s the holiday season is now upon us, I would like to wish you and your family a very Merry Christmas, Happy Hanukah and Happy New Year. As you may have noticed, Aging Tree Illustrated has changed its name. Our goal has always been to provide educational content that supports you to help make better decisions. Well, now we have taken things a step further. We are continuing to provide educational information in our articles but in addition, we are now becoming Aging Tree Illustrated’s "Legacy" Magazine and for good reason. There is no better time of year than the holidays and the New Year, to contemplate good fortune, great love, and hosts of family traditions and memories. Which made me ask myself, what’s in a legacy? Sure, it’s the things we inherit such as money and heirlooms. However, what we are truly capable of ensuring we forever leave behind? An everlasting legacy is one in which an idea, concept, or tradition is passed from one generation to the next. A good legacy leaves a family proud and eager to enlist younger generations to the good fortune of knowing something familiar or a sense of pride. After all, our legacy is truly the only part of us we leave behind. It is our mark here on this planet. It is our stamp on the universe that says, “I was here” and “I had purpose”. Perhaps it says, “I have loved with all of my heart”, “I have done my best”, or “I have been a faithful servant”. All-in –all, our personal legacy truly gives meaning to our lives. What's in Your Medicine Cabinet?

Part 3: Vitamins and Supplements By Cheree M. Albert MSN, ARNP, CPNP

In this last part of our three-part series titled “What’s in Your Medicine Cabinet?” We're talking about Vitamins and Supplements. Check out the October and November issues in this series: 6 Concerns for Your Prescribed Meds and Over the Counter Medications as well as the companion articles in the Living Well Branch for more information on this important topic.

week does not go by without one of my clients asking if there is a supplement they can take for their symptoms. Perhaps you have wondered about this too, since it seems there is something for everything. An estimated more than one-third of all Americans take a multivitamin supplement (MVM). This is big business as sales of dietary supplements in 2014 totaled an estimated $36.7 billion ($14.3 billion-MVM containing). Given the increasing popularity and marketing strategies attached to this growing trend let’s have a conversation about common questions surrounding quality, safety and efficacy. What is a dietary supplement? The Dietary Supplement and Education Act of 1994 considers supplements to include herbals (botanical products), complementary nutritionals (ex. Amino acids) and micronutrients (vitamins and minerals). Homeopathic products or topically administered herbals are often used by consumers for complementary health but do not

fall under the regulatory definition of dietary supplements. Who oversees the quality, safety and efficacy of these supplements? The Food and Drug Administration (FDA) has oversight of dietary supplements though the regulatory framework differs from that for prescription or overthe-counter drugs. Manufacturers of dietary supplements containing products that were introduced after October 15, 1994 are required to notify the FDA before marketing and to provide a rationale for the safety of the ingredients often historical use. What is often surprising is that neither safety testing nor FDA approval is required before marketing. You may be familiar with the disclaimer present on supplement labels which states “These statements have not been evaluated by the Food and Drug Administration. This product is not intended to diagnose, treat, cure or prevent any disease”. This is consistent with the National Institute of Health fact sheet updated on July 8, 2015 that states that multivitamins do not appear to reduce overall chronic disease risk

though several nutrients included might benefit certain population groups. With no requirement for safety testing or FDA approval how can I tell if a supplement is safe? If a dietary supplement is found to be unsafe the FDA can have the manufacturer remove the product from the market. There is criticism that the laws governing recall are not strong enough. A study of product recalls published in 2013 in the Journal of the American Medical Association found that 274 supplements recalled by the FDA between 2009 and 2012 contained banned drugs. A 2014 report found that more than two thirds of the supplements purchased six months after the recall still contained the banned drugs. Post market reporting of adverse events is required for only the most serious events including those resulting in hospitalization, significant disability or death. Unlike prescription and over the counter medications there are no requirements to identify adverse events on the dietary supplement packaging. RDAs, AIs and DVs: what do they mean and how do I know what is the right daily amount for each vitamin or mineral? food. The Institute of Medicine (IOM) of the National Academy of Sciences set the Recommended Daily Allowances (RDAs) for selected nutrients and where no RDA is established the IOM has developed Adequate Intake levels (AIs). The Daily Values (DVs) are set by the FDA and were updated in July of 2016. The DVs may not still reflect the latest intake recommendations by the IOM as the large manufacturers and small manufacturers have been given until January 1 of 2020 and 2021, respectively to update their labels with the new DVs. Is it possible to get too much of a nutrient? Yes, which is why the Upper Tolerable Intake Levels (ULs) were set by the IOM. This means that there is an increased risk for adverse events when the UL is exceeded. There are times when the UL may be exceeded based on the specific medical

treatment needs supervised by a health care professional. This is an important area for public education as the “more is better” belief may be applied to vitamins and minerals. Thoughts that “the excess will just be excreted” assumes that kidney function will be sufficient. Recently upon review of my client’s vitamin C intake we discovered an excess well above the UL of 2000mg. Her dietary intake from food was more than sufficient with an RDA of 75 mg for her age, though food sources of vitamins do not cause the adverse effects experienced with lab created compounds. These side effects can include nausea, vomiting, heartburn, inflammation that might damage the esophagus (esophagitis), a blockage that keeps food or liquid from passing through your small intestine or large intestine (intestinal obstruction), stomach cramps, fatigue, headache, sleepiness, diarrhea, insomnia and skin flushing. In some people, oral use of vitamin C can cause kidney stones. nationally representative sample of emergency departments from 2004 through 2013 revealed that dietary supplements were implicated in an average of 23,000 emergency department visits and 2000 hospitalizations annually. Children, millennials (ages 20-34) and seniors are reportedly most at risk with seniors most likely to be hospitalized. Child resistant packaging is not required for dietary supplements except for iron, though it was iron supplements that scored second in unsupervised ingestion by children. Weight loss supplements were most implicated in the 20-34 age group. Swallowing issues were implicated in many of the seniors. What can I do to limit unwanted side effects? Even though this is an unregulated industry you can make safe and effective decisions related to your choices for supplements. As always communicate with health care professionals about what you are taking and the need for additional supplements. Here is a partial list of what I teach my clients to consider when adding a supplement to their plan for health: 1. Is the product NSF® Certified and/or Certified for Sport™? This voluntary certification assures that there are no undeclared ingredients in the product that are not listed on the label. Is there additional quality assurance testing to assure that there are no contaminants or bacteria? How frequently is that testing done? Know the limitations of the organic labeling. Since contaminants can be introduced at any stage of the growing and manufacturing practice I feel most confident with end point testing to assure quality and purity. 2. Does it contain substances that have no proven benefit for example, fillers or dyes? |9|


3. Are there mega doses of selected ingredients above the Upper Tolerable Intake Levels (ULs)? 4. Does the supplement contain "whole foods" or food concentrates which are the closest to the nutrients from foods in nature? Does it have a food label or a supplement label? 5. Does the manufacturer make fantastic claims about their product? If you hear or see the words "miracle," "cure," "ancient remedy," "secret formula," and testimonials about diseases that were cured take caution. It is one thing for an individual to share their personal experience with a supplement and quite another to make testimonials in direct contradiction to the disclaimer present on the label. 6. Ask to see the research. Primary research, conducted on people who are taking the same product that is in the bottle you have purchased, is not required by the FDA or other organizations like is required for prescription pharmaceuticals. I always ask to review this type of research on any vitamins and supplements I recommend. Surprisingly there may be very little, if any documentation of quality or efficacy. There may be borrowed research, for example a product contains vitamin C so the claim is that their product will perform the same as referenced in the research, though none was conducted on their product. Where the research is published is also important. Independent, third party, double blind placebo control research, published in peer reviewed medical journals vs private publications, provides the framework to ensure that the data is credible and collected without bias. The Journal of the American College of Cardiology, Journal of Clinical Periodontology, Medicine and Science in Sports and Exercise and the Journal of the American College of Nutrition are some examples. According to the 2015-2020 Dietary Guidelines for Americans, “nutritional needs should be met primarily from foods. Foods in nutrient-dense forms contain essential vitamins and minerals and also dietary fiber and other naturally occurring substances that may have positive health effects”. Vitamins and mineral supplements can play an important role in meeting nutritional requirements when a medical condition requires more than can be met with food alone. Become an informed and empowered consumer when this is the case to ensure that you are receiving optimal benefit from this investment in your health.

The aromatic method is commonly used and a good introduction to essential oils. The delivery method could be via a diffusing device or a few drops in the palm or on a cotton ball that is placed near the nose and mouth in order to inhale the scent.

Clark explains essential oils can be applied to the feet, ears, chest, back of neck, or other troubled areas. To avoid skin sensitivity, the essential oil is mixed with a “carrier oil” such as coconut oil. A few drops of peppermint essential oil, for example, can be added to a carrier oil, then massaged into achy joints.

While the benefits are questioned and still being researched, Clark says, certain high-quality, pure essential oils can be ingested. One drop of oil in a glass of water is believed to cleanse and purify the body. Do you get discouraged when you hear your telephone ring? Do you avoid using your phone because hearing difficulties make it hard to understand the person on the other end of the line? For many Americans the telephone conversation – once an important part of everyday life – has become a thing of the past. Because they can’t understand what is said to them on the phone, they’re often cut off from friends, family, doctors and caregivers. Sourcing & Potency. The sourcing of the plant matters when it comes to how effective the oil will be. Some companies do set and self-impose high standards. doTERRA, says Clark, has third-party tested and trademarked its essential oils as “certified pure therapeutic grade.” Clark says it’s also important to become self aware and notice patterns during use. “You can’t just make a blanket statement that lavender essential oil [used aromatically] is good for everyone with anxiety,” she explains. “That’s a starting point, but you need to allow for your own body’s interpretation in regard to the types of oils and proportions.” Clark’s father, who is in his 80s and suffers with COPD and asthma, topically applies a doTERRA essential oil “respiratory blend” designed to open airways. He applies this blend directly to his chest as needed throughout day. In addition, he uses a pain relief blend on his legs to alleviate cramps and frankincense and cypress essential oils on the bottoms of his feet to encourage circulation. Finally, he puts a drop of frankincense under the tongue to enhance brain health. Most essential in the use of essential oils? Clark says, educate yourself!

This will help you become more educated in the use of essential oils. 3. Essential oils, also referred to as plant-based medicine, is powerful, says Clark, but know that this is just one part of a holistic lifestyle. If you’re still eating cheeseburgers every day, even the best essential oil in the world will be limited in how it can help you. 4. s we approach the end of another year, I would like to review some critical issues we have covered this year. ONE – Please remember Microsoft (or any other company associated with them) WILL NEVER

COLD CALL YOU! If you receive a call about your computer running slow or it needs an update – HANG UP THE PHONE IMMEDIATELY – it’s a ‘SCAM’, plain and simple. Reputable hardware and software companies simply do not make unsolicited phone calls to check your computer! I have heard about this happening 5 times in the last year and the victim(s) who fell for this scam lost an average of $375.00! TWO – If you think your computer needs work, get a second estimate! It may be inconvenient, but you can save hundreds of dollars. You do not necessarily need a new computer if you believe yours is not operating ‘up to par’. All you may really need is a good, deep and thorough cleaning. Computers that are no more than 7 years old can be brought back to a reliable performance level with a proper

cleaning. To be honest, we seniors may not need all the ‘bells and whistles’ that a new system may have, but, we may want them. THREE – If you are not paying for virus protection, your computer may not be protected! Even though some virus programs offer to scan your system for free, they may not be doing a respectable job – they scan at a very high level and may not detect a potential problem. Also keep in mind, ‘malware and viruses’ are NOT THE SAME. Make sure you have a tool that detects both. Some are free, some are a little more expensive than others, but which ever one you choose, make sure you check to see if that software addresses both ‘malware’ and ‘viruses’. FOUR – If you are looking for computer training, ask what the

Whichever holiday you celebrate, most enjoy showing their affection by giving gifts to loved ones. For larger families, these gifts can amount to a lot of money each year. And that’s wonderful, but if you might need to apply for Medicaid long-term care benefits, you need to be careful. Giving away money or property can jeopardize your eligibility. Here’s why you need to speak with an experienced elder care/elder law attorney about gifting. If you give assets away to someone other than your spouse with-

in five years before applying for long-term Medicaid, you might be ineligible for benefits. Medicaid pays for some or all your care at home, in an Assisted Living Community, or in a Nursing Home. The length of time you’ll be ineligible depends upon how much you give away. Even small gifts affect eligibility. The 2017 IRS rules allow gifts up to $14,000 a year, but Medicaid rules allow the government to deny benefits anyway. And there is no exception for gifts to charities. So, gifts for holidays, weddings, birthdays, and graduations could all cause ineligibility. If you buy something for a friend or relative, this could also result in a denial. If you face this problem, you can overcome it, but you’ll need help. To overcome a denial, you’ll

bout a month or so ago I wrote an article just hours before I departed on my journey to South Africa. This month, I am writing this column in just under 16 hours before I take my Annual Thanksgiving Cruise. I literally cannot wait to go on this cruise! I have a group of 70 going with me from Volusia County (primarily) and we are splitting between two buses to make our way to Miami tomorrow. I am often asked why I travel for the holidays, the answer is simple: All of the enjoyment, none of the mess! You see, I am the cook in my family. I love to make a huge family dinner and have everyone over. But my house never looks the same after everyone leaves. Social Security's Gift to Children is Security During the holiday season, most of us, regardless of religion or beliefs, focus on the children we love. Caring for children is one of the best ways to safeguard the future. And we at Social Security know a thing or two about helping children. The application for a Social Security number and card is sometimes overlooked in the paperwork that parents fill out in preparation for a child’s birth. Typically, the hospital will ask new mothers if they want to apply for a Social Security number for their newborn as part of the birth registration process. This is the easiest and fastest way to apply. The Social Security card typically arrives about a week to ten days after that little bundle of joy! You can learn about Social Security numbers for children by reading our publication, Social Security Numbers for Children, available at www.socialsecurity. gov/pubs. A child needs a Social Security number if he or she is going to have a bank account, if a relative is buying savings bonds for the child, if the child will have medical coverage, or if the child will receive government services. You’ll also need a Social Security

number for a child to claim him or her on your tax returns. If you wait to apply, you will have to visit a Social Security office and you’ll need to: • Complete an Application for a Social Security Card (Form SS-5); • Show us original documents proving your child’s U.S. citizenship, age, and identity; and • Show us documents proving your identity. A child age 12 or older requesting an original Social Security number must appear in person for the interview, even though a parent or guardian will sign the application on the child’s behalf. Children with disabilities are among our most vulnerable citizens. Social Security is dedicated to helping those with qualifying disabilities and their families through the Supplemental Security Income (SSI) program. To qualify for SSI: • The child must have a physical or mental condition, or a combination of conditions, resulting in “marked and severe functional limitations.” This means that the condition(s) must severely limit your child’s activities;

one are the halcyon days of skipping down to the local drug store to pick up your prescription and a chocolate malt. Not only are we hard pressed to find a drug store where you can still get a malted

milkshake, but you need to pass a fairly comprehensive background check just to get nasal spray these days! For those participating in VA healthcare, the pharmacy at the Daytona Beach VA outpatient clinic can be a helpful benefit, but still, sometimes we don’t always have transportation or a lot of time to wait. For many years, local military retirees have traveled to nearby military installations such as Jacksonville Naval Air Station and Patrick Air Force Base to have pre-

scriptions filled. While this may have been fun for a while, as we get older we tend to be less willing to travel great distances. Fortunately for many veterans, there’s an alternative. Prescriptions right to your door or mailbox! Did you know that VA, CHAMPVA and TRICARE participants can all get their prescriptions delivered? It’s easy to get started and saves many people much time and effort. While there may be a small co-pay for certain types of delivered medications, the cost is negligible in comparison to driving from our region to one of Florida’s military bases or waiting an hour or more at a clinic to accomplish the same thing. Receiving medications by mail helps the veteran and the VA. Not only can a veteran spend their time on more leisurely tasks, but VA facilities benefit from reduced patient levels, increased parking, less sensitive inventory to control and for those veterans that continue to use the pharmacy services at the VA clinic, the wait times become much shorter due to decreased demand at those locations.

Italian Bow Knot with Sprinkles Cookies Each year on Cookie Day, we make these cookies. We can thank Aunt Anita for this recipe. They are a favorite because they are not too sweet and not your run of the mill cookie. They take a little time but are so worth it! You will need a surface to roll the dough on. I have my mother's maple cutting board and it is such fun to use and think of her. You will need an heavy duty mixer for this. I use my kitchen-aid with a dough hook.
