H Bristol, North Somerset and South Gloucestershire CCG This year, GP practices in the Gordano area of North Somerset formed a new primary care network to help join up health and social care on behalf of local patients. Now, the four practices – Clevedon Medical Centre, Portishead Medical Group, Harbourside Family Practice and Heywood Family Practice – will be working collaboratively as Gordano Valley PCN to improve the physical health, mental health and social well-being of their 50,000 patients.
H Cardiff and Vale University Health Board NHS Wales procured OpenEyes Electronic Patient Record during the pandemic to support the Digitisation of Eye Care in NHS Wales. The EPR enables the delivery of ‘shared care’ to support ophthalmic services between the acute and primary care sectors. In 2019 Cardiff and Vale UHB had two optometrists training for a higher qualification in the treatment of Glaucoma, which was increased to 42 in training this year. The UHB has also helped the School of Optometry Science and Vision create training placements for the Higher Certificate in Glaucoma and Medical Retina for 21 optometrists.
H Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust Collaborative Newcastle’s vision is to work collectively and creatively to improve the health, wealth and well-being of everyone in Newcastle. The partnership, which also includes Newcastle upon Tyne Hospitals NHS Foundation Trust, Newcastle City Council, Newcastle Gateshead NHS Clinical Commissioning Group, is amongst the first of its kind in the country and is underpinned by a ground breaking legal agreement between the four key health and social care organisations in the city.
H Manchester University NHS Foundation Trust As of 1 April, North Manchester General Hospital formally joined Manchester University NHS Foundation Trust. This follows on from a 12 month period during which time the trust had been responsible for the delivery of services at North Manchester General Hospital through a management agreement. Extending the benefits of MFT and building on the existing expertise at NMGH will enable the provision of better, safer and more consistent care to the people of Manchester, Trafford and wider communities.

H Royal Berkshire NHS Foundation Trust Health literacy levels have a huge impact on how individuals manage their health. Royal Berkshire and Berkshire Healthcare NHS Foundation Trusts collaborated on a pilot project to promote health literacy to those aged 16+ in five local schools. Age specific health literacy training was developed focusing on understanding health literacy, evaluating information sources, and improving appointments with health professionals.
