n la práctica diaria nos enfrentamos a un sin fin de enfermedades que son portadas por nuestros pacientes, algunas de ellas conocidas y otras de nueva aparición, de cualquier manera, es necesaria una atención profesional por el servidor de la salud, que tiene el deber moral “tal y como aceptó en su juramento profesional”, de tratar y buscar respuestas ante las nuevas incógnitas que se le presenten. Los avances preventivos, diagnósticos y terapéuticos se consiguen tras realizar investigaciones básicas y clínicas, es así como se demostrará si en su preparación académica se le fomentó el hábito de la investigación o no, ya que una vez en el campo profesional debería ser capaz de formular una hipótesis de trabajo, establecer objetivos, emplear material y métodos adecuados, recopilar resultados, discutir los datos obtenidos y obtener conclusiones de su trabajo. Sin este tipo de formación los padecimientos y enfermedades pasarán desapercibidas y no se tendrá la oportunidad de combatirlas con programas preventivos o correctivos.

Con lo que concluimos que incentivar al alumno a realizar una investigación, aun cuando no sea una tesis como requisito para graduarse, sino aprovechar el periodo de formación donde se podrían realizar varias investigaciones para que el alumno aprenda a hacerlo y a transformarlo en un hábito. Considerando que las universidades son el principal pilar del sistema científico-tecnológico de cada país, les corresponde estimular la creatividad, así como la iniciativa científica de los profesionales que forman. Se debe implementar que en las carreras de Odontología el perfil del alumno tenga objetivos dirigidos hacia la investigación, para que los futuros profesionales contribuyan a solucionar y mejorar los problemas de la salud bucal y calidad de vida de las personas. La competencia en investigación adquirida durante los años de estudios universitarios de pregrado permitirá a los profesionales de ciencias de la salud investigar a lo largo de su vida profesional y así contribuir al desarrollo de su ambiente laboral y de su propio país ya que la investigación en todos los ámbitos es signo de progreso y desarrollo.

n daily practice we face an endless number of diseases that are carried by our patients, some of them known and others of new appearance. In any way, professional attention is needed from the health server, who has a moral duty, "as accepted in his professional oath", to try and find answers to new questions that arise. Preventive, diagnostic and therapeutic advances are achieved after conducting basic and clinical investigations.

This is what will demonstrate, for professionals, weather the habit of research was encouraged or not during their academic preparation. Once graduated, in the professional field he should be able to formulate a working hypothesis, establish objectives, use appropriate material and methods, collect results, discuss the data obtained and draw conclusions from their work. Without this type of training, the illnesses and diseases will go unnoticed and it will not have the opportunity to fight them with preventive or corrective programs. With which we conclude that encouraging the student to carry out research is important; and not just a thesis as a requirement to graduate, but during the training period, where several researches could be done so that the student learns to make this a habit. Considering that universities are the main pillar of the scientific-technological system of each country, it is up to them to stimulate creativity, as well as the scientific initiative of the professionals they form. It should be implemented in dentistry careers that the profile of the future student has objectives aimed at research, so that future professionals contribute to solving and improving the problems of oral health and quality of life of people. The research competence, acquired during the undergraduate university years, will allow health sciences professionals to carry out research throughout their professional life and thus contribute to the development of their work environment and their own country, since research in all areas is a sign of progress and development.

Resumen

Abstract

Introducción: Se pueden considerar los postes prefabricados de fibra de vidrio y los postes anatómicos, como alternativas para la reconstrucción de dientes con tratamiento de conductos, ya que permiten la reconstrucción del órgano dental para devolverle estética y funcionalidad. Objetivo: Comparar cuál de los dos postes de fibra de vidrio resiste más a la fractura por compresión, uno con la técnica de conformación anatómica versus la técnica de cementación simple. Métodos: Se constituyeron 30 muestras de ensayo en dos grupos de quince cada uno, en el primer grupo se anatomizaron 15 postes de fibra de vidrio marca Anthogyr® Fibio, en cubos de acrílico de la marca Densply® Maillefer; mientras que en el segundo grupo se usó la técnica de cementación simple, se realizaron las pruebas de resistencia a la fractura por compresión utilizando el Instron modelo 5567, se aplicó una fuerza compresiva mediante la punta de un vástago a una compresión de 0.5 mm por segundo hasta su fractura.

Resultados: Aplicando el análisis estadístico t de student la media de resistencia de los postes de fibra de vidrio con la técnica de conformación anatómica fue de 671.06 MPa, con una DS± de 116.98 MPa y de los postes de fibra de vidrio con la técnica de cementación simple fue una media de resistencia de 618.05 MPa con una DS± de 126.61 MPa. Observándose que no existe una diferencia estadísticamente significativa a la resistencia a la fractura ante las fuerzas de compresión. Conclusiones: Se concluye que no existe diferencia estadísticamente significativa entre ambas técnicas utilizadas en este estudio obteniendo un nivel de confianza de 95%.

Introduction: The prefabricated fiberglass posts and the anatomical posts can be considered as alternatives for the reconstruction of teeth with root canal treatment, which allow their reconstruction to restore the aesthetics and functionality of the dental organ. Objective: To compare which of the two fiberglass posts resists more to the compression fracture, one with the technique of anatomical conformation versus the simple cementing technique. Methods: 30 test samples were constituted in two groups of fifteen each, in the first group, 15 Anthogyr® Fibio fiberglass posts were anatomized, in acrylic cubes of the Densply® Maillefer brand and in the second group the technique of simple cementation was used, compression fracture resistance tests were performed using the Instron model 5567, a compressive force was applied by the tip of a shank at a compression of 0.5 mm. per second until its fracture. Results: The result of this study regarding the average resistance of the glass fiber posts with the anatomical conformation technique was 671.06 MPa., With a DS ± of 116.98 MPa. and of the fiberglass posts with the simple cementing technique it was an average resistance of 618.05 MPa. with a DS ± of 126.61 MPa. Observing in these results that the anatomical or anatomized post and the fiberglass post with the simple cementing technique, there is no statistically significant difference to the resistance to the fracture before the compression forces.

Conclusions: It is concluded that there is no statistically significant difference between the two techniques used in this study, obtaining a confidence level of 95%.

Introduction

La rehabilitación de dientes con retenedores intraradiculares es un procedimiento que se ha realizado durante muchos años, en principio los retenedores intraradiculares eran estructuras metálicas coladas, pero en la actualidad aparecieron los retenedores intraradiculares estéticos prefabricados, ambos tipos de retenedores se siguen utilizando ya que tienen diferentes indicaciones de acuerdo a la pérdida de estructura coronaria comprometida.1

The rehabilitation of teeth with intraradicular retainers is a procedure that has been carried out for many years. At first, the intraradicular retainers were cast metal structures, but nowadays there are prefabricated aesthetic intraradicular retainers, both types of retainers are still used since they have different indications according to the loss of compromised coronary structure.1

Pierre Fourchard, en 1728, describió el uso de “tenons” que eran pernos y coronas que se anclaban en los restos radiculares. Los dientes eran coronas de animales o humanas talladas dándole la forma del diente a reemplazar.1 Los pernos en un primer momento fueron realizados en madera, pero, por su alta frecuencia de fracturas, fue reemplazada por la plata. Claude Moutoun, en 1746, diseño una corona de oro sólidamente unida a un perno para ser insertado en el conducto radicular.2 Fauchard también reporta en 1747, que sus restauraciones en oro y en plata se mantenían en boca por largos años sin desplazarse, por la implementación de aditamentos de retención endoradicular.1,2 Casius M. Richmond, en 1880, diseñó la corona-perno conformada por tres elementos: el perno intraradicular, el respaldo metálico y la faceta cerámica; esta técnica se convirtió en la aportación más importante hasta ese momento.3 Gracias a Duret, en 1988, surgieron los postes de fibra de vidrio, quien también definió las características ideales del poste, el cual debería presentar forma similar a la estructura dental perdida, propiedades mecánicas similares a las de la dentina (por ejemplo, modulo de elasticidad de 18 GPa), exigir mínimo desgaste de la estructura dental y ser resistente para soportar el impacto masticatorio.4,5 La finalidad única de los postes o núcleos intrarradiculares es la de suministrar una buena retención de la restauración coronal definitiva, por lo que su necesidad depende estrictamente de la cantidad de estructura coronal disponible. Por esto es necesario que el clínico se familiarice con las diferentes formas de construir el núcleo, las indicaciones apropiadas, los tipos de postes existentes, los materiales de relleno y de la cementación para aumentar la durabilidad de las restauraciones.6 El poste muñón colado ha representado, desde Fouchard, la solución de mayor elección y fue considerada hasta la década de 1980 la mejor técnica para reforzar y reconstruir la corona de un diente con tratamiento de endodoncia. La estabilidad del poste, con una adaptación extremadamente precisa a la morfología del lecho endodóntico preparado con los postes metálicos estaba

Pierre Fourchard, in 1728, described the use of "tenons" as bolts and crowns that were anchored in the radicular remains. The teeth were carved animal or human crowns giving the shape of the tooth to be replaced.1 The bolts at first were made of wood, but because of the high frequency of fractures they were replaced by silver. Claude Moutoun, in 1746, designed a gold crown solidly attached to a bolt to be inserted into the root canal.2 Fauchard also reported in 1747 that his gold and silver restorations were kept in the mouth for many years without moving, due to the implementation of endoradicular retention abutments.1,2 Casius M. Richmond, in 1880, designed the crown-bolt made up of three elements: the intraradicular bolt, the metallic back and the ceramic facet; this technique became the most important contribution until then.3 Thanks to Duret, in 1988, fiberglass posts emerged, and it was also he who defined the ideal characteristics of the post, which should have: a similar shape to the lost dental structure; mechanical properties similar to those of dentine (for example, modulus of elasticity of 18 GPa.); require minimal wear of the dental structure; and be resistant to withstand the masticatory impact.4,5 The unique purpose of intraradicular posts or cores is to provide good retention of the definitive coronal restoration, so that their need depends strictly on the amount of coronal structure available. This is why it is necessary for the clinician to become familiar with the different ways of constructing the core, the appropriate indications, the types of existing posts, the filling materials and the cementation to increase the durability of the restorations.6 The stump post has represented, from Fouchard, the solution of choice and considered until the 1980s the best technique to reinforce and reconstruct the crown of a tooth with endodontic treatment. The stability of the post, with an extremely precise adaptation to the morphology of the endodontic bed prepared with the metal posts, was guaranteed by an active intrinsic primary retention, however, there are some works of clinical evaluation where the

Así demuestra que las fuerzas concentradas especialmente en la región apical pueden ser la causa de las fracturas longitudinales.8 Un sistema de reconstrucción de dientes con tratamientos endodóntico con resinas reforzadas con fibra de carbono data desde 1983 cuando Lovell propuso la utilización de estas fibras.8 Malquarti propuso el uso de filamentos de carbono sumergidos en una matriz de composite poliepoxídico para reformar paralepípedos de 2×2×50 mm, que al ser recortados mecánicamente, producían conos de 1mm de diámetro en la base. Se propusieron estos tipos de composite con fibras de carbono cortas para la elaboración de pernos en el laboratorio.8 Postes metálicos Están representados por sistemas intraconducto de diferentes aleaciones metálicas como son: latón, acero, liga de plata, cromo cobalto, aleaciones de oro, titanio. Pueden presentar superficie lisa, espiral o una rosca retentiva. No proporciona una retención activa en el interior del conducto radicular.7 Postes cerámicos Los postes preformados de óxido de zirconio. Permiten eliminar problemas biológicos y estéticos, pero no resuelven los problemas estructurales de la reconstrucción por su rigidez intrínseca.

These types of composite were proposed with short carbon fibers for the preparation of bolts in the Laboratory.8 Metal posts They are represented by intraconduct´s systems of different metallic alloys such as: brass, steel, silver-league, cobalt chromium, gold alloys, titanium. They may have a smooth surface, a spiral or a retentive thread. It does not provide active retention inside the root canal.7 Ceramic posts The Preformed zirconium oxide posts. Allow to eliminate biological and aesthetic problems, but they do not solve the structural problems of the reconstruction due to their intrinsic rigidity. They create a high and non-uniform tension

Postes anatómicos Con la técnica del poste anatómico se logra copiar la morfología interna del conducto radicular utilizando resina compuesta sobre un poste de fibra de vidrio, de esta manera se logra reducir el espesor del cemento mejorando su adaptación y por consiguiente, mejor retención.

En otro estudio realizado por MacLaren y cols. en el 2009, compararon la resistencia a la fractura en dientes tratados endodónticamente restaurados con 3 postes distintos: poste reforzados de fibra de cuarzo, de vidrio (Light Post y Snowlight), y poste de acero inoxidable (ParaPost XP). Encontrando la rigidez y la carga de la fractura de los dientes restaurados con los postes ParaPost XP más altos en comparación con los postes de los grupos reforzados con fibra y observaron que los postes de acero inoxidable tuvieron una incidencia de 25% de fracturas en la raíz, no observando fracturas radiculares con postes reforzados con fibra.9

In another study by MacLaren et al. in 2009, fracture resistance was compared in endodontically restored teeth with 3 different posts: reinforced quartz fiber post, glass post (Light Post and Snowlight), and stainless steel post (ParaPost XP). They found the stiffness and fracture load of the restored teeth with the higher ParaPost XP posts compared to the posts of the fiber-reinforced groups and observed that the stainless steel posts had an incidence of 25% root fractures, not observing root fractures with fiber reinforced posts.9

El objetivo principal de este estudio es comparar cuál de los dos postes resiste más a la fractura por compresión, el de fibra de vidrio con la técnica de conformación anatómica o los postes de fibra de vidrio con la técnica de cementación simple.

The main objective of this study is to compare which of the two posts resists more to compression fracture: fiberglass with the anatomical conformation technique or fiberglass posts with the simple cementing technique.

Material y métodos

Material and methods

El presente estudio fue de tipo Experimental, Observacional, Descriptivo, Transversal y Comparativo. Se constituyeron las 30 muestras en dos grupos de quince muestras cada uno, los grupos “A” y “B”, en el primer grupo se anatomizaron 15 postes de fibra de vidrio marca Anthogyr Fibio en los cubos de acrílico de la marca Densply® Maillefer y en el segundo grupo se cementaron los otros 15 postes de fibra de vidrio marca Anthogyr Fibio, en los cubos de acrílico de la marca Densply® Maillefer, en los cuales se realizaron las pruebas de resistencia a la fractura por compresión, para realizar estas pruebas se utilizó la maquina universal de pruebas mecánicas (Instron) modelo 5567, (misma que se encuentra en el laboratorio de materiales dentales de Posgrado de la Universidad Nacional Autónoma de México), donde se aplicó una fuerza compresiva sobre la muestra mediante la punta de un vástago que incide sobre la parte central del poste, esta fuerza se llevó a carga única de compresión de 0.5mm por segundo, hasta su fractura.

The present study was Experimental, Observational, Descriptive, Transversal and Comparative. The 30 samples were constituted in two groups of fifteen samples each, the groups "A" and "B"; In the first group, 15 Anthogyr Fibio fiberglass posts were anatomized in the acrylic cubes of the Densply® Maillefer brand and in the second group the other 15 Anthogyr Fibio glass fiber posts were cemented in the acrylic cubes of the Densply® Maillefer brand, in which the compression fracture resistance tests were carried out. To carry out these tests, the universal mechanical testing machine (Instron) model 5567 was used (which is found in the postgraduate dental materials laboratory of the National Autonomous University of Mexico), where a compressive force was applied to the sample using the tip of a stem that impinges on the central part of the post; this force was carried at a single compression load of 0.5 mm per second, until its fracture.

Preparación de los cubos de acrílico Una vez que se adquirieron los 30 cubos de acrílico de la marca Densply® Maillefer, se procedió a permeabilizar el conducto que viene preestablecido en el cubo, conformándolo con limas K file de la marca Densply® Maillefer de primera y segunda serie, para posteriormente usar el drill de color negro de Ø1.50 mm que incluye el kit para postes de fibra de vidrio marca Anthogyr fibio, a una longitud de 13 mm. Usando una regla y un tope de caucho.

Se polimerizó durante 5 segundos con una intensidad de 800 mW/cm2, posteriormente se retiró del conducto y se terminó de polimerizar usando la lámpara de fotocurado Bluephase N® de Ivoclar Vivadent fuera del cubo a una intensidad de 800 mW/cm2 por un tiempo de 20 segundos dando un total de 16000 mW/cm2. Posteriormente se eliminó el excedente de vaselina del interior del conducto conformado cubo con agua, aire y por último alcohol, secando con puntas de papel, se colocó nuevamente Variolink Esthetic DC® Ivoclar Vivadent en el interior del conducto y se introdujó nuevamente el poste ya anatomizado, se cementó dejando los 2 mm restantes fuera del cubo, aplicando nuevamente por un tiempo de 20 segundos por cada uno de los cuatro lados una intensidad de 800 mW/cm2 con la lámpara de fotocurado Bluephase N® de Ivoclar Vivadent.

Cementación de los postes de fibra de vidrio.

Con los 15 cubos ya conformados con las cavidades y los 15 postes de fibra de vidrio marca Anthogyr fibio en color rojo de Ø1.25 mm y 15 mm de longitud, se les colocó monobond plus (silano) con microaplicadores sobre la superficie para romper las fuerzas de tensión y tener una mejor adhesión del cemento resinoso con la superficie de éstos. Dejándose durante 60 segundos y desecándose con un ligero chorro de aire, para después colocar Variolink Esthetic DC® Ivoclar Vivadent en el interior del conducto mediante las puntas mezcladoras, después se introdujo el poste de fibra de vidrio hasta los 13 mm la longitud, el poste se cementó dejando los 2 mm restantes fuera del cubo, aplicando por un tiempo de 20 segundos por cada uno de los cuatro lados del cubo una intesidad de 800 mW/cm2 con la lámpara de fotocurado Bluephase N® de Ivoclar Vivadent.

Máquina Universal de pruebas Instron® Es un modelo 5567, calibrada el 24 de abril del 2017 con una vigencia de un año por la empresa Metrolab, en ella se colocó muestra por muestra en una base plana y una espiga o vástago, misma que se mando a elaborar especial y a la medida para que incida verticalmente sobre el centro de la parte superior del poste con un desplazamiento continuo de 0.5 mm por segundo hasta la fractura, registrando la medición automáticamente conforme al programa establecido de dicha máquina.

Universal Instron® testing machine Model 5567, calibrated on April 24, 2017 with a validity of one year by the company Metrolab, each sample was placed on a flat base and a spike or stem that was ordered to be made special and tailored to impact vertically on the center of the top part of the post with a continuous displacement of 0.5 mm per second until reaching the fracture, registering the measurement automatically according to the established program of the machine.

Resultados

Results

Se llevó a cabo el análisis de 30 mediciones divididas en dos grupos (anatómicos y estéticos), dando como resultado un total de 15 mediciones por cada muestra analizada. El estudio se llevó a cabo en el laboratorio de materiales dentales de posgrado de la Universidad Nacional Autónoma de México.

The analysis of 30 measurements divided into two groups (anatomical and aesthetic) was carried out, resulting in a total of 15 measurements for each sample analyzed. The study was carried out in the postgraduate dental materials laboratory of the National Autonomous University of Mexico.

En la Taba 1 se muestra la media, desviación estándar y los valores máximos y mínimos de los postes utilizados en este estudio.

Con respecto al de tipo anatómicos este presentó una media de 671.06, una desviación estándar de 116.98 y valores mínimos y máximos de 512 y 865.9 respectivamente. En relación al poste estético presento una media de 618.05, una desviación estándar de 126.61, un valor mínimo de 391.6 y un máximo de 813.

En la Tabla 2 se puede observar la distribución de las mediciones realizadas en los diferentes postes, en donde se puede apreciar que ambos postes cuentan con porcentajes variados en cada una de las mediciones tomadas, teniendo por lo tanto una distribución diferente en cada una de sus muestras sin encontrar un patrón característico.

Para el análisis de las mediciones realizadas con cada uno de los postes, se llevó a cabo un análisis de medias por medio de la prueba de “t” debido a que la normalidad de los datos no se cumple satisfactoriamente (n<30), con el fin de conocer si existía diferencia significativa entre las medias de los sistemas de medición. Asimismo en la Tabla 3 se puede apreciar el cálculo de la prueba de normalidad de los datos que se le realizó por medio de la prueba de shapiro wilk en la cual se puede apreciar que para el grupo de los anatómicos se obtuvo un valor de 0.2858 y para el grupo de los estéticos de 0.5926, obteniendo como resultado que en ambas mediciones se cuenta con una distribución normal de los datos.

En la Tabla 4 se puede observar el análisis de las medias (prueba t de student) de las diferentes mediciones llevado en los diferentes postes, en donde se puede observar que se obtuvo un

With respect to the anatomical type, it presented an average of 671.06, a standard deviation of 116.98 and minimum and maximum values of 512 and 865.9 respectively. In relation to the aesthetic post, it presented an average of 618.05, a standard deviation of 126.61, a minimum value of 391.6 and a maximum of 813.

In the Table 2 it can be seen the distribution of the measurements made in the different posts, where it can be seen that both posts have different percentages in each of the measures taken, resulting in a different distribution in each one of his samples without finding a characteristic pattern.

For the analysis of the measurements made with each of the posts, an analysis of means was carried out through of the "t" test because the normality of the data is not satisfactorily fulfilled (n <30). This was carried out in order to know if there was a significant difference between the means of the measurement systems. Also, in Table 3 it can be seen the calculation of the normality test of the data that was made by through the test of shapiro wilk in which it can be seen that for the group of anatomies a value of 0.2858 was obtained and for the aesthetic group, 0.5926, which leads to the conclusion that in both measurements there is a normal distribution of the data.

In Table 4 it can be observed the analysis of the means (student's t-test) of the different measurements carried in the different posts, in which a calculated t-value of 1190 and a p-value of 0.2438 was obtained. It is concluded that

Discusión La literatura reporta algunos estudios de postes y materiales de reconstrucción en los que se evalúan diversos aspectos como, la fuerza ejercida, la forma de aplicación de la carga, el material del poste evaluado, el material del relleno del composite, el uso o no de coronas y de dientes naturales, entre otros.10-12 En el presente estudio in vitro se midió el comportamiento de los postes de fibra de vidrio con la técnica de conformación y postes de fibra de vidrio con la técnica de cementación simple marca Anthogyr fibio, mediante la aplicación de fuerzas de compresión, sometidos a fuerza comprensiva con la maquina universal de pruebas Instron hasta su fractura, con un desplazamiento de 0.5 mm por segundo.

Discussion The literature reports some studies of posts and reconstruction materials in which various aspects are evaluated such as the force exerted, the way of applying the load, the material of the post evaluated, the material of the filling of the composite, the use or not of crowns and of natural teeth, among others.10-12 In the in vitro study, the behavior of glass fiber posts with the technique of forming and posts of glass fiber was measured with the simple cementing technique, Anthogyr fibio brand, using the application of compression forces, subjected to comprehensive force with the Instron universal testing machine until fracture, with a displacement of 0.5 mm per second.

Al analizar los resultados, en el presente estudio se observa que coincide con lo publicado en 2016 por Hsuan-Wen WANG, YenHsiang CHANG y Chun-Li LIN que nos dice que los resultados indican que el valor promedio y la desviación estándar para la flexión de tres puntos analizados para el poste de fibra SGFR anatómico y X-Post. La prueba t mostró una diferencia significativa de (p=0,00259) y no se encontraron diferencias significativas para (p=0,006) entre el poste de fibra SGFR anatómico y X-Post.13 en este estudio al igual que el presentado no se encontró diferencia estadísticamente significativa.

When analyzing the results, in the present study it is observed that it coincides with that published in 2016 by Hsuan-Wen WANG, Yen-Hsiang CHANG and Chun-Li LIN, which tells us that the results indicate the average value and the standard deviation for the flexure of three points analyzed for the anatomical SGFR fiber post and X-Post.

The t-test showed a significant difference of (p=0.00259) and no significant differences were found for (p=0.006) between the anatomical SGFR fiber post and X-Post.13 In this study like the one presented, no statistically significant difference was found.

El patrón de fractura de los postes de fibra de vidrio se concentró en el área cervical y media radicular en comparación con los postes colados que se concentró en la zona apical haciendo desfavorable su retratamiento restaurador.14

Conclusiones

Conclusions

En este sentido en base a la culminación del estudio podemos decir que los postes de fibra de vidrio con la técnica de conformación anatómica y los postes de fibra de vidrio con la técnica de cementación simple resultó que no existe diferencia estadísticamente significativa a la resistencia a la fractura ante las fuerzas de compresión con base en los resultados del estudio, ya que éstos obtuvieron una media de 671.06 MPa, mientras que los de fibra de vidrio obtuvieron una media de 618.06 MPa.

In this sense based on the culmination of the study we can say that the posts of glass fiber with the technique of anatomical conformation and the posts of glass fiber with the technique of simple cementation do not show statistically significant difference to the resistance to the fracture before the compression forces based on the results of the study, since these obtained an average of 671.06 MPa, while those of fiberglass obtained an average of 618.06 MPa.

Resumen

Abstract

Introducción: El término cáncer infantil se aplica a los niños antes de los 10 años. representa entre un 0.5% y un 4.6% de morbilidad. Objetivo: Obtener la frecuencia de tumores malignos en cabeza y cuello (TMCC) en niños atendidos en el Servicio de Otorrinolaringología y Cirugía de Cabeza y Cuello (SORLyCCC) del Hospital Central Militar (HCM) durante el período 2011-2014.

Material y Método: Mediante un estudio observacional, transversal y retrospectivo se revisaron los expedientes con diagnóstico histopatológico de tumores malignos en niños atendidos en el SORLyCCC, mismos que fueron analizados. Resultados: Se obtuvieron 109 expedientes con diagnóstico histopatológico de tumor maligno de pacientes menores de 10 años, de los cuales 97(88%) estaban localizados en diferentes regiones anatómicas, con excepción de cabeza y cuello y 12 (11.%) localizados en la región de cabeza y cuello, el 75% correspondieron al sexo masculino y 25.0% al femenino, con una razón de 3:1, con la media de edad de 5 años. Los tumores malignos estuvieron representados por los linfomas de hodgkin, linfomas no hodgkin y meduloblastomas con el 16.6% respectivamente. Mientras que el astrocitoma anaplásico, astrocitoma, carcinoma epidermoide, carcinoma mucoepidermoide, germinoma y retinoblastoma representaron el 8.3% cada uno. Conclusiones: Los TMCC presentaron una distribución muy similar a la reportada en la literatura donde los tumores del sistema nervioso central y los del tejido linfático fueron los más frecuentes con el 66.6% en conjunto.

Introduction: The term childhood´s cancer applies to children before 10 years of age. It represents between 0.5% and 4.6% of morbidity. Objective: To obtain the frequency of malignant tumors in the head and neck (MTHN) in children treated at the Otolaryngology Service Service and Head and Neck Surgery Service (ENTS&HNSS) of the Central Military Hospital (CMH) during the period 2011-2014. Material and Method: Through an observational, cross-sectional and retrospective study, It was reviewed the records with histopathological diagnosis of malignant tumors in children treated in the ENTS&HNSS, which were analyzed. Results: It was obtained 109 records with a histopathological diagnosis of malignant tumor in patients under 10 years of age, of which 97 (88%) were located in different anatomical regions, with the exception of head and neck and 12 (11%) located in the region of head and neck, 75.0% corresponded to the male sex and 25.0% to the female, with a ratio of 3:1, with the mean age of 5 years. Malignant tumors were represented by hodgkin lymphomas, non-hodgkin lymphomas and medulloblastomas with 16.6%, respectively. While anaplastic astrocytoma, astrocytoma, squamous cell carcinoma, mucoepidermoid carcinoma, germinoma and retinoblastoma accounted for 8.3% each.Conclusions: The MTHN presented a distribution very similar to that reported in the literature where tumors of the central nervous system and those of the lymphatic tissue were the most frequent with 66.6% as a whole.

Introducción

Existen diferentes clasificaciones para englobar a los niños utilizando su edad, una de ellas es la que propone el Instituto Nacional de Salud Pública de México donde agrupa a los niños preescolares como personas del hogar con edad entre 0 y 4 años cumplidos, niños escolares como las personas del hogar con edad entre 5 y 9 años cumplidos, adolescentes como las personas del hogar con edad entre 10 y 19 años cumplidos y adultos como las personas del hogar con edad mayor a 20 años cumplidos.1

There are different classifications to include children using their age, one of them is the one proposed by the National Institute of Public Health of Mexico, which groups preschool children as persons from the household aged between 0 and 4 years old, school children as persons of the household aged between 5 and 9 years old, adolescents such as persons of the household aged between 10 and 19 years old and adults such as persons of the household aged over 20 years old.1

Las estadísticas mundiales relacionadas con los tumores de cabeza y cuello se basan principalmente en neoplasias malignas en adultos, por ser el cáncer un problema de salud pública. El término «cáncer infantil» generalmente se utiliza para designar distintos tipos de cáncer que pueden aparecer en los niños antes de los 15 años. Éste es poco frecuente, pues representa entre un 0.5% y un 4.6% de la carga total de morbilidad por esta causa. Las tasas mundiales de incidencia oscilan entre 50 y 200 por cada millón de niños en las distintas partes del mundo.2

Global statistics related to head and neck tumors are mainly based on malignancies in adults, because cancer is a public health problem. The term "childhood´s cancer" is usually used to designate different types of cancer that can appear in children before age 15. This is rare, since it represents between 0.5% and 4.6% of the total burden of morbidity due to this cause. The world incidence rates range between 50 and 200 per million children in different parts of the world.2

Según estadísticas del Programa de Vigilancia Epidemiológica y Resultados Finales (PVERF) del Instituto Nacional del Cáncer en Estados Unidos, la incidencia del cáncer en niños varía según la edad, el sexo, la raza y la localización geográfica, por lo que se estima una tasa anual de 12 a 14 casos por cada 100.000 habitantes menores de 15 años, de ellos, un 60% corresponde a tumores sólidos y un 40% se diagnostica de leucemia o linfoma, teniendo mayor predilección el sexo masculino en estos últimos (Tabla 1).3 Asimismo, sugiere que la cabeza y el cuello están involucrados en el 12% de todas las neoplasias malignas de la infancia.4 Los tumores malignos de cabeza y cuello (boca, laringe, faringe, glándulas salivales, nariz) excluyendo los que se desarrollan en la glándula tiroides y en la piel, comprenden el 3% de todos los cánceres en Estados Unidos (2014) y el 4.9% en México (2002), de los cuales el 65% de los carcinomas de vías aerodigestivas superiores se diagnostican en etapas avanzadas con un pronóstico de supervivencia y funcional pobre.5,6 La distribución de los tumores de cabeza y cuello es muy compleja porque las características clínicas, aspectos etiológicos, comportamientos biológicos, impactos funcionales y respuestas al tratamiento son particulares a cada una de las neoplasias. En México la localización más frecuente es el tumor laríngeo con el (42%), seguido por la cavidad bucal (37%), fosas nasales y senos paranasales (9%), cáncer bucofaríngeo (6%), nasofaríngeo y el hipofaríngeo con el 3% respectivamente. La glándula tiroides representa el 10% de todas las neoplasias en esta región. Otras tumoraciones menos frecuentes se localizan en el cerebro (7.5%), en la piel de cabeza y cuello como son los melanomas (5%), los carcinomas de las glándulas salivales (1.8%) y aquellos que se desarrollan en el ojo y anexos con el 1.7%. According to statistics from the Surveillance, Epidemiology, and End Results Program (SEER) of the National Cancer Institute in the United States, the incidence of cancer in children varies according to age, sex, race and geographic location, therefore, an annual rate is estimated of 12 to 14 cases per 100,000 inhabitants under 15 years; of them, 60% corresponds to solid tumors and 40% to leukemia or lymphoma, having a greater incidence for males in the latter (Table 1).3 It also suggests that the head and neck are involved in 12% of all malignant neoplasms of childhood.4 Malignant tumors of the head and neck (mouth, larynx, pharynx, salivary glands, nose) excluding those that develop in the thyroid gland and skin, comprise 3% of all cancers in the United States (2014) and 4.9 % in Mexico (2002), of which 65% of upper aerodigestive tract carcinomas are diagnosed in advanced stages with a poor survival and functional prognosis.5,6 The distribution of head and neck tumors is very complex because the clinical characteristics, etiological aspects, biological behaviors, functional impacts and responses to treatment are particular to each of the neoplasms. In Mexico, the most frequent location is the laryngeal tumor with (42%), followed by the oral cavity (37%), nasal and sinus pits (9%), oropharyngeal cancer (6%), nasopharyngeal and hypopharyngeal cancer with the 3% respectively. The thyroid gland represents 10% of all neoplasms in this region.

Other less frequent tumors are located in the brain (7.5%), in the skin of the head and neck such as melanomas (5%), carcinomas of the salivary glands (1.8%) and those that develop in the eye and attachments with 1.7%. As can be seen in Table 2, deaths from head and neck cancer were 8.2% with respect to all deaths from malignant tumors (58,599) and morbidity was represented by skin tumors with 62% followed by thyroid cancers (10.2%).6 In the United States, mortality from head and neck tumors in children has been declining since 2001, although when analyzed by race, African Americans die more than caucasians.

In Mexico, these tumors are very important due to the high lethality and the serious aesthetic and functional sequelae derived from the treatments. It is estimated that the lethality in skin cancer is 5.4%, while melanoma is 29% and that of the thyroid is up to 24%.5 During the period from 2011 to 2014, 123 cases of cancer in the head and neck were reported in children and adolescents in Mexico (under the age of 10 years), 61% being male and 39% female, with a ratio of 1.5: 1. El nivel socioeconómico es otra variable que se reporta en los estudios epidemiológicos relacionada con presencia de ciertos tumores como, por ejemplo: el retinoblastoma, que se ha reportado con mayor frecuencia en países en vías de desarrollo (India, 13%), así como en un estudio realizado en el México (12%) en donde la mayoría de las ocupaciones de los padres fueron obreros (68.7%) y de las madres, dedicadas al hogar (76.1%). 7,8

Although there is no direct cause leading to the onset of cancer, the World Health Organization (WHO) through the International Agency for Research on Cancer (IARC) in 2015 proposes the classification of carcinogens in three major groups: 1. Physical carcinogens: ionizing and ultraviolet radiation.

2. Chemical carcinogens: tobacco, alcohol, asbestos, arsenic, aflatoxins.

3. Biological carcinogens: infections caused by certain viruses, bacteria or parasites.

En cuanto al sexo, éste difiere de acuerdo con el tipo de tumor y a la región geográfica en donde se estudien, sin embargo, en la mayoría, el sexo masculino predomina con una razón de 1.6:1.29.

Socioeconomic status is another variable that is reported in epidemiological studies related to the presence of certain tumors, such as, for example, retinoblastoma that has been reported more frequently in developing countries (India, 13%), as well as in a study carried out in Mexico (12%) where the majority of the occupations of the parents were laborer (68.7%) and of the mothers, dedicated to the home (76.1%).7,8

Squamous cell carcinoma

Con respecto a la edad, los menores de 5 años presentan mayor incidencia de tumores embrionarios (meduloblastoma, retinoblastoma, neuroblastoma, rabdomiosarcoma), a diferencia de aquellos niños que se encuentran en el grupo de edad de 5 a 9 años en donde los linfomas, sobre todo el de Hodgkin, predominan, seguido por los tumores óseos.

Carcinoma epidermoide

En México son muy importantes estos tumores por la alta letalidad y por las graves secuelas estéticas y funcionales derivadas de los tratamientos. Se estima que la letalidad en cáncer de piel es del 5.4%, mientras que el melanoma es del 29% y el de tiroides alcanza hasta el 24%.5 Durante el período 2011 al 2014 se reportaron en el Registro de Cáncer en Niños y Adolescentes (México) 123 casos de cáncer en cabeza y cuello en menores de 10 años, siendo el 61% en el sexo masculino y el 39% en el femenino con una razón de 1.5:1. Los tejidos blandos de cabeza y cuello se afectaron más en niñas (60.4%) que en los niños (34.7%), seguidos por las fosas nasales con el 8.1% en ambos sexos y la piel del cuero cabelludo con el 6.5%.

Exposure to these agents, in combination with environmental and genetic factors, as well as unhealthy lifestyles that include habits such as smoking, the intake of alcohol and foods rich in fats and carbohydrates, as well as sedentary lifestyle, are the causes mostly associated with the development of the disease. However, in children not all have an explanation, only 5% have as origin a hereditary mutation that may or may not be associated with certain family syndromes, genetic mutations during fetal development, exposure to ionizing radiation to environmental compounds such as those derived from benzene and pesticides (exposure also in parents), electromagnetic fields and some viruses.9,10

La exposición a estos agentes, en combinación con factores ambientales y genéticos, así como estilos de vida poco saludables que incluyen hábitos como el tabaquismo, la ingesta de alcohol y de alimentos ricos en grasas y carbohidratos, así como el sedentarismo, son las causas que más se asocian al desarrollo de la enfermedad.

Sin embargo, en niños no todos tienen una explicación, sólo el 5% tienen como origen una mutación hereditaria que puede estar o no asociada a ciertos síndromes familiares, a mutaciones genéticas durante el desarrollo fetal, a la exposición a radiación ionizante, a compuestos ambientales como aquellos derivados del benceno y plaguicidas (exposición también en los padres), a campos electromagnéticos y algunos virus.9,10

This study was observational, cross-sectional and retrospective in the database of the ENT Service and Head and Neck Surgery Service (ENTS&HNSS) of the Central Military Hospital (CMH). The type and size of the sample was conventional, all of the clinical files of patients with a confirmed diagnosis of malignant tumor were used and among these, only those who were located in the head and neck region during the period of 2011 to 2014 treated in the ENTS&HNSS of the CMH. The inclusion criteria were all the complete clinical files, the independent variables were the patients, and the dependent ones were the frequency of malignant tumors, sex and age group, state of residence, anatomical location, proportion of children under 10 years of age with head cancer according to the classification of the National Institute of Public Health of Mexico, according to the current state, proportion of children under 10 years of age with head and neck cancer, according to the different treatments received and the frequency of children under 10 years of age, head and neck cancer, according to the year of diagnosis.

Hypothesis Malignant tumors of the central nervous system are the most frequent tumors of the head and neck in children treated in the ENTS&HNSS of the CMH during the period 2011-2014.

Objective To determine the frequency of malignant tumors in the head and neck in children of the ENTS&HNSS of the CMH during the period 2011-2014.

Materiales y Métodos: Results Este estudio fue de tipo observacional, transversal y retrospectivo realizado en la base de datos del Servicio de Otorrinolaringología y Cirugía de Cabeza y Cuello (SORLyCCC) del Hospital Central Militar (HCM). confirmado de tumor maligno y de éstos los que se localizaron en la región de cabeza y cuello durante el período de 2011 a 2014 atendidos en el SORLyCCC del (HCM). Los criterios de inclusión fueron todos los expedientes clínicos completos, las variables independientes fueron los pacientes y las dependientes fue la frecuencia de tumores malignos, sexo y grupo de edad, entidad federativa de residencia, localización anatómica, proporción de niños menores de 10 años con cáncer de cabeza de acuerdo a la clasificación del Instituto Nacional de Salud Pública de México, según el estado actual, proporción de niños menores de 10 años con cáncer de cabeza y cuello según los diferentes tratamientos recibidos y la frecuencia de menores de 10 años con cáncer de cabeza y cuello según el año del diagnóstico.

Hipótesis Los tumores malignos del sistema nervioso central son los más frecuentes de cabeza y cuello en niños atendidos en el SORLyCCC del HCM durante el período 2011-2014.

Objetivo Determinar la frecuencia de tumores malignos en cabeza y cuello en niños del SORLyCCC del HCM durante el período 2011-2014.

(88%) were located in different anatomical regions except for the head and neck and 12 (11.0%) located in the head and neck region (Figure 1).

Malignant tumors were represented by hodgkin lymphomas, non-hodgkin lymphomas and medulloblastomas with 16.6%, respectively. While anaplastic astrocytoma, astrocytoma, squamous cell carcinoma, mucoepidermoid carcinoma, germinoma and retinoblastoma accounted for 8.3% each.

Distribution by sex The sample showed that the majority of patients who requested care for the disease were male (75%) with a masculinity index of 3: 1. (Figure 3).

Distribution by sex and age group With respect to the distribution of children by age group, it is observed that, although the male sex prevails in both groups, the ratio varies from 5: 1 for children under 5 years of age, and 2: 1 for those who have between 5 to 9 years of age (Figure 4).

En el presente estudio se obtuvieron 109 expedientes con diagnóstico histopatológico de tumor maligno de pacientes menores de 10 años que fueron atendidos en el SORLyCCC del HCM durante el período 2011 al 2014, de los cuales 97(88%) estaban localizados en diferentes regiones anatómicas con excepción de cabeza y cuello y 12 (11.0%) localizados en la región de cabeza y cuello (Gráfica 1).

Los tumores malignos estuvieron representados por los linfomas de hodgkin, linfomas no hodgkin y meduloblastomas con el 16.6% respectivamente. Sexo y Tipo de tumor Los tumores más frecuentes en el sexo masculino fueron el linfoma de Hodgkin, linfoma no Hodgkin y el meduloblastoma con el mismo porcentaje (22.2%) a diferencia de los que se presentaron en el sexo femenino como el astrocitoma anaplásico, el carcinoma epidermoide y el germinoma con el 33.3% cada uno (Gráfica 6).

Edad y tipo de tumor En la siguiente gráfica se muestra la frecuencia de tumores malignos por grupo de edad y tipo de tumor. Como podemos observar, la mayoría de los tumores que se presentan dentro del primer y cuarto año de vida fue el meduloblastoma, asimismo, el que se presentó con mayor frecuencia entre los 5 y 9 años, fue el Linfoma de Hodgkin, ambos con el 33.3% (Gráfica 7).

Tipo de tumor y localización anatómica En la Tabla 3, se muestra la distribución de casos con cáncer de cabeza y cuello en menores de 10 años por tipo de tumor y localización anatómica. La mayoría de los tumores se encontraron en el encéfalo y en los ganglios linfáticos de cabeza, cara y cuello, presentando un total de 4 casos cada uno; mientras que en la glándula parótida, retina, seno maxilar derecho y tejidos blandos del cuello se reportó solo un caso.

Tratamiento Como se puede observar en la Gráfica 8, la mayoría de los niños fueron tratados con la combinación de cirugía, radioterapia y quimioterapia. Con respecto a la cirugía, sólo se reportaron del sexo masculino con el 33.3%.

Sex and Type of tumor The most frequent tumors in the male sex were Hodgkin's Lymphoma, Non-Hodgkin's Lymphoma and Medulloblastoma with the same percentage (22.2%) unlike those that occurred in the female sex as Anaplastic Astrocytoma, Squamous Cell Carcinoma and Germinoma with 33.3% each (Figure 6).

Age and type of tumor The following chart shows the frequency of malignant tumors by age group and type of tumor. As we can see, the majority of tumors that occur within the first and fourth year of life was Medulloblastoma, also, the one that occurred most frequently between 5 and 9 years of age, was Hodgkin's Lymphoma. Both with 33.3% (Figure 7).

Type of tumor and anatomical location In the Table 3, the distribution of cases with head and neck cancer in children under 10 years of age by type of tumor and anatomical location is shown. As we can see, most of the tumors were found in the encephalon and in the lymph nodes of the head, face and neck, presenting a total of 4 cases each; while in the parotid gland, retina, right maxillary sinus and soft tissues of the neck only a single case was reported.

Treatment As can be seen in Figure 8, most of the children were treated with the combination of Surgery, Radiotherapy and Chemotherapy. Regarding surgery, only men were reported with 33.3%.

Type of tumor and Treatment Table 4 shows that children with Hodgkin lymphoma and squamous cell carcinoma received chemotherapy and radiotherapy for their treatment in 50% respectively and children with non-Hodgkin's lymphoma, mucoepidermoid carcinoma and retinoblastoma (33.3% each) were treated with surgery.

Actual state With respect to the current state of the patient, 58.3% are alive with tumor activity and 16.7% died having tumor activity (Figure 9).

De acuerdo con el análisis que se presenta de los datos, se puede observar que el comportamiento de los tumores en cabeza y cuello en niños entre los 0 y 9 años de edad es muy similar a lo reportado en la literatura.1

En un estudio realizado en población abierta en México durante el período 2007 al 2012 en 55 unidades hospitalarias acreditadas para el tratamiento de menores de 18 años con cáncer, se reportó que la mayor incidencia fue en el 2012 con el 57.1% en niños menores de 5 años y de 45.2% entre los 5 y 9 años de edad,7 lo que es similar a lo encontrado en el HCM en ese año con el 77.7% de frecuencia. La razón mostró una relación de 1.2:1 en el estudio de población abierta en donde el sexo masculino predominó11 al igual que en este estudio; sin embargo la distribución fue mayor en este último con la razón de 3:1. Asimismo los tipos de tumor que se encontraron en la población abierta después de las leucemias correspondieron a los linfomas y las neoplasias intracraneales11 lo mismo que se reportó en el HCM con el 33.3% para cada uno de los tumores.

El comportamiento de los datos reportados en este estudio fue similar a lo encontrado en una investigación realizada por el Instituto Mexicano del Seguro Social (IMSS) durante el período 1996 a 2001 en niños residentes de la Ciudad de México.12 Aunque el sexo masculino predominó en ambos estudios la distribución fue diferente con una razón masculino/femenino de 1.1:1 para el IMSS y de 3:1 para el SORLyCCC del HCM. Con lo que respecta al tipo de tumor y el sexo, los niños del IMSS presentaron linfomas (15.6%) y tumores del sistema nervioso central (9.5%) predominantemente en el sexo masculino similar al HCM que reportó linfomas y meduloblastoma con el 22.2%.

Discussion

In a study conducted in the open population in Mexico during the period of 2007 to 2012, in 55 hospitals licensed for the treatment of children under 18 with cancer, it was reported that the highest incidence was in 2012 with 57.1% in children under 5 years, and 45.2% between the age of 5 and 9 years of age,7 which is similar to that found in the CMH in that year with 77.7% frequency. The ratio showed a 1.2: 1 ratio in the open population study where the male sex predominated11 as in this study, however the distribution was higher in the latter with the ratio of 3: 1. Likewise, the types of tumor that were found in the open population after the leukemias corresponded to lymphomas and intracranial neoplasms,11 as was reported in the CMH with 33.3% for each of the tumors.

The behavior of the data reported in this study was similar to that found in an research conducted by the Mexican Social Security Institute (MSSI) during the period of 1996 to 2001 in children living in Mexico City.12 Although the male sex predominated in both studies, the distribution was different with a male / female ratio of 1.1: 1 for the IMSS and of 3: 1 for the ENTS&HNSS of the CMH. With regard to the type of tumor and sex, the children of the MSSI presented lymphomas (15.6%) and tumors of the central nervous system (9.5%) predominantly in the male sex, similar to the CMH that reported lymphomas and medulloblastoma with 22.2%.

For the female sex, the MSSI found 13.8% in brain tumors and 7.5% in germ cell tumors,12 which resembles what was found in the ENTS&HNSS of the CMH, where girls presented 33.3% in Astrocytomas, Germinomas and Squamous Cell Carcinomas. In relation to age, children under one year of age of the MSSI reported Retinoblastoma with greater frequency (17.4%), in the group of 1 to 4 years, tumors of the central nervous system (11-0%) and 5 to 9 lymphomas with 16.2%, which was similar to what was demonstrated in this study in which children from 1 to 4 years old presented Medulloblastoma with 33.3% and Retinoblastoma with 16.7% followed by the group of 5 to 9 years with Hodgkin in 33.3% and non-Hodgkin lymphoma in 16.7% .12 With regard to treatment, it has been shown in research that surgical resection produces 62% of the cures in patients

Para el sexo femenino el IMSS encontró el 13.8% en tumores cerebrales y el 7.5% en tumores de células germinales12 lo que se asemeja a lo encontrado en el SORLyCCC del HCM en donde las niñas presentaron el 33.3% en astrocitomas, germinomas y carcinomas epidermoides. En relación con la edad, los menores de un año edad del IMSS reportaron el retinoblastoma con mayor frecuencia (17.4%), en el grupo de 1 a 4 años, los tumores del sistema nervioso central (11-0%) y de 5 a 9 los linfomas con el 16.2% lo que fue similar a lo demostrado en este estudio en donde los niños de 1 a 4 años presentaron meduloblastoma con el 33.3% y retinoblastoma con el 16.7% seguidos por el grupo de 5 a 9 años con enfermedad de Hodgkin en el 33.3% y linfoma no Hodgkin en el 16.7% .12 Con respecto al tratamiento se ha demostrado en investigaciones que la resección quirúrgica produce el 62% de las curaciones en pacientes con la enfermedad, siendo la radioterapia que otorga el 25%, la quimioterapia el 4% y la combinación de todos ellos agrega el 9%.13 En los resultados obtenidos en este estudio la cirugía, radioterapia y quimioterapia representó el mayor tratamiento otorgado con el 58.3%, seguido por la cirugía con el 25.1% y la quimioterapia, radioterapia con el 16.6%.

Conclusiones

Conclusions

Los resultados de esta investigación demostraron que el comportamiento de los tumores malignos en cabeza y cuello en niños menores de 10 años son similares a lo reportado en la literatura, siendo los de origen del sistema nervioso central junto con los del tejido linfático los que ocuparon los primeros lugares con el 66.6% en conjunto. Sin embargo, es necesario que los datos hospitalarios sean constantes en el registro para garantizar la toma de decisiones en cuanto al diagnóstico oportuno, el tratamiento y el pronóstico de sobrevida de los derechohabientes.

The results of this research showed that the behavior of malignant tumors in the head and neck in children under 10 years of age is similar to that reported in the literature, being those of the central nervous system along with those of the lymphatic tissue the ones that occupied the first places with 66.6% as a whole. However, it is necessary that the hospital data be constant in the registry to guarantee the decision making regarding a timely diagnosis, the treatment and the survival prognosis of the rightholders.

El panorama epidemiológico del cáncer en menores de 10 años en cabeza y cuello que son atendidos en el SORLyCCC del HCM que se obtuvo en esta investigación es el primer esfuerzo de trabajos futuros necesarios para identificar la magnitud del problema de salud en los niños derechohabientes del ejército que conduzcan a mejorar la calidad de la detección oportuna y la atención.

The epidemiological outlook of cancer in children under 10 years of age in the head and neck treated in the ENTS&HNSS of the CMH obtained in this research is the first effort of future work necessary to identify the magnitude of the health problem in the children entitled to the army that lead to improve the quality of timely detection and care.
