ow many prescriptions do you take every day? In asking that question, I realize your first answer may not even include supplements, vitamins or herbals that you ingest, not to mention any over-the-counter pain relievers or aspirin regimens. It's a lot to keep up with, isn't it?

According to a study conducted a few years back by Georgetown University, more than 131 million people – that's 66% of all adults in the United States – take prescription drugs. Chronic conditions require more prescriptions than most health issues, of course. As an example, while the average adult has nine prescriptions, people with diabetes typically have 34 prescriptions filled annually, and those with heart disease have 30. Not far behind are the number of prescriptions filled annually by people living with hypertension (25), arthritis (21), and cancer (19). If you take even a fraction of the number of prescriptions most adults do, it can get quite overwhelming to organize and remember to take them. Folks have come up with many creative ways to efficiently remind us to take medications, including dispensing systems, do-it-yourself pill packs, and phone alarms. If you don't have a tried-and-true way of organizing and scheduling your medications, I recommend you take care of that as soon as possible! Here are a few suggestions for better pill-taking: 1. Learn all you can about your medications. It will help you better understand how they work and why they’re important. This alone can be a strong motivator for staying on track. 2. Organize your pills in pill boxes. You can easily find them at most pharmacies. Look online to find some creative dispensing options, too. 3. Load a pill reminder app into your smartphone. A quick search in my phone’s app store resulted in many, many options, and most are free! 4. Or, get an automated medication dispenser. Electronic Medication Dispensers help to organize and secure your medications while providing an audible alarm as a reminder. Call Aging Tree for more information. There are two things you can always count on “going up” in life...taxes and the price of prescription medication. According to the article “Why Our Drugs

A sure sign of life change can be what your medicine cabinet becomes stocked with as you get older…Instead of Scope mouthwash, denture cream…Instead of ibuprofen, rheumatoid meds…and possibly more than in previous years, an increased number of those clear, orange-ish, white-capped bottles containing everything from high blood pressure meds to heart pills... Cost So Much,”¹ the cost of prescription drugs in America are the highest in the world. One of the main reasons? The United States allows drug companies to set their own prices. If you’re relatively healthy or only have to pay for an occasional prescription for a temporary illness, be prepared for “sticker shock”-- even with insurance--if you develop a chronic, serious, or rare condition, warns the article. If you have Medicare, you have access to prescription drug coverage. Just be aware that if you don’t sign up for coverage when you’re first eligible that you may pay a late enrollment penalty unless you have other creditable coverage or receive Extra Help through Medicare. There are two ways to get drug coverage through Medicare: Medicare Prescription Drug Plan (Part D) and Medicare Advantage Plan (Part C). The Medicare website explains the Part D adds drug coverage to Original Medicare and some other plans. Part C, which is like an HMO or PPO, is available to those who already have Part A and Part B.

Once you’ve been diagnosed, your physician should discuss not only your new prescription but the potential interaction with other drugs you may be taking that have been prescribed by that doctor or others; supplements you may be taking on your own or on the advice of another doctor or health provider; and even certain food or beverages, including alcohol and milk, that you consume. In addition, various lifestyle choices can interact with certain medications, such as how much sun exposure you get each day. Pharmacist Dr. Roger J. Accardi of Accardi Pharmacy advises asking your pharmacist to specify the use of each drug. “For example, if your prescription reads ‘take 1 tablet twice a day,’ ask what that actually means. Do you take the second dose at lunch, dinner, or bedtime?” Accardi says that even the correct diagnosis and the correct medication could result in “disastrous outcomes.” “A prescription for Ibuprofen to address your inflammatory pain, if taken on an empty stomach, may result in bleeding ulcers,” he explains. cont. pg 10 |8|

Some drugs can mess with cognition and even mimic dementia. These can include anxiety and insomnia medications, corticosteriods such as prednisone, pain medications, chemotherapy drugs, and cholesterol-lowering statins.² Potential interactions can be checked by your pharmacist. This is easier and more accurate if you fill all of your prescriptions at the same pharmacy. Certain pharmacy websites offer a drug interaction checker, although this is a quick reference tool and doesn’t take the place of medical advice. One online drug interaction checker says that an interaction has the potential to “change the way a drug is absorbed, metabolized or eliminated; make drugs stronger or less effective; and increase the drug's side effects.”³

Storage, Expiration, Safety

Storage Most of our readers live in Florida and know that the humidity and warm weather they enjoy that invite mold and mildew growth and provide a happy habitat for insects. You are probably used to keeping food and other products in the refrigerator or the coolest and driest place you can find in your home. Although the “medicine cabinet” we are most familiar with is the one located over the bathroom sink, it’s actually not be the right choice for storing medication. What medications should you store in your medicine cabinet? “The answer is none, if the medication is taken by mouth,” says Accardi. “Light, heat, and moisture are a pill’s enemy. They will destroy the effectiveness of oral medication way before the stated expiration date on the label.” Accardi says that oral meds are

usually intended to be stored at 50F/86F. In the bathroom, especially during a hot shower, temperatures can exceed 100 degrees with excessive humidity. In addition, lighting tends to be too intense in most bathrooms. Similarly, oral medication should not be kept near the kitchen sink. However, storage of prescription creams, ointments, lotions, and gels are fine in those places, as long as temperatures don’t routinely exceed 86F, Accardi explains. Here are some common guidelines, but check with your pharmacist or the pamphlet that comes with your prescription for any special storage instructions such as refrigeration. • Keep your medicine in its original pharmacy container. • Store medication in a cool, dry location. • Don’t set your medicine bottles anywhere that receives sunlight. Expiration Pay attention to expiration dates, and be aware that improper storage of medication can make these dates obsolete. Check the expiration dates on prescription, over-the-counter, and supplements regularly, especially those you don’t refill or purchase on a regular basis. “On all your medications, you should have a yearly review of the expiration dates,” Accardi says. “Personally, I set it up to do on January 1. A conservative, safe way to make that determination is to discard all medication one month prior to the stated expiration.” Medication that is past its due date can lose its potency or degrade, which means it won’t work the way it should and could cause a risk to your health. According

to the Food and Drug Administration (FDA), medications like painkillers that are just sitting in your cabinet tend to be “highly susceptible” to misuse and abuse by other people with access to your home and medicine storage.⁴ Expired meds? Proper disposal is important. According to the FDA, pills should be mixed together with substances such as coffee grounds or cat litter and sealed in a plastic bag before being thrown in the garbage. Many cities also schedule specific days when people can bring their expired medication in for proper disposal. (Although the FDA website says some meds should be flushed down the sink or toilet, others sources say this practice compromises the public water supply.) Another option is to ask your pharmacist, some of whom will dispose of prescription medication for you. Accardi is of that opinion also, he says, remove your personal label, and discard in the pill container, with some water or you can take it to Accardi Pharmacy to be destroyed. Safety Chances are you have children in your home on occasion. If so, it’s imperative that medications are out of reach of or locked away from small hands. Unfortunately, it’s a sad fact, too, that your medication could be improperly used by others who may have access to your home and supply of medication. According to the National Institute on Drug Abuse, prescription drugs are among the most commonly used drugs by 12th graders after alcohol, marijuana, and tobacco. The online article “What is the scope of prescription drug misuse?” states that “the NIDA’s Monitoring the Future survey of | 10 |


substance use and attitudes in teens found that about 1 in 13 high school seniors reported past-year nonmedical use of the prescription stimulant Adderall® in 2015, and nearly 1 in 23 reported misusing the opioid pain reliever Vicodin®.”⁵

Virtual Medicine Cabinet

Keep a list of your most recent prescriptions and their dosages and the name, location, and phone number of your pharmacy that is easily accessible to your spouse or a close and trusted family member. In addition, include any supplements, including vitamins, you may be taking. These should always be discussed with your doctor as some herbal and other supplements may interact with prescribed and over-the-counter medications. This list can be life saving, especially in the event of a medical emergency. It’s also helpful to have this information handy when you’re traveling and you or a caregiver needs to quickly reference your medications. Often, as we age, the number of medications prescribed can increase, making it even more imperative to be proactive in keeping an accurate record of what you’re taking. Don’t assume your doctor or pharmacy knows everything about your day-to-day healthcare. Accardi tells patients that it is advisable to keep a medical journal of questions. For example, when do I take my meds, or can I take with grapefruit juice? Take the journal with you to your primary care physician. This will allow you time to review your questions with your doctor. For those of you who take advantage of technology, a variety of mobile phone apps can help keep track of this information. Some have been developed by pharmacies such as CVS and Walgreens to provide easy access to your records and to allow you to refill your prescriptions at the tap of a smartphone screen. What’s in your medicine cabinet is just as important as educating yourself about your medications and how to keep yourself, and the ones you love, healthy and safe. Because cholesterol is needed to produce vitamin D, which helps with immune function, building strong bones, the absorption of calcium and many other things, it is important to know your vitamin D levels. These are a few of the nutritional considerations related to common prescription medications. Share with a health care provider your specific questions and concerns. Health Wise Be a Bosom Buddy - You May Save a Life

October is Breast Cancer Awareness Month. This reminded me By Kim Denoff of some special women in my life. I called my best friend to make sure she has had a recent mammogram. I told her that it is breast cancer awareness month and that I was thinking about a high school friend that was diagnosed with an aggressive stage III breast cancer during a screening mammogram. A year later, at 39 years old, her only goal was to live long enough to turn 40. Unfortunately, she passed away later that year. Her death breathed a strange new awareness of life into me. I suddenly valued days I had once taken for granted. I felt that a wasted day was such an insult to her life when her only wish was to be 40 years old. My friend listened intently and then revealed that she hasn’t had a mammogram in 15 years. She added that she has been too afraid of finding something wrong. I told her that I would be making us a buddy appointment tomorrow. I assured her that not knowing is more worrisome and that I would be there with her. Reluctantly, she has agreed. So, let me encourage you! If you have had your mammogram this year, you’re doing great. Now, call your best friend and make sure she has too. The mammogram buddy system may just save the life of someone you love. Be a Bosom Buddy! If you find a lump, call your doctor. If you need a physician, please call an imaging facility that offers mammograms. Many facilities offer discounted pricing in October.

diseases often have no symptoms. A comprehensive dilated eye exam is different from the basic eye exam or screening you have for glasses or contacts. By dilating the pupils and examining the back of the eyes, we can detect eye diseases in their early stages, before vision loss occurs. Jerry, an Army vet with heart disease, loved to talk about his military experiences. As his illness progressed, he relived those days over and over. VOTRAN SERVES THE COMMUNITY THROUGH HURRICANE IRMA In September’s issue, we discussed the importance of having a disaster plan and knowing where you and your loved ones would go during an emergency. We also highlighted the role Votran plays during an evacuation order. Little did we know that in just a few short weeks, all that preparation would be tested! In early September, Hurricane Irma barreled its way up the state of Florida, directly affecting Volusia County. When an evacuation order was issued for residents on the beachside, in low-lying areas, and in RVs, mobile and manufactured homes, Votran prepared to assist those residents, and all others requiring transportation, to a shelter. Saturday, Sept. 9, Votran operated regular bus routes, providing free transportation to facilitate any transportation needed to prepare for the hurricane. Votran coordinated with buses provided by Volusia County Schools to get evacuees into public shelters. Votran Gold service transported over 150 individuals to Special Needs Shelters, which serve physically, mentally or sensory disabled persons needing medical assistance and those who need supplemental oxygen. By noon on Monday, Sept. 11, Irma had passed and it was deemed safe for Votran to resume operations. Buses were ready for the all-clear and immediately began transporting evacuees out of schools and back to their homes. Throughout Tuesday, Votran operated without charging any fare on normal fixed route service and on a case by case basis using Votran Gold vehicles and vans. Evacuees were traveling both to the Ocean Center seeking shelter and home from the shelters throughout the day. Votran resumed collecting fares Wednesday; however, passengers traveling to or from a shelter were not charged a fare. Travel Smart: Vaccinations, Currency Exchange, Insurance, & International Travel

I

’m writing this column with exactly 19 hours before I bring a group of 20 on the trip of a lifetime to Africa. I’ve been planning this journey for over two years and have painstakingly and passionately custom-designed every single aspect of this trip with a very trusted Africa specialist to ensure our group will get the best of the best in African experiences. Tomorrow, we’ll be flying into Cape Town, South Africa, and spending four days along the waterfront where we’ll take in noteworthy sights like: Table Mountain, the Cape of Good Hope, and Robben Island to name a few. Then we’ll proceed onward to Botswana and Zimbabwe, where we’ll enjoy game drives for Big Five game viewing, helicopter rides over Victoria Falls, elephant back safaris, and even an adventure where we’ll walk alongside a pride of lions. Let's travel smart. When it comes to taking adventures, some major questions come up:

Do I need shots?

• We always recommend staying current on your normal vaccinations as recommended by your doctor, but the best resource to use is the Center of Disease Control and Prevention (CDC). They have a travel health web page on which you can put your destination country and check off the type of traveler you are. The shots or medications may be different depending if you’re traveling with children, are immune-compromised, are pregnant, or traveling to visit family or friends. As always we recommend going directly to the health department, your doctor, or a local pharmacy that offers travel vaccinations.

Do I need a passport or visa?

• You must have a valid passport to travel to another country and it should be valid for 6 months past the date of your

By Amanda M. Vallone return. A traveler should check with his or her travel provider or the embassy if he’ll need a Visa to get into the country of destination as well. My group of explorers won’t need Visas to get into South Africa, but we will need them for Botswana and Zimbabwe. My travel agency often recommends a Visa Service called CIBT Visas when we are helping our clients with international travel. If a traveler does not have the appropriate documentation to enter a country, he will be denied boarding his flight from the USA. So cross those T’s and dot those i’s early.

What type of currency do I need?

• Every country is different, Europe is the easiest in my opinion. The best advice we give our explorers is to exchange a small amount of currency in advance at a local bank, mall, or the airport. The traveler will pay a premium on the exchange rate this way, but it’s worth it if you’re a stressor over cash. We remind travelers to call their banking institution or credit card in advance of international travel as to not incur any issues when trying to make purchases. When using a credit card it must have a | 26 |


chip and should have a 4 digit pin (many countries require this, even for credit cards).

Is there a language barrier?

• Absolutely, when you travel internationally. You’re traveling to another country and

experiencing their way of living. You’re not supposed to feel completely comfortable, it’s all a part of the experience. That being said, you can often take English speaking tours and most major travel areas are truly accommodating to English speaking travelers.

Is it safe?

• Well, it depends on where you’re going. Our rule of thumb is that some places in the US aren’t safe. If you live your life thinking that everything that the news portrays about international incidents cannot just as likely happen right here, then you will never go anywhere. Though difficult, I may be the best person possible to write this article for you. You see, I lived through and fled from several terrorist bombings in 2008 when I was living in India. I was scared to death. But this did not make me afraid of the rest of the world. Nor did it make me fear for my safety of traveling to India in the future. Opposite of that,

these specific incidents made me realize how we all need to be smart travelers. Go with a trustworthy tour operator. Book through a trustworthy agency

or company who knows either the destination or the company they are putting you on. And register your international trips through our Embassy’s Smart Travel Enrollment Program (STEP) located at step.state.gov.

What about travel insurance?

• ALWAYS! YES! Why would you go without it? I am 33 years old and have few to no health issues, but I purchase travel insurance. It protects my investment of a trip of a lifetime. It provides 24/7 emergency support. I’ve had to use it before, and I am a believer.

You should travel smart, be knowledgeable, and know your surroundings. Most of all – plan to go with people and companies who know what they’re doing. We’re currently planning a Christmas group trip to Madison & Stone Mountain, Georgia, and Gatlinburg and Pigeon Forge, Tennessee, with the Aging Tree. The Balanced Budget Act of 1997, introduced the term Medicare+Choice Part C of Medicare, Medicare Prescription Drug, Improvement, and Modernization Act of 2003 renamed +Choice "Medicare Advantage." I sincerely hope that everyone is OK after Irma – I have been living in Florida for over 30 years and must say, ‘this one had me on edge’! Benefits of participation in Lifelong Learning In their article “Cognitive benefits of participation in lifelong learning institutes,” Simone & Scuilli reference the fountain of youth that eluded Ponce De Leon and how brain function can be improved through physical exercise and cognitive stimulation. Rowe and Kahn (1998), their groundbreaking investigation into positive aging, suggest three keys to successful aging: (1) maintaining high cognitive and physical function, (2) staying engaged with life, and (3) avoiding disease. Although attending school used to be seen as age-based, i.e.,intended to prepare children and adolescents to become adults and begin a career, that

has been largely replaced by the newer idea of lifelong learning. A recent study published by the Journal of Gerontopsychology and Geriatric Psychiatry examined the impact of lifelong learning on a sample of students who attended the University Program for Older Adults (PUMA) at the Autonoma University of Madrid between 2007 and 2011. Pre-post comparisons showed that participating in school helped older adults increase their overall level of activity and improve memory functioning over time. While there is not enough research to determine the long-term benefits associated with active aging, we are certain that it can slow

Feeling like Fall... Delicious Pumpkin Chili

As the nights gets cooler, we start thinking of cold weather comfort foods. Chili always pops into my mind. This chili adds the always popular pumpkin flare. It can even be served in a pumpkin bowl!! DIRECTIONS:

Heat olive oil in a Dutch oven over medium heat. Add red onion, peppers carrots and jalapeño. Sauté for 10 to 12 minutes or until just starting to brown. Add garlic and spices and sauté for 1 minute. Add pumpkin, tomatoes, vegetable broth and beans. Bring to a boil and reduce heat to low. Simmer 30 minutes partially covered, stirring occasionally. Season with salt and pepper. Taste and adjust seasonings accordingly. VARIATIONS: Cubed cooked chicken or browned ground turkey can be added. OPTIONAL TOPPINGS • Chopped scallions, Sour Cream, Shredded Cheddar, Fresh Cilantro

Recipe: Pumpkin Chili From the Kitchen of: Mama Lisa Ingredients: 2 tbsps olive oil 1½ c chopped red onion 1 red bell pepper, seeded & chopped 1 green pepper, seeded & chopped 1 c chopped carrots 1 jalapeño, seeded & minced 5 garlic cloves, minced 2 tbsps chili powder 1 tbsp cumin 1 tsp oregano 1 tsp ground cinnamon ½ tsp crushed red pepper (optional)

Serves: 8-12

1 (15 oz) can pumpkin puree 1 (14.5 oz) can diced tomatoes w/liquid 3 cups vegetable broth 1 (14 oz) can black beans, drained & rinsed 1 (14 oz) can Cannellini beans, drained & rinsed 1 tsp sea salt ½ tsp black pepper

Your Social Security number is your link to Retirement or Disability benefits since we use it to record your wages and earnings. 2. There is no charge to obtain a Social Security number and card. This service is free.

3. We keep your records confidential and don’t disclose your number to anyone, except when the law requires, or when your information connects you with other government health or social services programs. 4. To prevent identity theft, keep your Social Security card in a safe place with your other important papers and be careful about sharing your number. If asked for your number, find out why your number is needed, how it will be used, and what happens if you refuse to provide it. 5. While you need a Social Security number to get a job or for other services, you often don’t need to show your Social Security card. Many organizations can verify your Social Security number directly with us.

6. If your Social Security card is lost, you can replace it up to three times a year with a lifetime limit of 10 replacement cards. Legal name changes and other exceptions will not count toward these limits. 7. You can request a replacement Social Security card with the ease and convenience of our online services if you have a my Social Security account and meet our qualifications. What Does Your Medicine Cabinet Say About You?

I

wonder if you can tell anything about a person by what’s in their medicine cabinet? Some people seem to think so and go to great lengths to clean them out before company comes, lest any snoopers should decide to see what ails them these days. But have you taken stock of your own cabinet lately? What pain relievers reside there? Are there any hearing aid batteries lurking in the nether regions? Any old, frayed Ace bandages used to support that bum knee or ankle? Anybody got an old school, rubber hot water bottle for that aching back? For veterans that have endured the rigors of military service, many of these are standard items. Many of us continue to patch ourselves up and carry on with our lives because that’s what we’re taught to do; carry on. And I commend you for that; it is that indomitable spirit that has won the battle on many occasions. But the next time you peek into that often forgotten space in your home, reflect on your military service and think about how your service to the country may have led to the ailments you have today. Those seemingly insignifi-

cant events from yesteryear, when you sprained your ankle during physical training, or the time you forgot your hearing protection at the artillery range; these events may very well have contributed

to today’s arthritic conditions and hearing loss, for example. With that in mind, I encourage you to visit us at Volusia County Veterans Services and let our counselors explore whether any of these present day conditions can be connected to your service. Healthcare and compensation may be available to those that still suffer from in-service illnesses and injuries. These benefits are given by law and like many federal benefit programs, can be a little difficult to navigate. Although veterans seldom like to admit to any weakness, sometimes the contents of our medicine
