WELCOME BACK! We are so glad for this opportunity to learn, reconnect, and celebrate—in person—as we are MOVING FORWARD TOGETHER at our 73rd annual Convention & Exposition, November 15-17, 2021. At this year’s convention, you can expect timely education— as we learn from experts and from each other; keynote speakers who will engage and inspire; opportunities to meet with valued business partners; recognition of our award recipients and Leading4Life graduates; and of course, time to celebrate and connect! We look forward to having meaningful conversations as we share and learn from the many events of this past year—the pandemic, civil unrest, a growing awareness of diversity, equity, and inclusion; and of course, new licensure laws that are now reshaping assisted living. Specifically, here’s what you can expect: • 59 exceptional learning sessions, spread out over three days, that will provide new insights and information ranging from assisted living licensure to appreciating healthcare heroes. Our keynote speakers will set the tone for our convention—delving into relevant topics that will educate and inspire. • It wouldn't be an exposition without vendors who support the Association. On Monday, there will be a reception in the expo hall—which is open to all attendees. Grab something to eat and check out the vendors who will be showcasing their latest products and services. Come back again on Tuesday for additional discussions with these valued business partners and enjoy a FREE lunch in the expo hall, sponsored by Care Providers of Minnesota Service Corporation. • After Monday’s welcome reception, Monarch Healthcare Management will once again be hosting a great event for our first night of convention—ROCKFEST 3.0. It will be a great way to unwind after a first full day of education—and we know you are going to enjoy it! • All awards and Leading4Life recognition will take place after Tuesday’s keynote presentation, with a reception immediately following in the Mystic Lake showroom foyer—this, too, is open to all attendees and we hope you will stay and celebrate! Our 2021 convention theme of MOVING FORWARD TOGETHER reflects changes—and we’ve made a few. We are going green—or as green as we can—with our convention app (my.yapp.us/CPMCON21). The app will bring you the daily schedules, session information and
handouts (please download and/or print handouts for the sessions you plan to attend), venue maps, and much more— right to your phone. Download it ahead of time to create your personal schedule with sessions and events you want to attend. Secondly, we are moving to a CEU tracking system that digitally tracks the sessions attended to earn CE credits. From a mobile device (smartphone, tablet, laptop, etc.) go to the Care Providers of Minnesota CEU URL, log in, and then add the sessions as you attend them. BONUS—you can fill out session evaluations online too. Care Providers of Minnesota will add the CEUs earned at the convention to the member records. Which brings up a third change—individual convention registration. We’ve been on track to do this for a few years, and last year—our virtual convention—provided the perfect opportunity to implement. Individual registration provides the Association a better snapshot of who’s attending, which helps us to curate future education sessions. CARE PROVIDERS OF MINNESOTA CODE OF CONDUCT Care Providers of Minnesota (CPM) is committed to providing a safe, productive, and welcoming environment for all program participants and CPM staff. This policy applies to all CPM meetingrelated events, including those sponsored by organizations other than CPM but held in conjunction with CPM events, on public or private platforms. All participants, including, but not limited to, attendees, speakers, volunteers, exhibitors, CPM staff members, service providers, and all others are expected to abide by this in-person programs code of conduct. CPM has zero-tolerance for any form of discrimination or harassment. If you experience harassment or hear of any incidents of unacceptable behavior, CPM asks that you inform staff at the registration desk or Nancy McCoy, Director of Office Management, at: nmccoy@careproviders.org or (952) 851-2481, so that we can take the appropriate action. CPM reserves the right to take any action deemed necessary and appropriate in response to any incident of unacceptable behavior, including immediate removal from the event without warning or refund. CPM reserves the right to prohibit attendance at any future event, virtually or in person.

Join us for 59 breakout sessions, two keynotes, 75.75 hours of education, and earn up to 13.25 clock/contact hours.
HOW TO REGISTER
BADGES
Register online at careproviders.org/convention; click on the registration icon and fill out the form. Once your registration is received, you will receive a confirmation via email. At a later date, you will receive instructions for session preselection. (Session preselection is an important part of the convention preparation.)

Each convention attendee must register separately. You will need to register online so make sure you have your Care Providers of Minnesota username and password. (Don’t know your login? Go to: careproviders.org/Loginhelp.)

HOTEL RESERVATIONS
A block of rooms have been reserved at the Mystic Lake Hotel at a special rate for Care Providers of Minnesota. Rates start at $154.00 per night, plus tax, until October 22, 2021, or until the block is sold out. Reservations received after the cut-off date will be accepted on a space availability basis. You can now make your guest room reservation online at https://bit.ly/CPMCon21Hotel or by calling Mystic Lake at 1-800-813-7349. You can also go to https://mysticlake.com and use access code CAREMN to bring up the block while booking a reservation. The rate that you will receive is the current booking rate at time of call. You must mention Care Providers of Minnesota to be booked into the hotel. Please make every effort to stay at Mystic Lake Hotel. (In order to keep our event costs as low as possible, we rely on meeting our room block commitments with Mystic Lake Hotel.)
PARKING
There are multiple locations to park for FREE at Mystic Lake. More than 700 parking spaces are available in the Mystic Lake Center parking lot, located on the north side, by the Mystic Lake Center entrance. (At the time of printing, valet parking is unavailable.)
A name badge will be mailed to you in advance of the convention for registrations received by November 1, 2021. Name badges for registrations received after that date will be available for pick-up at convention. Name badge holders and specialty ribbons will also be available on site. All attendees must be wearing a name badge to gain admittance to the education sessions, convention events, and exposition areas.
It is the goal of Care Providers of Minnesota to hold professional events where barriers of any type do not exclude people from attending or participating. If you have a condition that requires special accommodations, please contact Care Providers of Minnesota before the event.
CANCELLATION POLICY/NO SHOW POLICY
Refunds will be issued to registrants who cancel by October 28, 2021, minus a $25 processing fee. We are unable to offer any refund after October 28, 2021, for cancelled registrations. We encourage you to find someone to take your place if possible. No shows are charged the full registration fee and will receive access to the online education materials. Care Providers of Minnesota reserves the right to cancel the Convention & Exposition. In the event of such a cancellation, all fees will be refunded. All pre-publicized individual convention events (special events, breakout session topics, speakers, etc.) remain subject to change until the event.
PRIVATE MEETING ROOM
For your convenience, the Chaska Room 1 will be available during select times for private meetings during the convention • Monday, 2:15-4:15 PM • Tuesday, 8:00 AM-3:00 PM • Wednesday, 8:30 AM-12:30 PM The room can be reserved for one-hour blocks of time only. Please make your reservation at the registration desk. Maximum 24 persons, set in conference-room style.
REFLECTION ROOM
Chaska Room 2 will be open during regular convention hours as a reflection room—a quiet space designated specfically for reflection, meditation, and prayer.

All participants, including, but not limited to, attendees, speakers, volunteers, exhibitors, CPM staff, Mystic Lake Center staff, service providers, and all others are expected to follow any federal, state, or local health and safety standards, as well as any health and safety standards that may be implemented by Mystic Lake Center after your registration, up to and through the end of the convention. The protocols may include but are not limited to social distancing, wearing a mask, providing proof of a COVID-19 vaccine. Since there may be significant changes in the pandemic between now and the convention, Care Providers of Minnesota will work with health experts, Mystic Lake Center staff, and others to have health and safety protocols in place with the well-being of convention participants in mind. The convention health and safety protocols will be emailed to all convention participants and posted on site. The finalized protocols will align with both the CDC and state MDH guidelines in place this fall.

CONGRATULATIONS 2 0 2 1 Q UA LIT Y AWA RD RECIPIENT S The American Health Care Association/National Center for Assisted Living (AHCA/NCAL) National Quality Award Program is a progressive program that is based on the Baldrige Criteria for Performance Excellence. Participants move from the bronze, to silver, and then ultimately to the gold level. Bronze recipients must demonstrate a groundwork for excellence by completing a self-assessment that outlays their organizational priorities and goals. Silver awardees outline their systematic approaches and demonstrate sustainable organizational and process results linked to their key customer requirements, success factors, and challenges. Gold recipients represent an elite group by meeting all of the demands of the Baldrige criteria. IN 2022, CARE PROVIDERS OF MINNESOTA WILL CELEBRATE ITS 75TH ANNIVERSARY—75 YEARS AS A NON-PROFIT MEMBERSHIP ASSOCIATION WITH THE MISSION OF LEADING MEMBERS TO EXCELLENCE. In 1947, the long-term care profession faced challenges that called for an effective Association to provide advocacy, resources, and business support. Over the many years, the profession evolved and grew, and the Association both responded to and led these changes. We provided—and continue to provide—what our members need to succeed! Our 2022 celebration gives our members—and the profession as a whole—a perfect opportunity to reflect on the past and set our sights on the future!
Today, the Association represents a widely diversified group of providers along the full spectrum of care and services for seniors and persons with disabilities. While we provide membership services in a far different manner from our beginning years, what hasn’t changed in 75 years is the reason for our existence—at its heart, it is still the relationship between the caregivers and the consumers of care that matters.
