Karma Mobility unveils ergonomic wheelchair
Mobility solution manufacturer Karma Mobility has unveiled a new ergonomic wheelchair which reduces the effort needed to maintain an upright posture and enables the user to enjoy social interactions with more comfort.
The Ergo 3 self-propelled wheelchair is a lightweight crash-tested model that incorporates Karma Mobility’s S-Ergo seat system, providing good levels of support and comfort.

Disability aids sold online to include VAT
Mobility retailers supplying disability aids on online marketplaces must include VAT, HMRC has confirmed.
An HMRC spokesperson confirmed to THIIS: “HMRC will challenge the zero-rating of any supplies that are not supported by evidence. In addition, where a seller operating on an online marketplace has failed to account for the correct amount of VAT, HMRC has the power to issue a notice to the online marketplace requiring them to remove the seller from their platform or otherwise holding them jointly and severally liable for the VAT.”
Julia Phipps, Managing Director of Easier To, a supplier of mobility and daily living aids on Amazon, said she received confirmation from HMRC in May 2024 that her prices must include VAT. If the customer is eligible to purchase the item with VAT relief, then the customer must complete a form which must be returned to the seller. The seller must then refund the VAT element to the customer, reducing their next VAT return by the amount of VAT in the order which has been refunded to the customer.
Julia comments: “It is now probable that Amazon and eBay sellers will have to spend days/weeks re-pricing their items on marketplaces, before HMRC starts to undertake VAT compliance inspections which will now likely take place within this sector.”

RETAILERS COUNTING THE COST FROM AUTUMN BUDGET PROPOSALS
Retailers will be counting the cost of the recent Budget, delivered by the Chancellor of the Exchequer Rachel Reeves last month, according to Helen Dickinson, Chief Executive of the British Retail Consortium.
Helen said: “Retail employs three million people and 2.7 million more across supply chains, driving investment in jobs, communities and, ultimately, economic growth, right across the country.
"For a low margin industry, this Budget will hit hard, with the odds now stacked firmly against growth and investment in the short term. These new costs also risk increasing the prices customers pay at the till."

Tackling the above issues is not as difficult as you might think. You just need to take one step at a time and have a plan. Look at what will make the most significant difference, and do it now. If you do things quickly and systematically, you’ll start to see results. There are no shortcuts – it takes action and persistence.
But anything worth doing is worth doing well, and it should set you up for the future.

Independence Mobility has been supplying specialist healthcare equipment in Leeds since 2004. The business supplies a wide range of specialist seating, wheelchairs, beds, children’s cycles, pressure care products, moving and handling equipment, and more, for both adults and children.
As well as its commitment to consumers, Independence Mobility also works closely with physiotherapists, occupational therapists and other clinical professionals. Its vast experience in the healthcare industry means that its staff are well placed to provide advice and support to ensure its clients receive the best equipment for their needs.
Anthony Mitchell, Managing Director of Independence Mobility, initially set up the business as a small mobility products supplier providing entry-level
wheelchairs, scooters and beds directly to end-users with fairly basic needs.
Anthony comments: “After trialling the business at a local Morrisons store and then at Leeds Outdoor Market in 2004, we moved into our first indoor retail premises in 2006. This was on York Road in Leeds. We were there for around three years before the business expanded further. Since then, we have moved twice more into increasingly larger premises in Meanwood in Leeds.”
The business has changed phenomenally over the years, says Anthony, and looks very different today, with a much broader range of products that also cater for customers with more acute and complex needs.
Anthony adds: “A large part of our work is supporting occupational therapists, physiotherapists and other healthcare professionals in prescribing
mobility solutions for their clients, so we are predominantly visiting clients at their homes or in care settings.
“Prior to visits, we will have spoken to the OT about their client’s condition, so we can take a suitable range of equipment along to the appointment for the client to try.”
This, says Anthony, is much more convenient for clients rather than them having to come to them. Although the business does now

have an assessment zone set up at a rehabilitation unit in nearby Morley, where customers can come if they prefer.
The assessment zone showcases a range of equipment, including wheelchairs and specialist seating and it is used, by appointment only, for OTs and customers wishing to have assessments and product demonstrations there, rather than in their homes.
EXPANSION PLANS
Anthony has also set up Medical Hire, a business which provides medical props to hire for TV and film production. With both sides of the business continuing to grow Anthony explains that they are looking to move to larger premises as soon as possible. This will enable the business to better accommodate its existing stocks and allow it to continue to expand its product ranges.
Anthony shares: “In our new premises, we are planning to have a larger workshop area for our engineers to carry out more equipment servicing and maintenance work.
“We appreciate that mobility equipment is a big investment for our customers, so we want to help them get the best out of their piece of equipment throughout its expected lifetime.
“A larger premises will also enable us to set up an on-site showroom, as well as maintaining access to our off-site assessment zone. This will give customers more access for product demonstrations and assessments.”
Not that having a showroom has been an issue for the business so far.
Independence Mobility has always been successful in attracting new customers through word-of-mouth and recommendations.
“We are very well-known in our immediate local area and are becoming increasingly known in further surrounding areas,” says Anthony. “We are lucky to have a fantastic marketing team who continually raise awareness through social media, events, and other digital communications.”
Due to the breadth of its product range Independence Mobility sees a wide range of customers, including adults and children – many with fairly moderate needs and others with much more complex needs.
In terms of where customers are based, Anthony explains that the business can be limited in some cases as to where it can offer certain products. This is due to some of its suppliers having defined geographical areas for their dealers.
Anthony continues: “Where we are not restricted, we do support customers further afield as much as we can. As our team grows and our product range further develops, we hope to be able to do that much more.”
At some point in the future, Anthony says that the plan is to sell some products online. This will enable it to supply products even more widely.
Anthony adds: “Our website is very important to us as it portrays who we are and showcases what we do.”
GIVING BACK
Independence Mobility has a built a strong connection with its local community simply by the strong
working relationships that it has built up over the years, both with its customers and the healthcare professionals that it supports.
“We have many customers who repeatedly come back to us when they need new equipment, or they need help with their existing equipment,” says Anthony. “We are proud to be able to support them throughout their lives from childhood through to adulthood with the products that we offer.”
Staff at the business have also been supporting the local community through various fundraising initiatives. As Anthony explains, staff will often gather donations together to give to local shelters and food banks. Over the last year, they have been doing some fundraising to raise money for the mental health charity Leeds Mind. Next year they are planning to support a wider range of charities by pledging their time in many different ways.

opportunities and showcase the positive impact they have on people living with disabilities.
Most recently, staff completed a ‘2020 Challenge’, the last in its initiative to raise funds for Leeds Mind. Anthony says that the idea came about as part of its 20th birthday celebrations. He explains: “Each team member has been undertaking a challenge that incorporates the number 20, whether that’s 20 times, 20 minutes, 20 days, and so on. The challenges have been something that’s personal to them. For example, they have included meditating for 20 minutes a day for 20 days; driving to 20 destinations that would normally be avoided as a nervous driver; spending 20 hours baking to make items to sell.”
The target for Leeds Mind for the year was £1,000, so the 20-20 Challenge has been really helping staff to reach that target, and the good news, says Anthony, is they are very nearly there!
Recently staff at Independence Mobility came together to map out their core company values.
“These values help to guide us in delivering the very best for our customers and are a reminder of who we are and what we stand for,” says Anthony. “We are passionate about changing lives for the better through the provision of mobility equipment.”
Some of these values, he says, include being honest and reliable with customers to ensure that they get the best solution for their needs.
As a business, Anthony says, staff set out to be detail orientated, knowing that small details make a big difference to customers. It is also about staff
using their problem-solving skills and thinking outside of the box to find innovative solutions, and sharing their knowledge to support healthcare professionals to support their clients.

The recently published DHSC Policy Paper 'Design for Life Roadmap' concerns the building of a circular economy for medical technology, but are its timescales ambitious enough?
The 2024 policy paper from the Department of Health and Social Care (DHSC) entitled 'Design for Life Roadmap – Building a circular economy for medical technology'1 contains 30 ‘actions’ across six ‘problem statements’ to set out the fundamental challenges which the DHSC aims to address to develop and embed a circular system. By addressing these challenges, the programme will aim to deliver a circular system by 2045. 2045?
Many medtech companies are already well down that path: it is the behemoth of the public sector which needs to change, and with a much greater sense of urgency!
The arguments for a circular economy are strong.
• The DHSC calculates that a functioning circular system will mean that more of the £10 billion the NHS spends on medical devices could be spent in the UK.
• By reducing reliance on volatile supply chains, and growing local capacity to meet demand, circularity will protect health systems from global supply shocks.
• Circular approaches to medtech will reduce waste and carbon emissions – the paper quotes, for example, that the NHS throws away 133,000 tonnes of plastic each year, which is roughly the same weight as all the household waste generated in Leicester, but
the NHS only recovers a ninth as much of the plastic2 .
• Circular devices are often more cost-effective than single-use. In several case studies, savings of more than 50% could be realised compared with conventional single-use equivalents
SENSE OF URGENCY?
The roadmap’s 30 actions have been divided into how ‘mature’ they are, where indicatively high-maturity actions could be completed in 2024 to 2027, medium-maturity actions in 2027 to 2029, and low-maturity actions in 2029 to 2031. The outcome is that there’s only three actions under the high maturity heading, the reason being that the DHSC deems the others to require significant research activity before they can commence.
The three actions to complete over the next three years are bland statements to collaborate with other policy-making bodies, present a full ecosystem roadmap, and to have the circularity of medtech embedded in communications, engagements, and strategies.
The paper states that the most relevant problem for clinicians and patients is behavioural change, but the action to develop a behavioural change plan is not until the 2029-2031 window. The paper spells out clearly that it is behavioural changes that will drive the rest.
Other actions which are kicked down the road into the same time window include: “Develop a decontamination infrastructure framework” and “Establish a materials recovery and recycling framework”. These structures are already in place within local councils through their recycling pathways and their loan stores upon which the NHS could base these actions within a much shorter timescale.
Under the Commercial Incentivisation platform, the paper hits right at the core of the problem. Procurement is beset by short-term restrictions prescribed from above: short-term budget restraints, shortterm cost savings and lower up-front costs rather than life-time value.
While these drivers are in place, procurement behavioural changes will not occur. NHS Supply Chain agreements will continue to be based on what they have always done, and on how things are described, and not on outcomes – neither clinical nor lifetime cost benefits.
WHAT DO WE OFFER ALREADY?
Much of the paper concentrates on the acute sector and what hospitals are doing with the procurement, use, and disposal of their instruments. Looking back over the 40 years where I’ve been involved with both assistive products and with equipment decontamination I see many areas where those of us in the medtech sector have already been offering solutions right across the board in the paper’s circular economy system (Figure 1).
RECYCLE
Looking at products that BES Healthcare offers, as an example, that can be recycled we have been selling washer-disinfection machines for nearly 30 years, and when the user decides to upgrade their model we have either refurbished the old machine for reuse by another site, or have stripped it down so that the stainless steel can be recycled.
Excitingly, we have recently taken on the line of NEST mattresses and cushions (Figure 2)3 where, unlike most PU foam cushions, the core material can be washed, disinfected, and reused for up to eight years. Once its useful life has ended, it can be shredded into small pieces that can be recycled back into the original core plastic.
Many loan stores have skips being filled with metal items that have ended their useable life, ready to go off to
the scrapyard to be recycled. The community side is already ahead of the game in many respects.
REMANUFACTURE
Children’s equipment needs change regularly as the child grows. Our industry has, for many years, been providing equipment that can be adjusted to meet these changing needs, and then later also be refurbished for re-use with another child.
About 35 years ago, John Prout identified that the original Scandinavian Tripp Trapp high chair could be modified to make it more adapted to and safer for youngsters with behavioural or mild physical challenges - which he incorporated into the Breezi range (Figure 3)4. The chair can be adjusted for the individual as they grow and their needs change. Thereafter, the modularity of the components and accessories have meant that an original chair can have items replaced or upgraded to meet the specific needs of the next user, without the need for a completely new item. As we have updated the designs of these elements
we have worked to make sure that they are retrofittable to the original models.
REPAIR AND REUSE
Wheelchair services and loan stores have long been providing repair and reuse services, and so again the community side of the NHS and social services have led the way: this was streamlined in the 1990s with the creation of the Integrated Community Equipment Stores. It was these that fitted themselves out with automated washer-disinfectors for labour-efficient and reproducible cleaning and disinfection of the loan equipment, and at the same time protecting staff from the risk of infection from the aerosols created by hand-scrubbing or steamcleaning the equipment.
The tightening of regulations and
standards has moved forward the decontamination side of reuse. The MDR has made it explicit that CEmarked medical devices can only be decontaminated by chemicals and equipment that are themselves designated as CE-marked medical devices. New standards such as those in the BS EN ISO 15883 series5,6 provide the requirements which this equipment shall meet.
On the re-use front, having products that can be adjusted and modified as the user’s body shape or needs change or that can be adapted to another user is a route already available in the primary care market. What is valuable here is that there’s a plethora of manufacturers of ‘aftermarket’ accessories that can be used to broaden and extend the use of the original products – items that have been designed and CE-marked for this purpose.

One aspect that is not really tackled in depth in the paper is the usable life of a product – spending twice as much in the first place might source a product that lasts ten times as long!
Examples from my personal experience go back over 30 years. Foam cushions have been in use for a very long time and are great pressure care products when new, but their shelf life is quite short, and the foam develops a ‘set’ quite quickly. Cascade Designs came along with the Varilite range of cushions8, using the technology from their popular ThermaRest camping mattresses, and placed foam into an adjustable airtight bag that provided the envelopment provided by the trapped air with the stability of foam, but without the foam getting fully compressed, which extensively extended the life of the foam.
When we introduced the Bodypoint range of belts and harnesses, the first range to meet the ISO 168403 performance tests9, I received an apology from the clinical engineer at what was then Stanmore’s wheelchair service that their ordering had decreased since the belts lasted the ‘lifetime’ guarantee, and then could still be reused on other chairs. Paying a little extra for quality on day one provides savings over a lifetime.
The Matrix seating system, now also available in the updated FreeForm seating system (Figure 5)10, is made up of small adjustable components which allow for regular adjustment to meet a client’s changing needs for their life, and in a more compact and lightweight alternative to the likes of foam carved or moulded seating systems – and in a system that often can be built at the client’s first fitting. The components can later be re-used in other seating systems.
Unlike foam cushions and mattresses that cannot be readily cleaned and disinfected if soiled, the NEST cushions and mattresses3 can be decontaminated as often as needed in a washing machine or washer-disinfector, extending their life considerably, and then tick the sustainability box at the end of life by being shredded and recycled.
CONCLUSION
Along the roadmap for a circular economy, the primary care sector is already well down the road, and the acute sector could make accelerated progress by learning from what is already on offer. The main obstacles that need to be addressed are the deep rooted behavioural ones around product specification and prescription, and the short-term drivers of our procurement processes. These issues
need to be tackled right at the outset to give change a chance: adopting a pilot approach might be a useful way forward to reduce the perceived risk of transformation? However, how many pilots have we seen in the NHS where the Not Invented Here syndrome permeates the system? And why not be more ambitious and target this for three years’ time rather than seven years?

What do the proposals outlined in the recent Autumn Budget by Chancellor Rachel Reeves mean for your retail business?
“As anticipated, the new Government’s Autumn Budget has introduced significant changes. However, for many, it feels like using a sledgehammer to crack an egg, particularly when examining its impact on businesses – mobility dealers in particular – who have faced unprecedented challenges over the past four years.
“The COVID-19 pandemic, for instance, forced businesses to shut down, and many are relying on CBILS loans to survive. They include rising shipping costs and surcharges, coupled with delays which have doubled product lead times. The RussianUkrainian war, with its recent Red Sea disruptions as well as conflicts in the Middle East have also created further instability. These challenges have left businesses navigating a rocky road, and the Autumn Budget’s measures only add to the uncertainty.
“From a business perspective, the measures are necessary but overly severe.
"For instance, many businesses are still repaying CBILS (Coronavirus Business Interruption Loan Scheme) loans, with at least 18 months left for most to clear this debt. They are also battling rising staff wages, coupled with an increase in the minimum wage and higher National Insurance Contributions, which will all significantly impact short-term growth and investment.
“Another concern is with regards to health and social care funding. While £190 billion has been allocated to healthcare and social care, the focus should shift to utilising these funds more efficiently to reduce waste and improve outcomes. Throwing money at the problem without addressing systemic inefficiencies will not yield quick solutions.
“There are a few short-term
implications too. Confidence in investment will be delayed as businesses await signs of growth and stability. Most companies are likely to hold off on expansion or hiring plans.
“Hiring will also be scrutinised more critically, with business owners weighing whether new roles are truly essential. For high-end private sector businesses like Recare, there may be short-term benefits as individuals seek to maintain their quality of life amidst NHS and social care challenges.
“Rapidly injecting funds into the public sector won’t resolve staffing shortages. Training doctors, nurses, and other medical staff takes time, meaning short-term solutions remain elusive.
“The next two years are likely to be challenging for businesses as they navigate ongoing loan repayments, rising operational costs and a cautious approach to investment and hiring.
“However, amidst the uncertainty, there is an opportunity for the private sector to bridge gaps in healthcare and mobility services, providing solutions to customers seeking quality-of-life improvements.
“It seems likely to me that most businesses will adopt a wait-and-see approach over the next 12 months.
Employment decisions will focus on critical needs rather than wants and investment will be delayed until there is greater confidence in the future.
“In conclusion, while the Autumn Budget addresses urgent issues, its implementation poses significant challenges for businesses. A balanced, strategic approach is crucial to ensure long-term growth and stability.”

“Let us be very honest, the treatment of small and medium enterprises (SMEs) and charities by the new government in terms of the Employers National Insurance threshold reduction has been a total disgrace. SMEs are told by successive governments that they are the lifeblood of the economy – generating 25 per cent of the nation’s total GDP – yet will be hit the hardest by the announcement made in the Budget.
“Anyone who understands the sector knows many SMEs already survive on

“The increase in the minimum wage combined with the added national insurance contributions by employers is in real terms, another tax. The added blow for the removal of winter fuel payments for elderly and disabled people will hit mobility dealers, as potential customers think twice about purchasing large ticket items.
“To put the added costs into perspective, on a personal level the added expense is equivalent to a 66 per cent increase in the three properties we rent. Alternatively, it is the same costs as employing a new member of staff, but getting nothing for it. In product terms, Thistle Help need to sell an additional 28 recliners just to cover the chancellor’s increase.
“What people forget too, is the increase of 1.2 per cent for employer national insurance is really more

“Despite this, SMEs have been treated the same as large corporations that have the profits and resources to shield themselves from tax rises, less favourably than micro businesses with fewer than 10 employees and indeed Government Departments. Is this really fair and ethical?
“Ahead of the Autumn Budget, the business community was braced for some tax rises together with an increase in National Living Wage (although 6.7 per cent was probably two per cent higher than most businesses budgeted for). Indeed, most SMEs want to pay their staff well and pay their fair share of taxes.
"However, the real shock was the reduction in Employers NI earnings threshold from £9,100 to £5,000 for each employee, equating to an increase of £615 per year.
"That is, £6,150 for 10 employees, £61,500 for 100 employees and for an SME employing 250 staff a staggering £153,750.
than this when you consider the reduced thresholds combined with the minimum wage increase. It will mean it is more like a two per cent increase. I did say at the time of the general election, businesses like ours would be hit heavily and sadly, my prediction has come true.
“I do fear for the whole economy as everything is going to go up in price. Thistle Help are already looking at our margins to try and claw back some of the added expense caused by the recent budget. We run a tight ship and there is very little costs we can cut back on.
“SME businesses are grappling with other financial burdens on top of the unexpected reduction in the threshold, not least ever-increasing operational costs, the relatively recent rise in Corporation Tax, increased employers pension contributions, together with the Budget’s 1.2 per cent increase in overall Employers NI to 15 per cent and the rise in NLW. All combined, will inevitably see many SMEs disappear.
“In summary, whilst the Chancellor takes a Macro view, she has chosen to turn a blind eye to the devastating implications of the reduction in the NI threshold on SMEs, which will inevitably see a proportion disappear.
