Community Care Nursing Service
The Community Care Nursing Service provides comprehensive and person-centred care to people living at home or in their local community. Community Care Nursing Service works in close partnership with local hospitals, doctors, nurses, other health professionals and community and non-government organisations.
Anyone can make a referral through Community Care Intake Service (CCIS) t 1800 654 324.

Child And Family Health Nurse
A home visit is offered within the first 14 days. Routine examinations continue to be offered at different developmental stages. Child and Family Health Nursing role supports families through the journey of early parenthood from birth to five years.
Assistance is offered with:
• Breastfeeding and infant nutrition
• Parenting and parent’s wellbeing
• Sleep and settling
• Growth and development
• Immunisation
When using this service, children may be identified to need other supports and referrals can be made.

Chronic Kidney Disease Nurse
Benefits of early detection of CKD in the community include slowing CKD disease progression, decrease in death from kidney failure and reduction in health system cost. The CKD nurse provides education, care and support to people with kidney disease. CKD nurses monitor the progression of kidney failure in the individual and fosters pathways based on stage of kidney function.

Community Care Nursing Team
This service aims to prevent hospitalisation and provide ongoing care once clients are discharged from hospital and in the comfort of their home.
Care options including:
• Wound care
• Medication administration
• Catheter care
• Post-operative recovery care
• Health education, support and advice
• Clinical nursing support for aged care clients to continue to be independent at home

Diabetes Educator
The diabetes service supports and helps individuals, families and carers to understand and manage their diabetes, whether that be Type 1, Type 2, or Gestational.
The goals of the service are to:
• Prevent diabetes
• Promote early detection
• Promote self-management
• Reduce diabetes acute and chronic complications

Palliative Care
Palliative care refers to caring for people of all ages with a life limiting illness, nearing the end of their life. The care is targeted to the needs of the individual and their family. Many people indicate a preference to die at home, and as such Palliative care can be provided at home, in hospital, or in a residential aged care facility.
Palliative Care Nurses can help with:
• End of life planning
• Pain and symptom management
• Access to equipment
• Referral to allied health services such as dieticians and occupational therapy
• Providing information and advice to carers and access to respite for carers
• Bereavement support and access to counselling services for family members

RESPIRATORY (CHRONIC) AND HEART FAILURE SERVICE
Consumers who have Chronic Respiratory Disease, Chronic Asthma, Chronic Obstructive Pulmonary Disease (COPD), Chronic Bronchitis, Asthma, Interstitial Lung Disease, Bronchiectasis or Chronic Heart Failure.
The service provides support and education to empower consumers to self-manage and assist in optimising their quality of life and minimise exacerbations which result in hospitalisation. Services include:
• Recognising and monitoring symptoms
• Supporting you with a monitoring app for your condition on your mobile phone
• Daily management
• Managing your medications and equipment
• Preventing and managing symptoms and supporting you with an action plan

WOMEN’S HEALTH SERVICE
The Women’s Health Service health care services with a focus on reproductive health, health screening tests, education and advice. Clinics are available across the District with specially trained Women’s Health Nurses who can assist females from puberty to menopause and beyond.
Women’s Health Service can help with:
• Cervical screening
• STI screening
• Contraception including long acting reversible contraceptives
• Breast health and pelvic floor education
• Menopause education
• Pregnancy testing, counselling and referrals
