STEP 2: Complete the Emergency Information form leaving no blanks and provide a copy of your child’s current immunization records. This information can be turned into the NHS office or your child’s AMPM program. Children may not attend without this information. STEP 3: Staff will process your registration, charge 1st weeks fees and email a receipt for these charges. PAYMENT: A $20 non-refundable registration fee is required for all participants. *See parent handbook for detailed information. NOTE: School offices and AMPM sites can NOT accept registrations. Starfish Swimming Instructor (15+)
The Starfish Swim Instructor Course provides swim instructor candidates the training needed to teach courses in the SAI Water Safety Program. Instructor candidates will learn how to help students develop correct stroke technique, develop an understanding of how to teach students at different levels, develop water safety techniques, and learn how to evaluate participants’ progress. $150 Res. / $175 Non-Res.

Junior Lifeguard Course (13-15) This course is a requirement for those wishing to be City of Peoria Junior Lifeguards during the season, including returning Junior Lifeguards. Aquatics Programs Swim Team & Dive Team Registrations are first come, first serve and teams do fill up quickly. You will need your username and password to register. To obtain this information, please call (623) 773-7137. The City of Peoria offers four recreational swim teams: Sunrise Red Sharks, Sunrise Green Sharks, Centennial Stingrays, Peoria Piranhas and three recreational dive teams: Sunrise Marlins, Sunrise Gators, and Peoria Dolphins. Practices are held Mon. - Thur., May 25 - July 17, with meets typically once a week. Fees for Swim Team $90 Res. / $150 Non-Res. Fees for Dive Team $60 Res. / $70 Non-Res. Tues., April 21, 7:30 a.m. - 5:30 p.m.: Peoria residents returning to their same team from 2019 may register in-person at the Development and Community Services Building (9875 N. 85th Ave.). Registrations will not be taken for any new swimmers/divers or at any other location on this day. Your spot on the team is guaranteed that day, but practice times are first come, first serve. Mandatory Coaches Meeting on Sat., Feb. 1, 2020 at RVRC; 9 a.m. Team Formation Teams are formed based on the number of registrations received. There’s absolutely no guarantee of placement based on requests provided at the time of registration. No exceptions due to the high volume of requests provided. We are unable to switch children to a different team once they have been placed due to the many moving parts and large amount of requests received. Participants will be contacted by their volunteer coach after the pre-season coaches meeting on Feb. 1, 2020. Practices/Games All teams will be provided one, one-hour practice per week Mon.-Thur. The day of the practice will be chosen by the volunteer coach. Practices will start the week of Feb. 10. All games will be on Sat. starting Feb. 22. Game times and locations will be determined based on
venue availability and total registration numbers. Age Groups (Playing Up) Please make sure you are registering for the correct age / gender / division for your child. Participants may not play in a younger age group than what they are eligible for. Participants may play up an age division if they meet the following criteria: 1. Meet the minimum age requirements for the youngest age division for the sport. 2. A re less than 12 months from the age cut-off for the age division they want to play. (Ex. An 8 year old may register for the 9 & 10 division.) The age cut-off date for the 2020 Spring season is Sat., Feb. 22 (1st day of games).

DES/DDD FUNDED PROGRAMS
The City of Peoria Adaptive Recreation Program contracts with the Department of Economic Security, Division of Developmental Disabilities, to offer a variety of Day Treatment and Training Services. All programs are staffed on a 1:4 staff-to-participant ratio, so a moderate-to-high level of independence is required. After School Recreation Program provides age appropriate activities for students 12-17 and 18-21 years of age. Program days follow the Peoria Unified School District calendar. Adult Day Recreation Program is designed to provide unique and challenging activities in a small group setting. To inquire about openings at our two sites, contact Jodi at (623) 773-7436. Elementary Summer Recreation Program is an 8-week program for individuals 6-11 years of age. This program runs 8 a.m. - 3 p.m. five days a week as an integrated program in conjunction with the City of Peoria’s Summer Recreation Program. This program uses DTS hours. High School Summer Recreation Program is an 8 week program for individuals 12-17 and 18-21 years of age. This program runs 8 a.m. - 2 p.m. five days a week as a self-contained program and uses a combination of DTS and RSP hours. These programs are first come first serve so call (623) 773-7436 and ask for Jodi or Luke to request an application. Make sure you ask your support coordinator for DTS paperwork for the summer!

• All bulky items should be placed in the gutter/curb no later than 6:00 am of the zone collection date. Please do not place bulky items out more than one day in advance of zone collectioin date. Bulky items placed out after a street has been serviced or exceed the maximum collection size (approx. 6’ long x 4’ wide x 3’ feet tall) will incur a charge of $50.00 per 30 minutes of loading time. • Neatly place bulk trash alongside the curb in front of your property. Do not place items on your property or the sidewalk. Please do not combine bulk trash into one large community pile. Service will not be provided if bulk trash is improperly placed, deemed unsafe to collect, or contains unacceptable items. • Loose yard clippings and pet waste must be bagged and securely tied to be collected. • Cut tree limbs, branches and wood panels into 3' sections and stack in the same direction. • Place cacti pieces in a sturdy box. • Remove doors from appliances or secure with heavy tape.

Welcome to Lake Pleasant Regional Park. One of the most scenic water recreation areas in the “Valley of the Sun,” this northwest Valley park is a recreationist’s dream! If you are not completely satisfied with the activities and programs offered, we will assist you in the following ways:
Transfer you to another activity or class immediately. Credit your account in the computer immediately, which then can
be used by any family member for any recreation program in the future.
Refund your money. If you paid by credit or debit card, the amount
will be credited to your account. There will be no cash refunds. If you paid by cash or check, refunds take approximately three to four weeks to process and will be mailed to the address on the account. Refund checks will be made payable to the head of the household unless otherwise directed.
Class General Policies
• Assume you are registered and attend the first class. • NO confirmations will be sent. • Other than Parent Tot classes, parents remain outside the classroom. • Non-registered individuals are not allowed to attend classes.
Active Adults General Policies
Registration for trips is on a first-come, first-served basis. PAYMENT IS DUE AT TIME OF REGISTRATION. Times and, occasionally prices, are subject to change. Cancellations: For events where a ticket has been purchased or space booked from an outside agency (theater, travel company, etc.), we cannot issue a full refund, we can only try to resell your ticket, or you may sell it yourself. In some cases, a partial refund will be mailed to you in approximately 3 - 4 weeks. Transportation is by school bus, city van or charter bus. All trips require a minimum number of registrations. Arrive at least 15 minutes before departure time.
NO refunds, credits or transfers will be issued after the program or session has concluded. Refunds will be given for registration fees only.

Resident Rate
If where you permanently reside or live is located within the corporate limits of the City of Peoria and you pay property taxes to the City, you are eligible for the resident rate. Proof of residency is required at the time of registration. Acceptable proof is a copy of your property tax statement showing the physical location of your residence or we can look this up at www.maricopa.gov.

The City of Peoria Community Services Department strives to make its programs, services and facilities accessible to individuals with disabilities.
