For individuals and families that need help affording a Kroc Center membership, The Salvation Army Kroc Center is pleased to offer a scholarship program to help provide access to the facility and programs.

HOW TO APPLY FOR A KROC SCHOLARSHIP

To apply for a Kroc Scholarship, download an application at kroccda.org/scholarships or pick up an application from the front desk and return it completed with copies of your household income documents for verification. If your application is approved, you will be enrolled into a Scholarship Orientation.

SCHOLARSHIP ORIENTATION

All Kroc Scholarship applicants must attend a Kroc Scholarship Orientation. During this hour long presentation, you will have the opportunity to learn about the large variety of activities, education classes, and personal enrichment programs that are offered at The Kroc Center. You will also be able to redeem your scholarship membership immediately following the orientation.

Ensure you have completed the Life Skill requirement. Download an application at kroccda.org/scholarships or pick up an application from the front desk and return it completed with copies of your household income documents for verification.

HOW TO OBTAIN LIFE SKILL CREDITS

As part of the eligibility requirements all scholarship renewal applicants must have obtained two Life Skills credits. A completion form must be submitted for credit. Download at kroccda.org/scholarships. Each activity is worth one credit. Adult member(s) must complete the life skill credit. • Attend a Kroc Class (Drop-in Classes do not qualify) • Volunteer at The Kroc Center or in the Community for two hours • Obtain an annual Wellness Exam • Attend a Kroc Church Sunday Service • Attend a Free Kroc Fit45 appointment and workout one time • Donate Blood

We are pleased to provide scholarships for up to 50% off a class/ program. Complete the Program Scholarship Form and submit to the front desk or email to info@kroccda.org

KROC PAYMENT, REFUNDS, & CANCELLATIONS • The Kroc Center accepts cash, check, Visa, Discover, MasterCard and American Express. Full payment is expected at the time of enrollment unless otherwise specified. • The Kroc Center may cancel a class with low enrollment 48 hours prior to the start of class. • No cash refunds are given unless the program is cancelled by The Kroc Center. If the program is cancelled by The Kroc Center, you will be given the choice of a full credit or a cash refund. • Schedules, pricing, programming and policies subject to change without notice. • If you request to cancel your class enrollment five or more days prior to the first class, you will receive a full credit on a Kroc Center

gift card minus a $5 processing fee or you may choose to transfer to another session if available. Other cancellation fees may apply. • Requests made less than five days prior to the start date are not eligible for a refund or credit, except in the case of personal emergencies. No credits or pro-rated credits will be issued for missed days of camp or class due to illness, partial attendance, behavior issues, or any other reason. Drop-off/Pick-up in Huckleberry Hollow Huckleberry Hollow is where our most curious and active age group meets. Kids will have plenty of fun while learning meaningful Bible Lessons. Bible verse memorization is encouraged through simple movement. Children must be fully potty trained.

THE STAFF AT PLAY CARE LOVE LUCCI FIACCO, AND LUCCI’S MOM LOVES EVERYTHING ABOUT KROC PLAY CARE. Lucci is 15-months old and until May of this year, his mom Myra, a writer and publisher working from home, had been unable to leave him with a daycare because all Lucci wanted to do was cry. “Other daycare programs called me after 10 minutes of his disruptive crying," remembered Myra. Myra needed at least a few hours a day of immense concentration for her company. “I heard great things about Kroc Play Care. I found the staff flexible enough to proceed with a plan based on my parenting preferences. I was confident Lucci was in a safe, secure, loving, and learning environment.” He cried for about a week, the staff loved on him and soon Lucci was fully acclimated to his new play care. “All of the ladies in Play Care strive to pour love, care and kindness into each child,” comments Carolyn Meyer, Play Care Manager. “They work with each of the children to meet their needs, recognizing their individuality while respecting the families wishes and parenting plan.” Lucci is now fully acclimated to play care and loves coming to The Kroc! He and his sisters Phoebe and Lola attend regularly, Myra has the hours she needs for work and has gotten “hooked” on the gym. “Kroc Staff are truly our ‘extended’ family.”

This course will provide youth who are planning to babysit with the knowledge and skills necessary to safely and responsibly give care to children and infants. This training will help participants develop leadership skills, learn how to develop a babysitting business, keep themselves and others safe and help children behave. They will learn about basic child care, how to care for respiratory and cardiac emergencies and basic first aid. Participants will receive a Pediatric First Aid/CPR certification. HYBRID* AMERICAN RED CROSS FIRST AID/CPR/AED

The American Red Cross First Aid/CPR/AED program helps participants recognize and respond appropriately to cardiac, breathing and first aid emergencies. The courses in this program teach the knowledge and skills needed to give immediate care to an injured or ill person and to decide whether advanced medical care is needed. This class is taught in a blended learning format, which consists of an independent online class and test followed by an instructorled skill session. Each participant will be contacted prior to class and will be given the instructions for accessing online course work. Upon successful completion of all course requirements participants will be awarded American Red Cross certification. This class does not meet the requirements for healthcare providers or professional rescuers. See prerequisites. *What does Hybrid mean? These classes have both online and hands-on coursework. Prerequisites: Online course work must be completed prior to start of class. After registration, you will be contacted via email with additional instructions. The Kroc Swim Academy takes the water safety skills and developmental progression of traditional Red Cross swim lessons and molds them into a more streamlined, user friendly program. We now offer all levels at the same time to provide more flexibility and we have removed age requirements allowing your child to progress based on skill development. The Kroc Swim Team now serves as the capstone for the academy and incorporates the skills traditionally taught in levels 4 and 5. Not sure what level to sign up for? BEGINNER: PARENT INDEPENDENCE

The Beginner Level is the precursor to our level system. It is designed for the child’s first lesson away from mom and dad with the goal of becoming comfortable in the water. Average age 3–5.

INTERMEDIATE: BASIC WATER INTRODUCTION

Intermediate Level is a basic water introduction course for those children who are more independent. Getting comfortable in the water is the main goal. Things such as blowing bubbles, submerging the head, and floating will be taught. Most skills will be done with assistance. Average age 3–5.

LEVEL 1: AQUATIC FUNDAMENTALS

Level 1 teaches the basic fundamentals of swimming and lays a foundation with skills such as floating, front and back streamline, and introducing basic arm and leg actions. Average age 4–6.

LEVEL 2: GAINING INDEPENDENCE

Level 2 is where the child gains independence in the water. Fears of the water, both deep and shallow, are overcome, and the basic concepts of swimming are introduced. Assistance during skills is also slowly taken away. Average age 4–7.

LEVEL 3: STROKE DEVELOPMENT

Level 3 is where the child begins to learn freestyle (front crawl) and the basics of the other strokes. By this level, the child is fully comfortable in the water, moving freely by themselves, in both deep and shallow water. Average age 5–9.

The Kroc Swim Team serves as the capstone to our Swim Academy. As a part of the team swimmers will have the opportunity to improve strokes, build endurance, and foster friendships. As they progress, swimmers gain competitive meet experience in an introductory environment as well as an introduction to water polo.

SQUID SQUAD: BEGINNER

Squid Squad is the next step after completing our learn-to-swim levels. To be successful your child should be able to swim freestyle with side breathing and backstroke as well as display a basic understanding of Breaststroke kick and Dolphin kick. AMERICAN RED CROSS LIFEGUARDING

Prerequisites:

Instructor candidates will learn the skills necessary to teach water safety, including Basic Water Rescue, Personal Water Safety, and a suite of learn to swim levels. Candidates will learn to identify key elements and strategies for planning and preparing effective and safe swim lessons. Candidates will also learn techniques to adapt teaching to meet unexpected challenges including behavior issues and making accommodations for those with disabilities. THE BENEFITS OF AQUATIC EXERCISE:

• Buoyancy: This water property allows people to do exercises that are difficult on land. Depending on the water level you will only be impacting 20–25% of your body weight. No pounding or jarring, just less impact. • Resistance: There is continual resistance to every move you make. The water alone offers 12%–14% more resistance than when you exercise on land. No equipment is necessary to increase strength. • Heart Rate: The hydrostatic pressure of water pushes equally on all body surfaces and helps the heart circulate blood by aiding blood flow back to the heart. This results in lower blood pressure and heart rates during deep-water exercise verses similar exercise on land. Consequently, your heart rate is an estimated 10–15 beats lower per minute during suspended water exercise than for the same effort applied on land. • Cooling Effects: Water disperses heat more efficiently, so there is less chance of overheating. The water continuously cools the body. Exercise in the water is cooler and more comfortable than it is on land.

POLICIES & SAFETY FAMILY MEMBERSHIP DEFINITION

One or two adults living in a household and dependents who are eligible up to the age of 19. An individual with a disability or an elderly parent who is financially dependent on and is living in the same household with the primary member will remain eligible for the Family membership. Verification of family status and residency may be required. For example: tax return, utility bill, health insurance, etc.

MEMBERSHIP PLANS

Membership Plans are subject to change, when the members' age does not match the current plan's age range. The Kroc Center will automatically change your plan to fit the current plan for your age group. You will be notified by mail when the change takes place.

MONTHLY PAYMENTS/ONE-TIME ANNUAL PAYMENT

Membership fees can be paid monthly via recurring credit card charge or electronic funds transfer (bank account debit). Pro-rated fees will be due at time of enrollment and may vary depending upon enrollment date. To complete the electronic funds transfer process, a voided check must be provided with completed membership form. Membership fees for the following month will be automatically deducted from the member bank account around the 20th of the current month. Any requested changes must be submitted by the 10th of the month in order to be processed for the following month. Annual payments may be paid in full by cash, check, major credit/debit card. Fees are equivalent to 12 monthly payments. The annual fee and required registration fee are due at time of enrollment.

INSUFFICIENT FUNDS POLICY

There is a $20 charge for each insufficient funds transaction. This places your membership on hold until payment is received. Payments received after a plan has expired will be charged an additional $5 reinstatement fee.

FINANCIAL ASSISTANCE/SCHOLARSHIP PROGRAM

Scholarships are available for Kroc memberships, classes and programs. Eligibility is income based. See page 7 to learn how to apply.

EQUIPMENT

Free use of athletic equipment including volleyballs & basketballs. Available in the Gym.

GROUP/PERSONAL USE & COACHING POLICY

SAFETY INFORMATION Kroc Center. The Kroc Center will determine if a class needs to be cancelled due to low enrollment 48 hours prior to the start of class. If the program is cancelled by The Kroc Center, you will be given the choice of a full credit or a cash refund. If you request to cancel your class enrollment five or more days prior to the first class, you will receive a full credit on a Kroc Center gift card minus a $5 processing fee or you may choose to transfer to another session if available. Requests made less than five days prior to the start date are not eligible for a refund or credit, except in the case of personal emergencies. No credits or pro-rated credits will be issued for missed days of camp or class due to illness, partial attendance, behavior issues, or any other reason.

NURSING POLICY

The Salvation Army Kroc Center is a place for families and welcomes nursing moms. Couches are provided in the ladies lounge adjacent to the East wing restrooms along with a diaper changing station. Moms are permitted to nurse in all common areas but are asked to be discreet out of consideration for Kroc guests of all ages and backgrounds.

SERVICE ANIMAL POLICY

We love all furry companions however, for security and health reasons, only animals who remain on the floor at all times and under full control are allowed. A service animal is a dog that is individually trained to do work or perform tasks for a person with a disability. Sorry, pets and comfort animals are not permitted.

GIFT CARDS

Gift Cards are available for purchase at the front desk and can be used by recipient to pay for memberships, class and program fees, facility rentals, merchandise, and cafe items. They cannot be used for class materials, theater & performance tickets, vendor events or non-Kroc sponsored events. Gift cards must be used within two years of purchase date, and are nonrefundable and cannot be exchanged for cash.

DAY CAMP POLICIES DAY CAMP VISITORS

Any day camp visitors for all children and staff will be required to sign-in, and will be given a name tag to identify the individual as a visitor of Camp Kroc.

LOST AND FOUND

Public drop-in access is intended for inclusive recreational use. Any external personal or group use/training/coaching should contact the rental department for specific group requests.

Lost and found items will be kept for a period of two weeks after a session, then donated to the local Salvation Army Thrift Store. The lost and found is located at the front desk of The Kroc Center.

LOCKERS

MEDICATION DISTRIBUTION

Lockers are available at no charge. Guests are responsible for supplying their own lock and clearing locker after each use. The Salvation Army is not liable for lost or stolen property. Lockers must be cleared nightly. Locks will be removed by Kroc personnel once center is closed. Membership fees are nonrefundable. In order to cancel a membership, members must submit a written request by the 10th day of the current month to cancel the automatic payment for the following month. If a member cancels or if membership payments lapse for more than 30 days, the appropriate registration fee will be charged to reopen the membership account. There is no fee charged for cancellation.

In the case of medication distribution, parents must complete a Right to Distribute Medication Form. This releases The Kroc Center from liability for medication that is required for distribution for the health of a child. Medication must be dropped off to Camp Coordinators in the original prescription container with your child’s name printed on the label and placed in a plastic bag. Medication will be stored in Camp Coordinator’s office and administered at the time(s) and in the amount designated by the Right to Distribute Medication Form. Parents must sign out medication at the end of each day and check back in the following day. Any medication not brought in the appropriate container may preclude camp participation for your child on that day.

Any requested change that affects the billing of your membership must be submitted by the 10th of the month to be processed for the following month. If changes occur after the 10th of the month, please see the front desk for pro-rated amount. Membership downgrades, additions, and removal of family members are subject to a $10 change fee. Changes will take effect immediately.

PERSONAL GROUP/INDIVIDUAL COACHING & PROGRAM CANCELLATION POLICY

Cancellation of a one-on-one session or any personal or group coaching session requires 24 hours notice. Failure to do so will result in a forfeiture of any monies paid.

CLASS AND PROGRAM CANCELLATIONS

Full payment is expected at the time of enrollment. No cash refunds are given unless the program is cancelled by The

Day Camp is not designed or equipped to handle ill children. If your camper becomes ill while at Camp Kroc, our staff will make your child comfortable in an area away from the other campers. Our staff will then contact you (the parent) or one of the individuals you have authorized to pick up your child. If immediate medical attention is needed or if the severity of an injury cannot be correctly established, parents will be notified immediately. The Emergency Medical System (911) will be activated at the discretion of Camp Coordinators and/or Camp Counselors. If staff are unable to reach parents whose child is in need of immediate medical attention, he/she will be transported to the nearest hospital via ambulance and staff will continue to attempt to contact parents. Parents/guardians will be responsible for all emergency medical care expenses.

As part of our commitment to the safety of children and vulnerable persons, The Salvation Army Kroc Center reserves the right to consult public sources to determine whether any member or guest of any member poses an unreasonable risk of harm to its patrons, staff, or visitors. he Kroc Center constantly adapts and enhances safety proT tocols to adjust to needs. Members & guests are encouraged to report suspicious behavior. Forms for anonymous reporting, called Confidential Notices of Concern, are available at the front desk. The Salvation Army Kroc Center reserves the right to remove from the facility and terminate the membership of any member who fails to comply with any posted rules and regulation or otherwise breaches the Code of Conduct, in which case member will not be entitled to a refund of dues.

CODE OF CONDUCT & DISCIPLINE POLICY

In order to facilitate positive life experiences while at The Kroc Center, mutual respect between members, guests, and staff is required at all times. Failure to observe these policies will result in disciplinary action and/or termination of membership privileges. Kroc personnel have the right to refuse service to anyone deemed disruptive or abusive. The Kroc Center reserves the right to conduct background checks on all members and day pass users. 1. Children under the age of 11 must be accompanied in The Kroc Center. See age chart on page 3 for more information. 2. Refrain from the use of foul language, abusive actions, and inappropriate behavior. 3. Lost membership cards must be reported and replaced within 30 days. There is a $5 fee to replace a membership card. 4. No one under the age of 12 is allowed in the Fitness Area unless participating in a Kroc Center program or class. Members 12–14 may use the Fitness Area after successfully passing the Smart Start for Teens program or with a participating adult. 5. No swimwear or wet clothing is allowed outside of aquatic areas. Swimwear must be acceptable in a family setting. 6. Proper workout attire is recommended in fitness area. Clothing design, including logos or messages on apparel, and all visible tattoos must be acceptable in a family setting. 7. Bicycles, roller blades, skateboards, scooters or pets (excluding service animals) will not be allowed inside the facility. 8. Alcohol and/or drugs are not allowed anywhere on the premises. 9. Smoking and/or use of any type of smokeless tobacco products are prohibited in the facility or on facility grounds. 10. Weapons of any kind are not permitted on the premises. 11. With exceptions to special dietary restrictions, outside food is not allowed at The Salvation Army Kroc Center. 12. The Kroc Center reserves the right to terminate membership in response to any immoral or illegal activity by a member.
