SUMMARY An early and correct diagnosis of altitude disease is a challenge for any physician at high and extreme altitude. With the infrastructure of a hospital or while performing scientific studies the diagnosis may be easy. Principally it is simple, if appropriate equipment is available. But this is often missing when people climb the mountains of the world. With several cases treated in the field used as examples the paper presented here explains the procedure to make a valid diagnosis within a minimum of time and without technical equipment. Altitude diseases occur in subacute hypobaric hypoxia, if the subject is not acclimatized for the respective altitude. If the problem is not sufficiently cured the outcome may be fatal. ZUSAMMENFASSUNG Die schnelle und richtige Diagnose von Höhenerkrankungen stellt beim Aufenthalt in großen und extremen Höhen oft die Herausforderung für einen begleitenden Arzt dar. In einem Spital oder im Rahmen von Studien auf einer gesicherten Unterkunft ist ihre Diagnose nun wahrlich keine große medizinische Leistung, denn ihre Diagnose ist eigentlich sehr simpel bei entsprechender technischer Ausrüstung, die in den Bergen der Welt jedoch immer fehlt. Anhand eigener erlebter Kasuistiken wird dargestellt, wie ihre Diagnose schnell und sicher gestellt werden kann, trotz fehlender technischer Hilfsmittel.
