Understanding Menopause

An Interview with Jacqueline Linder, MS, CNM

JJacqueline Linder, MS, CNM, specializes in perimenopause and menopause at HerMD in Millburn. HerMD is a physician founded, female-led women’s healthcare company that focuses on f the unique health issues of women from puberty through menopause.

What is menopause?

We can think of menopause as being natural or induced. Natural menopause occurs after a woman’s menstrual cycle has stopped for 12 months, in the absence of other, underlying causes. Induced menopause occurs when the menstrual cycle stops due to surgical removal of the ovaries, effects of chemotherapy or radiation, or use of certain medications.

In the United States, the average age of menopause is 51, although some women enter menopause earlier and some later. A natural menopause diagnosis will typically be made based upon a women’s unique menstrual cycle history, although certain hormone levels, such as follicle-stimulating hormone (FSH), may be evaluated to confirm menopause. The time leading up to menopause—when the body makes a gradual transition to menopause—is known as perimenopause. Perimenopause often precedes menopause by 10 years, although women experience perimenopause at different ages. In fact, perimenopause can occur as early as a woman’s mid-thirties. During perimenopause patients will typically experience menstrual cycle irregularity and various menopause-like symptoms.

Historically, women have been misled about menopause. Despite spending approximately 40% of her life in some

type of menopausal state, lack of education, funding, gender bias, stigma, and taboo remain pervasive barriers to optimal menopause care. From a healthcare provider perspective, we receive very little training in care of the menopause patient, and as a result, women often experience dismissals and delays in receiving effective treatment. The need for formalized menopause education remains critical for optimal care of the menopause patient, and HerMD is addressing this gap by offering real-world menopause education solutions.

Why is discussing menopause often taboo?

As a society, we have developed a high tolerance for women’s pain and discomfort. We view pain as a normal process in a woman’s life. Patients may feel discomfort in talking about bothersome symptoms that occur from menopause.

The good news is that the tide is now changing. With increased education and support, women are becoming emboldened and empowered to advocate for their health needs. They are demanding acknowledgement and care for their menopausal symptoms. Women will continue to use their voices to demand change until the tide completely turns in menopause care.

What is your role as a Midwife?

My role as a midwife is to assess and diagnose menopause. I prescribe therapies that alleviate bothersome symptoms. Each treatment should be individualized, and your care provider should explain your best options. If you are deemed an appropriate candidate for treatment with hormone therapy, your provider will review with you the optimal type, route, and dose of hormone therapy specific to your needs. Additionally, non-hormonal therapies and lifestyle modifications are often incorporated into an individual’s care plan to optimize health.

Midwives treat the whole person—the biological— the actual physical symptoms. However, we also consider the psychological—the emotions the patient experiences— and the sociological—the effect of the woman’s environment. Sometimes, we recommend cognitive therapy.

What advice do you give to perimenopausal and menopausal women?

You do not have to suffer. Find someone willing to talk to you. At HerMD, we evaluate, diagnose, and treat perimenopausal and menopausal symptoms. Many patients tell me they feel heard and hopeful for the first time in seeking treatment.

HHappy anniversary, Livingston Gym! Last May, Jackie and Pasquale Ranalli, a couple with a deep passion for gymnastics and children’s development, purchased the gym. Their mission was clear-to establish a safe, learning, and fun environment for children ages nine months through 12 years. Jackie and Pasquale, parents themselves, found purpose in teaching young children the joy of movement through gymnastics in individual and group activities.

Pasquale, a former software engineer who trained in martial arts, and Jackie, a registered dietician and soccer player since age 3, bring much experience working with young children. In addition, the Ranallis understand youthful energy and how to channel it. Their four children are nine and younger and participate with other community children in the classes offered.

Children have a wide selection of exciting and fun classes tailored to their age and skill level. For the youngest ones, under three with a parent or guardian, there are music and sensory activities through movement. Slightly older toddlers get to tumble and roll in engaging tumbling activities, developing their coordination and balance in a fun and interactive way.

Children ages 3 through 12 learn gymnastics sequentially. The 3- to 4-yearold class introduces independence and enhances the fundamental elements of gymnastics; the Aspiring Gymnastics class includes children ages 4 to 6, and children aged 6 through 12 build skill and technique as they move through specific levels. There is also an “open gym” time for parents and children. During these two-hour sessions, parents and children have free play time together.

New this year is the Ninja Fitness class, a unique offering at Livingston Gym. This gymnastics-based class incorporates martial arts workout and safety techniques, focusing more on obstacle courses and functional movement. It perfectly combines physical activity and skill development to keep your child engaged.

At Livingston Gym, children don’t just learn gymnastics, they acquire life-long values. PPilates is for everybody. Exercise enthusiasts aged 16 through 80+ enjoy a workout tailored to their physical capabilities at Club Pilates. The Livingston studio franchise is part of the 1,000 Club Pilates nationwide.

Members begin their initial workouts on the Reformer. This is the the primary apparatus used to accomplish a complete workout using small movements and repetitions. “The workout is very focused and emphasizes form in doing the exercises correctly,” adds Josh Katsnelson, an owner. As they progress, students use other apparatuses to hone their workouts.

Club Pilates has ten certified instructors, offering clients group classes or private lessons. Open from weekdays 6am-9pm and weekends 8am-2:30pm, customers can register online daily for the classes they wish to take. Special courses include cardio segments added to the Pilates flow. All classes are 50 minutes with a ten-minute break.

“Everyone can make gains through Pilates,” says Katsnelson. “We help people become more in tune with their bodies.”

LLocated in the hub of Livingston, Mezza, a MoroccanTunisian-Israeli-style restaurant with a hint of Greek and Italian influence, offers a unique blend of Mediterranean cuisine with a focus on quality, service, and ambiance. Specialties include the Braised Short Rib Papardelle with Parmigiano Reggiano, grape tomatoes, peas, shiitake mushrooms, shallots, and truffle oil, and braised short rib ragu.

Another favorite is the Braised Colorado Lamb Shank served over creamy orzo and spinach, topped with homemade lamb jus. Seafood lovers enjoy the sautéed Bronzino with tomato, spinach, capers, olives, fingerling potatoes, sherry wine, and lemon, while steak enthusiasts savor the skirt steak.

For appetizers, Mezza recommends the Crispy Cauliflower, lightly fried and topped with raisins, scallions, and tahini. The Persian Shrimp is another delight, featuring lime, zaatar, aromatic herbs, yogurt, turnips, and pomegranate. The Mediterranean Fish Tacos are a must-try, with marinated branzino, Israeli salad, Jerusalem spiced yogurt, and dukkah served on toasted pita.

For dessert, indulge in the Pistachio Baklava made from phyllo dough, walnuts, pistachio cream, and pistachio gelato, or the Chocolate Soufflé served with Chambord cream.
