What’s Environmental Health?
Section 2: What is Environmental Health?
There are many elements that make up our environment and contribute to our overall health. The National Environmental Health Association (NEHA) defines environmental health as the following:
“Environmental health is the science and practice of preventing human injury and illness and promoting well-being by identifying and evaluating environmental sources and hazardous agents and limiting exposures to hazardous physical, chemical, and biological agents in air, water, soil, food, and other environmental media or settings that may adversely affect human health.”
The goal of environmental health as a practiced science is to prevent and/or treat environmentally related human health problems by analyzing the relationships between social and cultural factors on one side, and chemical, physical and biological factors on the other in order to:
+ Identify what in the environment is causing health problems
+ Identify how and where in the environment people are being exposed to health hazards;
+ Identify what can be done either to reduce or eliminate human exposure to these hazards; and
+ Monitor these health risks or hazards over time to ensure continued safety of the public.

Why Environmental Health?
Section 2a: Why is EH important to Tribal Communities?
Many traditional North American Native Tribal Populations maintain intricate and ecologically interdependent relationships with the natural environment that were cultivated over centuries.2 Because of this reliance on natural resources to maintain traditional diets, lifeways, customs and languages, there is a unique need for tribal-focused research to identify the impacts of pollution, dietary exposures, cumulative risks, environmental justice issues, and climate changes.3
American Indian (AI) communities face a number of environmental hazards such as living in remote and isolated areas that expose residents to severe climatic conditions, hazardous geography, disease carrying vectors (rodents/insects), limited availability of housing and health clinics, as well as exposure to unsanitary water and wastewater methods. In addition, Tribes are facing new challenges in dealing with chronic diseases that were not major issues in past years as well other challenges associated with social determinants.
A complex set of conditions, including longer life expectancy, dramatic lifestyle changes, changes in dietary practices, pollutants, and a variety of other environmental changes, contribute to new challenges in managing chronic disease and maintaining a community’s overall health.4 Tribal Institutions have increased emphasis on restoring and protecting the health and knowledge base of Tribal communities. This requires that environmental health hazards be identified to allow lifestyle adjustments and mitigation actions to be taken to reduce the health risks. Tribes must continue to promote and develop community resources and involvement to target environmental health promotion efforts at the local level. Community environmental health must be recognized as programs and services that extend outside of health clinics and into other community resources, such as homes, schools, clinics, restaurants, water sources, and many others.4 These compounded issues, make it even more imperative to focus on building the capacity of Tribal nations to navigate and improve their own environmental health programs to address the evolving climate of health in individual communities to better prepare for disasters and emergencies that may arise. Southern Plains Tribal Health Board (SPTHB) is committed to helping our Tribal partners identity, address, and respond the issues mentioned above.
Environmental Health Facts
One in Four AI/NA face food insecurity in the United States. 5
American Indians and Alaska Natives born today have a life expectancy that is 5.5 years less than the U.S. all races population (73.0 years to 78.5 years, respectively). 6
Diseases of the heart, malignant neoplasm, unintentional injuries, and diabetes are leading causes of American Indian and Alaska Native deaths (2009-2011).

Purpose of EH Resource Guide
Section 3: Purpose of the Environmental Health (EH) Resource Guide:
SPTHB developed this Resource Guide to assist Tribes and Tribal departments to better identify their capacity to recover, respond, and mitigate environmental health hazards that arise from natural disasters and emergency situations. Since Tribal citizens experience unique risks because of their traditional lifestyles and use of natural resources, environmental health programs and emergency management (EM) responses must be tailored to the distinct practices and exposures of each community.
The goal is for the Resource Guide to be used as a tool to enable the user to efficiently look up specific environmental health and emergency management resources covering a wide variety of topics. Tribal programs will have the resources to create or develop their own assessment(s) to identify environmental health hazards present in their communities and to then address the identified health hazards in their own policies, plans, outreach, education materials, and programs based off of resources provided.
The Resource guide has links to many EH and EM topics such as creating emergency management documents (CIO, EOP, and COOP), declaring natural disasters, addressing environmental health issues, improving data collection and reporting, climate change, environmental justice, links for future collaborations/partnerships, and funding opportunities.

Section 4: Background of EH Grant
In 2020, SPTHB was awarded the Supporting Environmental Health Response, Recovery,Mitigation, and Other Expenses Related to Declared 2018/2019 Disasters Grant from the Center for Disease and Control (CDC) on August 30th, 2019. The grant had a one year timeframe and was granted a one year continuation ending on August 31st, 2022.
The grant covered five (5) main objectives to address environmental health hazards:
1. Identify & partner with Tribes in areas with declared disasters and/or emergencies
– Subaward up to 7 tribal partners to strengthen their capacity to identify and respond to EH hazard needs and to implement programs and services based on those needs
2. Identify specific needs for strengthening capacity to respond to, recover from and/or mitigate environmental health hazards that resulted from or exacerbated by declared disasters and emergencies
– Develop a needs assessment for all Tribes to assess capacity to respond to, recover from and/or mitigate environmental health hazards that resulted from or exacerbated
– Disseminate needs assessment to partnering Tribes served by SPTHB (Tribes located in Oklahoma, Texas, and Kansas)
3. Support tribal efforts to respond to and address specific capacity needs identified
– Use results from assessment to determine support needed and provide subject matter expertise to improve capacity
– Identify trainings and additional resources needed based on results of assessments
4. Strengthen environmental health data use and informatics capacity
– Assess and improve information systems and informatics capacity to support identifying and characterizing EH that result from the declared disasters and/or emergencies with tribal jurisdictions in the area we serve
– Implement a text messaging system that provides up-to-date information and support to partnering tribes
– Identify and evaluate the utility of EH datasets for identifying hazards arising from the declared disaster
– Provide training to strengthen EH professional technical competencies in using data to identify and address EH needs or hazards arising from the declared disaster
5. Prevent, and control identified environmental health hazards
– Identify and implement interventions proven to be effective in mitigating the effects of EH that resulted from the declared disasters
– Use partnerships for collaborative approaches to identify and control EH related to the declared disasters
Expected Outcomes from the EH grant:
+ Use results to determine support needed and provide subject matter expertise to improve overall capacity to handle response, recovery, and mitigation of natural disasters and emergencies
+ Identify specific trainings targeted to the needs of the tribes who participated in the assessment
+ Identify programs and interventions specific to tribal needs
+ Use partnerships for collaborative approaches to identify and control environmental hazards related to declared disasters
+ Effective interventions developed and implemented to address disaster-related EH hazards
+ Interventions demonstrated to be effective and sustainable
+ Activities to strengthen the ability to meet public health needs in a comprehensive manner
+ Increased implementation of tools and processes that build operational capacity and effectiveness
+ Increased use of core and discipline-specific public health competencies among public health workers
+ Improved collection, maintenance, interpretation, and dissemination of tribal health data

Development of EH Needs Assessment

Creating & Disseminating
Section 1: Creating & Disseminating the EH Needs Assessment
One of the major objectives of the grant SPTHB received was to create and disseminate an Environmental Health and Hazards Needs Assessment to our Tribal partners. The goal of the EH Needs Assessment was to incorporate as many Tribal specific environmental health questions as possible into a survey that would take under 30 minutes to complete.
We wanted a comprehensive document that looked at which environmental health hazards may be present in the individual communities and what current policies and procedures are in place to address, respond to, and mitigate these hazards. In addition, we wanted to know how knowledgeable community members and the workforce were about the hazards present. The assessment also aimed to open discussions and collaborations regarding the environmental health of our participants.
One document that was essential in creating our needs assessment was the Southern Area Health Education Center & NM Department of Health Environmental Health Epidemiology Bureau’s Community Environmental Health Assessment Toolbox for New Mexico (2004). In this document, you will find many links, tables, examples assessment questions, and resources for creating your own document.
Another document that SPTHB utilized in creating the needs assessment was NACCHO’s Protocol for Assessing Community Excellence in Environmental Health (PACE EH) Guidebook. In this document, it listed 13 tasks in the methodology section to focus on while creating an environmental health assessment:

+ Task 3: Assemble a community-based environmental health assessment team
+ Task 4: Define the goals, objectives, and scope of the assessment
+ Task 5: Generate a list of community-specific environmental health issues

+ Task 7: Develop locally appropriate indicators
+ Task 8: Select standards against which local status can be compared
+ Task 9: Create issue profiles
+ Task 10: Rank the issues
+ Task 11: Set priorities for action

Major Steps in Creating the SPTHB EH Needs Assessment:
+ Identify the target audience completing the assessment
+ Choose which topics were the best indicators of environmental health programs
+ Word questions in a way to get the best results to analyze toward a specific outcome (figure out lexicon, qualitative vs quantitative questions, and assuring clear/concise wording)
+ Figure out how you are distributing and conducting the assessment (survey link via email, phone interview, in-person survey, etc.)
+ Choose how you are going to analyze the data and what the main outcomes you want from the data (survey programming)
+ Understand what follow up and deliverables are you going to be providing to the participants
+ How are you going to gain participation and buy-in with the community and programs? (ex: incentives)

Target Audience
Section 2a: Target Audience
The target audience for the needs assessment where Tribal leaders, council members, Tribal department heads, Tribal health clinicians, and Tribal emergency management personnel. SPTHB put together a comprehensive list of contacts for each tribe and reached out via email or phone to set up individual survey phone interview.
There was a total of 19 individuals from 14 separate Tribes who participated in the needs assessment.
Needs Assessment
Section 2b: Needs Assessment Description
The needs assessment consisted of 25 questions. The questions were multiple choice, check all that apply, open ended, and short statement questions on the survey. The survey took about 30 – 35 minutes to complete on average.
The surveys were conducted via phone interview with our Tribal participants. If the interview was conducted via Teams, then the interviews were recorded (with the participants consent). The phone interviews allowed us to dive into each question and response in a deeper, more comprehensive manner.

Interpret Results
Section 3: How to Interpret the Results of the EH Needs Assessment
The responses to the EH needs assessment were put into Survey Monkey to analyze them more efficiently and cohesively. Each topic and issue was then used to create a picture of the current landscape of environmental health in each of the Tribal communities that participated. The questions were also summarized together to get a general overview of the EH Tribal programs that are present currently.
The results were made into a comprehensive analysis report for internal uses. The analysis was used to create multiple infographic educational resources added to our online resource database to be shared with our social media accounts.
The results of each individual needs assessment were shared with the corresponding Tribal participant along with a detailed analysis of strengths, weaknesses, gaps, and opportunities found in the assessment.
In addition, each tribal participant was sent an in-depth document with resource, pricing, and links to any additional trainings that were identified in their needs assessment to better build the capacity of the tribe to respond, recover, and mitigate from natural disaster and/or emergencies.

How Knowledgeable are Community Members Regarding EH Issues?
13 participants (80%) identified that their community members were “somewhat knowledgeable” about the environmental health and hazard issues listed above in their communities.

+ 80% of Tribes who participated listed that they have a current Emergency Operating Plan (EOP)
+ 73% of Tribes surveyed listed that they have used their EOP in the last 5 years
+ 50% of Tribes listed that there is not a tool/procedure to assess which areas are impacted by EH factors after a natural disaster or emergency
+ 50% of Tribes surveyed stated that they currently have an Environmental Health Department
+ 100% of Tribes listed that they have the need for additional training on emergency management or environmental health issues. (For a full list of trainings identified by Tribal participants in Section 5 of this document)
+ 40% of Tribes reported that they do not have systems in place to track or monitor EH issues
+ 36% of tribal participants are “UNSURE”

Section 4: Subaward Project Identified from the EH Needs Assessments
Based on the analysis of the EH needs assessment responses, we were able to fund an additional 4 EH projects with our Tribal partners. (NOTE: names of tribes will remain confidential)
1. Created education and outreach opportunities for the tribal community members. At least two Tribal community outreach events where pre‐filled emergency preparedness backpacks were handed out. The backpacks included but were not limited to multi‐function emergency radio, light sticks, emergency whistle, first aid/trauma kits, survival blanket, poncho, hand warmers, waterproof matches, Swiss army knife, rope, masks, etc.
2. Purchased five (5) defibrillators; Six (6) portable generators; Two hundred and forty‐two (242) hand‐crank weather radios (one per household); CPR and AED training for five employees; and provide Data Analysis Training for three (3) people in the Tribe.
3. Installation of two (2) 8’ X 20’ Safe Rooms. The Safe Rooms can hold up to 55 individuals.
4. Purchased one (1) 10,000‐gallon vertical water storage tank; Twenty‐five (25) Portable generator inverters; Two (2) truck bed water tanks; Two hundred (200) customized cooler packs; Two‐hundred and fifty (250) 5‐gallon portable water containers; and materials (pamphlets) for education and outreach
