s una pregunta que me han realizado algunos pacientes que por cualquier causa han perdido sus dientes naturales y que acuden a consulta odontológica. Mi respuesta es: el mejor implante que usted puede tener es su propio diente natural.

En personas que han perdido sus dientes, los implantes de titanio son una buena alternativa, sin embargo, en personas que tienen dientes con problemas de caries, periodontitis apical, etcétera, la mejor opción será su conservación y no la extracción. En la Odontología existió una época obscura donde la especialidad de Endodoncia se denominó infección focal (1910-1928), siendo así la responsable de miles de extracciones dentarias, se afirmaba que los dientes despulpados eran causantes de enfermedades sistémicas. Hoy sabemos que esto es imposible.

In people who have lost their teeth, titanium implants are a good alternative, however, in people who have their teeth with problems such as caries, apical periodontitis, etc. the best option will be its conservation and not the extraction. In Dentistry there was a dark period, the specialty of Endodontics called focal infection (1910-1928), responsible for thousands of tooth extractions, t was claimed that the teeth that the pulp was removed its cause the systemic diseases. Today we know that this is impossible.

Con el avance científico-tecnológico de la Odontología y con el enfoque de la especialidad de Endodoncia, las investigaciones publicadas en revistas especializadas nos ha permitido tomar decisiones diagnósticas y terapéuticas basadas en la evidencia odontológica.

With the scientific-technological advance of Dentistry and with the focus of the specialty of Endodontics, the research published in specialized journals has allowed us to make diagnostic and therapeutic decisions based on evidence-based dentistry.

La globalización ha logrado que estemos más informados en todos los ámbitos tanto social, como científico y tecnológico y a partir de este auge en las comunicaciones podemos interactuar con especialistas altamente calificados o leer sus investigaciones, así mismo, las personas estan más informadas y por ende conocen la consecuencia de no tener dientes o de perderlos.

Globalization has made us more informed in all social, scientific and technological fields also from this boom in communications we can interact with highly qualified specialists or read their research, likewise, people are more informed and therefore know the consequence of not having teeth or losing them.

Ante el cuestionamiento planteado y el concepto de que el mejor implante es el diente natural, los investigadores y clínicos han propuesto desde hace algunos años al autotrasplante dental como alternativa para la conservación de los dientes. El glosario de términos de la Asociación Americana de Endodoncia define al autotrasplante dental como: el traslado del diente de un sitio a otro en la misma persona para sitios de extracción o cavidades preparadas quirúrgicamente.

Before the questioning raised and the concept that the best implant is the natural tooth, researchers and clinicians have proposed for some years to the dental auto transplant as an alternative for the conservation of teeth. The glossary of the American Association of Endodontists defines as autologous dental tooth transplant to transfer from one place to another in the same person for extraction sites or cavities surgically prepared.

El autotrasplante dental es una alternativa terapéutica que bien diagnosticado y tratado devuelve a los pacientes estética y funcionalidad.

Existen diversas indicaciones para un autotrasplante dental, la más común, es la del tercer molar para el espacio alveolar del primer molar; así como para el segundo molar, zona de premolares y de estos mismos para el segmento anterior. Cuando vamos a realizar un autotrasplante se deben contemplar factores como la edad del paciente, compatibilidad de dimensiones, ápice maduro o inmaduro, cicatrización (pulpar y periodontal, reabsorción radicular, desarrollo y perturbaciones radiculares.

The dental auto transplant is a therapeutic alternative that well diagnosed and treated returns to the patients esthetic and functionality. There are several indications for a dental auto transplant, the most common is the third molar for the alveolar space of the first molar; as well as for the second molar.

• Contenido / Content Debemos realizar un plan quirúrgico para evitar posibles complicaciones durante la cirugía y en los casos que se requiera, utilizar un modelo estereolitográfico diseñado por tomografía, porque está demostrado que su uso durante la cirugía reduce el tiempo en que el diente donador está fuera de boca.

We must perform a surgical plan to avoid possible complications during surgery and in cases that require a stereolithographic model designed by tomography should be used, because it is proven that its use during surgery reduces the time in which the donor tooth is out of mouth.

No debemos olvidar que un autotrasplante es un traumatismo dental controlado y que si el diente donador se manipula por mucho tiempo fuera de boca durante la cirugía, estámos propensos al fracaso por la técnica quirúrgica. Otro factor que debemos considerar es el empleo de magnificaciones y con esto me refiero al uso de microscopio clínico para una cirugía mínimamente invasiva.

We must not forget that an auto transplant is a controlled dental trauma and, if the donor tooth is manipulated for a long time out of mouth during surgery, we are prone to failure by the surgical technique. Another factor that we must consider is the use of

Por lo anterior, mi concepto es que: el mejor implante es el diente natural y por tanto, el autotrasplante dental es viable de ser llevado a cabo desde que los especialistas tienen el conocimiento teórico, la habilidad técnica y el respeto hacia el potencial de curación de los tejidos dentarios.

Therefore, my concept is that: the best implant is the natural tooth and therefore, the dental auto transplant is viable to be carried out since the specialists have the theoretical knowledge, the technical ability and the respect towards the potential of healing of the dental tissues.

RESUMEN

SUMMARY

Los quistes dentígeros son el segundo tipo más común de lesión quística de desarrollo en la mandíbula y han sido asociados con dientes no erupcionados. Son pocos los reportes sobre quistes dentígeros bilaterales o múltiples en pacientes no sindrómicos. Su tratamiento tradicional es por medio de la enucleación y en muy raras ocasiones se decide alguna alternativa de este tipo de tratamiento. Se reporta un caso clínico de un paciente de 11 años de edad, con quiste dentígero bilateral no asociado a síndromes tratado mediante la técnica alternativa de marsupialización y con seguimiento radiográfico a 7 meses sin presentar recidiva.

Dentigerous cysts are the second most common type of developmental cystic lesion in the jaw and have been associated with unerupted teeth. There are few reports of bilateral or multiple dentigerous cysts in non-syndromic patients. Its traditional treatment is by means of enucleation and on very rare occasions an alternative of treatment is decided. Its reported a clinical case of a 11-year-old patient with bilateral dentigerous cyst not associated with syndromes, treated by the alternative technique of marsupialization with radiographic follow-up at 7 months without recurrence.

INTRODUCCIÓN

INTRODUCTION

Los quistes odontogénicos se originan del componente epitelial del aparato odontogénico o de sus restos celulares que quedan atrapados dentro del hueso o en el tejido gingival que cubre a los maxilares. De acuerdo a su patogénesis se clasifican como lesiones del desarrollo o de tipo inflamatorio.1

Odontogenic cysts originate from the epithelial component of the odontogenic apparatus or from its cellular remains that are trapped inside the bone or in the gingival tissue that covers the jaws. According to their pathogenesis, they are classified as developmental or inflammatory lesions.1

literatura de casos de quistes dentígeros múltiples o bilaterales, siendo extremadamente raros y en casi todos los casos en relación con síndromes como displasia cleidocranial, síndrome de células basales y mucopolisacaridosis.5-8 El diente que se encuentra más frecuentemente relacionado a este tipo de lesiones es el tercer molar inferior, seguido por el canino superior y los premolares mandibulares.

El quiste dentígero usualmente se presenta como una lesión asintomática, pero puede causar aumento de volumen considerable por medio de la expansión de las corticales óseas y retardo en la erupción dentaria.3-5 En la literatura se han reportado casos que relatan la regresión espontánea de los quistes dentígeros,9-11 pero en la mayoría de ellos existen muchas controversias en cuanto a que este tipo de situaciones puedan ocurrir.12-14 El potencial agresivo del quiste dentígero influye en el tipo de procedimiento terapéutico a utilizar. Lesiones de menor diámetro principalmente en pacientes jóvenes o niños son removidos completamente para prevenir daño permanente en el diente y favorecer su erupción mientras que las lesiones de mayor tamaño son tratadas por medio de marsupilización o descompresión.2,15,16 Debido al alto potencial de desarrollo, los quistes dentígeros pueden llegar a presentar tamaños extremadamente grandes antes de ser diagnosticados, por lo que es importante resaltar que sí existe la posibilidad de causar lesiones a estructuras adyacentes o dejar la mandíbula debilitada incapaz de recibir cargas funcionales que causarían una fractura patológica, debido a lo cual se recomienda el tratamiento por medio de la técnica quirúrgica de marzupialización.

the literature of multiple or bilateral dentigerous cysts, being extremely rare and in almost all cases related to syndromes such as cleidocranial dysplasia, basal cell syndrome and mucopolysaccharidosis.5-8 The tooth that is most frequently related to this type of lesions is the lower third molar followed by the maxillary canine and the mandibular premolars.

The dentigerous cyst usually presents as an asymptomatic lesion but can cause considerable volume increase through the expansion of the bony cortices and delay in tooth eruption.3-5 In the literature, cases have been reported relate the spontaneous regression of dentigerous cysts,9-11 but in most of them there are many controversies as to whether this type of situation can occur.12-14 The aggressive potential of the dentigerous cyst influences the type of therapeutic procedure to be used. Small diameter lesions mainly in young patients or children are completely removed to prevent damage to the permanent tooth and favor its eruption while the larger lesions are treated by means of marsupilization or decompression.2,15,16

THE DENTIGEROUS CYST USUALLY PRESENTS AS AN ASYMPTOMATIC LESION BUT CAN CAUSE CONSIDERABLE VOLUME INCREASE THROUGH THE EXPANSION OF THE BONY CORTICES AND DELAY IN TOOTH ERUPTION. 6a Due to the high potential of development, dentigerous cysts can reach extremely large sizes before being diagnosed, so it is important to note that if there is the possibility of causing injury to adjacent structures or leave the jaw weakened unable to receive functional loads that they would cause a pathological fracture, due to which treatment is recommended by means of the marzupialization surgical technique.

REPORTE DEL CASO Paciente masculino de 11 años de edad sin antecedentes patológicos de importancia, lesiones cariosas y sintomatología. Admitido para tratamiento de ortopedia en el departamento de Odontopediatría de la División de Estudios de Posgrado de la Universidad Nacional Autónoma de México.

Male patient of 11 years of age with no relevant pathological history, carious lesions and symptomatology. Admitted for orthopedic treatment in the Department of Pediatric Dentistry of the Division of Postgraduate Studies of the National Autonomous University of Mexico.

In both the extraoral and intraoral clinical examination no signs of inflammation were observed (Figure 1-4). In the orthopantomography, two radiolucent zones are observed at the level

Se realizó interconsulta con el área de patología y de odontopediatría, se decidió realizar un tratamiento conservador, el cual consistió en la reducción de los quistes por marsupialización debido a la extensión de los mismos, para conservar los dientes permanentes y minimizar el riesgo de fractura mandibular debido a la extracción del tejido involucrado.

Internal referral was made with the area of pathology and pediatric dentistry, it was decided to perform a conservative treatment which consisted in the reduction of the cysts by marsupialization due to the extension of theme to preserve the permanent teeth and to minimize the risk of mandibular fracture due to the extraction of the tissue involved.

Con anestesía local mepivacaina al 2%, se realiza la marsupialización, extrayéndose también dientes temporales involucrados los cuales presentaban previo tratamiento pulpar y restauración con coronas de acero cromo en los dos cuadrantes (Figuras 6 y 7).

With 2% mepivacaine local anesthesia, marsupialization was performed, also involving temporary teeth which had previous pulp treatment and restoration with chrome-steel crown in both quadrants (Figures 6 and 7).

Durante el procedimiento de marzupialización se decide extraer una muestra de la cápsula de cada lesión, para enviarla al departamento de patología para su estudio histopatológico (Figura 8).

During the marsupialization procedure it was decided to extract a sample from the capsule of each lesion, to send it to the pathology department for histopathological study (Figure 8).

El reporte del área de patología menciona que dichas muestras son fragmentos de epitelio escamoso estratificado no queratinizado con edema intercelular en un estroma de tejido conjuntivo fibroso denso y laxo, con infiltrado crónico severo difuso, áreas de hemorragia reciente, presencia de colonias bacterianas y espículas óseas del espécimen izquierdo. La descripción microscópica del lado derecho reportó lo siguiente: el espécimen examinado se encontró formado por bandas de epitelio escamoso estratificado no queratinizado, con patrón arcoidal que varía de grosor y edema intercelular; así como fragmentos irregulares de tejido conjuntivo fibroso denso y laxo, abundantemente vascularizado con infiltrado inflamatorio crónico severo. Confirmando el diagnostico de quiste dentigero en ambos casos.

The report of the area of pathology mentions that these samples are stratified non-keratinized squamous epithelium with intercellular edema in a dense and loose fibrous connective tissue stroma, with diffuse severe chronic infiltrate, areas of recent hemorrhage, presence of bacterial colonies and bone spicules of the left specimen. The microscopic description of the right side reported the following: The examined specimen was formed by stratified non-keratinized squamous epithelium bands, with an arcidal pattern that varies in thickness and intercellular edema. Likewise, irregular fragments of dense and loose fibrous connective tissue, abundantly vascularized with severe chronic inflammatory infiltrate. Confirming the diagnosis of dentigerous cyst in both cases.

Mediante controles clínicos y radiográficos (Figuras 9 y 10), se observo la evolución de las lesiones, donde podemos ver la reducción de las lesiones, así como la erupción de los dos dientes involucrados.

Through clinical and radiographic controls (Figures 9 and 10), the evolution of the lesions was observed, where we can see the reduction of the lesions, as well as the eruption of the two involved teeth.

no erupcionados. Son pocos los reportes sobre quistes dentígeros bilaterales o múltiples en pacientes no sindrómicos. Su tratamiento tradicional es por medio de la enucleación. Algunos artículos mencionan el tratamiento de los quistes dentígeros tratados con marsupialización.

En este trabajo reportamos un caso de quiste dentígero bilateral en un paciente no sindrómico en el cual se realizó como tratamiento, la marsupialización de las dos lesiones con un acompañamiento posterior de 2 años, sin observarse recidiva de la lesión.

In this work we report a case of bilateral dentigerous cyst in a non-syndromic patient in which the marsupialization of the two lesions was carried out as a treatment with a subsequent follow-up of 2 years, without observing recurrence of the lesion.

Siempre es importante ofrecer un tratamiento conservador en este tipo de quistes de gran tamaño, ya que no compromete la integridad mandibular, con posibles complicaciones como es el caso de las fracturas patológicas por el debilitamiento óseo mandibular, además de obtener una formación ósea progresiva y mas organizada con mejor radiopacidad en el espacio ocupado por la lesión. Por lo que la marsupialización es una alternativa de tratamiento para la conservación además de los dientes permanentes.

It is always important to offer a conservative treatment in this type of large cysts, since it does not compromise the mandibular integrity, with possible complications such as pathological fractures due to mandibular bone weakening, in addition to obtaining progressive bone formation and more organized with better radiopacity in the space occupied by the injury. Therefore, marsupialization is an alternative treatment for preservation in addition to permanent teeth.

Resumen

Abstract

Introducción: El mecanismo y la intensidad de la energía que producen las fracturas del tercio medio facial afectan directamente al desplazamiento de los fragmentos óseos en el piso orbitario, en consecuencia, son involucradas diversas complicaciones en tejidos blandos de las estructuras orbitales.

Introduction: The mechanism and intensity of the energy produced by the fractures of the middle facial third directly affect the displacement of the bone fragments in the orbital floor, consequently various complications are involved in the soft tissues of the orbital structures.

Propósito: Presentar la revisión bibliográfica, caso clínico de la sintomatología, parámetros de las diversas clasificaciones, la introducción de nuevas tecnologías de imagen auxiliares de diagnóstico y el manejo de las fracturas del complejo orbito-cigomático.

Purpose: It was the literature review, clinical case of the symptomatology, parameters of the different classifications, the introduction of new auxiliary image technologies for diagnosis and management of fractures of the orbito-zygomatic complex were presented.

Caso clínico: Se presenta el caso de una paciente femenina de 20 años de edad que sufrió un accidente automovilístico, ocasionando fractura del complejo orbito-cigomático del lado derecho: se observan características clínicas, de imagen, técnica quirúrgica empleada y su evolución.

Case: It was the case of a 20-year-old female, who suffers a car accident, causing fracture of the orbito-zygomatic complex on the right side, clinical features, image, surgical technique and its evolution were observed.

Conclusión: Los trazos de fracturas del piso orbitario siguen estrechamente relacionados con los factores traumáticos, la zona anatómica involucrada y los contrafuertes en el maxilar que al permitir la trasmisión de fuerza, se conjugan para la solución de continuidad del tejido óseo. El objetivo fundamental del tratamiento quirúrgico es restituir el daño causado, realizar un adecuado reposicionamiento y fijación de los fragmentos con material de osteosíntesis que permita la estabilidad ósea que refuerce el proceso de cicatrización.

Conclusion: Traces of fractures of the orbital floor are still closely related to the traumatic factors, the anatomical area involved and the flying buttresses in the maxilla that allow the transmission of force, are conjugated for the solution of bone tissue continuity. The fundamental objective of surgical treatment is to restore the damage caused, perform an adequate repositioning and fixation of the fragments with osteosynthesis material that allows bone stability that promotes the healing process.

Introduction

El tercio medio de la cara es una zona que frecuentemente se encuentra expuesta a los traumatismos del macizo facial.

Siendo las fracturas nasales las de mayor incidencia, seguidas de las fracturas de la órbita. Esta situación se ha convertido en un problema de salud pública, afectando en gran medida a la población económicamente activa. Constituyen del 4 al 16% de todas las fracturas faciales. En ésta región facial existen sitios llamados arbotantes y contrafuertes, donde el tejido óseo presenta resistencias de mayor y menor grado, las primeras recogen la presión del golpe protegiendo diversas estructuras anatómicas y está fuerza la transmiten (matrices) que favorecen determinadas áreas para que se produzca una fractura.1 Los arbotantes serán la guía para la fijación rígida. El sistema contrafuerte se divide en verticales y horizontales: los verticales recogen las fuerzas de la masticación y las transmiten al macizo craneofacial para evitar la compresión en las zonas de debilidad; los arborantes horizontales se conectan entre sí y proporcionan soporte a los arborantes verticales. De los Verticales: Naso maxilar, Cigomáticamaxilar y Pterigomaxilar. De los Horizontales: Frontal, Reborde orbitario superior e inferior, Arco cigomático y Reborde alveolar del maxilar.

The middle third of the face is an area that is frequently exposed to trauma of the facial massif. The nasal fractures being the one with the highest incidence followed by the fractures of the orbit. This situation has become a public health problem, greatly affecting the economically active population. Constitute 4 to 16% of all facial fractures. In this facial region there are sites called buttresses and counterfoils, where the bone tissue has greater and lesser resistance, the former collect the pressure of the blow protecting various anatomical structures, and this force transmits it (counterfoils) that favor certain areas so that a fracture occurs.1 The flying buttresses will be the guide for the rigid fixation. The counterfoils system is divided into vertical and horizontal. The verticals collect the forces of mastication and transmit them to the craniofacial massif to avoid compression in areas of weakness. The horizontal buttresses are connected to each other and provide support to the vertical buttresses. Vertical: Maxillary-Nasal, Cigomática-maxilar, Pterigo-maxillary. Horizontal: Frontal, Upper and lower orbital flange, Zygomatic arch and Alveolar ridge of the maxilla.

La órbita está formada por siete huesos que constituyen una unidad funcional, donde los rebordes orbitarios protegen el contenido orbitario al ser fuertes y resistentes, mientras que las paredes caudales e internas al ser frágiles se rompen hacia afuera al recibir el trauma, debido a que está rodeada de cavidades huecas, como el seno maxilar, etmoidal y frontal, tratando de evitar que las estructuras perioculares y oculares sean lesionadas, el diagnóstico y tratamiento oportuno es decisivo para evitar secuelas irreversibles como la pérdida parcial o total de la vista, considerado el traumatismo la primera causa etiológica.

The orbit is formed by seven bones that constitute a functional unit, where the orbital rims protect the orbital content to be strong and resistant, while the walls inferior and internal are fragile to break out, when receiving the trauma, because it is surrounded of hollow cavities, such as the maxillary, ethmoidal and frontal sinus, trying to avoid periocular and ocular structures being injured, the diagnosis and timely treatment is decisive to avoid irreversible sequelae, as the partial or total loss of sight, considered trauma the first etiological cause.

El adecuado tratamiento representa un verdadero reto para el cirujano, debido a la variedad de lesiones que se pueden presentar por lo que cada caso requiere tratamientos individuales.

The adequate treatment represents a real challenge for the surgeon, due to the variety of injuries that can occur, as a result each case requires individualized treatment.

Dentro de las primeras referencias en el tema nos encontramos a Duverney (1751) que reporta dos casos de fracturas orbito-cigomática;2 Lang (1889)2 describe un traumatismo en la pared orbital y su tratamiento; en 1928 Bourquet introduce el abordaje transconjuntival;2 Smith y Regan (1957) acuñaron el nombre de fractura de escape y proponen lo que se conoce como la teoría “hidráulica” para fracturas orbitales;2 en 1971 Tenser modifica el abordaje transconjuntival. Pfeiffer describió en su libro el mecanismo de desplazamiento en fractura orbital interna la cual es la encargada del sostén de la parte posterior del ojo.2 Erling2 (1999) con el apoyo de la tomografía axial computarizada (TAC), menciona que el globo ocular es capaz de desplazar aproximadamente 2.5 cm el ápice orbital y superponer una parte del globo directamente hacia abajo del eje longitudinal de la órbita.2

Among the first references in the subject Duverney (1751) reported two cases of orbito-zygomatic fractures.2 Lang (1889)2 described a trauma in the orbital wall and its treatment. In 1928 Bourquet introduced the transconjunctival approach2 Smith and Regan (1957) coined the name of escape fracture and proposed what is known as the "hydraulic" theory for orbital fractures.2 In 1971 Tenser modified the transconjunctival approach. Pfeiffer described in his book the displacement mechanism in internal orbital fracture which is responsible for the support of the back of the eye2. Erling2 (1999), supported by computerized axial tomography (CAT), mentioned that the eyeball is capable of displacing approximately 2.5 cms the orbital apex and superimposing a part of the globe directly down the longitudinal axis of the orbit.2

Consideraciones Anatómicas

La órbita está compuesta por siete huesos: el frontal, esfenoides, maxilar, lacrimal, etmoides, malar y palatino.3 Posee un piso, una pared medial, un techo, una pared lateral, el ápex y el reborde orbitario.4 Tiene forma de pirámide cuadrangular cuyo eje mayor es oblicuo y está dirigido hacia atrás y adentro, lo que es consecuencia de la mayor extensión de la pared externa, siendo la interna más corta y paralela al plano sagital.3 Se encuentran simétricamente en el tercio medio de la cara de ambos lados de la línea media, separadas una de otra por las paredes laterales y perpendiculares del etmoides, el unguis, y la parte superior de las fosas nasales.

The orbit is composed of seven bones: frontal, sphenoid, maxillary, lacrimal, ethmoid, malar and palatal.3 It has a floor, a medial wall, a roof, a lateral wall, the apex and the orbital rim.4 It has a quadrangular pyramid shape whose major axis is oblique and is directed backwards and inwards, which is a consequence of the greater extension of the external wall, the internal being shorter and parallel to the sagittal plane.3 They are symmetrically found in the middle third of the face of both sides of the midline, separated one from the other by the lateral and perpendicular walls of the ethmoid, the unguis, and the upper part of the nasal passages.

Dentro de las clasificaciones podemos encontrar la Clasificación de Jackson:5 Cigomático-orbitarias, naso-orbito-etmoidales e internas. Puras: Son las fracturas que comprometen cualquier pared del reborde orbital. Impuras: Las que comprometen la pared el reborde orbito-frontal, orbito-cigomático-maxilar, naso-orbito-etmoidal. Internas: Lineales, Blow Out y Complejas.

Clasificación de Manson y cols:4 Se basa en el desplazamiento de las fracturas orbito-malares, evidenciado en la Tomografía axial computarizada (TAC): I. Fracturas de baja energía: Mínimo desplazamiento o ausencia de él. II Fracturas de media energía: Desplazamiento leve ha moderado, conminución variable. III Fracturas de alta energía: Severo desplazamiento, grave conminución en la mayoría de los casos, dentro de estas, encontramos las estructuras panfaciales.

Clasificación de Leipziger y Manson:6 Fractura naso-orbito-etmoidales la cual consiste en Tipo I: Fractura con fragmento único central, en este tipo esta preservada la inserción del ligamento cantal medial. Puede tener diversas características: a) Incompleta

Within the classifications we can find the Classification of Jackson:5 Zygomatic-orbital, naso-orbito-ethmoidal and internal. Pure: They are the fractures that compromise any wall of the orbital rim. Impurities: Those that involve the wall the orbito-frontal rim, orbito-zygomatic-maxilla, naso-orbito-ethmoidal. Internal: Linear, Blow Out and Complex.

Classification of Manson et al:4 Is based on the displacement of the orbito-malar fractures, evidenced in the computerized axial tomography (CAT): I. Low energy fractures: Minimum displacement or absence of it. II Medium energy fractures: Mild displacement has moderate, variable comminution. III Fractures of high energy: Severe displacement, severe comminution in most cases, within these, we find the facial structures.

Classification of Leipziger and Manson:6 Naso-orbito-ethmoidal fractures which consists of Type I: Fracture with central single fragment in this type is preserved insertion

unilateral. b) Completa unilateral. c) Completa bilateral “monobloque”. Tipo II: Fractura con segmento único o conminución externa a la inserción del ligamento cantal medial el mismo que se encuentra adherido a algún fragmento óseo de buen tamaño este puede ser unilateral o bilateral. Tipo III: Fractura con conminución del fragmento central, compromete la inserción del ligamento cantal medial, se encuentra frecuentemente adherido a varios fragmentos óseos muy pequeños, también puede ser unilateral o bilateral.

Clasificación de Zingg:4,7 A: Limitadas a solo uno de los contrafuertes del complejo cigomático. A1: Arco cigomático. A2: Pared lateral de la órbita. A3: Reborde orbitario inferior. B: Incluye a todos los contrafuertes. C: Fracturas complejas con conminución del malar.

Clasificación en base a 3D tomografía computarizada (TC):8 En el 2013 se reportó una nueva clasificación en base a TC. Tomando en cuenta la Imagen de la Sutura Frontocigomática y la zona infraorbitaria: Tipo cizalla: indica una brecha o desviación de la sutura frontocigomática. Tipo tallo verde: ausencia de brecha o desviación de la sutura frontocigomática. Tipo lateral: el hueso cigomático se gira en el maxilar superior. Tipo medial: el hueso cigomático se gira por debajo del maxilar superior.8 Existen también otros mecanismos por los que puede fracturarse la órbita: Blow out: Denominado fractura indirecta o por estallido. Ocurre cuando hay un aumento brusco de la presión hidráulica intraorbitaria. La órbita sufre una descompresión aguda por el estallido de las paredes más débiles. 4 Blow in: Ocurre cuando un fragmento óseo se desplaza hacia el interior de la órbita como consecuencia de traumatismo en regiones contiguas a la órbita.4,9

of the medial canthal ligament. It can have different characteristics: a) Unilateral incomplete. b) Complete unilateral. c) Full bilateral "monoblock". Type II: Fracture with a single segment or external comminution at the insertion of the medial canthal ligament, which is attached to a bone fragment of good size. This may be unilateral or bilateral. Type III: Fracture with comminution of the central fragment, compromises the insertion of the medial canthal ligament, is frequently attached to several very small bone fragments can also be unilateral or bilateral.

Classification of Zingg:4,7 A: Limited to only one of the buttresses of the zygomatic complex. A1: Zygomatic arch. A2: Side wall of the orbit. A3: Lower orbital flange. B: Includes all buttresses. C: Complex fractures with comminution of the malar.

Classification based on 3D computed tomography (CT):8 On 2013, a new classification based on CT was reported.

Taking into account the Image of the Frontozygomatic suture and the infraorbital area: Shear type: indicates a breach or deviation of the frontozygomatic suture. Green stem type: absence of gap or deviation of the frontozygomatic suture. Lateral type: the zygomatic bone is rotated in the maxilla. Medial type: the zygomatic bone is rotated below the maxilla.8 There are also other mechanisms by which the orbit can fracture: Blow out: An indirect or burst fracture. Occurs when there is a sudden increase in intraorbital hydraulic pressure. The orbit undergoes an acute decompression due to the bursting of the weakest walls.4 Blow in: Occurs when a bone fragment moves towards the interior of the orbit as a consequence of trauma in regions adjacent to the orbit.4.9

piso de la órbita. Existen mecanismos por los cuales el contenido orbitario se colapsa cuando hay fracturas y además se observa un ensanchamiento de la cavidad orbitaria. Según sea el mecanismo de la fractura puede haber o no atrapamiento de partes blandas periorbitarias, limitación de los movimientos oculares, enfisema, ptosis del globo ocular, enoftalmos, disminución del volumen orbitario y daños a estructuras orbitales.9,10 Dentro de las consecuencias del traumatismo en la región orbital podemos mencionar:10 Fractura del piso orbitario, incarceración tejidos blandos, hernia intrasinusal, afectación del nervio infraorbitario, sintomatología ocular como trastornos de la estática ocular, trastornos de dinámica ocular, contusión retiniana, neuropatía óptica traumática, hematoma órbita, hipema, lesiones penetrantes, diplopía monocular o binocular, desprendimiento de retina.11 Cuando las fracturas son el resultado de traumatismo directo sobre piso de órbita que es el techo del seno maxilar, el piso desciende con lo cual el globo ocular tiene más espacio para absorber el golpe y sobrevivir al trauma; esto se considera como un mecanismo de defensa.1

biocompatibilidad.14 La tendencia actual es usar mallas preformadas de titanio, para injertos óseos autólogos, debido a su maleabilidad que permite mayor adaptación así como la formación de tejido conectivo alrededor evitando así su emigración. La desventaja potencial es que si se tuviera que retirar, la técnica es muy complicada.14

Case report

Paciente femenino de 20 años de edad que sufrió un accidente automovilístico por exceso de velocidad e ingesta de alcohol, ingresa al hospital el mismo día con antecedentes heredofamiliares y personales patológicos interrogados y negados, con pérdida de la conciencia por algunos minutos. Al ingreso presenta contusión facial intensa, edema periorbitario y dermo escoriación del ojo derecho, así como herida supraciliar de 15 cm, herida a nivel del parpado inferior de 3 cm y herida lineal zona preauricular del mismo lado. Cráneo normocéfalo sin hundimientos, ni exostosis, pupilas normo reflexivas, narinas permeables, cavidad oral hidratada, tórax con campos pulmonares claros, ruidos cardiacos rítmicos, abdomen plano, blando, depresible, no doloroso, sin datos de irritación peritoneal, extremidades integras sin otras alteraciones en el resto del cuerpo. En la exploración física presenta edema cerebral manejado por el neurocirujano, el servicio de maxilofacial observa: movimientos oculares limitados a expensas de atrapamiento del músculo recto inferior, diplopía, hemorragia conjuntival severa, narinas permeables con sensación de obstrucción en narina derecha,

On admission, she presented intense facial contusion, periorbital edema and dermal excoriation of the right eye, as well as a 15 cm supraciliary wound, wound at the level of the lower eyelid of 3 cm. and linear wound preauricular area on the same side. Normocephalic skull without subsidence, nor exostosis, normo reflexive pupils, permeable nostrils, hydrated oral cavity, thorax with clear pulmonary fields, rhythmic heart sounds, flat abdomen, soft, depressible, not painful, without data of peritoneal irritation, integral limbs without other alterations in the rest of the body. edema severo en hemicara derecha, movimientos mandibulares conservados. Presenta tres heridas de tejidos blandos, la primera en la frente zona supraciliar que se extiende a la zona del temporal, la zona parpado inferior 3 cm y preriauricular.

Se observa parálisis facial parcial, por lesión a la rama cervico mandibular del nervio facial, y parestesia en tercio medio de la cara, por lesión a la rama infraorbitaria del nervio trigémino.

Depresión del arco cigomático y reborde infraorbitario del lado derecho (Figura 1 y 2).

En la tomografía axial computarizada (TAC) en corte coronal, se observó fractura del piso orbitario del lado derecho con fragmentos conminutos en seno maxilar y apariencia de hematoma, solución de continuidad en sutura fronto cigomática y cigomática malar (Figura 3).

Diagnóstico: Policontundida, Fractura correspondiente a complejo orbitocigomático y piso orbitario, asociado a fractura maxila, huesos propios de la nariz, presencia de hemoseno del lado derecho.

Plan de tratamiento: Reducción abierta con fijación con placas de osteosíntesis, de 1.5 y 2 mm. Una semana después del accidente.

Fase quirúrgica: Bajo anestesia general, se realiza abordaje por la sutura frontocigomática en cola de ceja, infraorbitaria e intraoral, disección roma por planos hasta el abordaje de los trazos de fractura para reducción y colocación de fijación rígida

of obstruction in the right nostril, severe edema in the right hemiface, preserved jaw movements. It presented three wounds of soft tissues, the first in the forehead supraciliary area that extends to the temporal area, the lower 3 cm and the pre-auricular eyelid. Partial facial paralysis was observed, due to injury to the cervical mandibular branch of the facial nerve, and paresthesia in the middle third of the face, due to injury to the infraorbital branch of the trigeminal nerve. Depression of the zygomatic arch and infraorbital rim of the right side (Figure 1 and 2).

In the computed tomography (CT) of the coronal section, fracture of the orbital floor of the right side was observed with minute fragments in the maxillary sinus and appearance of a hematoma, solution of continuity in the zygomatic frontal suture and malar zygomatic suture (Figure 3).

Diagnosis: politraumatized, with a fracture corresponding to complex orbito-zygomatic and orbital floor, associated with maxilla fracture, bones of the nose, presence of hemosinus on the right side.

Treatment plan: Open reduction with fixation with osteosynthesis plates, 1.5 and 2 mm. A week after the accident.

Surgical phase: Under general anesthesia, approach is made by the frontal zygomatic suture in eyebrow, infraorbital and intraoral suture, blunt dissection by planes

con miniplacas de titanio de 2.00 mm en sutura frontocigomática y miniplaca de 1.5 mm en reborde infraorbitario. Sutura (Figuras 5 y 6).

Fase posquirúrgica: la paciente presentó evolución favorable a los dos años de la cirugía, al no presentar complicaciones.

Discusión Las fracturas del complejo orbito-cigomático ocupa el segundo lugar del tercio medio de la cara, siendo el género masculino el de mayor incidencia,4 en nuestro caso se presentó en el género femenino y con un 30% relacionadas con el alcohol,15 igual que el caso presentado. Existen múltiples controversias acerca de cuál será el mejor tratamiento para la fractura de órbita y piso orbitario, así como el momento adecuado para llevar a cabo el procedimiento quirúrgico, todo dependerá de distintos factores.

Si requiere reducción abierta se plantea 7 a 14 días después del trauma. Esperando poder planificar adecuadamente, así como dar tiempo a la mejora de la inflamación asociada al trauma inicial, ya que en presencia de edema no puede haber una reparación efectiva.14 Olate refiere que las lesiones tratadas 3.5 días después del trauma, evolucionan satisfactoriamente.16 En nuestro caso la cirugía se realizó una semana posterior al trauma, esperando que el edema cerebral estuviera controlado. La prevención de secuelas como diplopía, enoftalmos, disestesia del nervio infraorbitario14 son prioritarios en el plan de tratamiento, en nuestro caso no tuvimos secuelas al respecto, el seguimiento postoperatorio de diversos autores lo llevan a cabo a los 3, 6, y 12 meses,17 nosotros tenemos dos años de control. Las diversas modalidades de tratamiento son difíciles de estandarizar, cada paciente debe ser evaluado indiviualmente.16 Así mismo el tratamiento quirúrgico debe ir encaminado a la recuperación de la función y prevención de complicaciones,18 considerando que el procedimiento empleado aseguré la vascularización del foco de fractura.19

Post-surgical phase: the patient presented a favorable evolution two years after surgery, as there were no complications.

Discussion The fractures of the orbitozygomatic complex occupy the second place of the middle third of the face, being the masculine gender the one of higher incidence,4 in our case it was presented in the female and 30% related to alcohol,15 the same as the case presented. There are many controversies about which will be the best treatment for fracture of orbit and orbital floor, as well as the right time to carry out the surgical procedure, everything will depend on different factors. If it requires open reduction it is posed 7 to 14 days after the trauma. Expecting to plan properly, as well as giving time to improve the inflammation associated with the initial trauma, since in the presence of edema there can be no effective repair.14 Olate reports that lesions treated 3.5 days after trauma evolve satisfactorily.16 In our case, the surgery was performed one week after the trauma, waiting for cerebral edema to be controlled. The prevention of sequelae such as diplopia, enophthalmos, dysesthesia of the infraorbital nerve14 are priorities in the treatment plan, in our case we did not have sequelae, the postoperative follow-up of several authors carried out at 3, 6, and 12 months,17 we have two years of control. The different modalities of treatment are difficult to standardize, each patient must be evaluated individually.16 Likewise, surgical treatment must be aimed at recovering function and preventing complications,18 considering that the procedure employed ensures the vascularization of the fracture site.19

Conclusión Conclusion Las fracturas del piso orbitario siguen estando estrechamente relacionadas con factores traumáticos de diversa etiología. El objetivo fundamental del tratamiento quirúrgico de la fractura de piso orbitario, sea cual sea el tipo, es delimitar dentro de lo posible el daño causado, para llevar a cabo un adecuado reposicionamiento de los fragmentos y así mismo tener una adecuada fijación, con el propósito de mejorar el pronóstico del paciente. Existen diversos métodos para la fijación y reparación de las fracturas de piso orbitario, evidentemente dependerá de la situación de la fractura la elección del material o tipo de terapia que se deberá utilizar.

Las diversas modalidades de tratamiento son difíciles de estandarizar, cada paciente debe ser evaluado en forma individual.

Fractures of the orbital floor continue to be closely related to traumatic factors of different etiology. The fundamental objective of the surgical treatment of the fracture of the orbital floor whatever the type, is to delimit as much as possible the damage caused by, carry out an adequate repositioning of the fragments and likewise have an adequate fixation in order to improve the patient's prognosis.

There are several methods for the fixation and repair of fractures of the orbital floor, obviously depending on the situation of the fracture, the choice of the material or type of therapy that should be used. The different treatment modalities are difficult to standardize, each patient must be evaluated individually.
