker. He explains OTC medications of all types may interact negatively with other OTC and/or prescription medication you’re taking. They may also interact with certain foods and beverages, as well as other illnesses or diseases you may already have. An example of a drug-drug interaction, according to the website, would be taking a pain medication such as Vicodin with an antihistamine such as Benadryl. Since each of these medications cause drowsiness, taking them together will multiply the effect.² Drug-food/beverage interactions are more common with prescription medications and include food and drinks like grapefruit juice, bananas, kale, milk, black licorice, salami, and walnuts, according to the Consumer Reports.³ But OTCs can interact with certain foods and beverages, too. When taking any medication, try to avoid alcohol. Combined with aspirin, especially with regular use and consumption of each, alcohol can cause gastrointestinal bleeding.⁴ Finally, drug-disease interactions occur with conditions like high blood pressure. Taking an over-the-counter oral decongestant like pseudoephedrine (Sudafed) or phenylephrine (Sudafed PE) “may increase blood pressure and can be dangerous.”⁵ The people most susceptible to mild, moderate, or even life-threat-

Medications Can You Wean off of Your Over-The-Counter Meds? LIVING WELL BRANCH

C

an’t I just take something over the counter instead of all these expensive medications? The number of adults and children taking prescription drugs over the last 20 years has increased 85% and there is a growing trend to figure out what is needed and what could be reduced. Fifty five percent of Americans take an average of four medications and 75% take at least one over the counter medication in addition on a regular basis. The decision to wean from your medications, take a lower dose or move to an over the counter medication FIRST requires conversation with your health care provider to avoid triggering a

serious problem that can occur with abrupt stopping. The plan will include a clear timeline for reducing the dose and follow up. Weaning from your medication may cause temporary side effects that are expected and not cause for concern. You will also learn the side effects that are serious and require a call to your health care provider. Discussion about nondrug options or the “Skills and Pills” model of healthcare will be extremely helpful. Antonio N. recently shared that he suffers with back pain and has been taking ibuprofen for many years. He was concerned about the long-term use of these over the counter medications and heard that there was some connection to kidney disease. Kidney disease caused by non-steroidal anti-inflammatories (NSAIDs) is often preventable according to the National Kidney Foundation. Here are a few things we can do to help keep our kidneys healthy: • Avoid prolonged use of analgesics that contain a mixture of painkilling ingredients, like aspirin, acetaminophen and caffeine mixtures in one pill • Increase the amount of fluid

Albert MSN, ARNP, CPNP you drink to six to eight glasses a day, approximately half of your body weight unless fluid restriction is recommended by your health care professional • If you are taking analgesics, avoid drinking alcohol • If you have kidney disease, consult your health care professional before taking an analgesic, particularly NSAIDs and higher dose aspirin. • Use NSAIDs with your health care professional’s supervision if you have heart disease, high blood pressure, kidney or liver disease, taking diuretic medications or are over 65 years of age Non-drug options for short and long-term pain management include: gentle stretches, yoga, tai chi, swimming, massage, acupuncture, spinal manipulation, electrical stimulation, physical therapy and cognitive behavioral therapy. Each one of these options or a combination of several has the potential to offer relief and the opportunity for a lowered dose. Deborah A. had burning pain in the center of her chest. At one point it became increasingly worse and was not relieved with rest. I consistently encouraged her to seek care and a cardiology evaluation. She agreed after her most recent episode. her with gastroesophageal reflux or GERD. It is extremely important to rule out cardiac issues as the symptoms are often confusing. The peace of mind she experienced was priceless. Treatment with a proton pump inhibitor (PPI) was ordered for a short period of time to allow the esophagus to heal. Long term use was a concern as she had heard the information about potential changes in risk for bone fractures, kidney disease, heart disease, low magnesium and B 12 levels. The role of stomach acid is an important one especially as it relates to absorption of nutrients and killing off the bad bacteria, which if decreased could make her more susceptible to severe diarrhea. She was especially concerned about the connection between PPI use and dementia. Motivated to find success with non-drug options she developed a plan with her health care provider to wean from her PPI. First, she agreed to identify and avoid or cut back on the foods that trigger the burn. This was a challenge and the temptation to “just take the pill”

and eat whatever she wanted was often there. Offending foods may include alcohol, garlic and onions, fried and spicy foods, citrus fruit, peppermint and chocolate. She found that smaller meals was ideal completing the last meal of the day a minimum of 2 hours, ideally 4 hours before bedtime. Other strategies are to lose weight, quit smoking and avoid tight fitting clothes. Sleeping on your left side and elevating the head of the bed can help as well. A plan for her

to have relief from the occasional heartburn and regular checks to determine progress was identified. Guidelines for when to revisit the over the counter or prescription medicine were firmly established. This included notification of her health care professional if she experienced episodes of heartburn twice weekly or more for 4 weeks or longer. One of the most stressful parts of the holiday season is coordinating schedules with ex- By Kim Denoff tended family members. During this era of blended families, the holidays can become hectic for the elders, as well as the grandkids. The traditions that surround the holidays are unique to every family. Those traditions are honored and cherished. As a parent, I dream of ways to allow my kids to remember something wonderful about the holidays. My hope is to give them something that they can pass along to their own children. I know that as they grow and have their own family, our traditions will have to adapt to allow them to extend the same wonderful gift of holiday memories to their own children. During the holidays, families strive to create strong feelings of love and togetherness. It doesn’t mean that we have to continue to do things the same way we always have. It’s the feelings of the people we love most that matters most. As kids and grandchildren grow, and especially with blended families, the grandkids get stretched to comply with extended family traditions. Often, having two parents’ homes to visit on the same holiday leave grandkids feeling road weary and stressed. For the older family members, it can be difficult to relinquish traditions as the family grows. We don’t want our older relatives to feel forgotten. Families have to find ways to create new traditions with their extended family members. Adapting to a family’s changing holiday needs is a sensitive topic. Roles may change and traditions may change, so long as the intention and reason for the season doesn’t become lost. Wishing you a Happy Thanksgiving with those that mean the most to you! By Scott Guimond this time each year we start scheduling Christmas Eve deliveries, so the surprise is kept until the big day. One of my first scooter deliveries made a profound impact on my life. I was asked to deliver a scooter around Christmas to a patient, something I had never done before. I was given a quick tutorial on how it worked before I left the warehouse. As I drive up to the house, the patient’s husband is waiting in the driveway for me, he couldn’t wait to get the new scooter for his wife who was stuck in the home. He helped me take it out of the truck and we hurried inside to present his

wife with the scooter. When I reached the living room she was sitting there with anticipation, and the second she saw it she began to cry. It was a happy moment for her because she was chained to her sofa by her inability to walk. We helped her get on the scooter and off she went, we took her outside, so she could get used to the controls. Once she got out the door she was off like a shot, she was so excited to get out of the living room without anyone’s help. That moment has stuck with me and is one of the reasons that I chose to work with mobility products for the last 25 years. Each time I provide a scooter or power chair I think of that lady and her intense reaction. Making people happy through giving the gift of mobility is what keeps me going through all the red tape and insurance questions. Alzheimer’s patients, the hippocampus of the brain is affected by the disease first. The hippocampus is the brain’s memory storage system and learning center. Medical professionals, notably neurologists, use several criteria to diagnose Alzheimer’s. They may include examining the patient’s medical history and lab test results, and reviewing a patient’s changes in day-to-day functioning. And though the symptoms of the disease can vary from person to person, Alzheimer’s Association states Alzheimer’s progression follows a relatively consistent pattern, from Stage 1 (no impairment) to Stage 7 (inability to respond to the environment). Age is the best-known risk factor for diagnosing Alzheimer’s; most progressive dementias are

Recipe

Best Homemade Stuffing

By Lisa Buckner

This recipe is a combination of my mother's Swedish recipe and my sister-inlaw's Southern Recipe. It combines the best of both for a real tasty dressing that can be either vegan or traditional with giblets. If a custardy texture is desired a couple of eggs can be stirred in the mixture before baking. DIRECTIONS:

Chop onions and celery into small chunks, by hand or food processor. Sauté in butter until soft. Cover and add a little water if necessary. Simmer until onions are clear, about 15 minutes Place crumbs into large bowl. Sprinkle crumbs with at least 1/2 box of seasoning. Add pepper and salt. Add cooked onions and celery and toss. Add broth to desired consistency, more for moist, less if you like it drier Add more seasonings after taste test, if needed. Place in greased 13x9 glass pan Cover pan with foil and poke holes to let out steam. Bake at 350 for 30 minutes and serve hot

What changes should you look for? Medicare Advantage (MA) plans, such as those offered through Aging Tree insurance partners offer additional benefits to traditional Medicare and can include dental, vision or Medicare Part D Prescription Drug Coverage. If you are an MA plan member, now’s the time to review that coverage as well. What are some of the possible changes you should be aware of for as you review your Medicare coverage? Primarily, be on the lookout for changes in coverage and cost. Here are some examples: 1. Review your plans carefully and read the materials sent to you about any plan changes. Specifically, review premiums, drug formularies, and how much the

"Aid and Attendance" an Under-Used Benefit

T

he Department of Veterans Affairs (VA) is reaching out to inform wartime veterans and surviving spouses of deceased wartime veterans about an under-used, special monthly pension benefit called Aid and Attendance. Although this is not a new program, not everyone is aware of his or her potential eligibility. The Aid and Attendance pension benefit may be available to wartime veterans and surviving spouses who have in-home care or who live in nursing-homes or assisted-living facilities. Many elderly veterans and surviving spouses whose incomes are above the congressionally mandated legal limit for a VA pension may still be eligible for the special monthly Aid and Attendance benefit if they have large medical expenses, including nursing home expenses, for which they do not receive reimbursement. To qualify, claimants must be incapable of self-support and in need of regular personal assistance. The basic criteria for the Aid and Attendance benefit include the inability to feed oneself, to dress and undress without assistance, or to take care of one’s own bodily needs. People who are bedridden or need help to adjust special pros-

thetic or orthopedic devices may also be eligible, as well as those who have a physical or mental injury or illness that requires regular assistance to protect them from hazards or dangers in their daily environment. For a wartime veteran or surviving spouse to qualify for this special monthly pension, the veteran must have served at least 90 days of active military service, one day of which was during a period of war, and be discharged under conditions other than dishonorable. If all requirements are met, VA determines eligibility for the Aid and Attendance benefit by adjusting for un-reimbursed medical expenses from the veteran’s or surviving spouse’s total household income. If the remaining income amount falls below the annual income threshold for the Aid and Attendance benefit, VA pays the difference between the claimant’s household income and the Aid and Attendance threshold. The Aid and Attendance income threshold for a veteran without dependents is now $21,531.00 annually. The threshold increases to $25,525.00 if a veteran has one dependent, and by $2,205.00 for each additional dependent. The annual Aid and Attendance threshold for a surviving spouse

NFL Thanksgiving Game Facts & Trivia

If you are any kind of football fan, you probably already know that the NFL traditionally hosts Thanksgiving Day games in both Detroit and Dallas. Since 2006, there has also been a Thursday evening game played at various locations. Here are some lesser know facts about the NFL’s holiday tradition.

• The Carolina Panthers and Jacksonville Jaguars have never played on Thanksgiving Day. The Houston Texans made their first franchise appearance in 2012 vs. the Lions. • The Detroit Lions have the most wins on Thanksgiving (34), as well as the most losses (38). • The tradition of the Detroit Lions playing on Thanksgiving Day began in 1934. The Lions have played on every Thanksgiving Day since 1934, except during 1941-1944 when no games were played due to WWII. • The Dallas Cowboys have the 2nd most Thanksgiving wins behind Detroit. The Cowboys are 29-16-1 all-time. • The tradition of the Dallas Cowboys playing on Thanksgiving day began in 1966 when the Cowboys were still part of the AFL. • The St. Louis Cardinals replaced the Dallas Cowboys on Thanksgiving during the 1975 and 1977 seasons. Dallas has hosted in every other year since 1966. • The Green Bay Packers have the 2nd most Turkey Day losses with 19. • Tampa Bay & Cincinnati are the only current NFL teams to play on Thanksgiving and not have at least one win. They are both 0-1. • The Vikings have the highest winning percentage of any team on Thanksgiving with more than 1 appearance. They are 5-1 all-time. • The Redskins are 1-6 on Thanksgiving for a .143 winning percentage. • The Chargers have not played on Thanksgiving in over 40 years (1969), and have not appeared since the AFLNFL merger. • Other than the Cowboys who have done it three times, the Dolphins are the only other team to win a Thanksgiving day game, and the Super Bowl in the same season. Miami did it in the 1973 season when they beat Dallas 14-7 on Thanksgiving, and then won Super Bowl VIII over the Vikings 24-7. • From 2006-2011 the NFL Network broadcast the Thanksgiving evening game.
