eceiving a diagnosis of Alzheimer’s is the beginning of a new, likely frightening, journey, but when you’re armed with information and resources, it’s a little

In this, the third installment of a three-article series on Alzheimer’s disease and dementia, Aging Tree Illustrated is exploring the research, medications (pharmaceutical and nutraceutical) and support available today that may ease the symptoms of progressive brain diseases and provide comfort to patients.

Though research continued in the early 1900s with the invention of the electron microscope and the development of cognitive measurement scales, the founding of the National Institute on Aging in 1974 propelled Alzheimer’s disease into the spotlight. Two years later, it was recognized as the most common form of dementia and a significant public health challenge. Research continued, and in 1984, researchers officially named beta-amyloid as a “prime suspect” in triggering nervecell damage that leads to Alzheimer’s (see the previous articles on agingtree.com for more information on causes of dementia). Treatments began emerging in the early 1990s, and 20 years later in 2010, The Alzheimer’s Association, in partnership with

Alzheimer’s disease research today

For decades, people have helped researchers and sponsoring pharmaceutical companies continue to develop medications that alleviate symptoms or reverse disease. The same need exists today for the many Alzheimer’s disease research studies being conducted in clinics across America, including right here in Volusia County. Qualified participants in medical trials typically receive free medication and healthcare as related to the study, compensation cont. pg 9

for time and travel, and are not required to have health insurance to participate. Although Alzheimer’s and progressive dementia cannot be reversed, some medications can slow their advancement. Clinical trials help scientists learn more about how the brain changes in healthy adults and those experiencing dementia. Studies require various ages, sexes, races, ethnicities and stages of disease to produce meaningful results. If you or a loved one is interested in participating in a clinical trial, talk to your healthcare provider. Researchers are constantly investigating the cause, diagnosis and management of dementia, and results are reported to the Food and Drug Administration through each drug’s various phases of study. Progressive Medical Research (PMR) in Port Orange recruits participants for a number of such studies. The clinic is currently searching for participants who have not yet been diagnosed with Alzheimer’s disease, but are showing mild cognitive impairment. A second trial underway at PMR is seeking patients with a mild Alzheimer’s diagnosis. One study uses an inhaled medication, while another uses an intravenous therapy. Medical Research Group of Central Florida, which has locations in Orange City and Sanford, also conducts drug trials for Alzheimer’s disease. The clinic asks prospective participants to contact the research team to schedule an initial evaluation. Pharmaceuticals (prescription medications)

Unfortunately, no medication on the market can stop dementia’s progression altogether. However, several drugs are available today to help ease symptoms or slightly slow the diseases’ advancement. Dr. J. Roger Accardi, president and owner of Accardi Clinical Pharmacy in Orange City, is a third-generation pharmacist who’s well versed in pharmaceutical and non-pharmaceutical options for treating dementia symptoms. He says treatment for mild to moderate Alzheimer’s disease is available in drugs known as cholinesterase inhibitors. The most commonly prescribed proprietary formulas for slowing the symptoms of Alzheimer’s are Aricept (generic: donepezil), Razadyne (generic: galantamine), and Exelon (generic: rivastigmine). According to the National Institute of Aging, cholinesterase inhibitors work to treat the disease by preventing the break-

down of acetylcholine, a brain chemical believed to be important to thinking and memory. A fourth medication in the marketplace, Namenda (generic: memantine), has shown some levels of success in slowing disease progression as well when used in collaboration with other pharmaceuticals, says Accardi. Additional medications are in various phases of study now, and may become in the future, says Accardi. Among them is Solanezumab, which works by a different mechanism than other dementia drugs available today, and is in phase three of testing. But, Dr. Accardi warns that though medications to treat symptoms of Alzheimer’s disease can benefit patients to a degree, they also can have significant side effects. “If the disease is progressing at a 10, the presence of these medications may slow it to a 7,” he says, adding their therapeutic values are not phenomenal, and most patients experience significant side-effects from the meds. Such side effects include nightmares, insomnia, and vomiting. And, as with many medications, higher dosages can create more significant reactions. cont. pg 10

Nutraceuticals (food-based supplements)

Nutraceuticals are foods that contain health-giving additives and have medicinal benefits. Dr. Accardi says testimonials reveal some nutraceuticals have been met with moderate success in Alzheimer’s patients, and lists ginkgo biloba, omega 3, vitamin E, Huperzine A, and Phosphatidylserine among them. He says that though these are available “over the counter,” patients should not assume nutraceuticals are infinitely safe. “Nutraceuticals work on a cellular level; they are chemicals,” says Dr. Accardi, “And, if you can purchase them over the counter, there’s a likelihood there’s not been extensive research on the formula. Research is expensive.” In short, always consult your doctor or pharmacist about all drug side-effects and interactions, no matter whether they require a prescription.

Beyond medication and supplements

Though the drugs listed above can provide patients with some comfort, Dr. Accardi suggests families also investigate several non-pharmaceutical and non-nutraceutical interventions to help Alzheimer’s patients enjoy the highest quality of life possible. “Focus on what brings them happiness and joy,” he says, “Help them deflect their frustration from the disease by assisting them in remembering what makes them happy.”

Dr. Accardi’s suggestions include: • Aromatherapy – Researchers have found aromatherapy to be an effective non-pharmacological therapy for dementia. It may have some potential for improving cognitive function, especially in Alzheimer’s disease patients. • Massage – Studies have found hand massage can reduce verbal agitation in Alzheimer’s patients, and the benefit can be sustained for up to one hour afterward. Further, researchers believe massage therapy also can help ease the effects of isolation, loneliness

and boredom, while encouraging feelings of worthiness and well-being. • Music therapy - Numerous studies have indicated the value of music therapy in the management of patients with Alzheimer’s disease, too. Music therapy can reduce anxiety and depression in patients with mild to moderate dementia. Dr. Accardi adds that there’s one very important aspect to caring for Alzheimer’s and dementia patients that won’t be tested in any drug trial anywhere: be kind.

Help them allay their fears and anxiety.” Adds Accardi: “The nature of the disease is gloom, so bring hope whenever you can.”

Support groups

The Alzheimer’s Association offers various methods of support to those who have been diagnosed with dementia or have loved ones who are in its grip. Online, the Alzheimer’s Association offers message boards and chat rooms that serve as forums for those affected by dementia to connect with others who can offer support, advice and a listening ear. The Alzheimer’s Association also organizes peer-led or professionally-led groups for caregivers and patients, and the groups are always g u i d e d by those trained to offer comfort and who understand the disease. mer’s Support Group offers support and resources for people who care for or know someone suffering from Alzheimer’s disease or dementia. The groups meets every third Wednesday of the month at Aging Tree. (For details about the Alzheimer’s Support groups at Aging Tree, contact Don Dalton at 386-3666129.) All of the VITAS support group services provided at Aging Tree are led by professional VITAS staff. Guests who attend support group sessions are never obligated to share their stories; people attend for different reasons. They may be seeking community resources, a listening ear, or simply connection or validation. For folks who may be intimidated at the idea of attending a group meeting, Shaunta Williams, admissions liaison for VITAS Healthcare, wants them to know these groups are safe places to get support. “I want them to know it’s okay to feel to feel how they feel,” said Williams. “I invite them to come to a group where it’s okay to hurt, it’s okay to cry and it’s okay to heal. National Cancer Survivors Day

What Cancer Cannot Do

T

he 29th Annual National Cancer Survivor’s Day on June 5th, is a treasured Celebration of Life held around the world for cancer survivors, family members, friends and medical professionals. It is designed to honor those recently diagnosed, provide support for families and outreach to the community. Around the globe thousands will gather to honor cancer survivors and show the world that life after cancer diagnosis can be fruitful, rewarding and even inspiring. In our own corner of the world I had the honor and privilege of having precious conversation with three cancer survivors who shared the story of their journey. Perhaps their stories will resonate with you or someone you know. Carol considers herself “one of the lucky ones” with her diagnosis of Ductal Carcinoma in Situ (DCIS) and subsequent mastectomy 15 years ago. She noted that with no family history and a healthful lifestyle some wonder, “Why me?” This thought quickly became “Why not me?” noting that cancer does not pick or choose. Carol acknowledged another thought I hear often, “Why bother (doing these healthful things)?” to which her son answered, “You are smarter than that!” The fact is that the investment we make in our health prepares us to meet the challenge of what is to come. A self-described “Dr. Google”, the need to be informed is essential for “knowledge is power” which enabled her to help a friend also diagnosed. A witty sense of humor and her belief to not take herself or anything too seriously was exemplified by Carol’s comment to her surgeon when after the guided needle biopsy was completed he said, “OK, I’m going to put a dressing on that” to which she replied,” OK, make it ranch!” The need to stay positive was echoed in her mantra, “where the mind goes the body follows”. Bob, a multi-talented and accomplished engineer, businessman and coach believes in doing things for the right reasons and understands the power of mentorship and education. We share the belief that our life’s story, whether it be financial health or personal health, is greatly influenced by our mentors as well as our choices including

whole food nutrition. Diagnosed 6 years ago with the “slow growing” kind of prostate cancer, he has taken charge and is successful at rewriting his legacy of health. The need to actively partner with health care professionals you trust is an essential component of Bob’s health story. When talking about the supports that have in some way been helpful and inspiring along the way he credits his parents, his grandchildren and his wife and best friend who is a “catalyst in keeping me on track”. Having hope and a “never give up” attitude is intertwined in all aspects of our lives. Bob noted that everyone has a purpose and it would be important to go through some sort of exercise to find out what that is. Kay describes her experience with thyroid cancer at the age of 24 as “baby cancer”. It provided an opportunity for ”learning and growing for family and demonstrated faith in action…what it means to die…not die and what it means to live”. She describes this time as a “miracle upon miracle” from the time of diagnosis when her husband was with her, to the woman who visited her in her hospital room and the surgeon who after sustaining the loss of his own daughter just the eve prior skillfully and successfully removed Kay’s cancerous thyroid. Fast forward 23 years to Kay’s diagnosis of breast cancer shortly after her mom’s diagnosis of breast cancer. Kay always had a holistic approach to health even before the cancers happened and noted there was “never a time when my body was sick that it was separate from what my psyche was doing”. Kay noted the “opportunity to grow into the seriousness of life, realizing that it is but a breath”. The new normal is “living with uncertainty and with hope”. Kay shared about a speaker who inspired her, who after experiencing 17 different types of cancers “quite possibly might die from cancer but that he doesn’t think about it much as we are called to live out each day and that is more than enough”. Kay also noted that our passion to care for one another is unique to each setting and person. The question that is the place of challenge and great opportunity is, “How do I love people well?” At some point each commented, almost

INSIDE ADVISOR People frequently fail to correctly fund their Revocable Living Trust. Funding a trust means an asset (or assets) is owned by the trust instead of an individual or jointly by multiple individuals. As a result, the asset must go through probate, which is expensive and takes a long time. Here’s an example of a conversation that often occurs in my office after someone loses a loved one: Me: I see that the home/bank account/investment account/ etc.) was owned only in your husband’s/wife’s/parent’s name, and that there are no written designations by the bank/investment house that says who gets the asset next. As a result, that asset will have to go through probate. Surviving spouse/adult child, surprised: What do you mean there has to be a probate? I/my spouse/ my parent was told there wouldn’t be any probate because he/she/they used a Revocable Living Trust. Me: I’m sure that’s true. However, did the other attorney advise you that creating a trust is just a portion of what has to be done to avoid probate?

Legal Surviving spouse/adult child: I don’t understand. Me: Well, the first part of making sure a trust will help avoid probate is writing the document. But, that’s only a part of it. The other part is making sure what you have is either owned by the trust while you’re living or immediately upon your death. Most attorneys and do-it-yourself options help with writing the trust, but they do little or nothing to help you avoid probate. What did the other lawyer do? Surviving spouse/adult child: I don’t remember/I don’t know. It was a long time ago, and my husband/wife/parent handled it all. Besides, back then, we didn’t own the items we’re talking about now. When we moved to this area, we got a new house and we transferred our investments from our original financial planner to someone we met here. Me: I’m sorry that you’re having this experience, but it’s too late to change it now. The only way for you to inherit something owned by an individual when there is no beneficiary listed is to prove that the will is authentic in court. That’s what probate means: the act or process of officially proving the authenticity and validity of a Will. So, what could this grieving widow/widower/adult child have done while their spouse/parent was living? They could have re-titled all of the assets on their own, or had assistance with doing so. In my experience, they should have had someone help them.

And assistance from a friend or loved one may result in the “blind leading the blind,” as they say. The best outcome will result from hiring a professional who frequently follows up to ensure the entire job is done correctly, and that the job stays done correctly. Use someone who will allow you to contact them at little or no expense when you obtain a new asset so he or she can help you make sure it doesn’t go through probate. So, when you see your lawyer, you ask him or her whether they will handle the re-titling of assets now and in the future. If they say, “No,” then you may want to consider hiring someone else. he third installment of a threepart series, “Alzheimer’s and Dementia,” is included in this issue of Aging Tree Illustrated, and I hope you’ve had an opportunity to read all the articles. This important series has included a host of information we should all know about progressive dementia – its causes, treatments and more. (In case you missed any articles

in the series, you can find them online at agingtree.com.) I’d like to take this opportunity to contribute to this dialogue and remind you about the importance of keeping your mind sharp… something you can do with a tool most you probably already use: your computer. Health experts who study brain function agree that one of the best things we can do to help stave off dementia is keep our minds as sharp as possible by consistently stimulating them. We must keep our brains active! Most of us have computers at our fingertips! We can experience an incredible amount of brain stimulation (up to 90 percent of what’s available “off screen”) simply by using these amazing machines. Let’s look at a small sampling of what is available online at no charge (or at a reasonable cost) that will unleash the power

of this tool (your computer) and help your brain stay healthy. Some games available on the internet are created to stimulate the visual and auditory cortex, as well as work the neuromuscular system. AARP, for example, offers “Staying Sharp Games.” Take a look at its offerings via https://stayingsharp.aarp.org/. Many other games exist online, so find the ones you enjoy and start playing! Some are free, and some require a paid subscription, so check the details as you browse. Music is another tool that can stimulate brain health and have positive effects on dementia, too. Experts don’t know exactly how it works, but listening to music from our generation and the past generations seems to ‘kick start’ our memories. (Though this may seem a little complex for the average user, Senior Computer Services can help, or even do this for you.) It’s a great way to enjoy your favorite shows time and again for no charge. A common misconception is that computers are too complex and frustrating to use. Perhaps your machine needs a tune up. Unless you have a very old system, there’s is a good chance that with a little clean up and maintenance, you can enjoy all of the internet services I mention above. If you have a newer computer model and it’s not running as well as you think it should, consider whether there may be other programs affecting its performance. We can identify and clear them up for you. The question I am most often asked is: Do I need a new computer? Most of the time the answer is no. Just like our brain, computers tend to slow down over time, and just need TLC to get back to past performance levels. Secondly, clients often tell me their computer is too complex to use. However, with some one on one training, most of us can can learn exactly what we want to know so we can use our machine effectively and efficiently. Symptoms & Prevention

Cataracts are on the primary eye diseases that an eye doctor looks for during an annual eye exam. A cataract is a clouding of the eye’s natural lens, which lies behind the iris and the pupil. The lens works much like a camera lens, focusing light onto the retina at the back of the eye. The lens also adjusts the eye’s focus, letting us see things clearly both up close and far away. The lens is mostly made of water and protein. The protein is arranged in an exact way that keeps the lens clear and allows light to pass through it. But as we age, some of the protein may clump together and start to cloud a small area of the lens, making it harder to see. Most cataracts occur gradually as we age and don’t become bothersome until after the age of 55. However, cataracts can also be present at birth (congenital cataracts) or occur at any age because of an eye injury (traumatic cataracts). Diseases such as diabetes or a long term use of certain medications, such as steroids can also cause cataracts. Cataract Symptoms A cataract will start off small that will have a little effect on your vision. You may notice a little blurred vision like looking through a cloudy piece of glass or a dirty window. As a cataract worsens you may experience the following: • Your blurred vision cannot be corrected with a change in your glasses prescription • Glare or halos around lights at night • Colors will appear faded

If you have limited income, your amount could be different and you may be eligible to get some assistance in the area. To fill in the gaps you ask yourself, should enroll in an Advantage Plan (Part C)? Or, should I take a Medicare Supplement with a prescription drug plan (Part D)? ith summer fast approaching I am reminded of the nursery rhyme, “Rain, rain go away, come on back some other day.” But a gardener might put their own twist to it and say “Rain, rain stay today and come on back another day.” With the hot days of summer

upon us giving our plants enough water is essential for growing in your garden at this time of year. You should never underestimate the importance of regular watering for your plants.

When someone comes to me and tells me they are having trouble getting their garden to do well one of the first questions I ask is, “How often do you water the plants?” A frequent response is, “Oh I water them every couple days.” While that may be enough for ornamentals it is a disastrous schedule for vegetables anytime of the year. In the summer it is often deadly. At our farm we water with all drip irrigation. This insures that our plants get the maximum benefit from the water and it minimizes evaporation. Even then we water twice a day in the fall and winter months. Once the temperatures get above 85 degrees we increase our watering to three times a day. And in the high nineties of the summer we got to 4 times a day. On average the pots are watered 2 to 3 minutes each time. This provides them with about a quart of water

by John Joslin each time. You may think this is too much but even with the drip irrigation the soil in the pots is warm and dries out quickly. Of course over watering can cause the same symptoms as under watering. Leaves will wilt and the plants may pale. If this occurs you can check the soil by simply sticking your finger a couple inches into the soil and feel for moisture. You can also purchase a moisture meter to measure the soils water content but I have found my finger to be accurate enough. If the soil does not stick to your finger or is powdery and dry you need more water. If you are watering by hand personal time constraints may make this difficult to do. I suggest a trip to your favorite hardware or box store and visit the irrigation section. For about seventy-five dollars you can set up a basic drip system with a timer valve that is more than adequate for the home garden and will let you water your plants regularly. That is how I got started. One last thing! Once you have taken care of your plants water needs don’t forget to pay attention to yours. If you work for more than 30 minutes in your garden during the summer you need to hydrate regularly too. I recommend at least 16 ounces of water before you go out in the summer heat and then one quart or 32 ounces every hour you are outside. By hydrating you will tolerate the heat much better and help avoid heat related injuries like heat stroke. If you find yourself getting thirsty it means you are already behind in your fluid intake, so drink up. Happy gardening and remember any day in your garden is a good day to have a good day. Summers are made for salads and this one is full of fresh greens, strawberries, nuts, cheese and topped with a delicious sesame dressing. Serve it as a side salad or add cooked chicken or sliced beef for a main course. Enjoy!

DIRECTIONS:

1 5oz. bag mixed greens of your choice 1 pint strawberries, hulled and sliced 1/2 cup pecans 1/2 cup feta cheese Dressing: 1/4 cup Italian dressing 1 tbl. honey 1 tbl. soy sauce 3/4 tsp. sesame oil

Combine all salad ingredients in large serving bowl. In small bowl, whisk dressing ingredients together until completely combined. Pour over salad and gently mix. Serve immediately. (This recipe adapted from Dinner Doctor original.)
